(NMD), in association with our team members, is motivated to have this opportunity to work with District VI Maintenance in providing Districtwide Repair and Rehabilitation Services. This project will involve an assortment of tasks in the specialty of bridge engineering and appurtenances to the maintenance department s bridge assets: Roadways, fenders, lighting fixtures, signs, and miscellaneous items. Towards this goal, our firm has the experience, drive, and know-how to prepare contract documents that conform to the FDOT s PPM, SDG, Standard Specifications, and Standard Drawings for Roadway, Bridge and Traffic Design. NMD is a local company with all of the resources necessary to deliver a District VI quality product. We understand District VI Maintenance from our many years of partnering within the Maintenance Department. Understanding that districtwide contracts tend to require various expertise of bridge and highway design, our team is tailored to offer the following: conventional bridges, post-tension bridges, movable bridges, special structures, roadway design projects, resurfacing projects, auxiliary structures and PD&E reports. It is important that the selected consultant team have the local resources to take on any assignment. From our working experience with District VI, we are aware that the main objective of this contract is to serve as an extension of the Department in order to have a seamless production of construction documents that work integrally with the Department s resources. NMD recently provided District 6 bridge inspections for over 350 local government bridges within Miami-Dade and Monroe Counties. This project included the inspection and condition assessment of various types of bridges including fracture critical, underwater, concrete, steel and movable bridge structures. He will provide monthly formal status reports and will keep Mr. Amigo up to date on all project issues. He will take the lead in organizing and documenting project meetings, telephone conversations/teleconferences and field reviews, etc. Coordination with Utilities Our goal will be to minimize impacts to existing utilities but at the same time adhere to all current safety criteria. We will notify the Department of any utility conflicts which may arise and coordinate with the Department in all relocation efforts. Coordination with Permitting Agencies The NMD Team has an excellent working relationship with the District s Environmental Management Office. We have extensive permitting capabilities to handle any project issues, should the District require permitting assistance. Coordination with the Public Community awareness is an important aspect in designing projects. We will support the Department by providing presentation materials for public information and meetings. The NMD Team will identify the project s stakeholders and establish points of contacts. Technical Approach Design Services This contract will involve different tasks including the update and/or revision of previously completed construction plans and specifications, as well as the preparation of construction plans such as: bridge design, roadway design, intersection and highway improvements, maintenance of traffic, utility relocation, structures design; sign support structures and details, drainage facilities, lighting design and justification reports; signing; signalization and justification reports, pavement markings and incidental items. The submitted package will include technical special provisions and supplemental specifications, as required. All reports, calculations and plans will conform to the latest AASHTO Standards, FDOT Standards and Specifications. CADD personnel will be assigned on-site to support the Department as required. Quantity computation books and preliminary construction cost estimates will be prepared for all phases of design. As part of the construction plans development, we will produce a Design Documentation Booklet with design notes, bridge load rating as necessary, field reviews and bridge inspection reports, calculations and pertinent information supporting our design conclusions. The NMD Team utilizes the latest approved software such as STAAD Pro V8i, LEAP, MDX Software, Mathcad and FB Multi Pier, etc. in developing the design documentation and calculations. The NMD Team is also experienced in the use of PEDDS (Project Electronic Development and Delivery System) and the Electronic Document Management System (EDMS). NMD holds several active Hummingbird software licenses. Construction cost estimates will be prepared

2 and periodically updated using the FDOT CES database system. We also have a sophisticated computer aided design department with full bridge modeling capabilities that can be available to the department as needed, as well as a conventional CADD drafting capabilities. ATEC can provide traffic studies, traffic counts, and analysis as required by the department. It is important to take into consideration Maintenance of Traffic. Our team will make TCP a top priority and will coordinate with Traffic Operations for all lane closures and MOT Plans. NMD has strong experience in developing specifications; our firm understands how to put technical special provisions and supplemental specifications together for any bridge/highway product. Our team has developed over years of experience a good working relationship with utility companies and owners that share of utilize FDOT roadways through our years of working on the MEI contract and