2 RADIATION ONCOLOGY SERVICES Mission Search s Radiation Oncology Division specializes in permanent and locum tenens placement of Radiation Oncology Physicians, Medical Physicists, Medical Dosimetrists and Radiation Therapists. We also offer remote dosimetry treatment planning services to accommodate the needs of our clients domestically and abroad. We work with free-standing cancer centers, community cancer centers, major medical centers and NCI designated Comprehensive Cancer Centers. We provide our clients with highly qualified professionals, from staff positions to directorships, for permanent placement and locum tenens assignments in the following specialties: Radiation Oncology Physicians Administrators Center Directors Program Managers Directors Medical Physics Medical Physicists Dosimetrists Radiation Therapists Research Compliance Oncology Nursing Leadership Vendor Services Remote Dosimetry Treatment Planning At Mission Search, we provide a full service approach to our Radiation Oncology Clients by offering Remote Dosimetry Treatment Planning in addition to permanent and locum staffing. After an assessment of your needs, an expert Mission Search Remote Dosimetry Implementation Consultant will discuss our program with your Radiation Oncologists and the members of your radiation oncology team. Our Consultant will work with your team to review the service and prepare your department to go live. We will tailor a program to meet your organization s specific needs. Using remote treatment planning is cost effective and allows your organization to be perpetually prepared for the growth of your practice. With our staff of highly qualified, well-trained and experienced Remote Dosimetrists you will handle increased caseload without adding FTE employees. Our Chief of Dosimetry Services will monitor the timeliness and quality of output and discuss these measures with you on a regular basis. You will receive a monthly report tracking your usage by CRT code. Your Dosimetry staffing and treatment planning issues will no longer be a roadblock to an effective, highly productive and financially viable radiation oncology process continuum. Mission Search Radiation Oncology offers a unique Locum to Remote program that will reassure your entire your Radiation Oncology team and solve your staffing dilemma
3 "The Radiation Oncology Specialists" "SOLUTION SAVINGS" STAFFING PROGRAM In order to provide a full service component to our clients, Mission Search offers a financially attractive program for your consideration. For each locum Radiation Oncologist, Physicist, Dosimetrist, or Radiation Therapist on assignment at your facility through Mission Search, we offer a significant credit to be applied toward the placement of a permanent Radiation Oncology professional at your facility. The amount of the credit to be applied toward a permanent placement is: Physicist Dosimetrist Radiation Therapist Interim Management Radiation Oncologist $500 for each full-time week on assignment* $400 for each full-time week on assignment* $300 for full-time each week on assignment* $500 for each full-time week on assignment* $500 for each full-time week on assignment* *Effective for all assignments up to 26 consecutive weeks.

5 SERVICE AGREEMENT DATE: CLIENT: CITY/STATE: SERVICES: As part of our service to our clients, Mission Search Contract Services, Inc. ( Service Provider ) provides written confirmation of our Services Agreement ( Agreement ). Service Provider has represented to Client that it has knowledge of and a business relationship with Healthcare Professionals ( Candidate(s) ) whom Service Provider will provide to Client on a contractual basis. Service Provider and Client agree that the terms and conditions of this Agreement shall continue to apply unless 30 day written notice shall be given by either party. FEES: Service Provider Fee. Service Provider shall provide service fee and expense amounts to client on Addendum A (see Attachment) prior to Client acceptance of candidate. Conversion Fee. In the event that Candidate s relationship with Client is converted from a Service Provider contractual relationship into a direct employment relationship with Client within one (1) year from the completion date of this Service Provider contract, Service Provider shall be due a Conversion Fee equal to twenty-five percent (25%) of the Candidate s first year annual base salary as a permanent employee of Client. TERMINATION: Client has the right to have Candidate replaced at Client s discretion and Service Provider has the right to replace Candidate within fifteen (15) working days after notification by Client of Candidate s termination. BILLINGS AND PAYMENT: Service Provider Fees and Expenses. On a weekly basis, Service Provider will bill Client for Candidate hours worked based upon Candidate s approved time sheet. Service Provider Fee and Expense invoices are due upon receipt. Conversion Fees. Conversion Fees will be invoiced upon conversion date. Conversion Fee invoices are due upon receipt. Service Provider Fee and Conversion Fee invoices that are greater than thirty (30) days old will be charged a late fee at the rate of eighteen percent (18%) per annum. LICENSES AND INSURANCE: All candidates provided by