1 Board of Pharmacy Practical Experience (Intern Hours) Exam Requirements Pharmacist Licensure Continuing Education Pharmacy Licensure Requirements Prescribing Authority Alaska State Law Review -5 pharmacist and 2 public members -Pharmacist members must be actively practicing -Pharmacists must have at least 3 years practice experience -Appointed by governor -Each pharmacist term is 4 years and is limited to 2 consecutive terms -Interns and Preceptors need to be registered with the Board -Intern registration fee: $25 plus one-time application fee of $50 -Intern renewal fee every 2 years: $25 (may only be renewed once) -Intern hours start after third year of a five- or six-year program -1,500 hours internship required by Board. Maximum of 1,000 hours completed in conjunction with educational requirement of the college of pharmacy. -Graduation from an accredited college of pharmacy -Pass NAPLEX and MPJE exams -NAPLEX score transfer is valid for 1 year -Licenses are issued by the Division of Licensing and Registration and approval of Board -Initial certificate fee: $200 plus one-time application fee of $50 -Biennial renewal (even years) fees change biennially. 3 Arizona State Law Review Board of Pharmacy Practical Experience (Intern Hours) Exam Requirements Pharmacist Licensure Continuing Education Pharmacy Licensure Requirements Prescribing Authority 6 pharmacists, 2 public members, and 1 technician Only the community pharmacist member must be in active practice Minimum number of years of practice experience requirement: 10 years Each pharmacist term is 5 years and the number of terms is unlimited Appointed by governor 1500 hours required Hours counted starting from first professional year Up to 1,500 hours for a PharmD program can count to board required hours Interns and preceptors need to be registered with the Board, but training sites do not Intern fee: $50 (renewal fee of $50 up to max of 6 years; after 6 years, must be re evaluated) Graduation from an accredited college of pharmacy Pass NAPLEX and MPJE State participates in NAPLEX score transfer program NAPLEX score transfer is valid for 1 year Licenses are issued by the state board of pharmacy Initial certificate fee: $180 plus $20 for wall license No minimum age at which initial license can be obtained Biennial renewal fee $180, license expires Oct 31, mailing for renewal 60 days prior to expiration Expiration year is odd numbered year for odd numbered license holders and even numbered year for even numbered license holders Temporary licensing not applicable License transfer fee: $300 Pharmacists must satisfactorily complete three (3) CEUs biennially of continuing professional education activities sponsored by ACPE or Board approved providers. At least 0.3 CEUs shall be pharmacy law subjects. Satisfactory proof of participation should be retained by participants for five (5) years. No carryover of credit is allowed. 11 Prescriptions Patient Counseling Facsimile Transmission Pharmacy Technicians Electronic Transmission of Prescription -Maintain records for 5 years -Only an offer to counsel is required to all patients (including Medicaid patients) for both new prescriptions and refills -Counseling must be performed personally and face-to-face by the pharmacist to the patient or patient s agent if the prescription is delivered within the pharmacy. If not, a written offer must be made with toll-free access to a pharmacist -Counseling must be offered unless a pharmacist deems it inappropriate or unnecessary; then it may be written, by telephone or as appropriate -Patient profiles are mandated -A faxed Rx is handled in the same manner as a phoned-in prescription except for C-II -Photocopying or immediately reducing to writing is required in order to produce a non-faded Rx -C-II fax is only allowed for home infusion prescriptions, LTCF inpatients, hospice patients -Need to be registered but not certified -Technician certification available -Beginning on January 1, 2010, within two years of becoming employed as a registered technician, must become certified by successfully passing PTCB or other Board-approved examination. Does not apply to pharmacy technicians hired prior to January 1, Registration fee $40 initially with annual renewal fee of $25 -Technicians are allowed to: enter prescription and patient information into computer, compound, and call for physician authorization on refills -Technicians may accept called-in prescriptions -Training and exam requirements -No CE requirements -Technicians are not allowed to: transfer prescriptions, check other technicians -Rules/regulations currently being developed/proposed and may alter the above statements -effective January 1 st, 2012, Schedule II controlled substances are allowed as long as transmission is in compliance with federal regulations

