1 WOOD COUNTY SCHOOL OF PRACTICAL NURSING FEDERAL STUDENT FINANCIAL AID PROCEDURE AND POLICY HANDBOOK

2 The Wood County School of Practical Nursing program has met the requirements necessary to participate in: PELL Grant Program, Vocational Rehabilitation Funding, Veterans and Worker s Compensation, HEAPS, and Workforce Investment Association. To maintain eligibility for PELL Grants disbursements students are required to maintain both satisfactory academic progress and satisfactory attendance in the program. Grading Policy: (Excerpts from Student Handbook - last revised/reviewed 05/14) The 12 month program is presented in a sequential manner from simple to complex and is divided into four nursing quarters: Nursing I, Nursing II, Nursing III, and Nursing IV. The student must satisfactorily complete the current nursing quarter in order to progress and remain in the program. A minimum "C" grade (80%) in each course of study and a "Satisfactory" clinical grade is required to remain in the program. Clinical grades are either "Satisfactory" or "Unsatisfactory" and are based on specific clinical objectives that must be met for each Nursing Quarter. A failing course or clinical grade will result in dismissal from the program. Students must pass on the first attempt; repeating a course grade or clinical grade is not permitted. Students must satisfactorily complete all assignments including make-up work. Students will receive a partial transcript at the completion of each quarter (Nursing I, Nursing II, and Nursing III) that will include course grades, clinical grades, and clock hours completed. A complete transcript will be issued upon completion of graduation requirements and payment of all fees. Attendance Policy: (Excerpts from Student Handbook - last revised/reviewed 05/14) Students will be terminated when absences exceed 48 hours and/or when the number of tardies exceed 7. Students may only miss one (1) clinical day in a specialty rotation. An absence due to court will be excused providing the student can produce a summons or subpoena and gives advance notice of the expected absence. A student will be excused for 3 bereavement days for a death in the immediate family. The student is responsible for obtaining class or clinical information that was given during an absence and to complete make-up work and tests in a timely manner. Students must complete the program in 12 months unless granted a leave of absence. Leave of absences are discouraged but may be approved for extenuating circumstances on an individual basis. Readmission must occur within one calendar year with completion of the program in 24 months from admission. Students receiving Title IV Financial Aid will pay all school fees from their PELL Awards and/or other awards or scholarships upon receipt of the money at the School. Self - pay students are required to pay half of all tuition and fees as well as the cost of books prior to or no later than the first day of class. The second half of tuition and fees are due by the last day of October of the same year. In the event that a student is unable to pay as stated above or financial aid does not cover the total cost of the class, the student can request a payment plan. The payment plan is set up as follows: One third of tuition and fees as well as the cost of books are due prior to or no later than the first day of class. The remainder balance will be paid in six equal monthly payments. We do not accept credit cards. Payment must be made with cash, check or money order. If the student