understand the importance of utility coordination and relocation is to the success of rehabilitation projects. The department will benefit from our knowledge and experience with utility owners. We will develop tracking as well as coordination efforts for all rehabilitation projects having utility conflicts. In addition, we may be required to provide expert witness services in any legal proceedings as required by the RFP. Design Support/Plans Review Plan reviews are a critical path item in all design projects and we will provide all plan reviews within the required schedule timeframes. Our plans review support to the District will be the review of construction documents, including plans, cost estimates, calculations and technical specifications at required submittals. We will perform comprehensive reviews and provide sound engineering comments to the in-house maintenance section and consultants working for the Department. The plans review process will ensure compliance with the scope of services, design criteria, FDOT standards and procedures and RFP. Design plans are reviewed at various stages of development along with special provisions, quantity calculations, and pay item lists to identify and minimize potential construction problems and claims. All review comments as well as recommendations to the project will be transmitted to the designer through the ERC system. In addition we will coordinate with the consultants that have developed the plans and maintain open lines of communication. Post Design Services The NMD Team is aware of the importance of supporting the Department during construction activities. The construction support services extend to reviewing specialty engineer s designs due to contractor s minor modifications and/or deviations from the contract plans, contractor s VECP to the project and review of errors discovered in contract plans during construction, including its impact on construction and making recommendations for cost recovery by the Department. Our Team will review shop drawings, material sheets and calculations submitted by the Contractor. In certain cases, depending on the complexity of the design deviation or a schedule related issue, we may need to conduct an independent analysis to assess the validity of the change. Additionally we can provide Value Engineering services. Delivering value on all rehabilitation projects is of the utmost importance to our firm. NMD has the expertise to provide Value Engineering services for any project the Department may want to review prior to biding. Emergency Response Process The NMD Team is very familiar with FDOT District 6 Maintenance, makes the commitment to District 6 to be there 24/7/365 and will furnish the Department with all emergency telephone numbers for all staff and key management personnel. NMD s Headquarters is within minutes of District 6 and its convenient location will facilitate task program development, data collection, and investigative work. Minor/Roadway /Traffic Control and Constructability Safety First is the number one goal of traffic control plan development. Another goal will be to develop traffic control plans that reduce local and regional traffic impacts. Several design factors our design team will consider are peak flow volumes, safety, construction efficiency, and impacts to traffic by local and seasonal events which may occur within the project limits. The design team will consider alternative FDOT approved construction materials and techniques, such as night time construction, that will expedite the work and reduce traffic impacts. Managerial Approach Mr. Ochoa will communicate and involve all necessary Team members to efficiently and quickly perform the task at hand. In most instances the basic principles of problem solving will be utilized in preparing the work plan for each assignment, whether it is a bridge or highway design. These principles include: the identification of the problem through a scope of work; identification of the required resources for the task; and finally, the implementation of the solution through and assigned staff ready to address the needs of the Department. The review/design process of assigned tasks will generally follow the steps in the flowchart above. It is the goal of the NMD Team to Deliver a Successful Complete Project Control of Project Schedule Our commitment to the schedule will ensure the assigned projects will be completed on time and within budget. The project schedules will be based on task and milestone delivery dates anticipated for the project and will identify critical path issues/tasks that drive the final deliverable dates. Control of Project Budget Our project team is proficient with FDOT s LRE program for cost estimating. We will produce a cost estimate early on for each of our assignments to assure that the proposed project scope will be within the Department s projected budget. Quality Control Process The NMD Team believes we are solely accountable and responsible for the quality of our deliverables. To ensure we achieve a high degree of excellence in our planning, design and construction plans preparation, a proven Quality Assurance and Control Plan (QA/QC) will be implemented for all project assignments. The design team s quality control efforts will involve detailed plans checking and reviews of all work products. Staffing Plan Our staffing plan is simple and straight forward: bringing together a team of highly motivated and qualified professionals with established