3 IMPORTANT INFORMATION ABOUT THIS HANDBOOK This handbook summarizes the GE Medicare Benefit Plans available to eligible GE retirees and is intended to provide easy-to-understand descriptions of significant plan provisions. It includes descriptions of the GE Medical Care Plan for Pensioners (MCPP), the GE Pensioners Hospital Indemnity Plan (PHIP), the GE Pensioners Prescription Drug Plan (PPDP) and the GE Medicare Insurance Plan for Part B Benefits (GEMIP). This handbook is intended to serve as the summary plan description for the GE Medicare Benefit Plans, and also includes information concerning GE Medicare Plus, the GE Pensioner Vision Value Option and the Elfun Medical Benefits Plan. While every attempt has been made to make this handbook as accurate as possible, full details of all provisions of each program or plan may not be included. Full details of each program or plan are contained in the official plan documents, which are available to you as described in Section 5.0, "Administrative Information," except for the Elfun Medical Benefits Plan, for which full details are provided in this handbook. If a provision described in this handbook differs from the provisions of an applicable plan document, the plan document prevails. Similarly, any oral or written representations by a Company or GE Elfun employee or agent, or any benefit estimates that you may receive, cannot override, reverse or supplement the provisions of the plan documents. You should understand certain important terms, such as "reasonable, necessary and customary," which are used throughout this handbook and which appear in the "Key Terms" section. This handbook does not create a contract of employment between the Company and any individual. For participants whose benefits arose under a collective bargaining agreement, participation in plans incorporated by reference into such agreement is only to the extent provided through the incorporation. For certain benefits whose specific terms and conditions are derived from a collective bargaining agreement, the terms and conditions of such collective bargaining agreement will govern should a conflict arise between this handbook or any programs or plans and the terms and conditions of such agreement. The Board of Directors of General Electric Company reserves the right to terminate, amend, eliminate or replace the GE Medicare Benefit Plans and any related program, plan or benefit at its discretion and at any time, and the Board of Directors of GE Elfun have reserved the same rights with respect to the Elfun Medical Benefit Plan, to the extent permitted by law. Your participation in any program or plan described in this handbook means that you have authorized your benefits-related data to be processed and transmitted by the Company, GE Elfun, its affiliates and any authorized suppliers anywhere in the world, in accordance with the GE Employment Data Protection Standards. Eligibility for these plans is based on the provisions in effect at the time of your retirement or termination. This handbook is based on the retiree s eligibility only and may not apply to other qualified dependents. 1

If you elect GE Medicare Benefit Plans, you may also enroll in the Elfun Medical Benefits Plan, which complements your coverage under the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan. GE Medicare Part B Premium Reimbursement Option For retirees who participate in Medicare Part B but elect not to participate in any GE Medicare Benefit Plans. You must be enrolled in Medicare Part B. GE Pensioner Vision Value Option Available with participation in all GE Medicare Benefit Plans. MEDICAL COVERAGE OPTIONS AT AGE 65 When you reach age 65, Medicare becomes your primary source of health care coverage. If you're eligible for GE medical coverage once you reach age 65, the GE Pensioner Health Plans provide you with four options: GE Medicare Benefit Plans These plans offer a traditional approach to health care coverage, in which Medicare provides primary health coverage and the GE plans provide supplemental coverage. The plans work together with your Medicare coverage to help you pay some of the health care expenses not paid by Medicare Parts A and B. If you are eligible, you may enroll in any or all of these plans, based on your need for coverage. Elfun Medical Benefits Plan This plan complements coverage under Medicare Part A and Part B, the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan. GE MedicarePlus GE MedicarePlus provides eligible retirees, who live in areas where the plan is available, with a single, comprehensive health benefit plan that includes the coverage you get with Medicare Parts A, B and D, plus additional benefits such as preventive care, dental, vision and hearing benefits. GE Medicare Part B Premium Reimbursement Option You can make an annual election to suspend your coverage under the GE Pensioner Health Plans and receive a monthly reimbursement from GE, equal to 50% of the estimated net cost of the GE Medicare Benefit Plans paid by the Company, up to a maximum of the Medicare Part B monthly premium amount. You must be enrolled in Medicare Part B to elect the GE Medicare Part B Premium Reimbursement Option. 7

