4 Introduction Congratuations......on the upcoming birth of your baby and thank you for choosing the Cork University Maternity Hospita services. This booket is used during our Preparation for Birth and Parenthood Education programme (aso caed antenata education or parent craft education). Most women attending the programme wi be 30 weeks pregnant or more, therefore we mainy incude information that is reevant to you from this point onwards in your pregnancy. We wi use this booket as a guide during this programme and you wi be abe to go over the information and skis you have earned at home. It contains information that other parents have found usefu. If you have further questions you can ask the midwives who present the Preparation for Birth and Parenthood Education programme. This programme is very informa and your birth partner is wecome to attend with you. It is run by midwives to give you information on birth and parenthood; to answer any questions you may have and to correct any misinformation. The booket wi give you information and sources of information that are avaiabe to you. It gives you a favour of many of the topics you wi be interested in whie pregnant and just after giving birth but is by no means an exhaustive source. You wi get more information and hep as you tak to other mothers, heathcare professionas and on-ine. Before the birth of your baby Preparation for Birth and Parenthood Education Programme Cork University Maternity Hospita offers a Preparation for Birth and Parenthood Education programme to a our expectant parents. We wi give you information and teach you skis that wi prepare you reaisticay for abour, birth and becoming a parent. The programme runs in the mornings and afternoons from Monday to Friday between 9.30am and 4.30pm. You can book a pace on the programme by teephoning (021) between 10am and 12 noon from Monday to Friday. The Preparation for Birth and Parenthood Education programme is run for the foowing groups: first time parents; teenagers and young aduts; parents expecting mutipe births - twins, tripets, etc; and parents expecting a second or subsequent baby who need a refresher education session. We aso hod individua sessions for parents with specific needs. Beow is a sampe timetabe of the Preparation for Birth and Parenthood Education Programme. 7 Having a Heathy Pregnancy Heathy Eating During pregnancy your unborn baby gets a its nutrients from you. Heathy food choices before, during and after pregnancy can hep you stay heathy and we. This wi aso hep your baby grow heathy and strong. Heathy eating during pregnancy may aso protect your baby against diseases in ater ife. 8 acoho There is no known safe eve of acoho use in pregnancy so stopping competey is best. Further information is avaiabe on: Taking Medicine Research shows that over-the-counter medicines you use in pregnancy can infuence your chid ater in ife. Even medicines that don t seem to do you any harm may be damaging to your baby. Take as few medicines as possibe, and ony where the benefit from taking them outweighs the risk. Aways te any doctor, dentist, midwife or pharmacist that you are pregnant. Remember to seek advice from your heath professiona before taking any medication or aternative therapies in pregnancy. Further information is avaiabe on: Smoking Stopping smoking is the singe most important thing you can do to protect your heath. If you are pregnant, giving up wi hep protect your baby s heath too. Cork University Hospita is a smoke-free campus which means that smoking is not permitted anywhere on the grounds incuding a the areas around Cork University Maternity Hospita. This incudes doorways, wakways, car parks, interna roads and bike sheds. It may be hepfu to contact your GP in advance of your stay at Cork University Maternity Hospita for advice on smoking cessation support options or nicotine repacement therapy. 9 Depression during pregnancy Most women have mood swings during pregnancy. This is perfecty norma. However, if you are constanty feeing down, angry or anxious, this may be more than a temporary bip and might need more carefu attention. If you fee that you are sipping into depression, tak to your GP or midwife at the antenata cinics. If you aready have menta heath probems, you are more ikey to become i again during pregnancy, or in the first year after giving birth, than at other times in your ife. Sometimes peope who have a menta heath probem stop taking their medication. If you do this without taking to your GP or midwife when you become pregnant, this can make your iness return or get worse, so it's important to tak to them first. From the minute you know you are pregnant, your feeings change: feeings about yoursef, about your baby and about your future. Your reationship changes: with your partner, other chidren and aso with your parents and friends. Deaing with these changes is not aways easy. If you are feeing concerned or anxious - for whatever reason - tak to your midwife or doctor as soon as possibe. Further information is avaiabe on: and see P, postnata depression. Sexua heath Sexuay transmitted infections (STIs) often have no symptoms, so you may not know if you have one. However, many STIs can affect your baby's heath during pregnancy and after the birth. If you have any reason to beieve that you or your partner may have an STI, go for a check-up as soon as possibe. You can ask your GP or midwife, or, if you prefer, go to a genitourinary medicine (GUM) cinic or sexua heath cinic. Further information is avaiabe on: page 5

