2 What is our mission? Promote health and minimize health disparities by increasing access to health care and other public benefit programs: Medical Insurance & Health Home Food Assistance (Basic Food Program/WIC) Breast, Cervical & Colon Health Program Energy Assistance Child Care Assistance Advocating for clients Working with community partners: City of Seattle, DSHS, Basic Health, Washington Health Program, Children s Alliance & WithinReach) Provide Trainings (Medical, Basic Food, Energy Assistance & Dental)
3 How do we do it? PeoplePoint is jointly sponsored by the City of Seattle Human Services Department & Public Health. Our goal is to help clients apply for all available programs at 1 time. Promotora Program uses culturally "tailored community" based outreach approaches, such as the Promotores de la Salud de los Niños (Children's Health Promoters) Program, which is comprised of volunteer Latino community health workers. Outreach is centered around the client. We go to where the clients are at. We have 30 regular outreach locations. And over 50 different agencies that we provide technical and material updates on a regular bases. ABCD Program collaborative effort between several partners in the public and private sectors with focuses on preventive and restorative dental care for Medicaid-eligible children from birth through age five with an emphasis on the first dental visit by age one. Team Members are trained, experienced, bilingual and bicultural staff who average about 10 years of experience. They help clients with eligibility questions, filling out application and follow-up on submitted applications. We advocate for the client(s) when necessary.

5 Strategies Direct outreach at various locations throughout King County. Train Community Based Organizations on how to process applications, understand eligibility and follow-up on applications. We provide technical assistance. Community Based Organizations (CBO) are trained on various programs but clients &/or applications are sent to Public Health for processing and follow-up back to CBO. 6 South King County Outreach South King County Public Schools Highline, Federal Way, Renton, Kent, Auburn, Enumclaw and Tahoma We do outreach with every school district in South King County. We hold regular scheduled meetings with school nurses, other school educators and officials in which we offer all of our services and assistance. That includes processing and follow up on applications, researching client eligibility, etc.. Highline School District Direct outreach to the Highline School District sites weekly where staff has been trained in the application process and our follow up for processing.

8 South King County Outreach- SPECIAL PROJECT Hunger-Free Communities Regional Coalition Focusing on increasing food security among low income children, seniors, immigrants and refugees South King County Child Nutrition Collaborative (CNC) Increasing participation in Federal Nutrition Programs (e.g. Basic Foods, SNAP, School Breakfast/Lunch, Summer Lunch Programs) and identifying innovative hunger relief strategies. Build on best practices for addressing childhood hunger in South King County
9 Outreach--Seattle/North King County/East King County/Tukwila Seattle Public Schools We train the Teen health centers & Wellness Centers staff in the Seattle/King County area. We provide direct outreach to 3 Teen Health Centers; Process the applications from 3 additional Teen Health Centers & 1 Wellness Center. SPS participates in the Free-Reduced Lunch Program. Tukwila School District Direct outreach at Foster High School weekly. We ve trained the staff at the Middle School to complete applications. Those applications are sent to us for processing & follow-up. Our Promotora s conduct outreach at the 4 elementary schools in the District. We participate in the annual Immunization event and Enrollment events. And we host the Latino Family Nights (2 each year). Tukwila participates in the Free-Reduced Lunch Program. Bellevue School District Direct outreach at Lake Hills Wrap Around Program; Outreach staff is on the Latino Family Group for Bellevue Schools; Free-Reduced Lunch Program Community College (North Seattle, South Seattle, Shoreline, Central Seattle, Seattle Vocational Institute & Everest College (2 locations).

11 ABCD Program King County What is ABCD? Access to Baby and Child Dentistry (ABCD) is a collaborative effort between several partners in the public and private sectors. ABCD focuses on preventive and restorative dental care for Medicaid-eligible children from birth through age five with an emphasis on the first dental visit by age one.
12 ABCD Program King County Why it Works Education is at the heart of the ABCD program. Partnerships - Training Increased access to dental care throughout King County Increased number of dental services (i.e. 3 fluoride varnish applications per year, 2 Family Oral Hygiene Instruction consultations per year per child, etc.)
13 ABCD Program King County Why it Works Assistance in finding a dentist that will fit your location, timing, and language preferences. Assistance in coordinating transportation to and from the dental office. Assistance in coordinating interpreter services for the dental visit. Outreach Efforts

16 Lessons Learned Developing outreach locations & relationships take time. Do not give up! Most of our locations were developed at least 3 years ago. New locations were added in Do your research are there other agencies working at that location. Create a good database that will allow your agency to capture data, contacts and create reports. Review the number of contacts and number of applications/enrollments from each site. Don t be afraid to change the strategy or change locations. Train staff to follow-up on all submitted applications.
17 Lesson learned continues Understand cultural differences. For example there is a difference between the Black American community & the African Immigrant community. Learn the differences. Hire staff that look and sound like the client if possible. Do out of the box outreach try something different. Create partnerships with DSHS & local government. If your outreach is based on funding, find a way to continue to work with the community when the funding is over. Developing partnerships requires time.