10 KEY THINGS TO KNOW ABOUT MEDICARE Medicare is the federal health insurance program for people age 65 and over, as well as for certain people with disabilities. The traditional Medicare benefit consists of three parts: Part A (hospital insurance), Part B (medical insurance) and Part D (prescription drugs). Medicare Part A pays some of the costs of hospitalization and limited skilled nursing home care. It is financed primarily by payroll taxes and is premium-free for nearly all beneficiaries. Medicare Part B primarily covers doctors' fees, most outpatient hospital services and certain related services. Those who enroll pay monthly premiums for Part B coverage. Medicare Part D covers prescription drug costs. Those who enroll pay monthly premiums for Part D coverage. In general, Medicare is available at the beginning of the month you turn age 65, whether you are retired or still working, or after you have been entitled to Social Security disability benefits for two years. ABOUT GE PENSIONER HEALTH PLAN OPTIONS GE Medicare Benefit Plans offer you the choice of two options for health coverage: the GE Medicare Benefit Plans or GE MedicarePlus (where available). You may also elect the GE Medicare Part B Premium Reimbursement Option under the GE Pensioner Health Plans. The GE Medicare Benefit Plans supplement your Medicare (Parts A, B and D) coverage. GE MedicarePlus replaces your coverage under original Medicare and your GE Medicare Benefit Plans with a comprehensive package of benefits through a Medicare Advantage Plan (formerly called "Medicare+Choice" and also known as "Medicare Part C") selected and qualified by GE. With both the GE Medicare Benefit Plans and GE MedicarePlus, you continue to pay your Medicare premiums to participate. With either option, you are eligible for the GE Pensioner Vision Value Option. If you elect the GE Medicare Part B Premium Reimbursement Option, you will be eligible for a monthly reimbursement from the Company. You continue to pay your Medicare premiums to participate, and you are eligible for the GE Pensioner Vision Value Option. ABOUT GE MEDICARE BENEFIT PLANS GE Medicare Benefit Plans work with benefits you receive from Medicare starting at age 65 to offer you comprehensive health coverage. The plans include: GE Medical Care Plan for Pensioners (MCPP); The GE Medical Care Plan for Pensioners supplements your Medicare Part A coverage and helps you pay Medicare co-pays for inpatient hospital admissions. Your number of years of continuous service, and other criteria, determine your cost, if any, for this coverage. GE Pensioners Hospital Indemnity Plan (PHIP); The GE Pensioners Hospital Indemnity Plan supplements your Medicare Part A coverage and helps you pay Medicare co-pays for inpatient hospital admissions. You pay the full cost of this coverage. GE Pensioners Prescription Drug Plan (PPDP); and The GE Pensioners Prescription Drug Plan helps you pay for prescribed medicines not covered by Medicare. Your number of years of continuous service, and other criteria, determine your cost, if any, for this coverage. GE Medicare Insurance Plan (GEMIP). The GE Medicare Insurance Plan reimburses 100% of Medicare Part B charges when combined with Medicare benefits after you meet a deductible. You pay the full cost of this coverage. 8

ABOUT THE ELFUN MEDICAL BENEFITS PLAN The Elfun Medical Benefits Plan is generally available to senior (retired) members of GE Elfun. The plan complements your Part A and Part B coverage provided by Medicare and the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan. You pay the full cost of this coverage. You also have the option to participate in the GE Pensioners Hospital Indemnity Plan (PHIP). Typically if you enroll in the Elfun Medical Benefits Plan, you should not enroll in the GE Medicare Insurance Plan (GEMIP). Since coverages under these plans overlap, enrollment in both plans is not cost effective. ABOUT GE MEDICAREPLUS GE MedicarePlus replaces your coverage under original Medicare and the GE Medicare Benefit Plans with coverage through a Medicare Advantage Plan made available to you by GE. GE MedicarePlus provides comprehensive health benefits through a single plan, including preventive care coverage and prescription drug