2 Life Sciences 2014 Capital Grant Program Phase II Application for Colleges & Universities Complete and submit this Phase II Application on-line as directed by your invitation from the Center to submit a Phase II Application by 12:00 p.m. EST on November 14, It is the sole responsibility of the applicant to ensure that this application is complete and is properly submitted to the Massachusetts Life Sciences Center (MLSC or Center). Funds awarded are public funds and any information submitted to the Center by the Applicant in response to this Solicitation or generated in relation thereto is subject to public disclosure requirements as set forth in the Massachusetts Public Records Act, M.G.L. c. 66 (Public Records Act), which governs the retention, disposition and archiving of public records. For purposes of the Public Records Act, public records include all books, papers, maps, photographs, recorded tapes, financial statements, statistical tabulations, or other documentary materials or data, regardless of physical form or characteristics, made or received by the Center. The foregoing notwithstanding, public records do not include certain materials or data which fall within one of the specifically enumerated exemptions set forth in the Public Records Act or in other statutes, including the Center s enabling act, M.G.L. Chapter 23I. One such exemption that may be applicable to documents submitted by the Applicant in response to the Solicitation is for any documentary materials or data made or received by the Center that consist of trade secrets or commercial or financial information regarding the operation of any business conducted by the Applicant, or regarding the competitive position of such Applicant in a particular field of endeavor (MLSC Trade Secrets Exemption). If the Applicant believes that documents it submits to the Center may be proprietary in nature, the Applicant must comply with the procedures for the handling and submission of such documents set forth in Section of the Solicitation, which can be found on the Center s website: Even if Applicant complies with all procedures described in detail in Section of the Solicitation to have such documents treated as confidential, the Center s receipt of such documents does not represent a finding by the Center or the Supervisor of Public Records that such documents fall within the Trade Secrets Exemption. 1

3 2014 LIFE SCIENCES CAPITAL GRANT PROGRAM PHASE II APPLICATION FOR COLLEGES & UNIVERSITIES All colleges and universities seeking to receive an award from the Life Sciences 2014 Capital Project Grant Program (Program), must have been invited to apply to Phase II. Pursuant to the Life Sciences Statute, the MLSC is authorized to make certain awards as part of its Life Sciences Capital Fund. The Life Sciences Capital Grant Program is being offered as part of that fund. THE LIFE SCIENCES CAPITAL GRANT PROGRAM IS COMPETITIVE AND THE MLSC BOARD OF DIRECTORS HAS FULL DISCRETION AND AUTHORITY TO DETERMINE (1) WHICH ORGANIZATIONS, IF ANY, ARE ENTITLED TO RECEIVE AWARDS UNDER THE PROGRAM AND (2) THE AMOUNTS OF ANY SUCH AWARDS. SUBMISSION OF AN APPLICATION DOES NOT GUARANTEE THE AWARD OR RECEIPT OF ANY AWARD OR OTHER BENEFIT UNDER THE PROGRAM. Contact information provided in your Phase I Application will be used. I understand that this Life Sciences 2014 Capital Project Grant Phase II Application may be disqualified if it does not contain all required information or if the Applicant does not meet the eligibility criteria required under the Program, and I further acknowledge and agree that the sample Agreement provided on the MLSC s website includes the MLSC s standard terms and conditions, but is subject to the inclusion of additional terms and conditions at the sole discretion of the MLSC that may be relevant and necessary to minimize the MLSC s risk(s) in making this investment. 4 PHASE II APPLICATION FOR COLLEGES/UNIVERSITIES: OUTLINE (Narrative form in PDF, not PowerPoint) THIS PORTION OF THE APPLICATION SHOULD BE IN NARRATIVE FORMAT AND NO LONGER THAN 6 pages (11 pt font) I. Elevator Pitch (no more than 5 sentences) Provide a short summary to describe your project, its value proposition to the applicant institution and to the broader community, and how the MLSC funding would leverage something that otherwise would not happen II. Summary of Training Program and Industry Collaboration Provide a concise narrative of the training program(s) for which the lab renovation/construction and/or equipment and supplies are needed. Items addressed in the narrative should include: A. Specific need and intended use of, as relevant, lab renovation/expansion, equipment, supplies and/or planning dollars; B. Connection between training and intended audience of students and employers; C. Description of any partnerships associated with the training, including how industry sponsor(s) will collaborate in the development and implementation of the program; D. If applicable, curriculum adaptation to meet the needs of industry; E. If applicable, description of any internship program related to the program; F. Efforts, or plan, to evaluate the success of the program in tracking: number of students entering and completing the training program, numbers of graduates, areas of certification/degree achieved as a result of training, placement in jobs within the life sciences sector by position, retention of skilled graduates in Massachusetts, and amount of other funds raised or leveraged. G. For private colleges and universities, please provide maps, photographs, drawings or any other graphics that will help the MLSC to appreciate the delineation of the project site and its context. NOTE: these visual materials may be attached to the application and will not count as part of the 6-page limit for the narrative portion of the application. This section does not apply to public institutions, which are working with the Massachusetts Division of Capital Asset Management and Maintenance on project plans. III. Performance Measurement Provide data in response to the categories in the order listed below (only as it relates to the program/project for which you are applying and not for the entire college). 1. Fall Enrollment Headcount (number of students at peak entry time) 2. Annual Enrollment Headcount (number of students in all terms throughout academic year) 3. Transfer Students (number of students who transfer to four-year institutions) 4. First Year Retention Rate (number of first-time, new FT students who return in the fall following their first year) 5. Fall-to-Spring Retention (percent of first-time and transfer degree-seeking students enrolled in a fall term) 6. Credit Course Completion Rate (percent of students enrolled in credit coursed that attempted to earn credit and successfully completed the course(s)) 7. Degrees Conferred (all degrees and certificates conferred during the previous fiscal year) IV. Budget A budget must be submitted with a breakdown of the lab renovation/construction equipment/supplies/other use of funds requested, their associated costs, and the anticipated matching cash or donated equipment from the industry partner(s), if any. Institutions can only 3

5 request reimbursement for costs associated with the approved Project as set forth in their Grant Agreement. Such costs may include supplies, equipment, planning, design and construction drawings, construction and renovation. Equipment installation and qualification costs also are reimbursable. However, indirect costs as well as maintenance, support personnel, and service costs associated with the utilization of the equipment are not reimbursable. Reimbursement for equipment rental is permissible, as long as it is for an operational lease (lessor is owner of the equipment and there is no finance charge) and not a capital or financial lease (lessee is the owner of the equipment and interest charges apply). The total amount for all budgeted items must equal the amount of funding requested. Projects must be ready for funding beginning in FY16 (July 1, 2015) and completion of the project must not extend beyond FY18 (June 30, 2018 ). Applicants may request single or multi-year funding. In addition to the budgeted items requested from the Center, Applicants should include in their budget section any other funds, whether from internal or external sources, presently committed to the proposed project. Please do not include any potential/uncommitted sources of funding. * For applicants requesting more than $1 million: In the event that the MLSC awards less than the requested amount of funding, how will this impact the project? How much, if any, of the project will still be feasible? For purposes of illustration only, assume that the MLSC provided 50% of the requested funds and 25% of the requested funds. 4
