1 STATE OF LOUISIANA DEPARTMENT OF TRANSPORTATION AND DEVELOPMENT SPECIFICATIONS FOR FURNISHING PEST CONTROL TREATMENT FOR THE VARIOUS DISTRICT 04 MAINTENANCE UNITS CONTRACT Contract to provide PEST CONTROL SERVICE FOR DISTRICT 04 MAINTENANCE UNITS, LOCATED IN COUSHATTA, HOMER AND MINDEN, LOUISIANA, in accordance with the specifications and special conditions outlined herein. This contract is for a period of TWELVE (12) months BEGINNING JULY 1, 2015 AND ENDING JUNE 30, INSURANCE Public liability insurance and workmen s compensation shall be carried by the contractor and a certificate of insurance shall be furnished within ten (10) days after notification. The limits of such insurance shall be shall be from a company licensed to do business in the State of Louisiana per the attached Insurance Requirements. SCOPE OF WORK The successful bidder will be responsible for the positive control of all listed pests in accordance with these specifications to include the following: Rats & mice, roaches (all types), mites, silverfish, spiders, and other pests, airborne or otherwise (excluding termites). Before each respective treatment, the contractor shall make arrangements with: Ray Parker (318) Coushatta Maintenance Unit #570, 1442 Ashland Road, Coushatta, LA Elmer Jones (318) Homer Maintenance Unit #520, 201 E 5th St., Homer, LA Ron Fortmann (318) Minden Maintenance Unit #530, Highway 80, Minden, LA \

2 All treatments will be coordinated in advance to provide notice to the District. Upon entering the area, service personnel must contact the District representative or designee to arrange access to the space to be serviced. The service personnel will give a service ticket to the District representative or designee before they leave the service location. All chemicals and treatments used must be in compliance with the Louisiana Structural Pest Control Law Rules and Regulations for Pest Control Work in Louisiana, as adopted by the Pest Control Operators Association, and must be odorless or low odor. The Department of Transportation and Development reserves the right to reject any chemical. Insecticides selected for use in treatment shall be labeled for that use. The pesticides shall be applied at the rate recommended on the label. The pesticides used for service calls shall be alternated at least every other application and have a residual effect. All pest control work shall be performed in a safe manner and in accordance with the modern and effective scientific pest control procedures. The contractor is required to furnish the Material Safety Data Sheet (MSDS) on all pesticides used if requested by the Department. Should there be any critical or persistent problem or unforeseen emergency such as infestation of nuisance animals such as bees, wasps, etc., the contractor must respond within twenty-four (24) hours of being notified excluding weekends and holidays. Services shall be increased to whatever degree found necessary to bring the pest under control at no additional charge to the department. Contractor shall be solely responsible for his supplies and equipment and the department will not be liable for destruction or theft of the property of the vendor through vandalism or any other cause. Contractor will be required to have on hand or immediately available to him, an adequate supply of pest control treatment material in order to meet the Department s needs. Service must be satisfactory and approved by the department. The personnel assigned by the contractor to perform services described within this contract shall be qualified to perform the assigned duties. The contractor assumes responsibility for its personnel providing services described herein.

3 The contractor shall not assign any interest in this contract and shall not transfer the same. On each visit, the service personnel are to leave a service ticket with a brief report which indicates the following: 1. The area(s) inspected and treated 2. Contract number and line number for location serviced 3. Contract price and extension Treatment shall be performed monthly and arranged so as not to interrupt normal routines of the department. All locations will be treated during normal working hours. The Department of Transportation and Development reserves the right to request a detailed plan of treatment prior to award. The successful bidder will be required to submit a copy of their license with the Department of Agriculture. All bidders should visit the site(s), verify all existing items specified and familiarize themselves with the working conditions, hazards, actual formations and local requirements involved. All bidders shall take these existing conditions into consideration and the lack of specific information or minor variances shall not relieve the supplier of any responsibility. Arrangement for a site visit should be made by calling Regina Conly at After inspecting the site, bidder should get the District Representative s signature on the site visit certification form (attached). Failure to have such signature may cause rejection of bid. Price per square foot for service if necessary to increase or decrease the service area: $ per square foot. (Note: Not a factor in determining award) EXTENSION OF CONTRACT At the option of the Department and acceptance by the contractor, contract(s) awarded may be extended for an additional two (2) twelve (12) month periods at the same terms, prices and conditions. Contract not to exceed thirty-six (36) months. Prior to exercising the Department s option to extend contract, the Department will determine if an extension is in the best interest of the Department, taking into consideration current market trends, cost factors,

4 price comparison with similar service in other States and various other factors as determined by the DOTD Procurement Director. The continuation of this contract is contingent upon the appropriation of funds to fulfill the requirements of the contract by the legislature. If the legislature fails to appropriate sufficient monies to provide for the continuation of a contract, or if such appropriation is reduced by the veto of the Governor or by any means provided in the Appropriations Act or Title 39 of the Louisiana Revised Statutes of 1950 to prevent the total appropriation of the year from exceeding revenues for that year, or for any other lawful purpose, and the effect of such reduction is to provide insufficient monies for the continuation of the contract, the contract shall terminate on the date of the beginning of the first fiscal year for which funds are not appropriated. TERMINATION OF CONTRACT The Department reserves the right to terminate this contract prior to the end of the contract period upon giving a thirty (30) day written notice to the contractor for unsatisfactory performance, unsatisfactory delivery, unsatisfactory service, failure to meet specifications, misrepresentation by the contractor, fraud, collusion, conspiracy or other unlawful means of obtaining any contract with the State, conflict of contract provisions with constitutional or statutory provisions of State of Federal Law, any breach of contract, and/or if it is considered in the best interest of the Department. All services provided prior to the effective date of such termination shall be paid for by the Department in accordance with the terms of the contract, whereupon all obligations of both parties to the contract shall cease.