benefits. In some areas, GE MedicarePlus may include dental, vision and hearing care benefits. You must live in the service area of a Medicare Advantage Plan (also known as Medicare Part C) offered through GE MedicarePlus to participate. See Section 3.0, "How GE MedicarePlus Works." If you enroll in GE MedicarePlus and later want to switch back to coverage you were previously enrolled in under the GE Medicare Benefit Plans, you can do so regardless of health status. ABOUT GE MEDICARE PART B PREMIUM REIMBURSEMENT OPTION GE Medicare Part B Premium Reimbursement Option allows you to suspend your GE Pensioner Health Plans and receive a monthly reimbursement equal to 50% of the Company's net cost for your health care, up to a maximum of the Medicare Part B monthly premium amount. You are eligible for this option if you are participating in (or are eligible for) the GE Medical Care Plan for Pensioners and/or the GE Pensioners Prescription Drug Plan and you are enrolled in Medicare Part B. ABOUT GE PENSIONER VISION VALUE OPTION The GE Pensioner Vision Value Option helps you purchase vision care services, including routine eye exams, eyeglasses, contact lenses and related vision care at discounted prices. After you reach age 65, you are eligible for the GE Pensioner Vision Value Option if you participate in any of the GE Medicare Benefit Plans, or if you have elected the GE Medicare Part B Premium Reimbursement Option. 9

12 KEY THINGS TO DO TURNING 65/RETIRING AFTER 65 Enroll in Medicare Part A, Medicare Part B and/or Part D when you reach age 65. At age 65, Medicare becomes your primary source of health care coverage. Review the enrollment materials you receive at retirement (or shortly before you reach age 65) and enroll if necessary. If GE MedicarePlus is available in your area, you'll receive more information about the plan and how to enroll. If more than one Medicare Advantage Plan is available in your area, compare which plan best meets your needs by reviewing all the benefit designs. In most cases, participation in the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan is automatic. If you are required to pay contributions, you will need to enroll. To participate in the GE Pensioners Hospital Indemnity Plan and the GE Medicare Insurance Plan, you must enroll. To participate in the Elfun Medical Benefits Plan, you must enroll. You must also be enrolled in the GE Medical Plan for Pensioners and the GE Pensioners Prescription Drug Plan. To enroll in a Medicare Advantage Plan, fill out a form and submit it to GE. Consider all your options GE Medicare Benefit Plans, Elfun Medical Benefits Plan, GE MedicarePlus (where available) or GE Medicare Part B Premium Reimbursement Option. Choose the option that best meets your health care needs. Present your ID card when you receive treatment and inform the provider that you have coverage that supplements Medicare. Present your ID card when you purchase prescription drugs and inform the provider that you have coverage that supplements Medicare. Use the vision care network to reduce your out-of-pocket costs for vision care expenses. See Section 4.0, "GE Pensioner Vision Value Option." For the GE Medicare Benefit Plans, you have a 90-day window of opportunity to enroll. Under the Elfun Medical Benefits Plan, you also have 90 days to enroll once you turn age 65 as defined by Medicare. See Section 1.1, "Who is eligible for the GE Medicare Benefit Plans?" for details. 10

13 Go to benefits.ge.com for benefits information, forms, transactions and more. 1.0 PARTICIPATION The GE Medicare Benefit Plans work with Medicare to provide you with health coverage starting at age 65. It's up to you to choose the coverage options that best meet your health care needs, choosing from among those for which you are eligible. This section describes the eligibility requirements for the GE Medicare Benefit Plans, Elfun Medical Benefits Plan, GE MedicarePlus, GE Medicare Part B Premium Reimbursement Option and GE Pensioner Vision Value Option. This section also provides information on your spouse's eligibility, when coverage begins and ends, and other provisions. REACHING AGE 65 For purposes of GE Pensioner Health Plans, participants "reach age 65" on the first of the month in which the participant's 65th birthday occurs, or on the first of the preceding month if the birth date falls on the first. 