1 YEARLY UPDATE REPORT FOR ADDITIONAL SUPPORT TEAM, FORENSIC NURSE PRACTITONER 1 P a g e

2 Introduction The purpose of this report is to discuss the period from April 2012 to Dec 2013 of the achievements and developments of Forensic Nurse Practitioner, Additional Support Team within the Adult Learning Disabilities Service. In addition the report will discuss future planned developments from November 2013 to April This will report will provide emphasis on prevention, social inclusion and least restrictive practices, within a positive, person-centred, individually-designed and risk managed framework. With achievements and developments of key objectives it will support positive partnerships between services such as wider Learning Disabilities and service within the Criminal Justice System and shared understanding and hence a concentrated effort to provide training about people with learning disabilities at risk of offending with wider work force. 3 Referral to Service From the period of April 2012 to March 2013, the service received 35 referral for individuals with a Learning Disabilities who are risk of offending or have committed an criminal offence. The referrals are broken down below into source of referral. Greater Manchester Probation Trust 65 % ( N= 23) Wigan Youth Offending Team 5% (N = 2) Prison Health Service 2% (N=1) 14% (N=5) Social Services 11% (N= 4) Stakeholder Through the service improvement/delivery the Additional Support Team has established effective close links with multi- agencies on local, regional and national basis. It clear from the information below the team has excellent links with Criminal Justice Agencies and secondary agencies. The list below details stakeholders which the service works in multiagency approach. 4 Secondary Agencies Wigan Alcohol and Drug Service Homeless and Vulnerable Person Team Connexions Services Employment Services Fire Brigade Housing Department Mental Health Services General Practitioner Wigan Learning Disabilities Partnership Board Education Services Acute NHS Trust Services High, Medium and Low Secure Hospital Services Religious Groups People First Advocacy Service Northwest Forensic Practitioner Forum Voluntary Agencies Links with Agencies on Regional and National Basis Northwest Secure Commissioning Team Prison Reform Trust Department of Health, Offender Health Team Risk of Offending Behaviour Pathway As part of service delivery/improvement, the Forensic Nurse Practitioner in partnership with Wigan Council, Neighbourhood Team(ASB) in 2012 reviewed of pathway for people with a learning disabilities who are risk of offending. In 2012, to support this process all personal from Neighbourhood team have received Learning Disabilities Awareness, Learning Offender Awareness Training in and awareness of service provision of Additional Support Team. This pathway will be reviewed with service planned for 2014/2015.The pathway demonstrates the risk of offending pathway 4 P a g e

6 Offending Pathway As part of service delivery/improvement, of Additional Support Team in partnership with Greater Manchester Police, HMP Prison Service, Greater Manchester Probation Trust, Wigan Youth Offending Team, Appropriate Adult Service, Wigan and Leigh Magistrates and Wigan Criminal Justice Liaison have developed the pathway for offenders with a learning disabilities throughout the Criminal Justice against national, regional and local guidance. To demonstrate areas of achievement within criminal justice agency are discussed below Greater Manchester Police (Wigan and Leigh Division) Neighbourhood Policing Team 2012/ Police Officers and Police Support Community Officer provided Learning Disabilities Awareness Training Review of Referral Pathway to Service Police Custody In 2013 partnership with Greater Manchester Police and Wigan Criminal Justice Liaison Team development and implementation of liaison and diversion scheme for people with Learning Disabilities. To the best of the knowledge of this report this only Learning Disabilities Scheme within Police Custody within the Northwest Regional. As part of this Scheme 15 Police staff and detention staff received Learning Disabilities awareness training. As part of this scheme all custody staff have been trained in use of Learning Disabilities Screening Questionnaire, as means of early identification Greater Manchester Probation Trust This service receives yearly Learning Disabilities Awareness, Learning Disabilities Offender Awareness, Skills to support people with Learning Disabilities and Service Provision Training. In 2012/ staff has received this training. 6 P a g e

7 In 2013, review of referral pathway to Additional Support Team from Probation Trust. As part of this process, probation officers are trained in the use of Learning Disabilities Screening Questionnaire as means of early identification. In partnership with Greater Manchester Probation Trust, the implementation of easy read documents for people with a learning disabilities. These easy read documents are what is community order?, Licence conditions and what is probation. At present the Additional Support Team have developed an offending programme for offenders with learning disabilities. This is on hold until the changes by the MOJ.have been completed. On weekly basis the Forensic Nurse Practitioner provides face to face support and advice to the probation staff at the Atherton Office,. In addition provides support and advice when required within the MAPPA meetings as Learning Disabilities Representative. Wigan Youth Offending Team. This service receives yearly Learning Disabilities Awareness, Learning Disabilities Offender Awareness, Skills to support people with Learning Disabilities and Service Provision Training abilities and Service Provision Training.. In 2012/ staff received this training. On two weekly basis the Forensic Nurse Practitioner provides face to face support and advice this case manager within this service Prison In 2012/2013 the referral pathway has been reviewed by the Additional Support Team. The clinical standard set by Additional Support team, if an individual with Learning Disabilities is sentenced to prison. Contact is made with receiving prison within 24 hour period for following 1.) Ensure that Prison are aware of the person Learning Disabilities 2.) Health Action Plan, Support Plan and Risk Assessment and Management Document are sent to prison 7 P a g e

8 3.) The Additional Support Team involved in resettlement of the individual back to Wigan Borough. Courts Since 2008, on a yearly basis, the Additional Support Team has offered free Learning Disabilities Awareness Training. However this has not be accepted due to financial contraints of the Courts service. This training will be reviewed on yearly basis is active objective to be achieved by the service. To support individuals with Learning Disabilities whom are known to enter the court system, easy read document is provided to enable to understand the process. The Additional Support Team actively supports the client through this process. In addition liaison with the Courts and legal staff to inform them of a person with learning disabilities. Wigan Criminal Justice Liaison Team All staff in this service have received Learning Disabilities Awareness Training. As part of this process, all staff are trained in the use of Learning Disabilities Screening Questionnaire as means of early identification. The Additional Support Team are an active member of Mental Health and Learning Disabilities Diversion Panel, which sits on monthly basis. Wigan Public Protection Unit (Bamflong Police Station) This service has been set up to deliver multi-agency approach to address the following offence Domestic Violence Sexual Exploitation/Violence Prolific Offenders The Additional Support Team in 2013/2014 will be This service receives yearly Learning Disabilities Awareness, Learning Disabilities Offender Awareness, Skills to support people with Learning Disabilities and Service Provision Training. 9 a referral pathway has been developed between this service and the Additional Support Team. In Jan 2014, the plan is for on a weekly basis for the Forensic Nurse Practitioner to provide face to face support and advice to the service. Forensic Assessment Pathway The Forensic Nursing Team have developed and implemented a Forensic Pathway which is clinical effective and evidence based, pathway of assessment utilised for individuals who are at risk of offending or offending with specific offending behaviour. As part assessment phase of this pathway, the clinical standard set by Additional Support Team is that the nationally/internationally recognised risk assessment and management strategies are competed within a 14 week period. For example HCR- 20, Risk of Sexual Violence Protocol, Short Term of Assessment of Risk and Treatability (START). All individuals commence person specific treatment/interventions by Week 15 of pathway. In 2012 the Core-LD outcome focused tool was implemented at specific stage of the pathway to establish the effective of the service provision. this information will be analysed in A copy of forensic pathway has been enclosed for information purpose. 9 P a g e
