1 2015 Annual FOMA Convention FULL Alpha Speaker Biographies Updated 2/5/2015 Asfa Akhtar, DO, FAOCD, is a 2000 graduate of Nova Southeastern University College of Osteopathic Medicine (NSU-COM). She completed both her internship and family practice residency at Palmetto General Hospital in Hialeah, FL. Dr. Akhtar also completed a dermatology residency at North Broward Hospital District in Ft. Lauderdale, FL. Dr. Akhtar is certified in dermatology and family medicine. Since 2007, Dr. Akhtar has been a staff dermatologist in the department of dermatology at Cleveland Clinic Florida in Weston, Florida. She also serves as a clinical associate professor for the Department of Internal Medicine and Dermatology at NSU-COM. Her interests include cosmetic dermatology, laser surgery, ethnic skin, and facial rejuvenation. Nicole Heath Bixler, DO, MBA, FACOFP, is a graduate of the Philadelphia College of Osteopathic Medicine and completed her internship and residency in Family Medicine at Frankford Hospitals in Philadelphia. While attending medical school, she earned her MBA in Health Administration at Saint Joseph s University in Philadelphia. After working 3 years in the Philadelphia area, Dr. Bixler relocated to Spring Hill, Florida where she practices at Immediate Medcare and Family Doctor of Spring Hill. Her clinical practice focuses on the geriatric population with outpatient and inpatient services. She has been an advocate of Osteopathic Medicine by participating in leadership roles at the national, state and local levels. She currently serves on the Board of Governors for the American College of Osteopathic Family Physicians and chairs the Task Force on Enhanced Student Chapter Services, she is a Past President of the Florida Society of the American College of Osteopathic Family Physicians, 1 st Vice-President of the FOMA, and President of District 13 of the FOMA. In 2010, she was appointed to the Florida State Physician Workforce Advisory Council by the State Surgeon General. In her community she serves as the Scholarship Chair for the Business and Professional Women of Hernando County. In 2011, she received her Fellow designation in the American College of Osteopathic Family Physicians and was awarded the Outstanding Young Osteopathic Physician of the Year. In 2012, she received the Family Physician of the Year Award from the Florida Society of ACOFP. She is proud to be in practice with her husband, Brian Bixler, MD and to be a role model for their three daughters. John Broussard, DO, is board certified in family medicine and osteopathic manipulative medicine by the AOBFP. He received his Bachelor of Science degree in Kinesiology and Human Movement Science from Louisiana State University in Baton Rouge, Louisiana prior to earning his Doctorate of Osteopathic Medicine from Nova Southeastern University in Ft. Lauderdale, FL. Dr. Broussard completed his residency in Family Medicine at Largo Medical Center in Largo, FL and is now completing a Sports Medicine Fellowship at Larkin Community Hospital in Miami, FL in conjunction with the Institute for Non-Surgical Orthopedics. Dr. Broussard provides sideline coverage for the North American Soccer League s local Ft. Lauderdale Strikers, Ft. Lauderdale High School, as well as countywide high school events. The embracement of the osteopathic principles is evident in his functional and integrative medicine application toward patient care. He is currently receiving training

2 in The Art and Science of Acupuncture for Physicians through University of Miami Miller School of Medicine s Integrative and Complementary Academic Medicine Program. Dr. Broussard has experience in international medical missions and will be traveling to Honduras for 2 weeks in March with the Hackett-Hemwall Foundation for extensive training in prolotherapy. Dr. Broussard s research interests include bioregenerative applications such as stem cell, PRP and prolotherapy for management of acute and chronic injuries as well as medical foods and hormone optimization for the prevention and treatment of disease. Ronald Burns, DO, FACOFP, graduated from what is now Ohio University Heritage College of Osteopathic Medicine in Athens. Following the completion of his degree, he fulfilled his postdoctoral medical training at the Doctors Hospital and completed his internship at Doctors Hospital of Stark County in Massillon, Ohio, and his residency at Florida Hospital East Orlando. Dr. Burns holds a license in both Florida and Ohio and is also Board Certified in Family Practice. He serves on