Service Provider on a contractual basis will have their own professional liability coverage and/or will have coverage provided by Mission Search. REFERENCES: Service Provider will perform reference and background checks and verification of education, certification and licensure of Candidate prior to Candidate performing services under this Agreement. CONFIDENTIALITY: Client agrees to hold in confidence all information concerning Service Provider Candidate(s) and agrees not to disclose such information to third parties. In the event that Candidate information is disclosed to a third party by Client and such third party enters into a service or employment relationship with a Candidate, Client shall be responsible for the payment of a Conversion Fee to Service Provider as calculated in FEES above. Service Provider agrees to hold in confidence all information of Client specifically identified by Client to Service Provider as Confidential Information. Any Candidate presented and/or referred by Service Provider to Client and not identified by Client as a known Candidate by Client prior to Candidate being interviewed (either via telephone or face-to-face) by Client, shall be considered a Service Provider s Candidate for one (1) year from the date of Service Provider s most recent communication to Client relating to the Candidate. If any such Candidate is hired/contracted by a third party, to whom such Candidate information was disclosed by Client, for any position within such one (1) year period, Client shall be obligated to pay Service Provider the applicable Service Provider Fee and/or Conversion Fee as provided herein. DIVERSITY POLICY: Service Provider works with all candidates and employers without regard to race, color, religion, sex, age, disabilities or national origin. Service Provider will not accept any assignments from employers or candidates based on the discrimination of any of the above qualities or characteristics of candidates or employers. TERMS AND CONDITIONS: Client s verbal approval and/or signature below or the Client s receipt of presented and/or referred Service Provider s Candidate(s) shall establish that the Client has accepted the terms and condition of this Agreement. If Client does not agree to the terms and conditions of this Agreement, please contact us immediately. Thank you for the opportunity to be of service. 7 CONTINGENCY SEARCH AGREEMENT DATE: CLIENT: CITY/STATE: POSITION: SALARY RANGE: CONTINGENCY SEARCH SERVICES: As part of our service to our Clients, Mission Search provides written confirmation of our agreement. As agreed, Mission Search s service fee is on a contingency basis. It would be payable only if a Mission Search candidate enters into an employment relationship with the Client or an organization affiliated with the Client within one year after Mission Search s most recent communication to the Client relating to the candidate. Mission Search s contingency service fee shall be twenty-five (25%) percent of the candidate's first year's base salary. Any candidate presented by Mission Search to the Client shall be considered a Mission Search candidate for one year from the date of Mission Search s most recent communication to the Client relating to the candidate and if hired by Client for any position will create a fee obligation from Client to Mission Search at the previously stated contingency service fee rate. GUARANTEE: Mission Search has a three-month candidate guarantee policy. If the employment relationship between the Client and the Mission Search candidate terminates before the ninetieth day, for any reason except for termination due to the position being eliminated or because of insufficient work, Mission Search will find a replacement candidate at no additional cost to the Client. Client must provide to Mission Search written notice of the termination and reasons for it within five (5) business days after termination and must have paid the contingency service fee when due. FEES: The contingency service fee will be invoiced when the Client and the Mission Search candidate agree to enter into the employment relationship, and will be due and payable in full within ten (10) days after the candidate s employment start date. Mission Search s guarantee is conditional upon receipt of payment when due. REFERENCES: Mission Search will perform reference checks and education verification of candidates prior to extension of offers. CONFIDENTIALITY: Mission Search candidates will be referred to the Client in confidence, and the Client agrees not to refer or identify them to any other company. Should the Client do so, and the other company enters into a service or employment relationship with the candidate, the Client is responsible for the payment of the contingency service fee. DIVERSITY POLICY: Mission Search does not discriminate in the presentation or referral of candidates on the basis of race, color, religion, sex, age, national origin, marital status, disability, or other protected characteristics. TERMS AND CONDITIONS: The Client s verbal approval and/or signature below and the Client s receipt of presented and/or referred Mission Search candidates shall establish that the Client has accepted the terms and condition of this Agreement.
