2 Introduction The importance of research and innovation for patient outcomes and quality Research and Development (R&D) is core NHS business National drive to raise the profile, quality and quantity of research careers in nursing and professions allied to medicine Large university teaching hospitals specifically target talented and interested nurses, encouraging and supporting them to develop research careers. But Although funding is available to support practice nurses to access R&D training, data demonstrates that attendance at R&D training/education, although improving, remains poor from primary care. However This is against the background of service change - from hospitals to primary care A time when there should be greater emphasis on developing the evidence for primary care, in primary care. This project specifically targets practice nurses, with the aim of introducing them to R&D, designing tools sensitive to work place, supporting accessing R&D training, and raising aspirations of developing a research careers 26/11/2012 East Midlands HIEC 2

5 Barriers to Implementation Primary care itself! Fragmented nature of primary care/primary care leadership Practice nurses work as small teams, employed by independent contractors, geographically dispersed, isolated from large scale programmes of professional development lead by Directors of Nursing within acute trusts. Traditional delivery of Research training Traditionally R&D training is delivered in a university setting. As the primary care service nationally reaches capacity, this is an increasing problem ; securing time away from the clinical setting is increasingly difficult. R&D in primary care R&D in primary care is still in its infancy, poorly understood and low starting point Lack of knowledge, confidence and competence of nurses from primary care 26/11/2012 East Midlands HIEC 5

6 Risk to Delivery Changing NHS architecture PCTs are going! There is no clarity at present where primary care R&D is going? New developing systems mainstream education There is no clarity where R&D s presence is in this? How the National Institute for Health Research (NIHR) training, including clinical research training and competencies, fits in with the mainstream education, or where the RCN is positioned in relation to this. 26/11/2012 East Midlands HIEC 6

7 Overcoming Barriers The project team Practice based research nurses and nurse researchers, clinical research nurses, all working in the field. Familiar with the environment and the challenges this presents. Support engagement and design sensitive to environment, support delivery as part of their role. Dr Jackie Parkes, lead for Nurse Educational Development at the University of Northampton, including primary care training and will support the development of identified training. A nominal group approach will be used, informed by the expert group, formulated into elements that will be tested with a focus group of practice nurses. From this specific training tools, interventions will be designed. The field team will then support delivery of the interventions within the practice setting and provide professional peer support. Gill Sarre from the NIHR EM Research Design Service has considerable knowledge and experience of developing evidence based tools to measure research culture and capability. Her expertise will support the evaluation of this project 26/11/2012 East Midlands HIEC Footer 7

8 Risk Mitigation Project team Project design This project does not use models that have previously been developed by NHS Trusts. It is not designed to be hierarchical, with a large expensive infrastructure that will require organisational commitment to continue. It is the belief of the steering group that this will not work in the primary care setting. The project is designed to grow organically, to use resources that are already part of the infrastructure, to work with the environment and be sensitive to its needs. Leadership Support Dr Sarah Whiteman, currently cluster Medical Director, Medical Director designate of the Local Area Team is in full support of this project. As a GP she recognises the importance of developing the primary care workforce and is committed to seeing this through the PCT changes. Engagement of the shadow LETB, main stream education system To be sustainable, it is vitally important that this project links into mainstream training and education provision. The project team is well integrated at university level with the developing LETB, engagement has been made with the Workforce Development Team, the project is being presented to the Northamptonshire Primary Care Subgroup tomorrow. 26/11/2012 East Midlands HIEC 8

9 Evaluation The academic team includes, Gill Sarre from the NIHR EM Research Design Service who has considerable knowledge and experience of developing evidence based tools to measure research culture and capability. Her expertise will support the development of a pre and post intervention tool to measure impact. Outcomes that will be measured will be number of practice nurses accessing any R&D training/nihr internships/fellowships or other master level programmes Number of NIHR/CLAHRC portfolio studies and recruitment to time and target 26/11/2012 East Midlands HIEC 9