the national Board of Osteopathic Medical Examiners. He served the Florida Osteopathic Medical Association (FOMA) in many capacities, including president from , trustee of FOMA District Society 3 and chair of many committees. Dr. Burns has received the FOMA Physician of the Year award and the Distinguished Service Award. He served on the Governor Bush Health Information Infrastructure Board, the Florida Medicaid Formulary Study Panel, the Pharmaceutical & Therapeutics Committee and many other statewide committees. He continues to serve as a Delegate to the AOA House of Delegates. He holds a teaching appointment at Florida Hospital. Dr. Burns has guest lectured for many prestigious, national organizations such as Harvard Medical School and is currently in private practice in Orlando, Florida. Dr. Burns was appointed to serve on the Florida Board of Osteopathic Medicine in 2007, where he served as Chairman. He serves as a Trustee to the American Osteopathic Association and was appointed by the AOA President to serve on various AOA Committees including Chair of the Bureau of Osteopathic Clinical Education and Research and Department of Educational Affairs. He is also the Co-chairman of the FOMA Committee on Continuing Medical Education. Richard Crank, Jr., DO is a graduate of Kirksville College of Osteopathic Medicine. He completed his orthopedic residency at Midwestern University, St. James Olympia Fields Hospital, Olympia Fields, IL. Dr. Crank completed fellowships in Adult Reconstruction at Florida Orthopedic Institute in Temple Terrace, FL and his Adult Reconstruction/Trauma at Orlando Regional Medical Center in Orlando, FL. Dr. Crank is Board Certified in Orthopedic Surgery. Currently, Dr. Crank is in practice at Lakeland Regional Orthopedic Associates in Lakeland, Florida. Hilda De Gaetano, DO, FAAP, FACOP, is Assistant Dean of Pre-Clinical Education and Professor of Pediatrics at Nova Southeastern University College of Osteopathic Medicine (NSU-COM). Dr. De Gaetano received her doctor of osteopathic medicine degree from the New York College of Osteopathic Medicine in

3 1992. She then completed an osteopathic rotating internship at Northlake Regional Medical Center in Tucker, GA. Subsequently, she completed her residency in pediatrics at the Medical College of Georgia, in Augusta. She is certified by the American Osteopathic Board of Pediatrics and the American Board of Pediatrics. She is a fellow of the American Academy of Pediatrics and the American College of Osteopathic Pediatricians. She currently practices in general pediatrics and oversees the second year curriculum at NSU-COM. Linda Delo, DO, is a 1986 graduate of Nova Southeastern University College of Osteopathic Medicine. Dr. Linda Delo is a past President of the Florida Osteopathic Medical Association (FOMA). She has been in private family practice for over 25 years and was recently certified as an NCQA Level 3 Patient Centered Medical Home. She also participates and serves on the board of one of the first Advanced Payment Model Accountable Care Organizations. She served as Vice-Chair of The Bureau of Socioeconomic Affairs 2011 to 2013 and currently chairs its Innovations sub-committee at the American Osteopathic Association (AOA). She serves as a board member or advisor for other associations and committees, at both state and national levels. Jacob Duong, DO, received his osteopathic degree from Touro University College of Osteopathic Medicine in 2011, in which he did many electives with Integrated NMM such as obstetrics, pediatrics and family practice. Dr. Duong completed his traditional rotating internship in Southern California at Arrowhead Regional Medical Center, where he rotated at Western University s OMM Department and at Loma Linda University s PM&R Department. Currently he is completing his Integrated FP-NMM residency at Larkin Community Hospital in South Miami. He completed the following clerkships. Yakima Valley Memorial Hospital in Yakima, WA in Obstetrics and Gynecology integrated Neuromusculoskeletal Medicine with a focus on OMM in Obstetrics; The Holonomic Institute in Sebastopol, CA in Family Medicine, Osteopathic Manipulative Medicine and Integrative Medicine; with a focus on Fulford Percussion Hammers, Cranial Manipulation and Nutrition. Dr. Duong s specialty interests are osteopathic manipulative medicine with a focus on obstetrics and pediatrics. He enjoys running, weight training, indoor rock climbing and is a second degree black belt with the Kukkiwon (World Taekwondo Headquarters). Enrique Gongora, MD, is currently the Medical Director, Adult Cardiac Transplant, Heart Transplantation and Mechanical Circulatory Support for the Memorial Cardiac and Vascular Institute, Memorial Healthcare System in Hollywood, Florida. Dr. Enrique Gongora is a board-certified cardiac surgeon. He earned his medical degree from the Universidad del Rosario in Bogota in Dr. Gongora completed residency training in general surgery and a fellowship in surgical critical care at Washington Hospital Center in Washington, DC. He then completed a residency in cardiothoracic

