5 Frequently Asked Questions What age division will I be in? A player's age division is determined by the athlete's age on August 1st of the year in which the tournament is being held. Example: If a player is 14 years old on August 1, they are eligible to compete in the 14&U division. If an athlete turns 15 on July 31, his/her age is 15 on August 1st and is therefore no longer eligible for the 14&U division. They are, however, able to participate in the 16&U division. How many coaches are there and why? We strive to have 4-5 coaches on the deck every practice. With so many talented and nationally recognized coaches volunteering their time we have the ability to have a coach to athlete ratio that is not equaled anywhere in the US. Because of this we can make sure that the athletes get the 1 on 1 attention that they need to improve their individual and team games the fastest. All of our coaches have college experience and most have sent athletes to the Division 1 level. Is there extra cost of traveling to out of state tournaments? Yes. Empire will provide the entry to the tournaments but traveling cost, hotel and food will not be covered. We will do our best to help coordinate rides, hotel rooms and food. We will be fundraising to defer the cost of large trips.

6 RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT I am the parent or legal guardian of the child named below and I give permission for my child to participate in all activities of Oregon Empire Water Polo Academy ( EMPIRE ). My child and I understand and acknowledge that participation in water polo can result in physical injury and even death. With full understanding of these potential risks, we assume the risks of participating in EMPIRE. Therefore, my child and I each (i) release EMPIRE and its employees, volunteers, and agents (each an EMPIRE Party ) from any and all claims, liabilities, obligations, costs and expenses arising from personal injury, death or damages of any kind, except those which result from gross negligence or willful misconduct of any EMPIRE Party; (ii) agree not to sue any EMPIRE Party for any of the claims, liabilities, obligations, costs or expenses that we have released; and (iii) agree to indemnify and hold harmless each EMPIRE Party from any claims made or liabilities assessed against him, her or it because of my actions or actions of my child. I execute this Release for and on behalf of myself and my child. I hereby bind myself and my child and the guardians, personal representatives, heirs, successors and assigns of each of us to the terms of this Release. I represent that I have legal capacity and authority to act for and on behalf of my child. 7 Empire Water Polo Playing Time Policy Purpose The purpose of this policy is to provide guidelines to coaches, players and parents regarding playing time for Empire Water Polo teams. This policy applies to all Empire Water Polo coaches, players and parents. Policy Empire Water Polo is a program striving to develop the full potential of each player in the club. Playing time however, should not be expected to be distributed equally whether it is in a developmental scrimmage, game, or competition or a select/special/invite or international competition. It is also understood that a player may receive less or no playing time in a game due to attendance problems, violating the Empire Water Polo Code of Conduct and / or Code of Ethics. Distribution of playing time shall be at the discretion of the Head Coaches. The Executive Head Coach has the authority to direct a Head Coach to address/adjust playing time if desired. It is the desire of Empire Water Polo that Head Coaches have the freedom to balance between goals of individual and team skills and giving the team a successful competitive experience. In order to achieve this balance, playing time will be based on the following factors: -The player s attitude, focus, sportsmanship -The player s effort and responsiveness to coaching instruction -The player s attendance -The number of players on the team available to play -The level of play for a competitive tournament and/or special competition These factors may increase or reduce a player s time, including no playing time at all, in any particular game, scrimmage series or tournament. Any questions regarding this policy should be directed exclusively to the Executive Head Coach and not the Age group Coach, or other Empire Water Polo parents. The Executive Head Coach will have the final say and may consult the Board of Directors if necessary. 8 Players Code of Conduct Empire Water Polo Club 1. Reliability- fulfill commitments; be on time to practice and games. For practices this is about five to ten minutes prior, for games it is about 30 minutes prior. If you know that you will miss a scheduled practice or event, please or contact your coach. If you need to leave early, please let your coach know upon arrival. If you leave the water for any reasons you must notify the coach on deck prior to leaving and make certain they know when you return to the water. 