1 GLEN OAKS COMMUNITY COLLEGE NURSING DEPARTMENT INFORMATION SHEET THE NEXT LEVEL I NURSING CLASS STARTS IN SPRING OF 2014 AND THE NURSING APPLICATION IS DUE ON NOVEMBER 29, 2013, ALONG WITH THE TEAS TEST PAYMENT. The Glen Oaks Community College Laddered Associate of Science in Nursing Degree Program begins yearly. The laddered program consists of Level I (practical nurse) and Level II (registered nurse). In addition to meeting the general requirements of the College for admission, there are requirements pertinent to each level of the program. If you are planning to pursue nursing, you must meet with an academic advisor prior to starting at Glen Oaks. The curriculum has been designed to accommodate beginning nursing students at Level I. Graduates of Level I may progress directly to Level II if requirements are met. It offers those students, as well as current Licensed Practical Nurses, the opportunity to continue their education at Level II to meet the requirements for R.N. licensure. Glen Oaks Community College admits to both levels concurrently. Qualified applicants may enter either of these levels as a means for meeting their educational goal. Completion of each level requires three semesters, with the first semester being a shortened 7 ½ week semester in Spring of each year. IF YOU ARE TRANSFERRING IN FROM ANOTHER COLLEGE, PLEASE HAVE YOUR TRANSCRIPTS SENT OFFICIALLY-HIGH SCHOOL TOO! YOU MAY BRING STUDENT COPIES TO YOUR INTERVIEW WITH THE DIRECTOR OF NURSING. There are basic prerequisite courses, which must be completed with a grade of 2.5 (C+) for each class. You must also complete the Accuplacer evaluation (given free at Glen Oaks) if you wish to transfer in courses 10 years or older, you must take Accuplacer evaluation and test into college level English and Math. These courses may be completed in high school (2 semesters-or 1 credit) or in college (1 semester). They are as follows: Prerequisite courses: take algebra prior to taking chemistry!!! passing grade of 2.5 (C+) in high school algebra (2 semesters = equivalent 1 credit) or its equivalent; for example NSM 100 Introductory Algebra at Glen Oaks Community College AND placement into NSM 104 Intermediate Algebra or higher on Accuplacer evaluation passing grade of 2.5 (C+) in high school chemistry (2 semesters = equivalent 1 credit) taken within the last 5 years or its equivalent; for example NSC 130 Chemistry at Glen Oaks Community College NSA 210 Principles of Anatomy & Physiology I, NSA 211 Anatomy and Physiology II SSP 210 Human Growth & Development COM 121 English Composition NSB 121 General Biology I ALH 218 Medical Terminology Acceptance into Level I of the nursing program is based upon: #1- successful completion of the pre-requisites for the program (listed above); #2-being ranked into the top 40 (there are a limited number of seats set by the Michigan Board of Nursing (at this time: LPN-40 seats)). Ranking is based on the numerical total of the score (percentage) on the preentrance test and the GPA of the prerequisite courses for Level I. The pre-admission exam (TEAS-Test of Essential Academic Skills) for Level I costs $50*. The test is comprised of four sections: English, Reading, Mathematics and Science and will take about four hours to complete. The GOCC bookstore has a study manual for the test, or go to #3-All health, including medical clearance, background check, drug screening requirements, etc. must be met by due dates established. (see Level I checklist). Entrance ranking score formula consists of the following: Please notify the Nursing Office if you have previously taken the TEAS test. 2 Info Sheet (cont.) 2 THE NEXT LEVEL II NURSING CLASS STARTS IN SPRING DEADLINE FOR APPLICATION IS January 17, Acceptance into Level II of the nursing program is based upon: #1-successful completion of the pre-requisites for the Level II portion of the program (see Level II admission criteria) #2-being ranked into the top 30 (there are a limited number of seats set by the Michigan Board of Nursing (at this time: RN-30 seats)). For Level II applicants, the ranking is based on the graduating GPA, plus prerequisites (25%) from the practical nurse program and the score on the Level II (NLN PN to RN ACE I) pre-admission exam. There is a requirement of 2 reference letters from your nursing clinical instructor if you are a current GOCC student,* or 1 letter from your DON and 1 from your nurse supervisor at your place of employment that will also be used to determine your entry into Level II. This exam costs $77* and the test date will is March 22, This test focuses on Nursing Care and a minimum decision score of 75 must be achieved. #3-All health, including medical clearance, background check and drug screening requirements must be met by due dates established. (see Level IIchecklist). After acceptance into either level of the program, documentation of having met health guidelines must be submitted and remain current for the duration of the program. Applicants who have met the prerequisites in high school or who are taking the prerequisite courses at Glen Oaks may utilize the time prior to admission to take non-nursing courses, which are part of the nursing curriculum. They are identified on the enclosed course sequence sheets for each level of the program. Taking courses prior to admission will not assure acceptance to the Nursing Program. However, if accepted, taking courses in