4 surgery at the Mayo Clinic, Rochester. He then pursued further fellowship training in adult cardiac surgery and thoracic transplantation/mechanical circulatory support at the University of Pittsburgh Medical Center. Prior to joining Memorial Healthcare System, Dr. Gongora served as Director of Thoracic Transplantation at Scott & White Clinic-Texas A & M Health Science Center College Medicine (Temple, TX) and as Surgical Director of Heart Transplantation and Mechanical Circulatory Support at Drexel University College of Medicine, Hahnemann University Hospital (Philadelphia, PA). Felix Grucci, III, DO is a 2010 graduate of New York College of Osteopathic Medicine. He completed his residency in Psychiatry at the University of Arizona College of Medicine at South Campus. Dr. Grucci is currently enrolled in IHI Open School for practicum in Quality Improvement and completed IHI Open School Basic Certification in Quality Improvement. Dr. Grucci personal interests include music, alternative medicine, comedy, cooking, historic ships and Disney parks. Robert Hasty, DO, FACOI, attended the University of Miami where he completed his undergraduate studies with honors as a triple science major in Biochemistry & Molecular Biology, Chemistry and Biology in He then went on to receive his DO degree at Nova Southeastern University College of Osteopathic Medicine in He subsequently trained at Mt. Sinai Medical Center where he completed his training in Internal Medicine while receiving the Resident of the Year award for three straight years as well as receiving the Howard Kane Resident of the Year, being the first osteopathic physician ever to win that award in over forty years of academic medical training at Mt. Sinai. He also served as Chief Intern as well as Chief Resident during his training. Dr. Hasty was an Assistant Professor of Internal Medicine at Nova Southeastern University College of Osteopathic Medicine and the course director for the Radiology and Cardiovascular System Courses at NSU. He served as the Founding Program Director for the Internal Medicine Residency at Palmetto General Hospital. Additionally, he is considered a thought leader and national speaker in the field anticoagulation. He was awarded the NSU Stuey Award for Co-Curricular Advisor of the Year in He was the recipient of the coveted Golden Apple Award by the graduation class of NSU-COM in 2008, 2009, and Dr. Hasty was also named as a member of the 40 under 40 by Gold Coast Magazine because of his service to the South Florida community. In 2013, Dr. Hasty became the Vice President of Medical Education and the Regional Associate Dean for Campbell University School of Osteopathic Medicine in North Carolina. Dr. Hasty is board certified in internal medicine and is a Fellow of the American College of Osteopathic Internists. Anna Hayden, DO, is a 1988 graduate of the University of Health Sciences-Kansas City, Missouri. She completed her internship at Humana Hospital of the Palm Beaches and her residency at NSU- COM in She is board certified by Family Practice and is credentialed by the American Academy of HIV Medicine as an HIV specialist since In Dr. Hayden completed the health policy fellowship sponsored by Ohio University/NYIT. Dr. Hayden was appointed to the Board of Osteopathic Medicine in 2010, served as Chair in 2011, and is a current member. She served as a delegate to the Federation of State Medical Boards in 2011 and is a current member. Dr. Hayden is past president of the Florida Osteopathic Medical Association. She has served as a trustee for FOMA District Society 6 and as a delegate to the American Osteopathic Association since In 1996 she served as the first female President of the Broward County Osteopathic Medical Association and

