No charge. Evidencebased model from Stanford University aimed at empowering patients and community members towards self-management.

4 BERTIE COUNTY Diabetes Program Area Agency on Aging 1385 John Small Avenue Washington, NC Contact: Beth Harrell, Aging Specialist, Mid-East Commission or Dana Hamill, Public Health Educator, Perquimans County Health Department Description: Living Healthy and Living Healthy with Diabetes are highly participatory workshops that take place once a week for 6 weeks. Each session is 2 ½ hours long, and all workshops are facilitated by 2 trained leaders (many of whom are volunteers) who follow a detailed manual, so that each workshop is highly consistent. These programs are not designed to help people to learn more about chronic conditions, but rather to learn the skills and tools to better manage chronic conditions. Participants will learn: techniques to deal with problems such as frustration, fatigue, pain and isolation, appropriate management of medications, skills for effectively communicating with friends, family, and health care providers, healthy eating guidelines and exercise tips, how to make informed treatment decisions, and more.

5 BERTIE COUNTY 5 Bertie Diabetes Support Group Contact: Dana Hamill, Public Health Educator at Perquimans County Health Department at Description: The Bertie Diabetes Support Group (Bertie DSG) focuses on people diagnosed with diabetes, those considered at risk, and their family members or caretakers. The goal of this group is to improve the individual s quality of life, decrease hospital stays, as well as death due to complications caused by this chronic disease. The Bertie DSG meets at Bertie County Health Department on the second Monday night of the month at 6:15 pm and the fourth Tuesday of the month at 10 am. The Living Healthy Chronic Disease Self- Management Program is a six-week program that focuses on people with chronic diseases to improve ability to manage pain, fatigue, depression, and frustration associated with the chronic disease, as well as increase fitness and self-confidence, use medications wisely, improve communication, choose foods for health, solve problems effectively, and set goals. This program is periodically offered to the participants during the support group meetings, as well as to the community.

9 CAMDEN COUNTY Healthy Carolinians of the Albemarle (HCOTA) is a community based network of agencies and citizens dedicated to improving the quality of life through health and wellness serving Pasquotank, Perquimans, Camden and Currituck counties. HCOTA offers several community programs aimed to reduce chronic diseases such as heart disease and stroke as well as promoting a healthy lifestyle through nutrition education and physical activity. Each of the programs provided by HCOTA and our partnering agencies can be scheduled at any time by contacting Amanda Betts, HCOTA Coordinator at or through at Amanda.betts@arhs-nc.org. Healthy Living - The goal of the Healthy Living program is to educate the community about the connection between nutrition, physical activity, and chronic disease using a self-management and social support framework. The program engages participants to expand healthy food preparation skills, increase fruit and vegetable consumption, increase daily physical activity levels, manage stress, as well as understanding their individual cholesterol, blood pressure, and blood glucose numbers. Know Your Numbers Train the Trainer - Two & 1/2 hour classes available to stakeholders in community groups. The emphasis is on recognizing health numbers e.g. blood pressure, cholesterol, blood sugar, BMI and knowing immediate and long-term interventions to prevent severe health outcomes. The curriculum includes informational hand-outs, video segments, individualized program planning and take-home binders for replication. Program can be adapted to time constraints of interested groups. Search Your Heart - Training session on The American Heart Association nine-week program with modules for stroke, nutrition, and physical activity. The information helps educate and raise awareness of the heart-health disparity issues and provides resources to live a healthier lifestyle. Participants who sign an agreement with The American Heart Association will receive background information and tools to implement the program. Eat Smart Move More Weigh Less - is a weight-management program that uses strategies proven to work. Each lesson informs, empowers and motivates participants to live mindfully as they make choices about eating and physical activity. The program provides opportunities for participants to track their progress and keep a journal of healthy eating and physical activity behaviors. The program can be offered to worksites, community groups, churches, and other groups interested in eating smart, moving more and achieving a healthy weight.

43 PASQUOTANK COUNTY 43 Healthy Carolinians of the Albemarle (HCOTA) is a community based network of agencies and citizens dedicated to improving the quality of life through health and wellness serving Pasquotank, Perquimans, Camden and Currituck counties. HCOTA offers several community programs aimed to reduce chronic diseases such as heart disease and stroke as well as promoting a healthy lifestyle through nutrition education and physical activity. Each of the programs provided by HCOTA and our partnering agencies can be scheduled at any time by contacting Amanda Betts, HCOTA Coordinator at or through at Amanda.betts@arhs-nc.org. Healthy Living - The goal of the Healthy Living program is to educate the community about the connection between nutrition, physical activity, and chronic disease using a self-management and social support framework. The program engages participants to expand healthy food preparation skills, increase fruit and vegetable consumption, increase daily physical activity levels, manage stress, as well as understanding their individual cholesterol, blood pressure, and blood glucose numbers. Know Your Numbers Train the Trainer - Two & 1/2 hour classes available to stakeholders in community groups. The emphasis is on recognizing health numbers e.g. blood pressure, cholesterol, blood sugar, BMI and knowing immediate and long-term interventions to prevent severe health outcomes. The curriculum includes informational hand-outs, video segments, individualized program planning and take-home binders for replication. Program can be adapted to time constraints of interested groups. Search Your Heart - Training session on The American Heart Association nine-week program with modules for stroke, nutrition, and physical activity. The information helps educate and raise awareness of the heart-health disparity issues and provides resources to live a healthier lifestyle. Participants who sign an agreement with The American Heart Association will receive background information and tools to implement the program. Eat Smart Move More Weigh Less- is a weight-management program that uses strategies proven to work. Each lesson informs, empowers and motivates participants to live mindfully as they make choices about eating and physical activity. The program provides opportunities for participants to track their progress and keep a journal of healthy eating and physical activity behaviors. The program can be offered to worksites, community groups, churches, and other groups interested in eating smart, moving more and achieving a healthy weight.
