Completed Financial Statement form (attached) along with verification of all assets reported (ex. bank and brokerage statements) 7. A letter from the shareholder(s) to the Board explaining the reason(s) for the loan 8. Employment verification letter stating position and salary. Self-employed applicants must submit a letter from an accountant or CPA verifying income and source. 9. $ Processing Fee payable to FirstService Residential, New York, Inc. 10. $75.00 Consumer Reports Fee (per report) payable to FirstService Residential, New York, Inc. If the new loan is for an amount EQUAL TO or LESS THAN your existing loan AND the monthly payments will be the same or lower only the following documents must be submitted: 1. Copy of new loan commitment 2. Copy of new loan application 3. Copy of current monthly mortgage statement 4. First five pages of appraisal report 5. Three (3) original Aztech Recognition Agreements (must be signed by lender & borrower(s) 6. $ Processing Fee payable to FirstService Residential, New York, Inc. In order to more efficiently process requests for a Certificate of Insurance, a new form has been created to collect the information required to process the request in a timely manner. The form can be accessed by going to our website, and by clicking on Insurance Certificates towards the bottom of the home page. A member of the Insurance Team will respond back accordingly.