1.1 WHO IS ELIGIBLE FOR THE GE MEDICARE BENEFIT PLANS? GE MEDICAL CARE PLAN FOR PENSIONERS (MCPP) AND GE PENSIONERS PRESCRIPTION DRUG PLAN (PPDP) You are eligible for the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan if you are age 65 or older and meet any one of the following sets of conditions: You retire directly from Company service at age 60 or older with at least 10 years of continuous service. You retire under the Special Early Retirement Option (SERO) of the GE Pension Plan. You receive pension benefits under the Plant Closing Pension Option (PCPO) of the GE Pension Plan: At age 50 or older and have at least 25 years of Pension Qualification Service (PQS) by the end of the calendar year in which your service is terminated because of the plant closing; or At any age and have at least 30 years of PQS by the end of the calendar year in which your service is terminated for plant closing. Your service is terminated because of a plant closing and you were age 50 or older with 25 or more years of continuous service as of the end of the calendar year in which your service was terminated. You are eligible for special benefits protection because your service terminated on or after June 27, 1988, because of a plant closing, transfer to a successor employer, or after you had been on layoff for 12 months during which your service was maintained, and at the time your service terminated you had at least 25 years of continuous service. You are eligible for special benefits protection because your service terminated on or after July 1, 1991, because of a plant closing, transfer to a successor employer or after you had been on layoff for 12 months during which your service was maintained, and at the time your service terminated you had at least 25 years of PQS. You belong to any other classification of pensioners, as determined by the Pension Board, who retire from a company acquired by GE. You retire on a Disability Pension from the GE Pension Plan. Employees who leave GE with a vested pension but without meeting one of these eligibility requirements do not qualify for coverage. See the definitions of "continuous service" and "Pension Qualification Service" in the "Key Terms" section. For the GE Medicare Benefit Plans, "reaching age 65" means the first day of the month in which you reach age 65 unless your birth date falls on the first of the month, then reaching age 65 means the first day of the preceding month. This "age 65" rule applies to you and also your eligible dependents based on when your eligible dependent's 65th birthday occurs. If you are eligible as described above, your eligible dependent becomes eligible for these plans when he or she reaches age 65. See Section 1.6, "Who are my eligible dependents?" 11

14 GE PENSIONERS HOSPITAL INDEMNITY PLAN (PHIP) AND GE MEDICARE INSURANCE PLAN (GEMIP) If you leave the Company at age 60 or older, regardless of your years of service, or if you meet any of the conditions for eligibility listed above, you also are eligible for the GE Pensioners Hospital Indemnity Plan and the GE Medicare Insurance Plan. Under both of these plans, you pay the full cost of your coverage. 1.2 WHO IS ELIGIBLE FOR THE ELFUN MEDICAL BENEFITS PLAN? If you join Elfun on or after January 1, 2001, and on or before July 1, Then you will be eligible to purchase coverage under the Elfun Medical Benefits Plan if you are: Age 65 or older and participating in Medicare Parts A and B; Eligible for and participating in the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan; and A senior (retired) member of GE Elfun who was in good standing for at least the last three consecutive years before retirement from the Company. If you became an Elfun member before January 1, 2001, the rules under which you retired will continue to apply. For the Elfun Medical Benefits Plan, "reaching age 65" means the first day of the month in which you reach age 65 unless your birth date falls on the first of the month, then reaching age 65 means the first day of the preceding month. This "age 65" rule applies to you and also your eligible dependents based on when your eligible dependent's 65th birthday occurs. If you are eligible as described above, your eligible dependent becomes eligible for these plans when he or she reaches age 65. See Section 1.6, "Who are my eligible dependent's?" 1.3 WHO IS ELIGIBLE FOR GE MEDICAREPLUS? You are eligible for GE MedicarePlus if: You are eligible for the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan; You are a participant in Medicare Part B (proof of enrollment may be required); You live in the service area of a Medicare Advantage Plan offered through GE MedicarePlus; and You meet the Center for Medicare and Medicaid Services (CMS) enrollment guidelines. 12