5 remains active on their Board of Governors. In 2000 she served two terms as Chief of the Family Medicine Department at Broward General Medical Center. She is the current chair of the Florida Medicaid Drug Utilization Review and Physician Prescribing Review Panel boards. She is involved in Community Health Center s and Broward General Medical Center s Quality Care Committee. She also serves on Community Health Services Pharmacy and Therapeutics Committee. Dr. Hayden has been caring for the community of downtown Ft. Lauderdale for over 20 years at Broward Health s Specialty Care Center. Leonard Hock, Jr., DO, is CMO of TrustBridge Palliative Care and Advanced Illness Management programs. He is a certified Medical Director and takes pride in serving the Florida Medical Directors Association as a member and contributor to the education process. He served as Senior VP of Medical Services for Covenant Hospice serving the panhandle of Florida and southern Alabama. Prior to that he was in private internal medicine practice in Kansas City for more than 25 years. He has a special focus and interest in geriatric patients and end of life care, with board certifications in internal medicine, geriatrics and hospice/palliative care. During his medical career in Kansas City, Dr. Hock was the health care reporter for FOX 4 Television News. For thirteen years he was seen daily on Housecalls, where he spoke on current and pertinent health topics. In 2009, Dr. Hock was elected a Master Fellow in the Gillum Society of Master Fellows. This membership is the highest honor awarded by the American College of Osteopathic Internists for outstanding contribution and service in his profession. Donald Jablonski, DO, FAODME, FACOFP, is a 1980 graduate of Chicago College of Osteopathic Medicine. Dr. Jablonski served as the President of the Florida Osteopathic Medical Association (FOMA) from and FOMA District Society IV President for 10 years. Dr. Jablonski is former Associate Professor of Family Medicine at the Ohio University-COM. He is past president of the North Carolina Society of the ACOFP and North Carolina Osteopathic Medical Association. Dr. Jablonski was recognized as the first osteopathic physician appointed to the North Carolina Medical Board in its 150 year history, he also served as President of the North Carolina Medical Board from Robert S. Juhasz, DO, AOA President, graduated from the Kansas City (Missouri) University of Medicine and Biosciences College of Osteopathic Medicine. He completed his internship and internal medicine residency training at what is now South Pointe Hospital. Dr. Juhasz is a fellow and diplomat of the American College of Osteopathic Internists, and a fellow of the American College of Physicians. Currently, Dr. Juhasz is the 118th president of the AOA. He also serves as president of Cleveland Clinic s South Pointe Hospital in Warrensville Heights, Ohio. One of Dr. Juhasz's goals as president is to facilitate more osteopathic medical research, which, he says, would provide DOs fact-based research to communicate their distinctiveness to their patients, their colleagues and the world. A pioneer in the adoption of electronic health records, Dr. Juhasz was actively involved with the implementation of the electronic medical record system at the Cleveland Clinic before many physicians used these systems. In 2005, he participated in a panel with President George W. Bush to discuss the benefits of electronic medical records.another project Dr. Juhasz has been instrumental

Dr. Juhasz served as an associate dean at OU-HCOM, where he is an associate clinical professor of medicine. He is also assistant clinical professor at the Cleveland Clinic s Lerner College of Medicine at Case Western University in Cleveland. Dr. Juhasz s commitment to providing excellence in patient care has not gone unnoticed. He was awarded the Phillips Medal of Public Service by OU-HCOM, received the Ohio Osteopathic Association s Trustee Award and was named an Honorary Alumnus of OU-HCOM in Joshua Lenchus, DO, RPh, FACP, SFHM, holds a pharmacy degree from the University of Florida, and graduated from Nova Southeastern University College of Osteopathic Medicine in Having completed a rotating internship at Broward General Medical Center in Fort Lauderdale, he pursued training in internal medicine at Jackson Memorial Hospital in Miami. He currently works as a hospitalist at Jackson Memorial Hospital for the University Of Miami Miller School Of Medicine. He has extensive hospital administrative and committee involvement and, in 2012, was elected the first osteopathic President of the Jackson Health System medical staff and chairman of its medical executive committee, in its then-94 year history. He was subsequently re-elected in June One of Dr. Lenchus