advance will reduce the number of credit hours the student will take each semester once the program begins. A grade of 2.5 (C+) must be achieved in each prerequisite course. If your career goal involves eventually completing a BSN degree, you are advised to check the curriculum at the university you plan to attend. While waiting for class at Glen Oaks to begin, you may want to take some non-nursing freshman and sophomore classes required in the university s curriculum that will eventually transfer to that university. Students in the nursing program must earn a grade of 2.5(C+) or higher each semester in each required course in the nursing curriculum, and maintain an overall GPA of 2.5 (C+). Students who receive final grades of less than 2.5 (C+) in a required course will not be allowed to progress until they have made up their deficiencies. Nursing students must meet GOCC requirements for graduation. A grade of 2.5 (C+) must be achieved in each prerequisite course. You may also stop by the Nursing Office in the Dwing, 200 level (RM.205) at any time or phone the Nursing Office at (269) , extension 333 or toll free to make arrangements to meet with the Nursing Director. An interview with the director is required. *Please Note: nursing students are responsible for their own expenses including, but not restricted to, travel, health care, and emergency treatment. Nursing students may be assigned to evening (3-11 p.m.) and night (11 p.m. 7 a.m.) clinical rotations, as it may not be possible for all students to be assigned to days (7 a.m. 3 p.m.). Travel is required to clinical sites in Indiana as well as various cities in Michigan. **IMPORTANT: *Costs of pre-entrance tests are subject to change at any time. Financial aid and scholarships are available if you qualify for the nursing program. For more information please contact the Financial Aid Office at (269) , extension 260. 3 GLEN OAKS COMMUNITY COLLEGE LEVEL II ENTRY CHECKLIST GOCC application for Admission, high school & college transcripts (official-transcripts must come in the mail from your college to Glen Oaks). Completed Nursing application received no later than the third week of January of the year you plan to enter the program. You must meet with the Director of Nursing before submitting your application. Proof of payment (receipt) of Level II Admission test (dated no later than February 14, After acceptance into the program these things will be required: Criminal Record Background Check & Drug Screening. Up to date American Heart Association CPR card for the Healthcare provider. (A current copy must be in your file throughout the program) Completed health (physical) form (no sooner than 6 weeks and no later than 4 weeks before your 1 st nursing class). Must be completed by the attending physician, PA or Nurse Practitioner and all information on front of form must be complete, including emergency contact information. Proof of Hepatitis B immunity, Rubella Immunity, Chicken Pox Immunity, Yearly TB test, TDap within 10 years, Influenza vaccination(s) completed in season. Copies of all immunizations must be given to nursing office immediately upon notification of acceptance into the program. You will not be allowed to start the program unless these things are complete in your nursing file. CURRENT LEVEL I STUDENTS (GOCC STUDENTS IN 2 ND SEMESTER OF LEVEL I PROGRAM) MUST ALSO SUBMIT: Nursing application and letter of intent (SEE NURSING HANDBOOK) to advance to Level II. References: Submit 2 clinical Faculty references dealing with the profession. Copy of current nursing license when received. You MUST sit for the NCLEX and pass the licensing test within the semester following graduation from Level I. *ADVANCED STANDING NURSES (LPN) Submit a typed letter to the Director of Nursing indicating interest in entrance to the Associate Degree Nursing program. Submit application to program by December 1 st and a copy of current nursing license. Submit a Letter from your employer indicating you have a minimum of 540 hours of employment as an LPN within the last year and a half. References: Submit 2 employer clinical reference s dealing with the profession. *Check the Level II Admission Criteria carefully and make sure you meet with the nursing director. January 13

4 GLEN OAKS COMMUNITY COLLEGE NURSING DEPARTMENT LEVEL II PRE-ADMISSION EXAMINATION (Nursing NACE-1 PN-RN Foundations of Nursing Entrance Test) As a part of the admission process, all applicants to the Glen Oaks Community College Laddered Associate Degree Nursing Program are required to take a Pre-Admission exam. A minimum decision score of 75 must be achieved to be considered for admission. The test is offered ONLY on Friday, March 21, 2014 in two sessions, 8am to noon and 1-5pm. Sign up will be available starting March 1, 2014 for the sessions. Call for a seat or stop by and sign in on the nursing board outside the offices in the D200 hallway. The fee for the exam is currently $77.00 but is subject to change. Please complete the form below and present it to the cashier in the bookstore. The bookstore is open from 8 a.m. until 4 p.m., Monday, Thursday and Friday, and 8 a.m. to 6:30 p.m. on Tuesday and Wednesday. The completed form will be forwarded to the Nursing office. The fee must be paid and this form must be received by February 14, 2014, however the bookstore will not take payments before February 1, If you have any questions, please feel free to call toll free and speak to the Interim Director of Nursing. *NOTE: Keep the cash register receipt (Ace I Test). 