A poster presentation profiling the factors and outcomes of hyperglycemia after cardiac surgeries in a community hospital. Rural medicine does not attract specialty providers, therefore access to care is limited. My research became a sustainable, cost effective program linking endocrinologists to our island diabetics, showing improvement in outcomes. Translating DASH diet into practice is an evidence-based project that aims to apply high-intensity counseling into primary care to improve compliance to hypertension treatment recommendations. Pediatric type 2 diabetes screening rate was increased by utilizing provider reminders, patient educational aids, and a screening protocol in the electronic medical record system. This presentation will focus on the HIV patient population and the emerging role for NP s in the transitions of care model. The purpose of this presentation is to introduce one model of care utilizing Nurse Practitioners (NPs) in the medicine service as Hospitalist. Type 2 diabetes in children is a growing problem that demands immediate attention. Children who are diagnosed with Type 2 diabetes face a possible lifetime of health issues, such as heart disease. The goal of this project was to assess a cohort of 7th-grade middle school students in rural community in order to refer at-risk participants for additional screening and testing and plan additional educational programming for students and parents. Implementation of a culturally sensitive diabetic education program is an effective evidence based intervention for increasing knowledge in adult Hispanics with type 2 diabetes.
