1 OPHTHALMIC MEDICAL ASSISTANT ADMISSIONS INFORMATION APPLICATION PERIOD: FALL 2015 CLASS Caldwell Community College & Technical Institute 2855 Hickory Boulevard Hudson North Carolina Program Information The one-year diploma program in Ophthalmic Medical Assistant prepares individuals to provide support services under the supervision of a licensed physician specializing in Ophthalmology. Course work includes both classroom and clinical instruction with an emphasis on taking medical and ocular histories and performing a variety of ocular measurements including lensometry, refractometry, keratometry, tonometry, muscle balance screening, and perimetry. Graduates of the OMA program will be eligible to take the Joint Commission of Allied Health Personnel in Ophthalmology (JCAHPO) national certification exam for Certified Ophthalmic Assistant (COA). Graduates are employed in medical institutions, clinics or physicians groups. Most disciplines of Health Sciences require registration, certification or licensure to practice in the health care field. Most examining boards require proof of good moral character to apply to take the registration, certification or licensure examination. JCAHPO can be reached at or jcahpo@jcahpo.org. Applicants should be aware that travel in out-of-service areas may be required for clinical placement. The deadline for applying and meeting all admissions requirements for the upcoming class beginning in the fall semester is December 1st each year. Each OMA applicant will be notified regarding his/her application status for the upcoming fall class. Class size is limited.

2 Admission Process Each applicant is tentatively admitted to the OMA program by the Consideration Date (the date that all admission requirements are met.) This is a chronological order of dates, firstcome, first-served process. Admission Requirements It is suggested that admission requirements be met in the following order. The deadline for completing all admission requirements is December 1st each year. 1. CCC&TI application. Submit an application to Student Services. Applications are available in Student Services or online at 2. Official documentation of high school graduation or GED completion. This is required even if the applicant holds a four-year degree. A high school graduate should request his/her high school to send to CCC&TI an official transcript indicating date of graduation. If the applicant is still in high school, an official transcript should be sent now only if SAT/ACT scores are to be used to waive the college placement tests; otherwise, an official transcript should be submitted after graduation. A GED or adult high school diploma graduate is exempt from sending a high school transcript, but an official copy of GED scores or official documentation of adult high school graduation must be sent to CCC&TI from the granting institution. Reproduced copies of personal documents are not acceptable. Request forms to obtain transcripts are available in Student Services at CCC&TI. 3. Official transcripts of any college work. The applicant should request colleges to mail transcripts directly to CCC&TI. If currently enrolled at another college, he/she should request a transcript now and another one at the completion of his courses. Transcript request forms are available in Student Services or may be downloaded from the college webpage, College transcripts must be received and evaluated in order to exempt the applicant from placement tests. 4. Placement tests. Placement tests are given at the Testing Center at CCC&TI in Hudson or at the Watauga Campus. Please contact the Testing Centers directly for testing appointments. Any applicant whose placement test scores fall below the required minimum for the OMA program must take the indicated developmental course(s). An applicant must complete all required developmental courses before the OMA Consideration Date will be given. Minimum acceptable North Carolina Diagnostic Assessment and Placement (NCDAP) scores for the OMA program and the corresponding exempted developmental courses: NCDAP Exempted Developmental Courses English/Reading Assessment DRE 096; DRE 097; DRE 098 Math Assessment DMA 010; DMA 020; DMA 030; DMA; 040, & DMA 050 Please see the Testing Center for comparable ASSET or COMPASS scores. Official test scores for these tests must be provided to the Testing Center if testing was done at

3 another college on or after 2/1/07. ASSET and COMPASS scores prior to 2/1/07 will not be accepted. Placement tests may be waived for persons who provide official documentation of transferable college credit, SAT, or ACT scores that shows proficiency in reading, English and math. SAT and ACT Guidelines: Students may be exempt from placement testing and from taking developmental courses based on the following ACT or SAT scores: English: ACT Reading 20 OR ACT English 18 SAT Writing 500 OR SAT Critical Reading 500 Math: ACT Math 22 OR SAT Math Information session/interview with program faculty. Attend an Ophthalmic Medical Assistant Information Session to receive current details regarding admission, academic, and program requirements. Please contact the OMA faculty at , ext to sign up for the next session. The Information Session Confirmation Form must be completed and returned to Health Sciences Admissions for completion of this admission requirement. A required criminal background check acknowledgment statement must be signed by the applicant during the information session/interview for the requirement to be considered completed. THE OMA CONSIDERATION DATE IS GIVEN WHEN REQUIREMENTS 1 5 ARE MET. NOTIFICATION PROCEDURE New students will be conditionally admitted to the Ophthalmic Medical Assistant Program in December (in preparation to begin the ophthalmic medical assistant program the following fall) and subsequent semesters according to eligibility. Once student seats have been filled, students by order of consideration date may be considered if an accepted student declines or is disqualified. All ophthalmic medical assistant applicants will receive a notification letter of his/her admission status in December each year. Applicants must respond to this letter either in writing, , or phone message, to keep their application active for the next admission period. All applications will be withdrawn from the system if a response is not received by the due date specified in the notification letter. An abbreviated re-application form may be obtained from Student Services.

