In the last few years, the number of technical and procedural developments in the world of medicine and healthcare have been steadily accelerating. With this rise, the types and varieties of private medical insurance (PMI) have increased as well. Indeed, as a consumer looking to cover yourself or your family, there has never been a time with a greater choice of options and benefits. As one of the leading providers of PMI in the UK, we ve written Healthcare Revealed as a guide to the choices you face. Within this booklet you will find information to help you choose the healthcare options that suit you best, including explanations of what private medical insurance entails as well as some regularly used terms. 4 Introduction Private medical insurance, or PMI as it is known, helps you get medical treatment when and where you need it, from a wide choice of professionals and private hospitals across the country. You can cover yourself, your partner and your children, to make sure you are all protected against the unexpected. 4 Healthcare Revealed

5 Why go private? In an emergency the NHS is still the best place to be and private health care does not try to replicate it. What it does do is offer an alternative to the NHS should you need non-emergency treatment such as a hip replacement, gall bladder removal or knee replacement where you may have to wait for treatment. aviva.co.uk/health 5

6 Trying to self-fund care if you are seriously ill could cost thousands. The table below gives some examples of the costs you could be faced with if you decided to self-fund private treatment. A PMI product may cover these conditions, but it would depend on your medical history and individual circumstances. 6 Healthcare Revealed

7 PMI may cover the cost of private medical treatment should you become ill, in return for a monthly or annual premium. How much cover you get and whether it includes treatment if you need to visit as well as stay in hospital, depends on the kind of plan you have bought. The cost has traditionally scared many people off, but the insurance industry has risen to the challenge and launched a range of plans to suit most pockets. Some 10.6% of the population at the end of 2013, had PMI with the majority of plans offered as a perk through their employer. 8 Why is PMI different? PMI is designed to pay for treatment to help make you better and is not the same as other types of insurance such as: l Critical illness cover which pays out a lump sum if you are diagnosed with a critical illness that meets the policy definitions and then survive for a specified number of days. Policies only cover the critical illnesses defined in the policy and no others. l Cash plans, which reimburse a fixed amount of cash to help with the cost of everyday treatments such as visits to the dentist or optician or payment towards a private consultation with a specialist. l Income protection insurance, sometimes known as permanent health insurance, which pays out a monthly benefit if you are unable to work because of sickness or accidental injury. Some PMI plans may however incorporate elements of these other insurances in them as extras. 8 Healthcare Revealed

9 Acute versus chronic PMI is designed to pay for medical treatment to help cure short-term conditions. Generally it does not pay out for chronic conditions, such as asthma or multiple sclerosis, which need ongoing treatment but cannot be cured. It is worth noting though that some top of the range plans will pay out a cash lump sum towards the cost of palliative care (care that makes you feel comfortable, but doesn t aid your recovery) in a hospice or for home nursing or for the acute stage of chronic conditions. The Association of British Insurers (ABI), which is the trade association for insurance companies, has published a number of industry wide standard definitions. It defines acute conditions as: A disease, illness or injury that is likely to respond quickly to treatment which aims to return you to the state of health you were in immediately before suffering the disease, illness or injury, or which leads to your full recovery. It defines chronic conditions as: A disease, illness or injury which has one or more of the following characteristics; l It needs ongoing or long-term monitoring through consultations, examinations, check-ups and / or tests, l It needs ongoing or long term control or relief of symptoms, l It requires your rehabilitation or for you to be specially trained to cope with it, l It continues indefinitely, l It has no known cure, l It comes back or is likely to come back. aviva.co.uk/health 9

10 Other exclusions Remember, no plan will cover you for every single health problem, as the cost would be prohibitive. All but the most comprehensive will usually exclude cover for: l Pre-existing conditions (unless included by the provider at the time of taking out the policy) l Normal pregnancy (but they may offer cover for complications) l Infertility treatment l Psychiatric or mental illness l Cosmetic treatments/surgery l Experimental treatments l Self inflicted injury l Treatments undertaken by a specialist without GP referral l Charges by GPs or specialists for filling out claims forms l Alcoholism, drug abuse or other addictive conditions l Routine medical examinations or dental treatments l Surgical or medical appliances l Out-patient drugs or dressings l Kidney/renal dialysis l HIV/AIDS and related treatments. 10 Healthcare Revealed

