8 Collec*on A]er delivery of baby but before delivery of placenta Wipe cord with sterile 4x4 gauze Removes maternal blood that could contaminate gra] Scrub distal end of cord for 10 seconds with Povidone Iodine Disinfects to prevent bacterial contamina*on With tubing clamp closed, introduce needle into umbilical vein. Release clamp. Prevents introduc*on of airborne organisms

12 Contraindica*ons for Collec*on Premature Birth (<34 weeks) Significant placental tears or signs of infec*on Malodorous placenta/fluid or suspicion of chorioamnioni*s Evidence of maternal or infant infec*ous disease Excessive maternal bleeding in labor Evidence of ac*ve genital herpes, suspected prodrome or significant vaginal/perineal HPV infec*on Mul*ple births Congenital abnormali*es S*llborn infant

13 Physical Assessment of Mother and Infant Cord bloods eligible for banking must be obtained from healthy infants of healthy mothers Document on paperwork if mother or baby exhibits signs of infec*ous disease

14 Physical Assessment of Mom Look for physical signs that may indicate high- risk behavior for, or infec*on with, a relevant communicable disease Evidence of sexually transmiied disease Evidence of nonmedical drug use (needle tracks) Evidence of recent taiooing or body piercing Evidence of HIV (disseminated lymphadenopathy, oral thrush, Kaposi s sarcoma) Unexplained jaundice, hepatomegaly, or icterus Evidence of sepsis(unexplained generalized rash or fever) Evidence of recent smallpox immuniza*on

15 Why Must I Par*cipate in Training? Proper training improves collec*on quality Proper training decreases the risk to recipient of transmission of a communicable disease FDA requires training of the individuals par*cipa*ng in the collec*on, labeling, and documenta*on involved in cord blood collec*on

17 Clarifica*ons 3. Was there excessive maternal bleeding in labor? Concern here is for possibility of mom s blood entering baby s circula*on. So excessive post partum bleeding is acceptable. Excessive bleeding prior to delivery is not acceptable.

18 Clarifica*ons 11. Did the mother receive more than two liters of IV fluid in the 48 hours prior to her blood samples being drawn? Yes No If yes, what IV fluid was given Was more than 2L given in the hour prior to blood samples being drawn? Yes No This ques*on refers to the maternal samples of blood drawn into tubes (not the cord blood). If more than 2 liters of crystalloid fluid is given in the hour before mom s samples are drawn, the samples are NOT acceptable for infec*ous disease tes*ng due to dilu*on with IV fluid. If possible, please consider having mother s blood redrawn.

22 Michigan Blood Cord Blood Program In cord blood units collected for our program 149 different clinicians participated in collections Average collection volume = 91mls Range of collection volumes: mls Minimum acceptable volume = 80mls 43% of units collected in 2010 were <80mls More volume is always better.
