1 Government of Western Australia Department of Commerce Renewal of registration Building surveying contractor (individual) Form 63 Use of this form This form is to be used by building surveyors who are currently registered as practitioners and who are registered as contractors with the Building Services Board and are applying for their contractor registrations to be renewed. If you decide that you no longer require contractor registration, please advise the Board. Combined practitioner/contractor, practitioner, partnership and company registration renewals cannot be made on this form. Please contact the Building Commission for the correct form. Registration as a building surveying practitioner In Western Australia a building surveyor who wishes to act as a nominated supervisor for a registered building surveying contractor or wishes to use a prescribed title such as registered building surveying practitioner must be registered as a building surveying practitioner. Registration as a building surveying practitioner does not authorise a person to provide services as a contractor. Registration as a building surveying contractor A person, partnership or company must be registered as a building surveying contractor to: contract with others to provide building surveying services; issue compliance certificates; or use a prescribed title such as registered building surveying contractor. Change of recorded details Inform the Board if any information shown on the renewal form is incorrect or out of date. Mark corrections on this form. Financial capacity Building surveying contractors must inform the Board if they have been an insolvent in the last three years. Duration of registration The Building Services Board offers you a choice of two renewal periods of either 18 months or three years. Registration renewal fee The registration renewal fee is: $ for a period of 18 months; or $ for a period of three years. GST is not payable on this fee. If the Board refuses your application the registration fee will be refunded. Registration expiry You must make your application to renew registration no later than the expiry date shown on your registration certificate. Your application should be complete including attachments and payment. You are encouraged to lodge your application well before the expiry date to avoid your registration expiring. If you do not have the required attachments, you should still lodge your application form and make payment by the expiry date. Please provide all outstanding information within 28 days of lodging this form. Your application will be finalised after receipt of the outstanding information. If you do not provide outstanding information in 28 days, the Board may not consider your application and your registration will expire. Late applications Avoid making a late application. If you do not lodge your application by the expiry date, you can still lodge a late application but it may not be accepted by the Board. A late application will incur a late fee and will require additional processing steps. For assistance call the Building Commission on before making a late application. A Form 63 Page 1

Yes / / No PROFESSIONAL INDEMNITY INSURANCE Building surveying contractors are required to have professional indemnity insurance with a minimum level of indemnity of $1,000,000 in any one claim and $2,000,000 in aggregate in any one period of insurance. Please state your insurance cover below. Sections 33(1) and (2) of the Building Services (Registration) Act 2011 require a registered building service provider to give the Board written notice of any change in circumstances that affect eligibility to remain registered. A change in the contractor s insurance arrangements could constitute such a change in circumstances. The notice to the Board must be given within seven days after the change in circumstances. 5 NOMINATED SUPERVISOR Each building surveying contractor is required to have at least one nominated supervisor who is a registered practitioner. As an individual practitioner/contractor you are required to be registered as a practitioner to perform the supervisory function.
