3 Strengthening the Safety Net Blue Cross interest Investing in free clinics and other communitybased safety net clinics that care for the uninsured, underinsured and newly insured at little or no cost to the patient Services may include medical, dental, mental health, vision or specialty care. Grantees that help people enroll in, re-enroll in, and understand their health insurance coverage. (Referrals can be made to effective partners for these functions, but must be monitored and reported.) Clinics that engage community volunteers to help provide service and have medical directors who routinely report to the Board on clinical quality.

4 New in 2015 Dental clinics may apply for up to $15,000 for operations. Medical clinics may apply for up to $10,000 for operations. Medical clinics may also apply for up to $10,000 for service enhancement. This requires a local match of at lease $5,000. Service enhancement program should promote health, link patients to services outside of the clinic or improve community health. Examples include health literacy, health promotion, population health, case management, emergency room diversion, or multicultural services. Applicants are required to provide a letter of commitment from a local donor by August 31, The funds must be new and designed for the program.

5 Eligibility 1. Be based in Michigan with a mission to serve lowincome, uninsured and underinsured patients. 2. Be recognized by the Internal Revenue Service as a 501 (C)(3) organization or be a communitybased nonprofit organization paired with a 501 (C)(3) organization. 3. Provide medical, dental, vision, mental health or specialty care services directly or refer patients for such services. 4. Provide services at little or no cost to patients. 5. Work with volunteer administrators and clinicians. 6. Have a physician serving as medical director. 7. Be in operation for at least two years. 8. Be in good standing on all previous Strengthening the Safety Net grants, if applicable.

8 Conditions of grant You must use your grant funds as indicated on the required budget form. You can shift up to 10 percent of your award between approved line items. If you need to revise your budget or if you need an extension, you will be required to send a written request. We plan to visit as many grantees as possible to learn about your work. The site visits are one of the conditions of your grant. Service enhancement grantees must submit a letter of commitment from a local donor for the match. Any unused funds remaining after the end of the grant period may be required to be returned. If your clinic is closing or merging, please notify us as early as possible. We will work with you to make sure the grant funding is used for the intended purpose.

9 Media and publicity Blue Cross will announce the Strengthening the Safety Net grants via a statewide media release in the fall of We also like to acknowledge the grants at several local media events each year and we may ask for your help on these. Please do not publicly announce the receipt of the grant until after these announcements are made. If you would like to use the Blue Cross logo on your materials or website, please send a request for approval. 10 Status reports Mid-year: The mid-year status report is due on May 1, 2016, and is a progress report on the services you provided between October 1, 2015 and March 31, This is a report on the amount of services you provided compared with the amount of services you expected to provide. In order to complete this report, you need to be sure that you are collecting the required information from the beginning of the grant period. Year-end: The year-end status report is due November 15, 2016, and covers the entire grant year. It includes the amount of services you provided compared with the amount of services you expected to provide, a financial report, and a narrative summary of accomplishments, up to three double-spaced pages. For your reference, the blank status report form is located on the Strengthening the Safety Net website, bcbsm.com/safetynet