s special areas of interest is patient safety. Aside from creating a crisis management simulation course, in 2007 he developed a novel curriculum centered about simulation-based invasive bedside procedural instruction. Now in its eighth year, the program has transformed the way procedures are taught and performed, decreasing complications thereby leading to safer patient care. He has conducted numerous workshops at various conferences, garnered nearly $1.5M in grant funding, and his team s work has been published in peer-reviewed journals. Outside the hospital, Dr. Lenchus is incredibly involved, serving as a leader, in multiple county, state and national medical associations, including the Florida Osteopathic Medical Association, Florida Medical Association, the American College of Physicians, and the Society of Hospital Medicine. He has a passion for health policy, serves on public policy committees, and has presented this topic in a variety of venues. He has received a number of teaching and University awards. In 2014, he was recognized as the Soref JCC Humanitarian of the Year, and as a Distinguished Alumnus of Broward College. Robert T. Leonard, Jr., DO, FISHRS, FAACS, graduated from the University Of New England College Of Osteopathic Medicine in He completed his internship at Metropolitan General Hospital and his Residency at Parkview Hospital in Toledo, OH. Dr. Leonard also completed a preceptorship in hair transplants surgery. Dr. Leonard has been recognized as a Fellow of the American Academy of Cosmetic Surgery and of the International Society of Hair Restoration Surgery. Currently, Dr. Leonard is in private practice as a hair restoration specialist in Cranston, Rhode Island, and is board certified in hair restoration surgery. He has served on a number of administrative boards and held leadership in several societies, including Past-President of the International Society of Osteopathic Physicians and as a Trustee of the American Board of Hair Restoration Surgery. Dr. Leonard provides his patients with a customized hair loss treatment. Jacqueline Miller, has worked with the Florida Coalition Against Domestic Violence (FCADV), for three years, as the InVEST Statewide Coordinator. InVEST is a coordinated community response program designed to reduce the number of domestic violence-related deaths. Jacqueline coordinates and provides technical assistance to 13 InVEST program advocates and their law enforcement

7 partners who partner together to reduce intimate partner homicides. Over the past year, Jacqueline served as a member of the technical assistance and training workgroup for the Peace of Mind (POM) Florida project. The Peace of Mind Florida, is a collaborative between FCADV and the Brain Injury Association of Florida (BIAF). Jacqueline came to FCADV from Chicago where she worked as an advocate for battered women and their children for 17 years. In Chicago, Jacqueline worked for the Connections for Abused Women and Their Children s (CAWC) Hospital Crisis Intervention Project (HCIP). She has provided training for approximately 1,500 medical providers on how to identify patients who are experiencing abuse. Florida communities have benefited from her, extensive, community organizing experience in efforts to increase survivor safety and batterer accountability. Doris Newman, DO, FAAO, graduated from the University of New England College of Osteopathic Medicine (UNECOM) in She completed a Traditional Rotating Internship and a PGY-2 year in internal medicine at Worcester Medical Center in Worcester, MA, and two additional training years in the Neuromusculoskeletal Medicine and Osteopathic Manipulative Medicine (NMM/OMM) residency program at UNECOM serving as chief resident in her final year. Fulfilling one of her life goals, Dr. Newman was honored to earn the Fellowship of the American Academy of Osteopathy (FAAO) in March Following residency, Dr. Newman was appointed as Assistant Professor of Osteopathic Principles and Practices (OPP) at UNE advancing to Associate Professor and serving as the interim Director of Medical Education, Residency Program Director for the NMM/OMM and NMM +1 residencies and Co-Program Director for the Integrated Family Practice and OMM residency. Since 2010, Dr. Newman has been at Nova Southeastern University College of Osteopathic Medicine (NSUCOM) after being appointed Associate Professor of OPP and more recently as Director of the Department of Rural and Urban Underserved Medicine. Dr. Newman has been very committed to her professional organizations