1 Job Description and Person Specification for Get Active Specialist WATFORD FC COMMUNITY SPORTS & EDUCATION TRUST JOB DESCRIPTION Post title: Get Active Specialist (Watford) part time (32 hours per week) Grade: 28,695 per annum, pro rata ( 24, for 32 hours per week) Contract type: Fixed term until 31 st July 2018 Responsible to: Mark Smith, Head of Education & Health MAIN PURPOSE OF THE JOB To be responsible for the successful delivery of the Get Healthy, Get Active Healthy & Active in Herts project in your allocated area. This role involves building strong working relationships with a wide range of primary and secondary care staff to ensure the programme has a high profile and is fully utilised. You will work very closely with GP Practice Managers, GPs and nurses to identify patients who meet the inclusion criteria. You will also build strong links with local community groups to promote the project to the local population. You will pre-screen new individuals to ensure suitability for the scheme, organising and delivering one-to-one brief interventions, maintaining a central referral database and feeding information about patients back to GP s Practice Nurses and other health professionals. The Get Active Specialist will be responsible for monitoring and evaluation and ensuring that the KPIs are met in line with the agreed requirements. GENERAL RESPONSIBILITIES AND DUTIES 1. Organisation and delivery of one to one brief interventions using motivational interviewing techniques to contribute towards positive lifestyle changes. 2. Responsible for working across a range of partners to coordinate and implement activities associated with the Get Healthy, Get Active Healthy & Active in Herts project. 3. Maintain all administrative duties including booking appointments, providing follow-up contact, and maintaining monitoring and evaluation data. 4. Recruitment of suitably qualified coaches / instructors with relevant experiences to deliver sessions for the referred individuals. 5. Provide direct, frequent support and guidance to coaches / instructors operating in your geographical area. 6. Develop networks with identified referral routes (e.g. GP Practice Managers, Physiotherapists, Occupational Therapists, Pharmacies, stop smoking services, and other primary/secondary care professionals). 2 7. Provide effective communication to all members of the project team and partner agencies and promote the work of the project. 8. Build relationships with local sports clubs and assist with club development and workforce development to increase capacity for new beginner members. 9. Engage with appropriate continued professional development opportunities. 10. Carry out any other related duties commensurate with the responsibility level of the post. 11. This post involves evening and weekend working on a weekly basis to enable the delivery of projects and initiatives. Please note: The hours of this post will be spread across the week. You will be based in 4 GP practices and one Mind Wellness Centre providing one to one support. You will be working as part of a team with other Get Active Specialists. Your annual work programme will focus on a number of the above responsibilities and duties which may change over the course of the contract and you may be required to undertake a range of other projects in subsequent years. SUPERVISION RECEIVED The Get Active Specialist will be line managed by your employer (Watford = Watford FC Community Sport & Education Trust; Stevenage = Stevenage Borough Council; Broxbourne = Broxbourne Borough Council; Hertsmere = Hertsmere Leisure Trust). You will also receive supervision and support from the Sports Project Officer (Get Healthy, Get Active Co-ordinator) and work as part of the Healthy & Active in Herts project team. You will be expected to operate with a substantial degree of autonomy in carrying out the above responsibilities and duties. SUPERVISION GIVEN The Get Active Specialist will line manage the work of a team of part time sports coaches that may be assigned to the project from time to time (Broxbourne and Watford posts only). RESPONSIBILITY FOR BUDGETS The post holder will plan and monitor income and expenditure on individual projects and produce regular monitoring reports. 6 Healthy & Active in Herts Background and introduction In the Autumn of 2014, Sport England announced a new funding stream called Get Healthy, Get Active. Sport England supports the drive to get Everybody Active, Every day and wants to explore the contribution that sport can make to decreasing inactivity and helping people to achieve the Chief Medical Officers (CMO) Physical Activity guidelines of 150 minutes of activity a week. The Get Healthy, Get Active fund aims to: Get Inactive people playing sport 1 x 30 mins per week Achieve a better understanding of sports contribution to improving public health and the prevention, treatment and management of long term conditions Enable sport & physical activity to be regularly commissioned to meet a wider range of agendas, but particularly focussing on reducing health and social care costs Achieve a reduction in health inequalities A 5m pot of funding was available for organisations to apply for funding between 100k and 500k. Applicants who could secure cash match funding before submitting their bids would add to the strength of their application. In Hertfordshire a range of partners came together to develop a project that would begin to address the gap between primary care and the sport and leisure sector. It is recognised that regular participation in sport and physical activity is beneficial for health. However, there isn t enough resource in primary care to enable inactive patients to be supported to make changes to their lifestyles to become regularly active. The Healthy & Active in Herts project aims to begin to address this gap. It will work with inactive adults who have one or more risk factors for cardiovascular disease &/or mild to moderate mental health conditions. The project will also aim to address health inequalities and project locations have been selected based on highest levels of deprivation and highest levels of inactivity. Hertfordshire were one of sixteen projects nationally to be awarded Get Healthy, Get Active funding. 500,000 was awarded from Sport England and 525,000 cash match funding has been committed from Herts Public Health, Herts Valleys CCCG, East & North Herts CCG and Broxbourne Borough Council. The total project fund is 1,025,000 over 3 years. The Herts Sports & Physical Activity Partnership is the accountable body. 7 Mind in Mid Herts Herts Mind Network University of East Anglia Project overview Get Active Specialists will be at the heart of the project and will manage the project for their locality. They will run Get Active clinics in GP surgeries and at Mind Wellbeing centres. Patients will receive one to one consultations to help them make positive lifestyle changes to become more active. Motivational interviewing will be used to help them make these changes. Each session will last up to 30 minutes. The number of sessions each patient receives will be decided on an individual basis. The Get Active Specialists will be very familiar with local provision of sport and physical activity opportunities that are suitable for previously inactive adults. They will build relations with group leaders to ensure all activities that the project sign posts to are appropriate for each individual. All Get Healthy, Get Active project have to be evaluated by an academic partner. In Hertfordshire, we are working with the University of East Anglia. Two different delivery models will be evaluated. The project will compare delivery model A and delivery model B. Delivery model A includes one to one sessions, free sport and physical activity sessions specifically for inactive individuals and a volunteer buddy scheme. Delivery model B includes the one to one sessions only. Patients will be signposted to community sport and physical activity opportunities. The research element of the project will be explained in more detail at the interviews. Project locations The project will delivered in four districts: Watford, Stevenage, Broxbourne and Hertsmere. Staff employed by the project The Herts Sports & Physical Activity Partnership (HSP) have employed a full time Project Coordinator and will shortly be recruiting a part time monitoring and evaluation admin assistant. Four Get Active Specialists will be recruited and will be in post by 1 st September Each Get Active Specialist will be employed by a local partner: Watford = employer is Watford FC Community Sports & Education Trust Hertsmere = employer is Hertsmere Leisure Trust Broxbourne = employer is Broxbourne Borough Council Stevenage = employer is Stevenage Borough Council (this post will not be advertised until September 2015) The line management for each post will be through the employing organisation. The Get Active Specialists will receive supervision from the Project Co-ordinator (employed by HSP). The Get Active Specialists will work as a team of instructors and will meet on a regular basis. Work patterns

8 The Get Active Specialists will be employed for 32 hours per week. This will be spread out over the week to include day time, evening and weekend working. The timetable of work will be put together over the coming weeks once room availability is confirmed. The Get Active Specialists will run clinics in four GP surgeries and one Mind Wellbeing centre.