12 Board of Pharmacy Practical Experience (Intern Hours) Exam Requirements Pharmacist Licensure Continuing Education Pharmacy Licensure Requirements Legend Drug Prescriptions Patient Counseling Facsimile Transmission Indiana State Law Review -6 pharmacists and 1 public member -Appointed by Governor -Each term is 4 years and there is no limit on the number of terms that a member can serve - Pharmacist members do not have to be actively practicing -Interns must be registered -Initial registration fee is $10 with annual renewal fee of $ hours of practical experience required by the Board - Internship/externship credit begins with the first professional year in a college of pharmacy -Graduation from an accredited college of pharmacy -Pass NAPLEX and MPJE exams -State participates in NAPLEX Score Transfer Program on a reciprocal basis with any other state that will accept its scores -No validity period for a NAPLEX score transfer -Licenses are issued by professional licensing agency -Individuals must be at least 18 years of age to obtain license -License expires June 30, mailing date is 60 days prior to expiration -Criminal background check required through the Indiana State Police. Separate fee for the police - Initial certificate fee $100 -Biennial renewal costing $160 -Fee for license transfer is $100 -Temporary licenses are not available -Pharmacists are required to complete thirty (30) hours of approved CE every two (2) years. -No more than six (6) hours of business- or computer-related CPE is accepted. -No carry over of credit is allowed. -At least half of total hours must be ACPE approved; any other hours must be approved by the Board. -There are 6 categories of pharmacy licenses -Non-resident pharmacies must register -Initial licensure/registration fee of $100 with biennial renewal (Fee $200) -Initial Controlled Substance fee is $100 with a $100 biennial renewal fee -Time limit on refills: 1 year (Six months for C-III and C-IV controlled substance prescriptions.) -Maintain records for 2 years -Can sell schedule V products over the counter (Must be sold by pharmacist only in pharmacy.) -Patient counseling must be offered for all patients for both new prescriptions and refills -It must be performed personally and face to face (when appropriate/applicable) by the pharmacist -Patient profile is mandated -Schedule III-V may only be sent from an institutional facility by the authorizing practitioner or an authorized agent. P Schedule II permitted only for home infusion prescriptions, long-term care facility (LTCF) inpatients, and hospice patients.

13 Electronic Transmission Prescribing Authority Pharmacy Technicians -Prescriptions may be transmitted electronically if the pharmacy passes through a Board-approved intermediary -Electronic transmission of the controlled substances is permitted given that it complies with DEA requirement -Pharmacists may administer immunizations under a drug order, prescription or physician-approved protocol for flu and shingles -Medical Doctors and Doctors of Osteopathy: unlimited, independent prescribing authority -Doctors of Dental Surgery, Podiatric Medicine, Veterinary Medicine: independent prescribing authority limited to their course of practice -Doctors of chiropractic have no prescribing authority -Doctors of homeopathy are not licensed -Physician assistants have no prescribing authority -Nurse Midwives, Nurse Practitioners, OB/GYN Nurse Practitioners, Pediatric Nurse Practitioners, Psychiatric Nurse Practitioners, and Certified Registered Nurse Anesthetists, Midwife: Dependent prescribing authority ( Advanced Practice Nurses must be certified under rules promulgated by the Board of Nursing and Medical Licensing Board. When certified, they will have dependent authority. Must have collaborative agreement with physician and may only prescribe within the scope of the physician s practice. The state does not necessarily recognize each listed nursing specialty. Hospital and private mental institutional Pharmacists may adjust drug therapies pursuant to protocol and under the supervision of a physician.) - Optometrists: Limited prescribing authority (Optometrists must be certified by a committee of the Pharmacy Board and use formularies to prescribe. They may not prescribe for controlled substances. May only prescribe drugs related to eye and adjacent structures.) -State certifies technicians and has training requirements -Registration fee $25 initially, with biennial renewal -Separate criminal background check fee for Indiana State Police -Technicians are allowed to: enter prescription and patient, compound, call for physician authorization on refills and accept new prescriptions from a physician as long as they are under immediate and personal supervision of the pharmacist. -Technicians cannot transfer prescription orders - Technicians must be under the immediate and personal supervision of the pharmacist.