serving on the Board of Trustees for the American Academy of Osteopathy having been elected as their President-Elect in March of 2014 and will automatically advance to the office of President in March 2015 as well as on the Postdoctoral Training and Review Committee (PTRC) for the American Osteopathic Association (AOA), on the National Faculty for the National Board of Osteopathic Medical Examiners (NBOME) in the Department of OPP. She lectures locally, nationally, and internationally on various topics concerning Neuromusculoskeletal Medicine and Osteopathic Manipulative Medicine with a particular emphasis in the osteopathic treatment of the pediatric patient as well as osteopathic medical education. Naushira Pandya, MD, CMD, is an Associate Professor and the Chair of the Department of Geriatrics at Nova Southeastern University College of Osteopathic Medicine. Dr. Pandya is the Principal Investigator and Project Director of the federally funded Geriatrics Education Center at NSU and is the certified medical director of long-term care facilities. Her most recent accomplishment is as a Fulbright Senior Specialist scholar in Geriatrics. She has an active nursing home and ambulatory care practice in Geriatrics and Endocrinology. Previously she has worked with the Arizona Long- Term Care System, served as Assistant Professor at the University of Missouri, Kansas City, and as a geriatrician at William Beaumont Hospital in Detroit, MI. She is a reviewer for geriatric journals, a member of the editorial board for the Journal of the American Medical Director s Association. She is a nationally recognized authority on diabetes in the elderly and has published in the areas of diabetes, osteoporosis, hypopituitarism, and anemia in the elderly. She is the past Chair of the American Medical Director s Clinical Practice Guideline Steering Committee and past Chair of the

8 Clinical Practice Committee. She is a member of the Education Committee, a board member of the Florida Medical Director s Association, and a member of the Governor s Advisory Committee on Cancer Control in Florida. Dr. Pandya earned her medical degree at the University of London, England. She completed her Internal Medicine residency at Maricopa Medical Center in Phoenix and her fellowship in Geriatrics and Endocrinology at the University of Michigan. She is board-certified in Internal Medicine, Geriatrics, and Endocrinology. Siddharth Pandya, DO, received his DO degree from the Des Moines University College of Osteopathic Medicine in of Health Sciences in Dr. Pandya completed his internship at Botsford General Hospital and his Residency in Family Practice in Michigan. He completed a residency in radiology at Providence Hospital in Michigan and a fellowship in Neuroradiology at UNM Department of Radiology in New Mexico. He currently holds licenses in Florida, Michigan and New Mexico. Scott Pearlman, DO, is a 2000 graduate of Nova Southeastern University College of Osteopathic Medicine. Before becoming a physician, he received his Bachelor of Arts in Psychology from the University of Pennsylvania. Dr. Pearlman completed his Neurology residency at the Stony Brook University Hospital and Medical Center in Stony Brook, NY. He also completed two fellowships at Stony Brook University Hospital including one in vascular neurology and the other in clinical neurophysiology. Dr. Pearlman is a diplomat of the American Board of Psychiatry and Neurology as well as an assistant professor and team neurologist at Florida Atlantic University. He currently practices in Boca Raton, Florida. Dennis H. Penzell, DO, MS, FACP, received his DO degree from the New York College of Osteopathic Medicine in He completed a residency in Internal Medicine at the St. Barnabas Medical Center, Livingston New Jersey in He is Board Certified and is a Fellow of the American College of Physicians. From he completed a National Health Service Corps Scholarship obligation at the Suncoast Community Health Centers, Inc, (formerly the Ruskin Migrant and Community Health Center) in Ruskin, Florida and remained as Medical Director from 1989 until January From 2006 to 2010, Dr Penzell served as a consultant for the Nova Southeastern University and University of South Florida Area Health Education Centers, the Florida Farmworkers Association, and the Florida Agency for Health Care. In September 2010, Dr. Penzell began his dream career position by joining the Internal Medicine faculty of Nova Southeastern University, College of Osteopathic Medicine as an Associate Professor of