1 HFA'S 21 ST ANNUAL LIVING WITH GRIEF PROGRAM (2014) "Helping Adolescents Cope with Loss" PROGRAM INFORMATION: This new program focuses exclusively on the issues that adolescents face as they cope with loss. Adolescence, broadly defined, from middle school years to emerging adulthood, is a significant developmental period during which adolescents form identity, independence, and intimacy. Illness and loss profoundly influences these processes. Moreover, adolescent encounters with loss are likely to be traumatic deaths by accident, suicide, and homicide. This program explores the ways that healthcare workers, hospices, educators, social workers, counselors, clergy, funeral directors, and other professionals can assist adolescents as they cope with loss. DATES / METHOD OF PRESENTATION: HFA s 2014 Living With Grief program will be available to registered sites in the following formats: LIVE via Streaming Video Webcast on April 10, 2014 from 2 to 4:30 p.m. ET. Archived webcast (online); available for 1-year, until April 9, A DVD of the program is also available (for an additional fee); the DVD will ship in l mid-june. The program is 2.5 hours, with an additional 30-minute local discussion (required for CE credits). Sites are urged to participate in the live program but can schedule their own dates and times (between the dates of 4/10/14 and 4/9/15) to show the program. PROGRAM FEES: A registered site is licensed to show this program at one, single location only (as defined by a physical street address). Webcast only Registration: $250 DVD only Registration: $ (Webcast + DVD) Registration: $330 **Webcast Registration options include access to the LIVE program as well as access to the Archived Webcast for 1-year. ***DVDs will not be shipped until mid-june, HOW TO REGISTER: Register online now (register.hospicefoundation.org), or complete the paper registration form (located on HFA s website). There are no refunds for registration after 30 days after registration is complete (or after materials are shipped).

In addition, all sites will receive a complimentary review copy of the 2014 companion book written by experts. DOWNLOAD YOUR MATERIALS Once you are registered for the program, you will have immediate access to the "Download Area" online. Download materials are located at register.hospicefoundation.org.* *To access your materials, simply log-in using the SAME address that you registered with. Then, click on "Downloads" (on the left-hand menu). PARTICIPANT FEES: HFA does not charge a participant registration fee, however some sites may choose to charge a small fee to cover costs associated with room rental, equipment, or food & beverages. HFA discourages this practice but understands economic necessity. There is a small fee if participants wish to receive continuing education credits, and CEs are now discounted for the first two weeks after April 10, Please see the Continuing Education section below for further details. TARGET AUDIENCE: The program is primarily for professionals already working in the field, but is practical for all levels of education entry level, intermediate or advanced. The information provided by the expert panel will be useful to clergy, counselors, nurses, nursing home administrators, social workers, case managers, psychologists, physicians, funeral directors, marriage and family therapists, caregivers, and those working with death, dying, grief or bereavement. Identify the three developmental issues encountered in adolescence and indicate the ways that encounters with death, grief, and loss can complicate adolescent development; 2. Describe the ways that the developmental issues of adolescence can both complicate and facilitate the ability of the adolescent to cope with life-threatening illness and identify issues that should be addressed in providing care for adolescents as well as their peers and siblings in life-threatening illness; 3. Discuss the ways that adolescents experience grief and cope with loss, noting the ways that schools and other professionals can offer strategies to help adolescents cope with loss; 4. Describe the ways that sudden and traumatic deaths complicate the grieving process, noting strategies and processes for schools and other organizations to respond to a tragedy or traumatic loss such as an accident, suicide, or violent event; 5. Discuss the value and approaches of interventive strategies with grieving adolescents, such as peer support groups, bibliotherapy, individual counseling, and expressive therapies. 4 AGENDA: The 3-hour presentation (including local discussion) includes the 2.5 hours LIVE streaming live video webcast presentation (also available as an archived webcast or via DVD). An additional 30-minute local discussion is required by each registered site location. CE credits are only available for the full 3 hours (program + discussion). The moderator will suggest an intermission about half-way through the program. You may show the program at any time between the dates of 4/10/14 and 4/9/15. When publicizing the event, make sure that the advertised time includes the registration time, introductions, and required 30-minute local discussion. Conclusion: Supporting Staff Working with Adolescents SEGMENT HIGHLIGHTS: The full segment highlights will be available to sites once they are successfully registered, and segment highlights will be printed in the program guides viewable for all participants. CE INFORMATION: Continuing education (CE) credits are required for many health care and other professionals in order to renew a license. Offering and publicizing the availability of CEs will help you attract audience members! This program is valid for three (3) hours of CE credit (available online only); until April 9, CE FEES: The CE cost is $15 per person (for the first two weeks after the live air date; until April 24, 2014). After this date, the CE cost is $25 per person (for the remainder of the year; until April 9, 2015).

5 CE APPROVED BOARDS/PROFESSIONS: A complete list of board approvals (for all national and state approvals) for this program is posted to HFA s registration site at Course Completion Requirements: Participants must sign in upon arrival and attend the entire 3-hour program (2.5 hour video presentation and 30-minute local discussion). Partial credit is not awarded. Participants must complete the entire CE process online, before the CE deadline of April 9, o Two codes will be issued to you and verified online; a Site # and the Course Code, both provided by the registered site organizer. o The online CE process includes a required evaluation form. o The online CE process includes a required exam. The exam must be completed at 80% or above in order to pass. However, attendees may re-take the exam as many times as needed. o Attendees will be able to choose the board they wish to receive credit from (from our list of board approvals) and will then be able to print their CE certificate immediately after completing all CE requirements (on HFA s CE site: o Payment is required online. HFA accepts Visa/MC/AMEX only. Organizations may also pre-pay for their attendees CE credits. In this case, a Payment Code may be used online, in place of a credit card payment. CE POLICY: CE credits are copyrighted by HFA. Participants are attending an event of Hospice Foundation of America (HFA). All CE certificates must be obtained directly through HFA no exceptions allowed. Boards approve the content developed by HFA, not by the viewing organization. Sites are not allowed to print their own Certificates (including a Certificate of Attendance). CE instructions will be provided to you on the download area online, and will be provided to attendees at the conclusion of the program. COPYRIGHT POLICY: HFA s program is copyrighted. Its use and dissemination is restricted and unauthorized duplication is strictly prohibited. HFA strictly prohibits any other dissemination of this program by any method, including internal WAN or LAN computer networks or telemedicine networks. A registered site is licensed to show this program at one, single location ONLY, as defined by the site s physical street address. If you plan to show this program at multiple geographic locations (e.g. satellite offices) each location must be registered separately and each site must have its own Site ID.