10 Sex during pregnancy You can have sex during pregnancy. Your partner's penis can't penetrate beyond your vagina, and the baby cannot te what's going on. It is norma for your sex drive to change during pregnancy though. Don't worry about this, but do tak about it with your partner. Your midwife or doctor wi probaby advise you to avoid sex if you've had any heavy beeding in pregnancy, since sex may increase the risk of further beeding if the pacenta is ow. We wi advise you to avoid sex if your waters have broken (rupture of membranes) as this can increase the risk of infection. If you're unsure, ask your midwife or doctor Further information is avaiabe on: and search sex in pregnancy. activity and exercise The more active and fit you are during pregnancy, the easier it wi be for you to adapt to your changing shape and weight gain. It wi aso hep you to dea with abour and get back into shape after the birth. Keep up your norma daiy physica activity or exercise (waking, piates, swimming, yoga or even just waking to the shops and back) for as ong as you fee comfortabe. Exercise is not dangerous for your baby there is some evidence that active women are ess ikey to experience probems in ater pregnancy and abour. Expecting more than one baby - mutipe pregnancy Casses are avaiabe at CUMH, taiored specificay for women/coupes expecting twins, tripets or more. 11 Teenage Parents Becoming a mum or dad is a pretty big dea, but being a teen mum or dad can be even more of a chaenge. From the time you find out that there is a baby on the way, there are many things you might want to find out. As part of our Preparation for Birth and Parenthood Education programme, CUMH runs a specia information session for teenagers and young aduts expecting a baby. You can aso find ots of information to hep you become the best mum or dad you can be on the website: Domestic Vioence Domestic vioence can be described as The use of physica or emotiona force or threat of physica force, incuding sexua vioence, in cose adut reationships the term domestic vioence goes beyond actua physica vioence. It can aso invove emotiona abuse, the destruction of property, isoation from friends, famiy and other potentia sources of support, threats to others incuding chidren, staking, and contro over access to money, persona items, food, transportation and the teephone. Pregnancy does not offer protection from domestic vioence. In fact, internationa research has found that 30% of women who experience domestic vioence are physicay assauted for the first time during pregnancy. The Rotunda Hospita, Dubin conducted research which found that one in eight women surveyed were being abused during their current pregnancy. It is recognised that the birth of a baby can be a stressfu time. Socia workers based at Cork University Maternity Hospita provide a counseing and support service for expectant parents. They work cosey with other professionas both within the hospita and with those based in the community. If you wish to speak to a socia worker, pease ask your midwife or doctor who wi make contact with the department on your behaf. 12 Covering the costs of pregnancy and birth The costs of hospita and GP care can be covered for you if you are attending Cork University Maternity Hospita as a pubic patient. This is provided by the Maternity and Infant Care Scheme. If you are attending as a private patient, check with your heath insurance company and with your consutant obstetrician for charges. Trave during pregnancy Car Trave: Pace the seat bet over your shouder and chest (between your breasts) without impinging on your abdomen. The ap strap shoud ie across your upper thighs. Neither strap shoud go over your bump. air Trave: Long-hau air trave is associated with an increased risk of venous thrombosis. Discuss fying, vaccinations and trave insurance with your doctor before you make your pans to trave abroad. For more information see: pregnancy care and ifestye on DOMINO Our combined midwifery and GP maternity service CUMH provides a DOMINO (Domiciiary care In and Out of hospita) service whereby a your care before, during and after your birth wi be shared by midwives who are members of the DOMINO team and your GP. You wi see a DOMINO midwife during your antenata appointments at the hospita. You wi meet the entire DOMINO midwifery team during your pregnancy so that when it comes to giving birth you wi be ooked after by a midwife that you have aready met. You wi have your baby in hospita and you can go home six hours after the birth, if a is we. The DOMINO midwife wi visit you and your baby at home up to five days after giving birth. You wi not need to see a hospita doctor during your pregnancy uness a compication occurs. 13 Some discomforts and conditions that may arise during pregnancy Some women may experience discomforts or iness during pregnancy. Pease consut your midwife or doctor to discuss any concerns that you may have. Beow is a ist of various discomforts and inesses that may come up during your pregnancy. These incude constipation, pies, heartburn and indigestion, varicose veins, stretch marks, thrush, vagina discharge and incontinence. For more information see: pregnancy care on Irish Maternity Eary Warning System (IMEWS) CUMH has an eary warning system in pace to pick up any change in your condition. This system is used aong with cinica assessment to detect any change in your condition to improve the decision making about the care you may need if you are i during pregnancy or in the first 42 days foowing the birth of your baby/babies. Urinary Incontinence Urinary symptoms such as frequency and urgency can be common in eary pregnancy, up to 18 weeks and after 30 weeks. This is due to pressure from the uterus in the pevis. Urinary incontinence on exertion is common as your pevic foor musces reax from the hormona changes of pregnancy and increased pevic pressure. A pevic foor musce exercise programme, avoiding excessive weight gain, constipation and heavy ifting wi hep. If the probem has existed since your ast pregnancy, is severe, or you have other badder probems outside of pregnancy pease contact the Continence Advisory Service in Cork University Maternity Hospita at page 9