15 Go to benefits.ge.com for benefits information, forms, transactions and more. 1.4 WHO IS ELIGIBLE FOR THE GE MEDICARE PART B PREMIUM REIMBURSEMENT OPTION? You are eligible to exercise the GE Medicare Part B Premium Reimbursement Option if: You are a participant in or eligible for the GE Medical Care Plan for Pensioners and/or the GE Pensioners Prescription Drug Plan; and You are enrolled in Medicare Part B (proof of enrollment may be required). 1.5 WHO IS ELIGIBLE FOR THE GE PENSIONER VISION VALUE OPTION? After you reach age 65, you are eligible for the GE Pensioner Vision Value Option if you participate in any of the GE Medicare Benefit Plans, or if you have elected the GE Medicare Part B Premium Reimbursement Option. 1.6 WHO ARE MY ELIGIBLE DEPENDENTS? For the purposes of this handbook, an "eligible dependent" is defined as: Your spouse; or Your same-sex domestic partner, provided that he or she was covered by you as a dependent under a GE Medical Care Option at the time of your retirement. Please Note Dependent coverage is not available for: Eligible dependents covered under any of the plans as an eligible Company retiree; Eligible dependents covered under a GE Medical Care Option as an eligible Company employee; or Same-sex domestic partners acquired subsequent to your retirement. If you get married while you are covered under the GE Medicare Benefit Plans, your new spouse must make an election within 31 days of your marriage, to be covered. If you are eligible for the GE Medicare Benefit Plans, you may enroll your eligible dependent for coverage in the plans (or one of the alternative options) you choose once he or she reaches age 65. If you are eligible for these plans and your eligible dependent is under age 65 and is not eligible for coverage under a GE Medical Care Option, your eligible dependent can be covered under the GE Medical Care Plan for Pensioners and receive the alternate benefits described in Section 2.1.5, "What alternate benefits are available?" When your eligible dependent reaches age 65 and becomes eligible for Medicare, regular coverage under the GE Medical Care Plan for Pensioners will begin. If you are a senior Elfun eligible for the Elfun Medical Benefits Plan, you can also choose to cover your spouse when he or she reaches age 65 or when coverage under a GE Medical Care Option ends, if later. If you are a senior Elfun under age 65, you may cover your eligible dependent if he or she is age 65 or older and participating in Medicare Parts A and B, the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan, or the GE Medicare Part B Premium Reimbursement Option. 13

16 1.7 WHEN DOES COVERAGE BEGIN? If you are eligible and you enroll (as needed), your coverage begins as described below. Please Note In most cases, retirees eligible for the GE Medical Care Plan for Pensioners and the GE Pensioners Prescription Drug Plan do not need to enroll for coverage under those plans; enrollment is automatic. You need only enroll for these plans if you are required to pay contributions for your coverage. There is no automatic enrollment for the GE Pensioners Hospital Indemnity Plan, the GE Medicare Insurance Plan and the Elfun Medical Benefits Plan; you must enroll to participate in these plans. If you enroll for GE MedicarePlus, you will not be covered under any of the GE Medicare Benefit Plans, because all of your coverage will be provided through the Medicare Advantage Plan in which you enrolled under GE MedicarePlus. Remember, you can switch back to traditional Medicare and coverage under the GE Medicare Benefit Plans during the Annual Enrollment period. You may elect the GE Medicare Part B Premium Reimbursement Option under the GE Pensioner Health Plans and receive a monthly reimbursement from the Company. You do not need to enroll for the GE Pensioner Vision Value Option. If you are eligible, your enrollment is automatic. If you are eligible and you retire or leave the service of the Company before age Then your coverage under the GE Medicare Benefit Plans begins when you reach age 65, provided you have enrolled as required and pay any required contributions. If you are eligible and you retire or leave the service of the Company after age Then your coverage under the GE Medicare Benefit Plans begins on your retirement or termination date, provided you have enrolled and pay any required contributions. If you are disabled and have extended coverage under a GE Medical Care Option... Coverage under the GE Medical Care Plan for Pensioners, the GE Pensioners Hospital Indemnity Plan, the GE Pensioners Prescription Drug Plan, the GE Medicare Insurance Plan, the Elfun Medical Benefits Plan and GE MedicarePlus ends: If you (or your surviving eligible dependent) do not pay any required contributions as scheduled; If your surviving eligible dependent remarries before age 65; For your eligible dependent, if you divorce him or her; or For your same-sex domestic partner, if your relationship ends or you no longer meet all the criteria for same-sex domestic partnership. In such case, you will be required to provide an affidavit of termination within 31 days of the event. 15