12 ADMISSION CRITERIA FOR LEVEL II Admission Criteria for Level II is divided into 2 categories. Part A is for students who are currently Level l students who have chosen to advance to the Level II program. Part B is for students returning to Level II who have completed Level l some time previously OR students who did not attend GOCC for their PN. Level II is limited to 30 students. A. CURRENT LEVEL I STUDENT (GOCC STUDENTS IN 2 ND SEMESTER OF LEVEL I): 1. Submit a formal typed letter signed by you, indicating a desire to advance to Level II after successful completion of the second semester of Level l, to the Director of Nursing. 2. Submit a Nursing Application to the Nursing office after successful completion of the second semester of Level l. Deadline the 3 rd Friday in January. 3. Take the NLN ACE-1 PN to RN exam for Level II and achieve the required minimum score of Submit a transcript(s) from other institutions of higher education you may have attended since starting the nursing program. 5 Student will request a recommendation from two instructors in the program. Recommendations will typically be based upon input from instructors and/or the Director and shall be based upon clinical competency, critical thinking, knowledge, attitude and ability to work with others. (The designation of "recommended" is required to be eligible to be ranked). This reference, in addition to your ranking score (GPA & test score formula**), will determine your entrance into the program. Faculty will review recommendations for each student. For these students we do not add the winter semester grades into the ranking, although the students must still pass with a 2.5 or greater in their last semester courses. Since we are a year round program we cannot wait until May for them to finish their coursework, as we would not be able to get ranking done in time to start the Level II program immediately again in June. 6. Negative Drug and Alcohol screen will be required. Additionally, a Level II verification form must be completed. 7. Applicants are ranked to determine selection Enrollment for Level II is limited to 30 seats. Applicants are ranked for admission Entrance ranking score **formula consists of GPA from first two semesters = 25%, NLN ACE l test score = to or greater than 75%. Acceptance into Level II is conditional until documentation supports you have met the following: a) A physical exam that must reveal no medical reason which would exclude the student from participating in the nursing program b) A record of current immunizations per immunization policy c) A current CPR for the health care provider card from the American Heart Association. Both the CPR card and the immunizations must be kept current for the duration of the program d) Copy of transcript(s) from 4 above B. CURRENT LICENSED PRACTICAL NURSES (ADVANCED STANDING NURSING STUDENTS): (students returning to the program from a previous GOCC class, or current LPN's from another program): 1. Meet with the Director of Nursing (DON) to submit a nursing application by the deadline of the 3rd Friday in January of the year you plan to attend AND submit a formally signed, typed letter indicating a desire for admission to Level II with a copy of a current unencumbered PN license to the DON. 2. Submit 2 references - References will be; one letter from your Employer DON and one letter from your nurse supervisor which speaks to your professionalism, clinical competency, critical thinking, knowledge, attitude and ability to work with others. Letters must be on Employer letterhead and signed. 3. Confirmation of 540 hours of employment as an LPN within the last year. Individuals who do not meet the employment requirements for admission will be evaluated by the DON and the faculty. 4, GPA for all Level II prerequisite courses must be a 2.5 or higher. These courses may be needed if you are coming from outside GOCC s program. a) High school Level Chemistry (2 semester or 1 credit), with a 2.5 or higher GPA, within 5 years. b) Nutrition course equivalent to our ALH 1O3. c) Have completed a Nursing Pharmacology course (3 to 4 credit.) with 2.5 (80%) or better. Graduates from another program with an integrated Pharm course may take a standard Pharm exam such as ATI Pharmacology and achieve a minimum score of 2.5 (80%) or better. This test can only be taken one time per year. The student is responsible for the cost of the test. If the test is not passed with a minimum of 2.5, the student must take a 3 or 4 credit Nursing Pharmacology course and pass with a 2.5 minimum GPA. d) Meet prerequisites for non-nursing courses required for Level II prior to semester in which that course is required in nursing curriculum sequence, i.e. Biology NSB 121 is a prerequisite to NSB 230 Microbiology, and NSA 210 Anatomy & Physiology I & NSA 211 Anatomy & Physiology II are prerequisites to NSA212 Pathophysiology. Any Criminal Felony or Misdemeanor charges on a public record must be reported to the DON with submission of your nursing application. Acceptance into Level II is conditional on a negative criminal background investigation and a negative drug plus alcohol screen. 6. The entrance ranking score formula is a combination of the NLN-ACE l test score, and the GPA of all Level II & PN program prerequisites.
