(STUDIO A&B) Classes Begin Monday, August 17 th ** = NEW TIMES & CLASSES Class Descriptions Tap: Tap is a fun and upbeat dance form where students will learn old school steps such as shuffles, flaps, time steps, rhythms & beats with a new school flair. Jazz/Hip Hop: Students learn popular hip hop dance moves as seen on the latest music videos while incorporating jazz technique such as pirouettes, shane' turns, kicks & leaps. Ballet: Ballet for ages 2yrs - 2nd grade is a "Princess" style of dancing. Students will learn beginner through intermediate levels of ballet technique such as ballet positions, plie', releve', turns & leaps to all of the latest and greatest Disney Princess music. ACRO: ACRO is a dance form of gymnastics which teaches balance and builds muscle strength in arms and legs. Students will learn forward rolls, cartwheels, round offs, front walkovers, back walkovers, back handsprings & back tucks. ACRO is offered in all ages and students are placed according level. Lyrical/Contemporary: Lyrical/Contemporary blends ballet and jazz dance. Lyrical is generally smoother and a bit faster than ballet but not quite as fast as jazz. Students will dance to music with lyrics which serves as the inspiration for movement and expressions. HipHop/Funk: HipHop/Funk includes a large range of hip hop styles, specific to the East and West coasts. In this dance form, students will learn how to express themselves to the high and low beats, song lyrics and rhythm using popular dance steps such as head toss, body rolls and sassy shoulder shakes. Musical Theatre: Musical theatre brings the exciting world of Broadway to life combining singing, acting and dancing. This class will build confidence and stage presence, a must for any triple threat performer. Street Tap: Street tap is hip- hop based. Students will learn old school moves to new school sounds. Tap lessons are choreographed to the latest music and tapping trends. Students are taught basic tap training to enhance coordination and rhythm.

Tuition IS NOT pro-rated for enrollments after the first of the month, although missed classes can be made up & we do encourage you to do so. A flat tuition is paid monthly to hold a students place in a class or classes. Tuition must be paid regardless of the students' attendance each month. Since tuition is paid for the space in the class, please do not ask for a reduction if your child is absent during the month. No refunds will be given!!

5 WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY (READ CAREFULLY BEFORE SIGNING) Program/Activity: In consideration of N.Y.C. Dance Company permitting (Student Name) to participate in the program/activity identified above, the undersigned parent(s) or legal guardian(s) of Participant hereby freely agree(s) to and make(s) the following contractual representations and agreements: I agree that it is my sole responsibility to be familiar with the physical and/or mental demands on the Participant that may be associated with this program/activity. With these demands in mind, Participant has no physical or medical condition which, to my knowledge, would endanger Participant or others participating in this program/activity, or would interfere with Participant s ability to participate in this program/activity. I also agree that Participant shall abide by any established rules/regulations while in this activity. I understand that situations may arise during the program/activity which may be beyond the control of the Releasees (defined below). I fully realize the risks and dangers to Participant associated with participation in a program/activity of this type and voluntarily assume all such risks and dangers, whether or not known or reasonably foreseeable at this time. I understand the risks and dangers include, by way of example and not by limitation, bodily injury disability and/or death that may result from participation in the program/activity, and resulting social and economic losses and/or damages. I hereby agree not to sue the Dance Studio, its owners, directors, officers, managers, employees, agents, guests, invitees, assigns or other participants in the program/activity (collectively the Releasees ) and release and forever discharge Releasees from all liability to Participant (and to the undersigned parent(s) or legal guardian(s) of Participant), my/our personal representatives, assigns, executors, heirs and next of kin and waive any and all claims and demands for losses and damages on account of any injury, including but not limited to the death of the Participant, or damage to property however caused, even if occasioned by or proximately caused by negligence on the part of the Releasees or otherwise. I agree, for myself and my successors, assigns, beneficiaries, executors, and my personal estate, that the above representations and agreements are contractually binding and are not mere recitals. I agree that my failure or refusal to sign other such agreements or releases shall in no way affect the validity of this agreement nor revoke or cancel any of the terms hereof. I and my successors shall be liable for all expenses (including legal fees) incurred by the Releasees in defending any claim or suit brought in violation of this agreement. 6 PAYMENT POLICY Registration Fee: There is a registration fee of $25.00 per child or $40.00 per family. Registration fees are non-refundable and non-transferable. Tuition: Tuition is a monthly flat rate. AS OF JANUARY 1 ST you are in a contract with N.Y.C. Dance & you understand that you are responsible for JAN-MAY Tuition! NO EXCEPTIONS!!! April & May Tuition are due on April 1st. (If May tuition is not paid between April 1 st -10 th a $10.00 late fee will be added on April 11 th ) The rate for 1 class is $ Tuition is due the 1 st 10 th of each month and is late the 11 th of the month. A credit card must be on file at all times. Debit cards are accepted but N.Y.C. Dance is NOT responsible for any over draft fees that your bank may charge you. A late fee of $10.00 will be charged for tuition received after the 10th. If you fail to pay your tuition by the 10 th of the month, your credit card will be billed for the tuition that is past due, along with a $10.00 late charge on the 11 th of each month. Tuition can be paid at the front desk or the drop box. You may also choose the Auto Pay program (tuition automatically charged to your credit card the 1 st of each month.) If paying by check, please write the student s name on the check if last name is different from parent/guardian. Tuition is non-refundable and non-transferable even if you or your child does not complete the month. A $25.00 service fee will be charged for all returned checks. Costumes: Costume Fee is due on November 1 st. Costume fees are non-refundable and non-transferable. Costumes will only be ordered for those who have paid the fee. If you choose the Auto Pay program, your Costume fee will be charged to your credit card on November 1 st in addition to November tuition. Exit Policy: If you plan to exit on or after January 1 st you are responsible for JAN. MAY Tuition! If you wish to exit a class, a class exit form must be filled out 30 days prior to the date you wish to exit that particular class or classes. If 30 days notice is not given the student will continue to be billed until written notice is received. (Ex: If you EXIT on the 23 rd of the month, you are responsible for the next month s tuition.) Recital: Recital tickets may not be purchased until May tuition and any account balances have been paid in full. Recital tickets may only be purchased with Cash or Credit Card.