14 Pregnancy Reated Pevic Girde Pain Pregnancy-reated Pevic Girde Pain (PGP) or Symphysis Pubis Dysfunction (SPD) is a common condition in pregnancy. Between 50 and 70% of pregnant women experience pain in the pevic joints or ower back during pregnancy. Women experience different symptoms and these are more severe in some women than others. If you understand how PGP may be caused, and how you can hep yoursef, this may essen the impact of PGP on your ife. The symptoms of PGP are often fet over the pubic bone at the front, beow your tummy or across one or both sides of your ower back or buttock. Symptoms You may have difficuty: Waking; Taking weight on one eg at a time e.g. cimbing stairs, dressing or getting into or out of the bath; Parting your egs for exampe getting in and out of the car; With hip movements for exampe turning in bed; Lying on your back or side; or With norma movement activities of daiy ife. Causes of Pevic Girde Pain It may be a combination of factors for exampe the igaments around your pevis can have increased axity due to your pregnancy hormones. This can ead to increased or uneven movement in your pevis. This extra movement in itsef can cause pain. As your baby grows, your posture changes. This eads to a change in the activity of the musces of your tummy, pevic girde, hips and pevic foor. This can increase the strain on your ower back and pevis. Due to increased movement in the joints of your pevis and the change in musce activity this can cause ma-aignment of the pevic girde joints. This can be a source of pain and discomfort. If you have a history of back pain your symptoms may worsen during pregnancy. Occasionay women experience pain due to the position of their baby. page 10

15 Genera tips for preventing or managing Pevic Girde Pain Standing Changes in your posture occur as you carry the weight of your baby at the front. This changes the position of your centre of gravity. Sometimes, the curve in the sma of your back increases and your shouders become more rounded. This can pace extra strain on your ower back or your pevis.your tummy musces can hep ift and support the weight of your baby. This can reduce the strain on your back and pevis. To fee these musces working, pace your hands on your ower tummy, beow your bey button. Breathe in normay and as you breathe out genty pu your tummy musces in, away from your hands, a very sma amount. Hod your tummy musces in genty as you continue to breathe. Practice tightening your ower tummy musces when you wak and stand. This wi ease the strain on your ower back and pevis. Aways maintain equa weight through both feet. Avoid putting weight onto one eg ony. Wear comfortabe fat supportive shoes. Rest when you can - you may need to rest and sit more often. Sit to get dressed and undressed. activities of daiy iving Be as active as possibe within your pain imits and avoid activities that make the pain worse. Take the stairs one step at a time. When going upstairs try eading with the ess painfu eg. Going downstairs, try eading with the more painfu eg and use the banister for support. Try to avoid ifting heavy things or doing heavy housework such as vacuuming. When ifting bend your knees and keep your back straight rather than stooping over. You may find it easier to sit when doing some house work such as ironing or preparing vegetabes. Take care when pushing a heavy shopping troey. Avoid turning quicky as this can strain your pevis. Instead move sowy as you turn. Ask for or accept hep with your daiy work. page 11

16 If you have todders or sma chidren Avoid ifting them as much as you can. Knee down to comfort them or sit and et them cimb up to you instead of ifting. When ifting a sma chid bend your knees and keep your back straight. Let them cimb into the car seat themseves if they can. Avoid bending or stooping as this paces strain on your ower back and pevis. Knee down or pace them at a height where you can stand or sit when caring for them. Sitting Sitting, souching or sitting very straight can strain your back and pevis. Aim for hafway between these two positions. Pacing a sma support such as a cushion or roed up towe at your ower back can hep to avoid souching. Sit we back into the chair and take its fu support. Don t perch on the edge as your musces wi tire more quicky. If using a computer bring your chair under the desk or the keyboard coser so your arms can rest by your side as you type. Adjust the monitor so it is at eye eve. When driving, adjust your seat so that your arms can rest by your side as you turn the whee. This heps prevent souching. When getting in and out of the car, move your egs in and out in stages rather than one big movement. page 12

