1 No. 137-AR HATBORO-HORSHAM SCHOOL DISTRICT HOME TEACHING (HOME SCHOOLING) - ELEMENTARY AND SECONDARY 1. Each home education program will-be supervised by the student s parent or guardian or such person having legal custody of the child. 2. The home education supervisor will file a notarized affidavit with the school district Superintendent by August 1 of each school year which includes: a. The name of the supervisor and a statement indicating that such person possesses a high school diploma or equivalent. b. The name and age of each child in the program. C. The address and telephone number of the program site. d. An outline of the proposed objectives for each of the required subjects. e. A statement indicating that the required subjects will be taught in the English language. f. Evidence of the child s proper immunization and a statement that the child will receive health services required for public school students. g. A statement noting that all adults in the home and persons having legal custody have not been convicted, within five years of the affidavit, of criminal offenses which disqualify persons for employment in public and non-public schools. 3. At the elementary level, the following courses shall be taught. a. English b. Spelling C. Reading d. Writing e. Arithmetic f. Science g. Geography h. History of the United States and Pennsylvania i. Civics j. Safety Education, including regular and continuous instruction in the dangers and prevention of fires. k. Health and physiology 1. Physical Education M. Music n. Art 4. At the secondary level, the following courses shall be taught.
2 137-AR. HOME TEACHING (HOME SCHOOLING) - ELEMENTARY AND SECONDARY - Pg. 2 a. English, to include language, literature, speech, and composition. b. Science C. Geography d. Social Studies, to include civics, world history, history of the United States, and Pennsylvania. e. Mathematics, to include general mathematics, algebra and geometry. f. Art g. Music h. Physical education i. Health and safety education, including regular and continuous instruction in the dangers and prevention of fires. 5. Instruction must include a minimum of 180 days of instruction or 900 or 990 hours of instruction at the elementary and secondary levels respectively. 6. In order to qualify for a Hatboro-Horsham diploma, a home educated pupil must meet the established Hatboro-Horsham School District graduation requirements. 7. The supervisor must submit required documentation at the end of the school year, or may be required to submit documentation during the school year which includes: a. A portfolio of records and materials. b. A log listing titles of reading materials. C. Samples of writing, worksheets and workbooks or creative materials used or developed by the students. d. Achievement test results in reading/language arts and mathematics in grades 3, 5, and B. The supervisor may select the test from a Pennsylvania Department of Education approved listing of seven nationally normed standardized achievement tests or may select a statewide administered test such as TELS. 8. At least annually, the supervisor must arrange for a written evaluation of the student s educational progress which shall be based on an interview with the child and a review of the portfolio. The evaluation shall certify whether or not an appropriate education is occurring and shall be submitted to the district Superintendent by July 1 of each school year. The supervisor has the following options in selecting the evaluator but in no event shall the evaluator be the supervisor, his/her spouse or the student s guardian. a. A licensed clinical or school psychologist. b. A teacher certified by the Commonwealth of Pennsylvania who possesses appropriate experience in evaluating student performance.

A nonpublic school teacher or administrator with at least two years experience within the past ten years in a Pennsylvania public or nonpublic school who possesses appropriate experience in evaluating student performance. 9. Special education children may participate in a home education program if the program meets the child s specific needs and is approved by a certificated special education teacher or a licensed clinical or certified school psychologist. Written notification of this approval must be submitted with the notarized affidavit. The supervisor of the home education program can request the school district or intermediate unit to provide required services for the specific needs of a special education child. Upon agreement to provide the services, they shall be provided in the public schools or in a licensed private school. 10. If the Superintendent has a reasonable belief that at any time during the school year appropriate education is not occurring, s/he may request that the portfolio be submitted within fifteen days (except for achievement test results) and that the evaluation be submitted within thirty days. 11. When a home education program site is relocating to another school district within this Commonwealth, the supervisor of the home education program must apply, by registered mail, 30 days prior to the relocation, to the Superintendent of the district in which s/he currently resides, requesting a letter of transfer for the home education program to the district to which the home education program is relocating. The current Superintendent of residence must issue the letter of transfer no later than 30 days after receipt of the registered mail request of the home education is not in compliance with Section , the Superintendent of the current district of residence must inform the home education supervisor and the Superintendent of the district to which the home education program is relocating, the status of the home education program and the reason for the denial of the letter of transfer. If the home education program is in hearing procedures, the Superintendent of the current district of residence must inform the home education supervisor, the assigned hearing examiner and the Superintendent of the district to which the education program and the reason for the denial of the letter of transfer. The letter of transfer must be filed by the supervisor of the home education program with the Superintendent of the new district of residence. In the case of pending proceedings, the new district of residence Superintendent shall continue the home education program until the appeal process is complete.

The program will be conducted at (address) The phone number at this site is (phone number) 2. I attest that the Home Education Program will be in compliance with the Public School Code. 3. I attest that the subjects listed in paragraph 4 below will be offered in the English language for a minimum of one hundred and eighty days of instruction or a minimum of 900 hours. 4. I attest that the following courses shall be taught: English, to include spelling, reading, and writing; arithmetic; science, geography, history of the United States and Pennsylvania; civics; safety education, including regular and continuous instruction in the dangers and prevention of fires; health and physiology; physical education; music; and art. 5. I attest that the education objectives in the home schooling program are by subject area as follows: (attach objectives). 6. I attest that has been immunized against the following diseases and I have attached evidence thereof a. Diphtheria b. Tetanus C. Poliomyelitis d. Measles (Rubeola) e. German Measles (Rubella) f. Mumps 7. I attest that has received the health medical services required by Article XIV of the Public School Code. Article XIV requires that every child of school age be given by methods established by the state s Advisory Health Board, an annual vision test, a hearing test, a measurement of height and weight, tests for tuberculosis under medical supervision and other tests required by the Advisory Health Board.
5 SECONDARY - Pg. 5 Children upon entry into school and in the sixth and eleventh grades must have a medical examination and comprehensive appraisal of health by a physician. Children upon entry into school and in the third and seventh grades must have a dental examination by a dentist. A comprehensive health record shall be maintained for each child.