2. Respect treat all people with respect at all times, as well as require the same of other athletes. No negative recruiting by making unduly derogatory statements about the members or coaches of another team. 3. Focus listen when a coach is talking. Direct your undivided attention to the coaches at all times. If you are talking at least three other players near you are not able to hear what the coach is saying. To maintain focus in a game situation we all need to pay attention during practices. Communication in the pool is key. If you have a question, ask. 4. Disrespectful Conduct don t engage in disrespectful conduct of any sort towards an athlete, coach, referee or any person participating in or conducting a Empire sanctioned event, including profanity, obscene gestures, offensive remarks of a sexual or racial nature, trash-talking, boastful celebrations, or other actions that demean individuals or the sport. 5. Respect Officials, Coaches treat all officials with respect: don t complain about or argue with official calls or coaches decisions during or after a practice or athletic event. 6. Role-Modeling remember, participation in sports is a privilege, not a right and that you are expected represent your club, coach and teammates with honor, in and out of the pool. Consistently exhibit good character and conduct yourself as a positive role model. Suspension or termination of the participation privilege is within the sole discretion of the Executive Head Coach. 7. Self-Control exercise self-control; don t fight or show excessive displays of anger or frustration; have the strength to overcome the temptation to retaliate. 8. Concern for Others demonstrate a concern for others; never intentionally injure any player or engage in reckless behavior that might cause injury to yourself or others. 9. Teammates help promote the well-being of teammates by positive counseling and encouragement or by reporting to coaches any unhealthy or dangerous conduct. 10. Play by the Rules maintain a thorough knowledge of and abide by all game and competition rules. 11. Possession of illegal substances, any weapons you shall not use, possess or provide alcohol or illegal substances or drugs (except as prescribed by a licensed physician) in violation of federal, state or

9 local laws. Firearms, ammunition, knives and all other items that could be construed as a weapon are prohibited. 12. Behavior on Team Trips you agree to obey all travel rules, including curfews, check-in and meal times, hotel rules, restricted areas, etc. You will also obey our team chaperones, be respectful to them and thank them for their support of our team. If you do not comply with these rules, you may be sent home early and at your own expense. When a player s behavior becomes unacceptable to the coach, the player will be given a verbal warning. A second reprimand will result in the player being asked to exit the pool and sit and watch the play from the deck. A third reprimand will result in the parents being called to take the player immediately home. If a player is removed from practice three times, they will be suspended from practice and all team activities for a specific amount of time, which will be determined by the Head Coach. If a player s suspension coincides with a tournament, all tournament fees will be forfeited, and no refunds will be granted. I have read and understand the requirements of this Code of Conduct. I understand I am expected to perform according to this code and I understand that there may be sanctions or penalties if I do not. 10 CONCUSSION POLICY & INFORMATION A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications, including prolonged brain damage and death, if not recognized and properly managed. You can t see a concussion. Most concussions occur without loss of consciousness. Signs and symptoms of concussion may show up immediately or may take hours or days to fully appear. If your child reports any symptoms of concussion, or if you notice any symptom or sign of concussion, seek immediate medical attention. Symptoms and signs observed by teammates, parents and coaches may include one of more of the following: Headache Pressure in head Nausea or vomiting Neck pain Balance problems or dizziness Blurred, double or fuzzy vision Sensitivity to light or noise Feeling sluggish or slow Feeling foggy or groggy Drowsiness Change in sleep patterns Amnesia Don t feel right Fatigue or low energy Sadness Nervousness or anxiety Irritability More emotional Confusion Concentration problems Forgetful/memory problems Repeating questions/comments Appears dazed Confused about assignment Unsure of game/score/etc. Answers questions slowly Changes behavior/personality Can t recall events after hit Vacant facial expression Forgets plays Moves clumsily/uncoordinated Slurs speech Seizures or convulsions Athletes with signs and symptoms of concussion will be removed from play immediately and must have the signed release of a physician before being allowed to return to play. Continuing to play with the signs and symptoms of concussion leaves athletes especially vulnerable to greater injury. There is an increased risk of significant damage from a concussion for a period of time after that concussion occurs, particularly if the athlete suffers another concussion before completely recovering from the first one. This can lead to prolonged recovery, or even to severe brain swelling (second impact syndrome), with devastating and even fatal consequences. If you think your child has suffered a concussion, close observation should occur for several hours. You should also inform your child s coach if you think he/she has suffered a concussion.