Medicine. Dr. Penzell has Tobacco Treatment Specialist Certifications in New Jersey and Florida, and his special interests include treatment of the tobacco addiction, health care for the medically underserved and the effects of pesticides. Bhagirathy Sahasranama, MD, is Medical Director of Henderson Behavioral Health, a large CARF accredited behavioral health organization in Florida that serves over 30,000 individuals each year. She is also the child psychiatrist consultant for several organizations such as Camelot Community Care, Children s Harbor and Kids in Distress. Dr. Sahasranaman has served on various committees and panels at the district and state levels. She has been a member of the expert panel for the Florida Medicaid Drug Therapy Management Program for Behavioral Health that has developed best

9 practice medication guidelines for treatment of various behavioral health disorders in children and adults. She has provided numerous presentations, workshops, and trainings and has received several awards and recognitions. Dr. Sahasranaman is Clinical Assistant Professor, Department of Psychiatry, Nova Southeastern University College of Osteopathic Medicine and is also on the faculty of the Department of Psychiatry, Florida International University College of Medicine. Dr. Sahasranaman is Board certified in general psychiatry and in child and adolescent psychiatry and is a Distinguished Fellow of the American Psychiatric Association. Stavros Papandreou, DO is a 2009 graduate of Nova Southeastern University College of Osteopathic Medicine. He served as Chief Resident his Emergency Medicine Residency at St. Mary Mercy Hospital Michigan State University and Sports Medicine Fellowship at Larkin in Miami. Currently, Dr. Papandreou is an Emergency medicine physician at Broward Health Medical Center, Broward Health Imperial Point. On a personal note, Dr. Papandreou was born in Dusseldorf, Germany, grew up in Athens, Greece, and moved to the United States in 1998 to pursue a career in medicine. Daniel Shaw, PhD, Med, is Associate Professor and Vice Chair of The Department of Psychiatry and Behavioral Medicine in the College of Osteopathic Medicine at Nova Southeastern University (NSUCOM) in Ft. Lauderdale, Florida. A near native of Miami, Dr. Shaw received his Bachelors of Science, Masters, and Ph.D. degrees all from the University of Florida (UF) in Gainesville (Go Gators!). While at UF, he studied under Dr. Art Combs (Perceptual Psychology), Dr. William Purkey (Invitational Theory and Practice) and Dr. Betty Siegel, President Emeritus Kennesaw State University. He has been a psychology professor for over 40 years with the last 25 as a medical educator working exclusively with student physicians, residents and faculty at NSUCOM. Dr. Shaw is married to the same wonderfully patient woman, and they have two sons; the oldest is a graduate of UF and the University Of Maryland School Of Law. Dr. Shaw has a wide variety of hobbies including long distance cycling, amateur astronomy, electric/acoustical guitar, metal detecting, digital photography and has played drums for 50 years. Joel D. Stein, DO, FAAO, FAOASM, graduated from Kirksville College of Osteopathic Medicine (KCOM) in He was an undergraduate fellow in the departments of Anatomy and Osteopathic Manipulative Medicine at KCOM. Dr. Stein completed his internship at Humana Hospital South Broward in Hollywood, Florida. He is board certified in Family Practice, Osteopathic Manipulative Medicine, and Sports Medicine. Dr. Stein is a fellow of the American Academy of Osteopathy, the American Osteopathic Academy of Sports Medicine, and the American College of Osteopathic Family Physicians. Dr. Stein is Past President of Broward County Osteopathic Medical Society. He is currently a Trustee for FOMA District Society 6, a delegate to the AOA House of Delegates and has served as a FOMA Director-at-Large. He also serves on the Board of the Florida Society, ACOFP. Dr. Stein is a past board examiner for the American Osteopathic Board of Neuromusculoskeletal Medicine (AOBNMM) and currently is the Vice-Chair for the Conjoint Board Committee in Sports Medicine. Currently, Dr. Stein is the Medical Director at the Institute for Non-Surgical Orthopedics and Center for Osteopathic Manipulative Treatment in Ft. Lauderdale, Florida. He is also Clinical Associate Professor at Nova Southeastern University - College of Osteopathic Medicine, Professor

