Wrote a grant to fund CHOICES, Inc. to deliver Independent Case Management and Flexible Supports. This is a mental health consumer-run organization that will deliver case management and skills development through billing Medicaid as a community mental health clinic. Develop Action Plan for establishing and developing the Office. Implement Action Plan which includes surveys, patients rights and ethics, consumer council, consumer education, special community education events. 1

Developed job description to meet ADA requirements. Created training for hospital administrators and management staff to meet legal requirements, developed interview questionnaire, assisted in interviewing and hiring process. Assisted in salary negotiations, contract preparation. Facilitated statewide monthly meetings to oversee implementation and development of new program. Monitored contract between State of Illinois and Mental Health Consumer Education Consortium (statewide consumer organization) for US DHHS, Center for Mental Health Services Consumer and Family Network Three Year Grant. Conducted focus groups in state hospitals to find out from patients items important to them to be included on survey. Developed pilot project protocol with CQI to implement on a limited basis. Developed monitoring instrument and protocol to monitor community agencies compliance with contract language for consumer participation in planning, developing, delivering, and evaluating services. Facilitated 298 consumers in focus groups in state hospitals and community agencies. Prepared detailed reports on results such as rehabilitation, managed care, satisfaction survey, medication education, and forensic services at high security state hospital. 2

Design/develop/implement hospital and community staff training on patient sensitivity training such: Positive Staff/ Recipient Interaction; A Recovered Person s Perspective on Medication; Involuntary Interventions; Facilitating Recovery. Worked with an established statewide work group with forensic professionals to design and implement training. Conduct trainthe-trainer events to develop a consumer/family network to make presentations in their local communities to educate staff and family members about forensic mental health services. Marketed self-help program in Chicago subburbs to meet grant requirements of contract with State of Illinois, and State of Delaware and Robert Wood Johnson Foundation funding requirements. Developed self-help groups and mentored leadership of individuals in groups. Co-founder and first President. Wrote successful $125,000 grant application. As Executive Director, designed and implemented peer support, advocacy and public education programs. First year project review was rated 12 th out of 46 statewide community mental health programs.
