Associated Youth Services of Peel Phone: Fax: Website: 160 Traders Blvd. E, Suite 100 Mississauga ON, L4Z 3K7 Serves residents of Peel Region DBT Program provides therapeutic services to youth aged years residing in Peel who have experienced suicidal thoughts, actions and/or self-harming behavior as well as quality of life-interfering behaviors and interpersonal issues o DBT treatment ranges from 6-12 months and includes weekly individual therapy sessions, skills training group, phone coaching and family counseling o Inclusion criteria youth who experience several of the following: life threatening behaviors, quality of life interfering behaviors, recurrent suicidal thoughts or attempts, difficulty in interpersonal relationships, frequent mood swings, intense fear of abandonment, confusion about self, family conflict Transitional Aged Youth Outreach Program case management and counseling program for youth aged 16 to 18 years residing in Peel who are experiencing on-going mental health challenges and are transitioning into adult services o Offers a community based service providing a consistent level of support to youth throughout the transition Youth Beyond Barriers Program provides confidential services for youth aged 12 to 18 years residing in Peel who identify as LGBT through a support/education group and individual counseling o Support can also be provided for parents/caregivers of the youth Tangerine Walk In Counseling walk-in counseling provided, in partnership, by Associated Youth Services of Peel, Peel Children s Centre and Rapport Youth & Family Services (www.tangerinewalkin.com) o Each partner delivers the service on a different day from Tuesday - Thursday o Tangerine Walk-in Counseling is available free of charge to children and youth under the age of 18 in Peel Region, as well as their parents or caregivers o Associated Youth Services of Peel offers Tangerine Walk In Counseling on Tuesdays from 9AM to 8PM Associated Youth Services of Peel also offers family programs, justice programs and group programs 2. Peel Children s Centre Phone: Website: 85A Aventura Court Mississauga ON, L5T 2Y6 Monday Thursday: 8:30AM 8PM; Friday: 8:30AM 5PM Serves residents of Brampton, Mississauga and Caledon All programs at the Peel Children s Centre are free Mental Health Services for Children and Youth (Centralized Intake) o Finds an appropriate program for children and youth under the age of 24 years residing in Peel Region who need help for a mental health problem o They will make an appointment with one of the following providers: 1. Associated Youth Services of Peel 2. Nexus Youth Services 3. Peel Children s Centre 4. Rapport Youth & Family Services 5. Trillium Health Partners 6. Building Bridges Day Program at Toronto East General Hospital Phone: ext Fax: Website: 825 Coxwell Avenue Toronto ON, M4C 3E7 Offers a 12-week full time school program for youth between the ages of 12 and 17 who are unable to attend school due to mental health concerns o The morning is academic and the afternoon includes mental health treatment o The program runs Monday 8:25AM 2:30PM, Tuesday to Thursday 8:25AM 3PM and Friday 8:25AM 12:30PM o Physician referral is required and forms should be faxed to Central Toronto Youth Services Phone: Fax: Website: 65 Wellesley Street East, Suite 300 Toronto ON, M4Y 1G7 All services are free of charge Community Counseling Programs offer outreach, individual counseling and group work to youth between the ages of 12 and 18 years (referrals accepted from professionals, agencies and youth) o Community Outreach Program offers services to hard to serve youth in their homes and communities in order to improve coping and life skills and help youth respond to a range of issues, including aggression, depression, anxiety, self-harm 14

15 behaviors, school related problems and family conflict o Group Programs groups address issues such as coping with anger, social skills, depression, anxiety and self-esteem and run for weeks New Outlook provides a range of programs for youth aged years recovering from serious mental illness, primarily psychotic disorders (referrals are accepted from health care or social service agencies and youth) o Day Program supports young people with serious mental illness, through a structured program including life and social skills training, recreation and academic studies (can earn high school credits) Confirmed diagnosis of a serious mental illness, case management and ongoing psychiatric/medical care are necessary for inclusion o Early Intervention supports young adults experiencing a first episode psychosis through counseling, skill building and advocacy o Community Support Program provides long term case management to youth with serious mental illness Pride & Prejudice offers programs including individual counseling and group work to support lesbian, gay, bisexual, trans, queer and questioning youth between the ages of 13 and Centre for Addiction and Mental Health Phone: Fax: Website: Queen Street West Toronto ON, M6J 1H4 Child, Youth and Family Program offer intensive and specialized services for young people with mental health and/or addiction problems, including: o Assessment o Medication consultations o Individual and family therapy o Specialized treatment groups o Day treatment programs o Parenting programs Gender Identity Service offer assessment and treatment to youth aged 3-18 years struggling with gender identity development and any related behavioral or emotional problems o Children/Youth across Canada are eligible for assessment o Referrals are accepted from any health care professional, parent or youth over the age of 16 forms should be faxed to o Assessments are covered by OHIP o Located at 80 Workman Way CAMH Queen Street site Mood & Anxiety Service offer assessment to children and youth experiencing anxiety or mood problems and recommend treatment when appropriate (may be able to offer time-limited treatment) o Physician referral is required forms should be faxed to o Located at 80 Workman Way CAMH Queen Street site 6. 16 o Individuals must not yet have received treatment for psychosis, or had less than one year of treatment from other mental health services 7.