11 Which plan? There are hundreds of different PMI plans on the market designed to suit every pocket and need. Later in this guide we look at how you choose a PMI plan. Typically plans are designed around giving complete cover for people requiring a stay in hospital known as in-patient or day-patient treatment what else you have depends on the type of plan you buy. The table below is a general overview of policies available on the market and what treatment they may/may not cover. Cover will vary from policy to policy and from provider to provider. Low-cost Mid-range High-end Cover for out-patient treatment may only apply if linked to an in-patient or day-patient stay Treatment may only be available in a limited number of hospitals Policy may only be able to be used if there is a waiting list of over 6 weeks at a local NHS hospital Will generally cover you for in-patient, day-patient and outpatient treatment Typically offer some cover for consultations and diagnostic tests May offer alternative therapies such as osteopathy and chiropractic All in-patient, out-patient, daypatient treatment, consultations and diagnostic tests Limited cover for psychiatric and mental conditions Overseas cover Policy may cover a finite list of conditions Private ambulance Wide range of hospitals for treatment Limited emergency cover abroad Some home nursing Cash payment if you stay overnight in an NHS hospital Cover for some pre-existing conditions Payment towards private pregnancy care Private dental and optical treatment aviva.co.uk/health 11

12 Keeping costs down Excess Many plans include the option of an excess the first part of a claim that you must pay, typically But some plans offer the option to increase the excess to 250, 500 or 1,000 for example, to reduce premiums significantly. Ask your adviser to find out the savings you could make as they differ between plans and providers. You need to check whether the excess applies per policy year or per course of treatment and, if you have a couple or family plan, whether it is per person or overall. There are plans that offer very large excesses as standard, as much as 5,000 a year, but have very low premiums. However, you need to make sure that you can afford such large amounts, potentially each year or for each course of treatment, depending on the policy wording. Split payment Other plans have no excess but split each bill with the consumer usually either 50:50 or 25:75 as a way of keeping costs down. Known as co-payment plans, you need to make sure there is a cap on the total amount of money you could be required to pay each year, or the policy could end up costing thousands. Limited hospitals One option is to limit the number of hospitals that you can have treatment at. Most medical insurers will reduce premiums if you agree to only seek treatment in a limited number of hospitals in the country you need to check which ones are available in your area before you sign up. The downside to this is that a specialist consultant you would like to use may not practice from one of the hospitals on your list. 12 Healthcare Revealed

13 Treatment limit Rather than limit the number of hospitals, you can limit the amount of treatments or conditions that are covered. You could choose a plan that only offers, for example, 1,000 worth of out-patient treatment each year but unlimited in-patient and day-patient cover. Or you could choose a policy that restricts when you can claim. For example, some policies offer an option whereby they will only pay for you to be treated in a private hospital if there is at least a six-week waiting list on the NHS. PMI is perceived to be expensive but there are ways of keeping costs down. Modular policies Many health insurance companies now offer modular policies that let customers pick and mix the cover that is important to them. All have core cover, usually in-patient and day-patient plus some out-patient cover. Customers can then choose to add extras or limit out-patient treatment to keep costs down. The benefit of this type of plan is that you have the option to only pay for the cover you want. No claim discount Many providers now offer policies with a no claim discount. If you don t make a claim during a policy year your discount will increase up to a maximum level. aviva.co.uk/health 13

