1 Thinking Nationally Acting Locally NatPaCT in the West Midlands March 2004 Issue 6 NHS Modernisation Agency National Primary and Care Trust Development Programme WELCOME The allocation of resources to support the implementation of policy, together with information about wider organisational development programmes, provides the content for this edition of NatPaCT in the West Midlands. We focus on AHP and PEC member development as well as highlighting some imminent activity around helping to develop the potential for future PEC members through succession planning. This last point has been a concern for members of the PEC Chair network both in the West Midlands and beyond and so we hope to bring some added support to this area for PCTs. Finally, NatPaCT's central function in Leeds continues to draw together a range of expertise and skill in key areas, and more expansive information on the support available is contained on our website. During late last year and early this year, Local Reference Groups comprising PCT, WDC and SHA representatives, have been looking at how to spend 50K across their respective geographies. In releasing the resource, NatPaCT was looking for local health economies to align the funds to affect real service redesign and change. Each of the three areas, agreed ways to make best use of the collective resource and here's what they are each doing: Shropshire and Staffordshire Used the funds across the PCTs to support the work needed towards the implementation of ngms Birmingham and Black Country Split the funds across the nine health economies to support clinical engagement West Midlands South Pooled the funds with resources from the WDC to provide a development programme around Agenda for Change. 2 Watch this space - further details out soon Partnerships, Primary Care and Out-of-Hours Collective resource gets agreement on use Each strategic health authority area of PCTs will from 1st April, receive 100K recurring to support the introduction of new posts focussing on critical policy imperatives. The PCT Improvement agenda, Out of Hours and the ngms contract have all been the focus of discussions over recent months, and each of the three areas in the West Midlands is now forging ahead with its plans. Shropshire and Staffordshire An appointment has been made for a Partnerships Support Officer in conjunction with the County Council, to facilitate cross boundary working and service development. The remaining funds are supporting two new fixed-term posts to focus on the Out-of-Hours agenda. Cannock Chase, Shropshire County and South Stoke PCTs are taking the lead in these arrangements. Birmingham and Black Country A recruitment and selection exercise is underway to bring in two new posts to work on the primary care agenda with an immediate focus on ngms and PMS arrangements, together with Out-of-Hours. The posts of PCT Primary Care Development Co-ordinators will cover both the Birmingham and Black Country areas respectively. South Birmingham and Walsall PCTs are taking the lead in these arrangements. West Midlands South Discussions are ongoing for a new Director of Primary Care to join the SHA, with a view to creating a cultural shift in the approach to primary care and the ngms contract. It's envisaged that the post-holder will be seconded from a PCT for a 1-2 year period and will work closely across boundaries to affect significant organisational change in this policy area. The work from these posts will be reported into the Local Reference Groups which exist in each strategic area, so that there is co-ordination with other organisational development and performance improvement activity. These Groups are due to next meet on: Shropshire and Staffordshire Through the Chief Executive network on 26th March Birmingham and Black Country 24th March West Midlands South 29th March So You Want to be a PEC Member? Coming Soon.an opportunity to consider succession planning in your PEC Through various networks and discussions amongst PCTs and wider clinical colleagues, the issue of where future PEC members come from, is keeping most people awake at night! In reponse to this, a programme is being developed which aims to support your succession planning activities and help to stimulate some thought for potential PEC members.