working relationships. Our organizational chart and key personnel resumes describe the management, technical and administrative individuals needed to successfully complete any assignment. NEW MILLENNIUM DESIGN CONSULTANTS, INC; Technical Approach Movable Bridges we will provide complete movable bridge support services whether minor or major items our team is able to handle the task. Post-Tensioned/Segmental Bridges - We will provide specialty engineering support the Department with respect to post-tensioned bridges and segmental bridge load rating, design review, rehabilitation, constructability and construction of major segmental bridges. Concrete Repairs/Restoration - To ensure a successful repair, the preparation of the steel and concrete to remain is of the utmost importance, whether cathodic protection, structural reinforcing systems like carbon fiber or typical concrete repairs are employed. If cathodic protection is required NMD will coordinate with the Department s Consultant in developing the optimal restoration procedure. Structural Steel Painting - Non-destructive testing is required to determine if lead is present and the necessary surface preparation and painting system. Mr. Ochoa s well-rounded experience and knowledge are invaluable to the management of any project. PROJECT EXPERIENCE: Miami International Airport Upper Vehicle Drive Bridge Rehabilitation and Load Rating, Miami-Dade County, FL. Project Manager Project consisted of complete professional engineering services for the restoration of the Miami-International Airport Upper Drive Bridge Structure (FDOT Bridge No ). Rehabilitation included replacement of expansion joints, repair and rehabilitation of structural steel bents and frames, repair of spalling wearing surface, painting of structural steel elements to protect from further damage. Project also, included preparation of rehabilitation plans, specifications, cost estimate and procurement services. SR-826 On Ramp to I-75, Miami-Dade County, FL. Responsible for the construction engineering portion of the project for the widening of a multi span three unit single box girder segmental ramp in Miami Florida. The project encompassed a single segmental box that had was on a 1000 ft horizontal curve and with spans in the 200 ft range. Responsible for the complete torsional analysis of the single box, which included time-dependant effects and the design of stability towers. Also responsible for the verification in localized stress areas for hold down devices for stability design. MIA Short Term Parking Bridge Shoring, Miami-Dade County, FL. Project Manager/Engineer of Record responsible for the development of a complete set of construction documents for the shoring of the existing short-term parking bridges (3062B & 3062C) in the upper drive. Existing one-span bridges span from the upper drive to the short term parking garage. The structural steel bridges consist of two through girders with reinforced concrete deck, supported on columns and roof slab of existing Consumer Services Building, Shoring system consisted of a two structural steel hollow tubes on spread footing. N.E. 2nd Avenue Bridge, Miami-Dade County, FL. Provided structural engineering services for the shoring design of the NE 2nd Avenue Bridge over the Little River Canal in Miami. The project consists of a several widened bridges multilane steel beam and prestressed concrete beams supported on pile bents and supported on concrete frame supported on wood piles. NMDC was responsible for providing emergency analyses for the stability of the bridge and the design of shoring system so that the bridge may continue to carry traffic while the canal is dredged. NE 15 th Avenue Bridge Over Snake Creek (C-9) Canal, Miami-Dade County, FL. Project Manager who provided Miami Dade County professional engineering services by conducting a peer review of the EOR s Superstructure plans for the Snake Creek Bridge Project that was under construction. Scope included field visits, review of superstructure plans, review as-built information provided by the County, and to develop a marked up revision set intended to assist the EOR in his revision & implementation of recommendations in bridge plans to meet AASHTO, Public Work Standards, FDOT Structural Design Guidelines, and, FDOT Standards as applicable. Civil Engineering, Northwestern University, 1960 Professional Registration Professional Engineer: FL # Special Inspector, FL# 274 Years of Experience: 46 Experience and Qualifications Mr. Tokay serves as Senior Structural Engineer and Project Manager for New Millennium. He has over 46 years of experience in the design of bridges, buildings and other specialty structures. Mr. Tokay began his career in Chicago. He has worked on both complex and major structures for bridges and buildings and has specifically 18 years of experience in the analysis and design of bridge and transportation structures including movable bridges. PROJECT EXPERIENCE: SW 2nd Avenue Bridge - Miami-Dade County, Florida. Responsible for the design of the structural elements of double leaf Bascule Bridge over Miami River, including the design of bascule piers, control tower, trunnion and machinery supports. Developed an orthotropic plate design for the sidewalks supporting wheel loads and provided wind and thermal analysis for bascule machinery design. Designed a concrete roof structure for the control tower providing maximum visibility for the bridge tender. The end spans of both ramps were