17 Seeping You may find ying on your side with a piow between your knees comfortabe. As your pregnancy progresses, pacing an additiona piow under your bump may prevent strain on your hips and ow back. To turn in bed easiy, bend your knees up, pu in your tummy musces and keep your knees together. Turn, moving your shouder and hip together in one movement. If you find your hips are sore or aching this may be because your mattress is very firm. Doubing over a singe duvet and pacing it underneath you, can hep to reieve this. To get out of bed, ro onto your side and drop your egs over the edge of the bed. Use your ebow and hands to push your body into a sitting position. When you want to get in to bed reverse this procedure. During Labour The majority of women with PGP cope very we with a vagina birth. 18 Pregnancy reated Carpa Tunne Syndrome What is the carpa tunne? The carpa tunne is a narrow passage in your wrist made up of sma bones and a tough band of tissue. Severa tendons and your median nerve run through the tunne to suppy movement and sensation to your fingers. What is Carpa Tunne Syndrome (CTS)? CTS is a condition where the median nerve in the hand becomes compressed at the wrist. Symptoms of CTS occur when the median nerve becomes pinched by pressure within the tunne. What causes CTS? Fuid retention in pregnancy increases the pressure in your carpa tunne. Symptoms are made worse if your wrist is bent for ong periods of time e.g. proonged use of a computer keyboard. If you had simiar, mider symptoms prior to pregnancy there may be other causes for this. Pease discuss this with your GP or physiotherapist. What are the symptoms of CTS? Tinging or numbness in a part of your hand (thumb, index, midde or ring fingers); Sharp pains that shoot from your hand or wrist up aong the arm; Burning sensations in your fingers; Numbness in your hand whie driving; Morning stiffness or cramping of your hands; Thumb weakness; Frequenty dropping objects; and Waking at night with hand pain and numbness. Your symptoms wi tend to be worse at night or first thing in the morning. This is because you are unaware of what position your hands are in when you are aseep. They may have been bent for a period of time causing pressure on the nerve. We wi give you a spint to wear at night to prevent you from bending your wrist and compressing the nerve. page 14

19 What can I do to hep improve the symptoms of CTS? Remember PRIME (Positioning, Rest, Ice, Movement, Eevation). Positioning - Avoid bending your wrists for proonged periods of time e.g. typing at a keyboard. When doing any activities with your hands, such as typing, try and keep your hands in a supported neutra position. Wear your spint particuary at night. Change the position of your hands reguary. Rest Minimise any activities that make your symptoms worse. Take reguar work breaks if you are doing a manua task. Ice When your symptoms fare up, cooing your wrists can hep reieve your symptoms. Run your wrists under the cod tap. Use a cod compress, such as an ice pack or ice wrapped in a towe. Do not appy ice directy to your skin as it can cause an ice burn. Movement Gente exercises wi hep to contro the symptoms. Shaking your hands when they are numb or tinging may hep. Eevation Where possibe, try to eevate your wrists/arms on piows, to hep reduce sweing. This may hep in the evenings before going to bed. Exercises for CTS Range-of-motion exercise: Cench fist tighty for 5 seconds, then reease, straightening your fingers a the way and keep them straight for 5 seconds. Repeat 5 times for each hand. Stretching exercise: Using one hand, bend the fingers and wrist of the other hand back towards you genty. Ony stretch it back as far as comfort aows. Hod for 5 seconds. Repeat 5 times for each hand. page 15

20 Wi my CTS improve after my baby is born? CTS usuay resoves after your baby is born. For some women it can happen immediatey, for others it can take a few weeks. Bring your spints to the hospita as your symptoms may be a itte worse in the first few days after your baby is born. This is due to extra retained fuid or sweing. Your symptoms shoud be gone competey by your six week check with your GP. If not, discuss this with your GP. Common vira inesses in pregnancy Chicken Pox Chickenpox is a mid and common chidhood iness that most chidren and sometimes aduts catch at some point. It causes a rash of red, itchy spots that turn into fuid-fied bisters. They then crust over to form scabs, which eventuay drop off. Chickenpox during pregnancy can cause compications, both for the pregnant woman and the unborn baby. However, the actua risk of any compications occurring is ow, occurring in approximatey three in every 1,000 pregnancies. Most pregnant women who get chickenpox recover, with no adverse effects on the baby. Seek advice from your GP or midwife immediatey if you re pregnant and: you think you may have chickenpox; either you've never had chickenpox or you re not sure, and you've been near someone that has it (even if you have no rash or other symptoms); or you get chicken pox within seven days of giving birth. page 16

21 Whooping cough Whooping cough (medicay known as pertussis) is a serious infection that causes ong bouts of coughing and choking, making it hard to breathe. Getting vaccinated whie you re pregnant may hep to protect your baby from deveoping whooping cough in the first few weeks of their ife. The immunity you get from the vaccine wi protect you and wi pass to your baby through the pacenta and provide passive protection for them unti they are od enough to be routiney vaccinated against whooping cough. The antibodies you pass to your baby in the womb decine rapidy in the first six months of ife so it is important your baby gets the routine chidhood vaccines (which incude whooping cough vaccine) on time at two, four and six months. Is it safe for pregnant women to be vaccinated for whooping cough? Yes. The vaccine is safe for pregnant women. The Nationa Immunisation Office has recommended this vaccine for pregnant women in Ireand. This whooping cough vaccine has been studied in pregnant women in the US and no risks have been found. 22 Section 2. Preparing to give Birth Preparing a Birth Pan A birth pan is a written pan that heps you to think about options you may ike to expore around your abour and birth.
