1 University of Arizona Nutrition Network (UANN) Online Partner s Log Instructions This set of instructions will help guide you through the completion of your online log (aka Teacher s Report). 1) Each month you will receive an from UANN staff that will contain a link to your online report. 2) If you are having trouble finding the link please contact your UANN staff member. a. The following is an example link. 3) After you click the link you will be taken to the welcome page of the online partner s log. (shown below) 4) Click the Login button to begin filling out your online partner s log. Note: If you are having trouble viewing the page because it is too small, you can hold control (ctrl) and use the mouse wheel to enlarge / reduce the size of the page. 2 5) You have the ability to save and return to your Partner s Log at a later date. This can be done at any time while you are filling out the form. To save, click the save button in the upper right hand corner of the survey. a. On the save page, enter your address into the text box and click send . This will send you a link to return to where you left off in the survey. Note: After you enter your and click send you can close the page. Look in your for the return link when you are ready to continue. 6) Click Next Personal Information Section (make sure this information is correct with proper capitalization) You will be asked about the following: Your Name (First and Last), address, whether or not you are a UANN Coordinator / Point Person, your title, and your site name (including your school district if applicable) For the glory of God John Rupp Page 2

3 7) Begin entering your personal information: a. Type your first name in the field labeled First Name b. Type your last name in the field labeled Last Name c. Type your Address in the field labeled Address d. Are you the UANN Coordinator (Point Person) at your site? i. If no click the radio box next to No ii. If yes click the radio box next to Yes e. What is your title at your school / site? i. Please select the closest to your own from the drop down menu. ii. If no title is close to yours please select Other 8) Click Next 9) If you selected Other as your title please enter your title in the open field (What is your title at your School / Site?) and click Next. a. 4 10) Next you will be asked to select your school district from a drop down list. a. If you are not part of a school district select Other on the school district question. 11) Click Next 12) Then you will be asked select your school / site from a drop down list. IF you selected Other in step 10 above, non-school sites will appear in this drop down list. 13) Click Next For the glory of God John Rupp Page 4

5 14) You must verify the information that you submitted is correct. Note: If you select No on this verification question you will be redirected to correct your responses. You will not have to re-enter any of the information from the previous steps, you will simply be given the opportunity to correct any errors. Audience Information You will be asked about the following: The grades you teach, your average class size, and whether or not this is your first activity with this group this year. 6 15) On the Classroom information page a. Please check the box next to each grade or age group that you teach. b. Select your average class size from the drop down list. Note: This question uses ranges, so pick the one that best suits your situation c. Is this your first activity with this group this year? i. If No click the radio box next to No. ii. If Yes click the radio box next to Yes. 16) Click Next Activity Information You will be asked about the following: Whether or not you participated in a special event this month, the number of times during the report month you taught each of the topics listed, the number of hours you spent on direct delivery, and planning /preparation during the reporting month, and how many hours of your total hours were devoted exclusively to special events For the glory of God John Rupp Page 6

7 17) If you did participate in any of the events listed, please check the box next to each of those events 18) If you participated in a special event, but it is not listed please select Other and type the event in the text box located to the side of this option. 19) If you did not participate in a special event this month please select None. 8 20) Enter the number of times you taught each topic for the reporting month. Note: The State requires physical activity topics to be integrated with nutrition topics. Therefore, the Benefits of Physical Activity topic must be taught with a nutrition topic. You will not be allowed to continue the form if you indicate that you taught more of this topic than the total number of nutrition lessons. The errors you will see if your responses are invalid are shown below. Note: This error will pop up under the Benefits of Physical Activity topic if your responses are invalid. 21) Click Next For the glory of God John Rupp Page 8

9 22) Enter the number of hours you spent on the following: a. Direct Delivery (including special events) examples include i. Nutrition education lessons ii. Nutrition integrated physical activity iii. Bell activities iv. Morning Announcements v. Food demonstration messages and lessons b. Planning / Preparation (including special events) examples include i. Photocopying materials for distribution ii. Planning/coordinating school wide nutrition or nutrition integrated physical activity events iii. Preparing foods for an in-classroom tasting iv. Lesson planning and/or curriculum review v. Hanging posters/creating nutrition displays c. Special Event(s) Enter the number of hours spent just on special events. Note: Special Event hours should have been included in the total hours entered. Note: The Direct Delivery and Special Events(s) questions are shown based on previous responses provided. Don t worry if you don t see either of them here; it just means you didn t enter those types of activities earlier in the form this month. For the glory of God John Rupp Page 9

10 23) Click Next Note: This report only accepts the designated month s hours (max 176). This error will pop up at the top of the page if your responses are invalid. Note: Special events hours must be less than the total amount of hours you entered. This error will pop up under the special event hours if your responses are invalid. 24) You must verify the information you entered in the activity information section. a. Click the check box next to Yes if your responses are correct. b. If your responses are incorrect i. Press previous and correct your responses. 25) Click Next For the glory of God John Rupp Page 10

11 26) Enter any comments, questions, and / or success stories you would like to share with UANN Staff. Note: These will be addressed by UANN staff as soon as possible. 27) If you would like to print your responses before submitting your form please click the radio button next to Yes. a. If not click the radio button next to No. 28) Click Next 29) If you selected Yes on the print responses question you will see the following page i. 12 31) You must provide an electronic signature by attesting that the information entered is correct and truthful. a. Click the check box next to Yes the information you entered is correct. 32) Click Submit 33) Congratulations! Your report has been submitted!