18 1.10 WHAT IF I ELECT THE GE MEDICARE PART B PREMIUM REIMBURSEMENT OPTION? Once you are eligible for coverage under the GE Medicare Benefit Plans and you have enrolled in Medicare Part B, each fall you will have the opportunity to elect the GE Medicare Part B Premium Reimbursement Option (effective the first of the following year) under the plans and receive a nontaxable, monthly payment instead. The payment will be equal to 50% of the estimated net cost of the GE Medicare Benefit Plans paid by the Company, up to a maximum of the Medicare Part B monthly premium amount. If you have elected the GE Medicare Part B Premium Reimbursement Option, but your Part B enrollment has not yet been accepted by Medicare, your reimbursement will not begin until you are enrolled in Medicare and paying the Part B premium. Note that you will automatically be enrolled in the GE Pensioner Vision Value Option, even if you elect the GE Medicare Part B Premium Reimbursement Option. If you elect the GE Medicare Part B Premium Reimbursement Option at age 65 you will be eligible to enroll in any of the GE Medicare Benefit Plans for which you were initially eligible during any subsequent fall enrollment period, with participation effective the first of the following year. To continue medical coverage under the GE Medical Care Plan for Pensioners, you may need to pay required contributions. Contributions are waived if you retired before January 1, If you are enrolled in the plan prior to January 1, 2009, and you had 15 or more years of continuous service when you retired... GE provides coverage under the GE Medical Care Plan for Pensioners for you and your eligible dependent at no cost to you. If you are enrolled in the plan prior to January 1, 2009, and you had at least 10 but less than 15 years of continuous service when you retired... Your contributions are 25% of the plan's cost to provide coverage for you and your eligible dependent, adjusted annually. If you enroll in the plan on or after January 1, 2009 You will pay a percentage of the plan s cost to provide coverage for you and your eligible dependents, adjusted annually. The specific contribution you pay is based on certain criteria as of January 1, If you had at least 25 years of Pension Qualification Service (PQS) or continuous service, and your service was terminated because of a plant closing, transfer to a successor employer or after 12 months on layoff during which your service was maintained... Your cost is 50% of the plan's cost to provide coverage for you and your eligible dependent, adjusted annually. If a federal, state or local government applies a tax or surcharge on health care services, benefits or enrollment, the tax or surcharge will be considered a covered expense, subject to the applicable benefit payment provisions. Contributions for participants affected by the tax or surcharge will be increased to pay for half of the added cost to the plan or the Company resulting from the tax or surcharge. The Company pays the other half HOW DO I ENROLL? If you elect the GE Medicare Benefit Plans, you do not need to enroll for coverage under the GE Medical Care Plan for Pensioners if you're eligible for coverage without contributions. You will receive enrollment information and forms shortly before you reach age 65. You must enroll within 90 days after becoming eligible. If you miss the enrollment deadline, you cannot enroll later, except under the following condition: You may enroll if enrollment occurs within 60 days of the termination of other employer-provided health care coverage. (Proof of loss of coverage may be required.) Your eligible dependent will receive enrollment information and forms shortly before he or she reaches age 65. Like you, your eligible dependent must enroll within 90 days after becoming eligible. If your eligible dependent misses the enrollment deadline, your eligible dependent cannot enroll later, except under the following condition: Your eligible dependent may enroll if enrollment occurs within 60 days of the termination of other employer-provided health care coverage. (Proof of loss of coverage may be required.) If you are required to pay contributions, you can enroll by filing a completed enrollment form and agreeing to make the required contributions. 21