3 fails to pay, they will be exited from the program and billed for the balance due. The U.S. Department of Education states that the responsibility for the cost of an education lies primarily with the student and/or spouse and/or parents. Therefore, the charges due the School are collected first with the remainder (if any) being disbursed to the student to help defray cost of living expenses. All fees due the School are collected from the first financial aid disbursement received at the School regardless of the source. Since all students in the Wood County School of Practical Nursing are adults, the School does not offer the option of budgeting cost of living money for the student. Any money remaining after the deduction of School tuition and fees will be disbursed to the student. The student is then responsible for budgeting their funds so that the cost of schooling and related fees (class dues, licensing fees, transportation costs, etc.) is readily available when such costs arise. Withdrawal From Program A student wishing to withdraw must make an appointment with the Coordinator. All library books must be returned and all business arrangements completed. Unless the student follows this procedure, he/she may not be eligible for readmission to the School, nor will they receive a partial transcript. An administrative fee of $ will be collected from any student withdrawing from the Program. This fee will be retained by the School from any tuition refund due the student or any agency from which financial aid was received. Students completing the Program do not have to pay this fee. The Wood County School of Practical Nursing requires any Federal Student Financial Aid recipient to make an appointment with the Financial Aid Administrator to discuss how the school's Refund Policy and the Federal Government Return of Funds Formula will affect their financial aid account. Students are advised that the school's refund policy is a separate calculation - a student may, upon their withdrawal from school, owe the Federal Government a repayment of Federal Aid received, and also owe the school for tuition and fees. Federal Student Aid which are considered to be "unearned" by the Return of Funds formula will be returned to the Pell Grant first. The school will make any required refund to aid programs within 45 days of the student's official withdrawal date. Should the Return of Funds formula result in a refund due to the student for tuition and fees paid, the amount to be refunded to the student will be applied to any student liability to the Federal Student Aid Programs first. Should a student find themselves owing a repayment of "unearned" Federal Pell Grant, the school will notify the student of the amount; the student will be given 30 days to make such repayment to the school's Federal Aid Account, and after that point, begin preparation to refer the account to the U.S. Department of Education for collection. Unpaid amounts of "unearned" Pell Grant are called "overpayments" - a student with an overpayment of Pell Grant will not be eligible for future Federal Student Aid until such time as the student repays, or makes arrangements to repay, the amount due. Students leaving the Program prior to graduation will not receive any Title IV funds due them for cost of living expenses. This money will be returned to the PELL Grant Account or lending institution. Federal Refund Policy for students receiving Financial Aid (Policy Review 4/15) Refunds of tuition and fees will be processed in accordance with the Federal guidelines governing the return of Title IV funds. The Financial Aid Office is required to recalculate federal financial aid eligibility for students who withdraw, drop out, are dismissed, or take a leave of

4 absence prior to completing 60% of a payment period or term. The following formula is used: Percentage of payment period or term completed = the number of days completed up to the withdrawal date divided by the total days in the payment period or term. (Any break of five days or more is not counted as part of the days in the term.) This percentage is also the percentage of earned aid. Funds are returned to the appropriate federal program based on the percentage of unearned aid using the following formula: Aid to be returned = (100% of the aid that could be disbursed minus the percentage of earned aid) multiplied by the total amount of aid that could have been disbursed during the payment period or term. If a student earned less aid than was disbursed, the institution would be required to return a portion of the funds and the student would be required to return a portion of the funds. Keep in mind that when Title IV funds are returned, the student borrower may owe a debit balance to the institution. If a student earned more aid than was disbursed to him/her, the institution would owe the student a postwithdrawal disbursement which must be paid within 45 days of the student s withdrawal. Refunds are allocated in the following order: Federal PELL Grants for which a return of funds is required Federal Supplemental Opportunity Grants for which a return of funds is required Other assistance under this Title for which a return of funds is required (e.g., HEAP). The Refund Policy for the Wood County School of Practical Nursing will also be enforced in addition to all Federal regulations. Wood County School of Practical Nursing Refund Policy (Policy Review 04/15) Refunds of tuition will be processed as follows: Start through 5 days of Course = the School will retain 10% of first half Over 5 days to 10% of Course = the School will retain 25% of 1 st half 10.01% to 25% of Course = the School will retain 50% of 1 st half tuition Over 25% of Course = there will be no refund of tuition. The following fees are non-refundable: preentrance testing fee, book and supply fees, printed course material fees, standardized examination fees, drug screening, user fee, and graduation fee. An Administrative Fee of $ will be deducted from refund due under either of these refund policies and retained by the School. If tuition and fees are collected in advance of the start date of a program and the institution cancels the class, 100% of the tuition and fees collected will be refunded within 45 days of planned start date. If tuition and fees are collected in advance of the start date of class and the student does not begin classes or withdraws on the first day of classes, the $100 seating fee will be nonrefundable. A student who has not visited the school facility prior to enrollment will have the opportunity to withdraw without penalty within three days following a regularly scheduled orientation. Refunds of any fees paid will be made within 45 days of planned start date. *Note: All refunds due to students will be made without requiring a request from the student. Verification Policy Verification is performed in accordance with title IV regulations and must be completed

5 prior to aid disbursal. Student will be given a letter explaining verification required if any. Once all processing and verification is complete, the student will be notified in writing of the amount(s) and type(s) of financial aid they are eligible to receive. All students are expected to pay tuition and/or fees due the school prior to each enrollment period unless financial aid processing and verification procedures are complete and arrangements for credit have been made.
