Some abbreviated program descriptions have been used in this publication in order to keep the focus on HIV/AIDS related services. They are flagged with an asterisk (*) after the program name. The directory is in a PDF format and is downloadable for use. It is organized alphabetically by agency name, with program listings underneath. It also has an alphabetical index to assist you in finding the agencies and programs you need. Layout Conventions: The Agency Name is in boldface, capital letters. Services are available for all ages and cover such issues as substance abuse, chronic health problems (HIV/AIDS, Diabetes), and relationship concerns. Counseling is provided by psychologists and by interns. Eligibility: No age limitations. Fees: Sliding scale from $20 or more for counseling services, depending on income level. 4 ALPHA PROJECT FOR THE HOMELESS 3737 Fifth Ave, Ste203 San Diego, CA (619) Service/Intake and Hospice for the Homeless Provides housing placement, case management, and support services to homeless and low income persons facing terminal illness/end of life issues through a partnership with the San Diego Hospice and Palliative Care. Individuals accepted into the program will have a case manager assigned to them. A personal plan of care will be developed by the patient s physician and San Diego Hospice team and may include hospice care, a physician/nurse, social worker, chaplain, home health aide, pharmacist, and bereavement counselor. Eligibility: Must be homeless or lowincome with a terminal illness. Must provide proof of terminal illness determined by a physician. Intake: By appointment with a case manager who determines eligibility on a case by case basis or by a direct referral from the San Diego Hospice. Phone for specific information. The Outreach team is trained in crisis response and de escalation. The team provides support services, transportation (wheelchair Accessible van), and referrals to a variety of services, including housing. Focus is on placement for chronically homeless seniors and individuals with chronic and /or terminal illness, and HIV/AIDS services. Eligibility: Ages 18 years and older. Intake: Walk in or phone. Agencies giving referrals are requested to call 1 or 2 days in advance to determine if service is available and to set appointments. Pickup and delivery of laundry is available for people living in the central San Diego area. People living outside of the central San Diego area may bring in their laundry. Medical equipment and linens are available for checkout at no cost to clients. Members may use a voucher to obtain free emergency clothing. Eligibility: Must have AIDS. Must be referred by case manager or social worker. Fees: No fees charged; however, donations are accepted. Intake: Referral required. For donations, phone or walk in. AUNTIE HELEN'S FLUFF 'N FOLD LAUNDRY PROJECT th St San Diego, CA (619) Service/Intake and BEING ALIVE 4070 Centre St San Diego, CA (619) Service/Intake and ADAP (AIDS Drug Assistance Program)** Federal/California prescription drug assistance program funded by the Ryan White CARE legislation and state funds. Provides HIV related prescription drugs and related medications to under insured and uninsured individuals living with HIV/AIDS. Provides therapeutics (including devices necessary to administer pharmaceuticals) to treat AIDS disease or prevent the serious deterioration of health, including treatment of opportunistic infections. Serves as gateway to broad array of healthcare and supportive services funded through the CARE Act and to other sources of coverage such as Medicaid, Medicare, and other private insurance. Documents: Required documentation list provided when intake appointment arranged. Eligibility: HIV positive or AIDS diagnosis; 18 years of age or older; San Diego County resident; Medi Cal ineligible or Medi Cal zero share cost plan, limited or no prescription coverage; income within program limits. Non perishable dried and canned goods are available on a monthly basis. Eligibility: Must be HIV symptomatic or have AIDS, be low income (SSI/SSA document, pay stub, etc), and be a resident of San Diego County. Must have photo identification. Provides the following services: a 3 to 4 hour break for the primary caregiver in the home; someone to be at home when a patient is discharged from the hospital and has no caregiver; and volunteers who provide assistance with chores such as packing and moving, basic housecleaning, grocery and prescription pickups, errands, and babysitting. Eligibility: Clients must be HIV symptomatic or have AIDS. For household moving services, must provide proof of income and a 2 week notice prior to move. Intake: Phone or walk in. Volunteer peer advocates (who have been affected by HIV/AIDS for 3 or more years) share experiences, provide emotional support, make home/hospital visits, and provide information and referrals to HIV resources. Provides transitional case management services and support groups. Offers library services, educational presentations, and training sessions to the HIV/AIDS affected community. Assists clients in finding permanent housing. Other services include referrals to substance abuse programs and mental health services. The client s needs are assessed during an initial intake appointment with a case manager. Documents: ID Card, Social Security Card, Proof of HIV Status (Letter of Diagnosis), Recent TB Test (PPD skin test and chest x ray), Proof of Income or lack thereof. Supportive services include mental health services, clothes closet, emergency food, hygiene products, peer mentor services, public benefits counseling (appointment required). Eligibility: Ages 18 years and older. Must be HIV/AIDS positive and have a co occurring disorder (substance abuse and mental illness). Low income must pay 30% of gross monthly income. CENTRO provides a transitional housing program for 28 homeless families. Case management services include childcare services, life skills training, medical services, and access to permanent housing. Also offers affordable rental housing to low income families. Marisol Apartments offers 21 permanent supportive housing units for individuals with AIDS or symptomatic HIV. Las Casitas provides 14 units of permanent supportive housing for families disabled due to drug and alcohol abuse and who are referred thru SARMS (a Superior Court program). A meal is served free of charge at each group session. One, Talking About Tina group is specifically for HIV+ men who use methamphetamine. This group is held on Tuesdays from 12 1pm.