subjected to fire damage after a gas fuel tanker went over the barrier railing on the southbound I-95 to westbound SR 836 ramp. Project involved repairing structural concrete elements to their original cross-sectional profiles and appearance. Fire-affected bearing pads were replaced by constructing steel jacking frames. Killian Parkway over SR Miami-Dade County, Florida. Project consists of the design of a trumpet entrance ramp from Killian Parkway to SR-974 in Miami Dade County. The bridge ramp consists of a two span continuous steel plate girder bridge with 142 ft spans. Assisted in the design verification of the steel plate girders. Design was done in accordance to AASHTO s LRFD Specification, the analysis was done utilizing STAAD Pro and the design was verified by hand. Generated loads by hand and optimized the section by hand. American Airlines Arena Seawall, Miami-Dade County Florida. Project Manager for the replacement of the seawall with docking facilities at American Airlines Arena. Services include programming and schematic design, design development, construction documents and post-construction administration. Involved in permitting from regulatory agencies including State EPA, Corps of Engineers, DERM and City of Miami. As Chief Design Engineer, Ms. Herrera has led the design of various types of roadway projects such as widening, milling and resurfacing, interchange design. Her experience also includes the design of bridge structures, retaining walls and sign structures. Ms. Herrera has also served as a Quality Assurance Manager, successfully implementing the policies and procedures necessary to ensure quality and performance. Project consisted of approximately 4 miles of new construction. Also responsible for the design of the culvert at the C-102N canal and miscellaneous strucutres including mast arm and lighting foundation. Miami International Airport Upper Vehicle Drive Rehabilitation Project., Miami-Dade County, FL. Project consists of complete professional engineering services for the restoration of the Miami-International Airport Upper Drive Bridge Structure (FDOT Bridge No ). Project included preparation of rehabilitation plans, specifications, cost estimate and procurement services. Roadway Design Engineer responsible milling & resurfacing plans, signing and pavement marking and traffic control plans for the rehabilitation effort. N.E. 2nd Avenue Bridge, Miami-Dade County, FL. Provided structural engineering services for the shoring design of the NE 2nd Avenue Bridge over the Little River Canal in Miami. Proposed bridge consists of a 38-4 single span bridge, with an overall bridge width of 36-0 to accommodate 2-11 lanes, shoulders and raised sidewalk. Superstructure consists of prestressed slab units with concrete overlay. Substructure consists of conventionally reinforced cast-in-place caps supported on shallow spread footers. Responsible for client coordination, permitting coordination, specification, and post design services. SW 127 Avenue over C-6 Canal, Miami, FL Senior Structural Engineer for the single span cast-in-place bridge over the C-106 Canal on conventional pile bents. Project included design of a bulkhead tie-back system retaining wall, environmental permitting and construction engineering design services. Structural Inspection and Assessment of the Indian Creek Bridge (Bridge No ), Miami-Dade County. Structural Engineer. The project involved the field investigation and analysis of the Indian Creek Bridge located in Miami-Dade County. This a thirteen~span reinforced concrete T beam bridge with fascia arch beams. Responsible for the bridge inspection, determination of deficiencies and load rating in order to gather all data required to ascertain the existing condition/capacity of the structure and estimate a preliminary construction cost that will serve as the basis for the production of the bridge rehabilitation document to repair and partially restore the capacity of the bridge s superstructure

Ms. Hernandez s has a solid background in the design phase services related to transportation projects with experience in highway/roadway design, horizontal/vertical layout, storm water management, drainage, maintenance of traffic, signalization, construction estimates, and permitting. Engineer of Record- This project consist on new construction of SW 137 th Avenue (4 Mile Corridor) which will be initially constructed as a four-lane divided facility between US 1 and SW 248 th Street and a two-lane undivided facility between SW 248 th Street and SW 200 th Street. This project includes horizontal and vertical design, drainage, traffic control, signing and pavement markings, lighting, environmental, specifications and coordination with DERM and SFWMD. Assisted in the rating of existing bridge structures throughout Miami Dade County and surrounding areas. Performed detailed analysis based on given bridge inspections and construction plans. Provided a Load Rating for recommendation to the Florida Department of Transportation. MDX Load Rating. Structural Engineer prepared and developed load ratings for 25 MDX Bridges. The Superstructure of such bridges included typical Prestressed Concrete AASHTO Girders, Reinforced Concrete Slab Bridges & Steel Plate and Rolled Shapes Girders. The geometry of the bridges was diverse so that some required a more elaborate load rating analysis since they had skewed supports, with the combination of AASHTO girders and continuous steel plate girders for different spans.