10 Emeritus at the University of Miami - School of Medicine, Visiting Clinician for the American Academy of Osteopathy, and serves on the AAO Bureau of Experts. David Thomas, MD, JD, EdD. Apart from his extensive military service in Vietnam as a former U.S. army major, Dr. David Thomas is a former elected member of the Florida State Legislature. His experience as a senior state official in the executive branch, which was partly responsible for Florida s largest HMO, has given him the ability to lead CBAP curriculum development in the intermediate and advanced levels for clinical preparation in all-hazards events. Dr. Thomas holds an M.D. degree from the University of Miami and was trained in general and chest surgery at that institution, as well as ophthalmology at the University of South Florida. He is also a practicing attorney with a J.D. degree from Stetson College of Law and a M.Ed. degree in health education from Nova Southeastern University. Dr. Thomas is the current NSU chair of the Department of Surgery and the Division of Correctional Medicine. Don Waechter, CPA, is a graduate of the University of South Florida with a degree in Accounting. After working several years as a CPA, Don moved into the investment advisory sector of the financial services industry where he functioned in roles of administration, compliance, business development, and product and division management. He has managed and successfully grown three separate investment advisory businesses. Upon entering the information security business, Don spearheaded an effort to serve the healthcare industry. He now serves as Managing Partner of Health Compliance Associates, LLC where his responsibilities include administration, management, and business development. Nathan Widboom, DO, is a graduate of Touro University College of Osteopathic Medicine in Mare Island, California. Dr. Widboom was involved in many campus clinics and clubs which included: Co-coordinator of the Osteopathic Manipulative Medicine Clinic for Berkely s free suitcase clinic and co-creator of the Touro University student Run Health Clinic. He completed an integrated family practice and neuromusculoskeletal medicine residency at Larkin Community Hospital in South Miami, Florida. Kavin Williams, CPC, CCP, is a Certified Professional Coder from the American Academy of Professional Coders (AAPC). Mr. Williams responds to member inquiries regarding correct coding conventions, denial of payment and in public and private payment policies. Mr. Williams reviews proposed CPT coding changes, attend the Relative Update Value Committee (RUC) meetings and coordinates responses between the AMA and the AOA Coding & Reimbursement Advisory Panel, and is responsible for monitoring changes in physician regional Medicare policies. He has extensive work experience in the physician practice and reimbursement areas. Jason D. Winn, Esquire, is a 1996 graduate of the University of Maryland and received his Juris Doctorate from Nova Southeastern University - Shepard Broad Law in Ft. Lauderdale, Florida. Mr. Winn was admitted to the Florida Bar in September From 2001 until 2004, Mr. Winn worked for the Assistant Public Defender in the Fifth Judicial Circuit where he conducted over 15 jury trials,

11 numerous non-jury trials, and many hearings including, violations of probation, restitution, and early termination motions for defendants in Juvenile, Misdemeanor and Felony Court. Mr. Winn was also an adjunct professor at Lake Sumter Community College teaching Business Law during this time. In 2003, Mr. Winn was appointed by Governor Bush to serve a one year term on the Judicial Nominating Commission for Judicial Compensation Judges. From Mr. Winn worked for the law office of Clyde M. Taylor, Jr. focusing on both state and federal criminal defense and parole violation hearings. In 2006, he opened his own practice, where he is managing partner and continues to focus on criminal, administrative, governmental, civil, family law, wills and trusts. Mr. Winn currently serves as general counsel for the Florida Osteopathic Medical Association (FOMA). He also currently serves as executive director and general counsel for the Florida Society of Hearing Healthcare Professionals (FSHHP). He is a member of the Florida Bar, Tallahassee Bar, Legal Services of North Florida, a lifetime member of the state Florida Association of Criminal Defense Lawyers, and a member of the Tallahassee FACDL chapter. As a member of the Tallahassee Bar, Mr. Winn volunteers his legal services to the Wakulla County Senior Citizens Center, Legal Services of North Florida, and is also a volunteer with Justice Teaching. Penelope P.