24 2.1.3 WHAT EXPENSES ARE COVERED? The plan pays benefits up to a combined lifetime maximum for you and your eligible dependent of $150,000. If you or your eligible dependent dies, then the survivor is eligible to receive benefits up to the amount of the remaining lifetime maximum balance in effect on the date of either your or your eligible dependent's death. The plan pays benefits for any admission covered by Medicare, even if you have other coverage. HOSPITAL STAYS The plan pays benefits for hospital stays that are medically necessary and covered by Medicare: FOR DAY... THE PLAN PAYS... 1 $ to 60 $0.00 (paid in full by Medicare) 61 to 90 $ a day 91+ $ a day If the hospital stay is for dental treatment, or the hospital is not certified by Medicare... Then the plan pays $40 a day for each day of confinement not covered by Medicare. If the hospital stay is for tuberculosis... Then the plan pays $10 a day for stays that extend beyond the period covered by Medicare. If the hospital stay is in a government psychiatric hospital... Then the plan pays $10 a day for stays that extend beyond the period covered by Medicare. If you exhaust your Medicare benefits, GE benefits can continue as long as your hospital stay is medically necessary or until you reach your maximum benefit under the plan, whichever is first. EXTENDED CARE FACILITIES The plan pays benefits for stays in an extended care facility when medically necessary and covered by Medicare: FOR DAY... THE PLAN PAYS... 1 to 20 $0.00 (paid in full by Medicare) 21 to 100 $50.00 a day $25.00 a day 22 If you exhaust your Medicare benefits, GE benefits can continue as long as the extended care stay is medically necessary or until you reach your maximum benefit under the plan, whichever is first. OUTPATIENT AMBULATORY SURGICAL CENTERS For charges not covered by Medicare, the plan pays up to $400 for reasonable, necessary and customary charges when treatment in an approved outpatient ambulatory surgical center would otherwise have required a hospital stay. See the definition of "reasonable, necessary and customary" in the "Key Terms" section.

As with all medical benefit plans, some expenses are not covered under the GE Medical Care Plan for Pensioners. The plan does not cover expenses: That you are not required to pay; For your eligible dependent if he or she is covered by this plan as a retiree, rather than as your dependent (benefits in that case are paid for your spouse on his or her own account); For custodial care services furnished primarily to provide room and board, education or other care for a mentally or physically disabled person in a monitored or protected environment who is not under specific medical, surgical or psychiatric treatment, when there is no reasonable likelihood that the care will result in functional improvement; and That are not reasonable, necessary and customary WHAT ALTERNATE BENEFITS ARE AVAILABLE? The plan pays alternate benefits, if you meet the age and service requirements for eligibility and: You or your eligible dependent does not qualify for Medicare because of residence or travel outside the United States or its possessions; or Your eligible dependent or surviving eligible dependent is under age 65 and not eligible for a GE Medical Care Option. ALTERNATE BENEFITS If you are eligible for alternate benefits, you will need to meet a $25 deductible each calendar year for covered medical expenses before the plan will begin to cover your eligible expenses. If you cover your eligible dependent under the plan, your eligible dependent needs to meet a separate $25 annual deductible. Two features help limit the number of deductibles you must meet: Yearly carryover Any covered expenses from the last three months of one calendar year used to meet that year's deductible are carried over to help satisfy the next year's deductible, as well; and Family accident limit If you and your eligible dependent are both injured in the same accident, you must meet only one individual deductible for your combined covered expenses related to the accident for that calendar year. After you meet the annual deductible, the plan pays: 100% of the next $225 of covered medical expenses; and 85% of any additional covered medical expenses, up to the lifetime maximum. Alternate benefits pay reasonable, necessary and customary charges for the diagnosis, care and treatment of non-occupational illness or injury, including: Semiprivate hospital room and board and related services; Emergency treatment and other special services required for medical or surgical care; Chemotherapy and radiation therapy; Special approved facilities and programs, such as extended care facilities, outpatient ambulatory surgical centers, hospices, home health care when services are performed by a home health care agency, home hemodialysis and home infusion therapy programs; Anesthesia and its administration; Surgeons' fees, including certain dental surgery; Diagnostic x-rays and other imaging procedures; and Local professional ambulance service, when necessary. 23
