Registration fee covers meals between sessions, dinners and conferences. Registration fee covers also accommodation only for participants to the HOPE Exchange programme 2008 and their accompanying persons. - Accompanying persons who do not want to attend the HOPE conferences and evaluation meeting pay a contribution of 25. The dinners they want to attend have to be paid separately. Accommodation The event organisers will provide three nights accommodation (from May 28 to May 31), including breakfast, in 2-star hotels in single or double occupation only for participants to the HOPE Exchange programme 2008 and their accompanying persons. Local co-ordinators will have to make their own hotel reservation. The central booking office ATI Congrès will help them to find a hotel. No optional social tour will be organized. The topic of the Exchange programme 2008 Improving Continuity of Care: the role of IT was chosen on purpose. A special session for HOPE Exchange programme will be organised. At the end of the conference day, participants will have the opportunity to visit exhibition areas. Agora participants not yet registered will have the opportunity to do it directly at HIT at the point Borne Evénements Associés (hall n 1) General presentations Reinforcing synergies The development of health networks in France as in other countries has led to the emergence of information systems for the coordination of care. This notion must now be extended to embrace all aspects of health care in private practice, in hospitals but also in the wider health and social context. The GMSIH (Grouping for the Modernisation of Hospital Information Systems) presents the results of a preliminary study on the information systems for coordination of care. The challenge of providing a continuum of care for chronic patients at home calls for the introduction of innovative organisational approaches and the support of technologies. An example taken from England shows how to set up protocols for the follow-up and therapeutic education of patients on a daily basis.