3 Driving the Agenda through Development Summarising the range of two years' organisational development activity Over the last two years, each PCT in the West Midlands has received 15K each year to align to organisational development work within their organisation. Some of this focussed on developing the internal facing spectrum of the organisation and some was used to support development across organisational boundaries. There are examples of both clinically and managerially led development programmes, bringing a range of experiences across the 30 PCTs. These were some of the feedback statements provided by delegates who had participated in a two-day PEC development programme. Aimed at AHP and wider PEC members, delegates were taken through a range of opportunities which focussed on their role, the organisational climate they work in, and the mechanisms to be harnessed to maximise individual effectiveness. A further two days are scheduled for the 20th and 21st April in Redditch and places are available for any AHPs or PEC members from across the region. We are also planning to hold a further two days in May and these dates will be issued as soon as they are confirmed. The development is jointly supported through NatPaCT, BBC and local SHAs, together with the Leadership Centre at the Modernisation Agency. Journey to the Emerald City A book of accountability, culture testing and accelerated change This book by Roger Connors and Tom Smith, introduces the notion of 'a culture of accountability' in organisations driven by the desire for change and success. It refers to accountability as one of 'freedom to do', rather than the traditional model of accountability we're used to. It presents a model of working at 4 different levels in an organisation - Results, Actions, Beliefs and Experiences - using the premise that unless you change experiences, your results will remain the same. There are useful culture self tests and models of looking at accelerated change through management of alignment, confrontation and different levels of feedback. Presented clearly and simply, the book is easy to dip in and out of and provides a different perspective on individualism and its impact on organisational culture and drive. 5 Working closely with Chairs and Non-Executives Appointments Commission listening to development needs A local Regional Training Facilitator from the Appointments Commission, Susan Bedward, came to post in November last year to work closely with local colleagues on the development needs of Chairs and Non-Executives. Susan will be supporting the work of the West Midlands Regional Training Board of the NHS Appointments Commission, which has been set up to facilitate high quality training in this area. She will be working closely with colleagues across the region to co-ordinate and facilitate training and development needs identified at the local level through the Regional Training Board, and has already found immense enthusiasm for the scope of her work: "I'm really enjoying my early days in post and everyone is keen to share their thoughts and ideas about the training needs. We'll need to take an overview across the region and I'll be looking at the early priorities that arise from our work." Leadership and Management Development Review draws finding together! Many of you will be aware of a short West Midlands South and Shropshire and Staffordshire SHA combined project, to develop a framework for a series of Leadership and Management Development activities for potential and new senior clinical leaders; that is PEC Chairs, Clinical Directors, PEC members and nurses, AHP and healthcare professionals at a similar level. Anthony Aggett the Project Manager would like to thank those of you who contributed to the work, and for your time and enthusiasm. The project is now at the report writing stage. Once approved by the SHA, then the framework will be piloted later this year, involving a cohort which is likely to provide 24 places across both patches. A Project Manager Co-ordinator will be appointed soon to take this work forward. Further details will therefore be out in the near future. West Midlands Primary Care Development Team (NPDT) Centre "This is what's helped to achieve real sustainable change!" Being one of eleven centres around the country and building on the work of the first four national waves of the Primary Care Collaborative - the West Midlands National Primary Care Development Team (NPDT) centre now has 267 general practices on Access and CHD throughout the West Midlands. To facilitate progress it now has a team of 26 Access Facilitators based in PCTs across the three Strategic Health Authority areas. Jenny Lee, Lead Manager for the West Midlands centre, was enthusiastic about the outcomes from the programme's work: "We've had impressive practice results in reducing patient waiting times. Patients initially waited 4.39 days to see a GP and this has now been reduced to an average of 1.42 days. For nurses we've seen patients waiting 4.49 days down to 1.92 days. This couldn't have been achieved without the local engagement and participation in quality improvement tools and techniques. This is what's helped to achieve real sustainable change within primary care teams." The centre is responding to the needs of ngms and will be launching some exciting new programmes of work such as Phase111, Quisp and LQIP. The NHS is under increasing pressure to meet its priorities, and is looking for solutions to help it to meet its targets. Are you aware of the potential in your local network of community pharmacists? These health care professionals based in the heart of communities can help to meet those targets. For example, a community pharmacy based minor ailment scheme, where the community pharmacist supplies medication on the NHS from a limited formulary free of charge to patients who are exempt from prescription charges, encourages more patients to consult with their pharmacist for minor ailments thereby improving access to the GP. So where can I find out more? The National Pharmaceutical Association (NPA) is a trade association which supports around 11,000 community pharmacies in the UK. Within the NPA, the NHS Service Development Department supports members, local pharmacy leaders and the NHS on all areas of community pharmacy service development. For example, the Department has recently launched Implementing a community pharmacy minor ailment scheme: A practical toolkit for primary care organisations and health professionals which can be accessed at (then 'Publications' then 'NHS Service Development'). There are also three regional NHS Service Development Managers in the Department. Their role involves working with local pharmacy leaders (e.g. local pharmaceutical committees) and the NHS (e.g. PCTs) to inform people of the implications of changes and developments in community pharmacy, to identify and spread best practice, and to help identify engaging with community pharmacy as a way to improve patient care and achieve targets.