14 Understanding your underwriting options When you first buy PMI you are given two choices either to be medically underwritten and declare all your medical history or to choose a moratorium policy which puts a blanket ban on any pre-existing conditions for a set period of time. Medically underwritten policies will require you to answer a series of medical questions about your current health and past conditions. The benefit of a medically underwritten plan is that you know exactly what is and isn t covered. The downside is that you may never get cover for a pre-existing condition, even if you have a long period of no symptoms. You need to make sure you give details of all your past medical history. Moratorium policies mean you don t have to fill out any medical forms or give any medical history, but cover is automatically excluded for any pre-existing conditions for which you have received treatment, diagnostic tests, medication, advice about or had symptoms of (whether or not diagnosed), usually during the five years immediately before you took out the cover. However, if you do not have any treatment, diagnostic tests for, medication or advice about that condition for two years after your policy starts, many providers will cover that condition. 16 How to choose a PMI policy It is important to seek professional advice from an insurance adviser who will be able to scour the market on your behalf to find the plan that best suits your needs. Advisers know the differences between the plans which may be missed if you try and do all the research yourself. Contact AMII, BIBA or IFA Promotion who all hold details of insurance advisers in your area. Alternatively if you know which insurer you d like to go with, you could contact them direct for further information on their range of products. Before then you need to decide: l Do you want it to provide cover for all in-, out- and day-patient treatment or are you happy to rely on the NHS for some things? l How much can you afford each month? This as much as anything will determine what plan you buy l What size excess can you afford? l Who do you want to cover? Just yourself or your spouse, partner or civil partner and other family members? Unmarried dependent children can usually be covered up to the age of 21, or older if still in full time education 16 Healthcare Revealed

17 l How do you want the plan underwritten fully underwritten or on a moratorium basis? l How will you pay your premiums? Some firms offer a discount to people willing to pay annually. There may be discounts for buying online as well l Find out what the insurer s premiums will be as you get older. Some companies increase premiums every five years, others may increase your premium each year based on your age, and medical inflation l How much flexibility is there? Can you switch plans within the insurer s range without the need for new medical underwriting or elongated moratoriums? l Do you want a policy that rewards you if you don t claim? 19 Switching provider Although you may stay with your PMI provider for many years, medical insurance is actually an annual policy. Each year premiums usually increase, taking into account factors such as your age and medical inflation. If for some reason you are not happy then you can look to switch providers or policies. You need to choose a company that will let you switch on the same underwriting terms if you can so all conditions that are covered continue to be so. If you can t switch, perhaps because you are in the middle of treatment, then talk to your insurer about changing or limiting cover to keep costs down. If you would like to learn more about switching your provider, speak to your insurance adviser. 20 PMI - frequently asked questions How much detail should I put on my application form when taking out PMI? Anything relating to your medical history should be included if in doubt it is better to include something irrelevant than exclude something that could result in the claim being denied or the policy cancelled. Will premiums go down if I stop smoking / drinking? Many insurers do not request this information on their application form so it has no effect on the premiums they charge. But as insurers move their focus over to keeping clients healthy it may have more relevance. If you are asked the question it is definitely worth contacting your insurer to see if your premiums can be reduced if you stop smoking. What is the difference between out-patient and day-patient treatments? l Being a day-patient means that you are admitted into a hospital or a day-patient unit for treatment but although you will need a period of clinically supervised recovery, you do not occupy a bed overnight. l Out-patient treatment is that given in a hospital, consulting room or out-patient clinic that does not usually require clinically supervised recovery and you are not admitted as a day-patient or in-patient. Typically it will be for diagnostic tests and consultations, often before or after in- or day-patient treatment. l In-patient treatment, as the name suggests, is where it is medically necessary for you to stay in hospital at least overnight, if not longer. 20 Healthcare Revealed

21 When will/won t a cancer claim be covered? You will need to speak to your insurer about what they will or will not cover as not all types of cancer are covered and some insurers may not pay for treatment. Some insurers have a specialist oncology team, which assess each claim on a case-by-case basis. Once the treatment becomes palliative, cover may be limited and may not be paid for at all. What is the difference between acute and chronic conditions? As explained more fully on page 6 of this guide, acute conditions are those that can be treated and the patient is expected to make a full recovery. Chronic conditions meanwhile, cannot be cured and will continue indefinitely. Will every eligible claim be paid in full? Not necessarily. If you choose to limit the number of hospitals you can be treated at, and then choose one outside the list, there may be extra costs. Some specialists may charge more than allowed on an insurer s fee scale so again you may have to fund the difference. Also some treatments will have limits on the amount you can claim. How can I fairly compare policies between providers? The best route is to contact an insurance adviser who will be able to review the market and choose the plan best suited to your individual needs. aviva.co.uk/health 21