4 AFTER ACCEPTANCE: 1. Physical examination. An accurate and satisfactory completed student medical form (health form) which includes appropriate documentation of immunizations by the health care provider or county health department and physical exam results must be submitted to Certified Background Check Medical Document Management by the deadline stated in the acceptance letter. The physical examination form may be downloaded from the Certified Background web site when the order is placed. Each student will be responsible for the cost of the electronic health form tracking. Requirements include current TB (1 step) skin test (within 12 months), mumps/measles/rubella blood titer or vaccine, proof of chicken pox immunity by vaccination or blood titer, and current immunizations for tetanus and diphtheria. The student is also encouraged to obtain Hepatitis B immunizations. Otherwise, the decision to decline the Hepatitis B immunizations must be formally documented. 2. Infant, Child and Adult CPR certification. One of the following is required: American Heart Association Healthcare Provider course OR American Red Cross Professional Rescuer or Healthcare Provider OR National Safety Council Healthcare Provider. A copy of the CPR certification should be submitted by the deadline established by the program director. 3. Criminal background checks/drug screening/ other expectations. Criminal background checks and/or drug screenings are required for the student to participate in the clinical component of each OPH course. A signed acknowledgement statement must have been submitted to the Health Sciences Admissions office as an admissions requirement (a copy of the form will be given to the applicant to sign during the interview with the program faculty see admissions requirement #7). All costs associated with criminal background checks and drug screenings are the responsibility of the student. The students will assume responsibility for all health care or emergency room costs they might incur during their clinical rotations. The student will assume the responsibility for travel to and from the clinical site and all associated costs. The student will complete a final evaluation of all clinical rotation sites, including faculty and educational experience. 4. Progression Requirements A minimum of 2.0 (grade of C) must be maintained in each of the Ophthalmic Medical Assistant classes AND in ENG 111,PSY 150, MED 121, and COM 120. A grade of D in any of these general education classes requires repeating the class and obtaining a C or above. Registration in each successive OMA course requires a grade of C or better in the preceding OMA course.

5 Please note: Each applicant is responsible for: completing admission requirements, confirming receipt of letters and documents in the admissions office, keeping CCC&TI informed in writing of any personal information changes, keeping copies of all documentation submitted to CCC&TI (i.e., CPR, physical exam). Applications are accepted for only one two-year health sciences program at a time. However, an applicant may apply for the one-year ophthalmic medical assistant diploma while also applying for a two-year health sciences program. Technical Standards for Ophthalmic Medical Assistant Program Functional Ability Standard Examples of Required Activities Observation Visual Hearing Normal or corrected visual acuity sufficient for accurate observation and performance of technician duties. Normal or assisted ability sufficient for assessment of patients See objects up to 20 feet away Read and interpret the electronic medical record and/or associated paper medical records. Read dials on ophthalmic instruments such as, but not limited to: keratometer, phoropter, lensometer, tonometer, biomicroscope Hear normal speaking level sounds Communication Communication Motor Function Gross Motor Skills Oral communication skills sufficient to communicate in English with accuracy, clarity, and efficiency with patients, their families and other members of the health care team, including nonverbal communication, such as interpretations of facial expressions, affect and body language. Gross motor skills sufficient to provide the full range of safe and effective care of the ophthalmic patient Give verbal directions to or follows verbal directions from other members of the healthcare team and participate in health care team discussion of patient care Elicit and record information about health history, current health state and responses to treatment from patients or family members. Convey information to patients and others as necessary to teach, direct and counsel individuals in an accurate, effective and timely manner Move within confined spaces such as a treatment room, examination room, or OR suite Assist patients with impaired mobility with safe transfers
