2 Fayette County Behavioral Health Administration, in partnership with Value Behavioral Health of Pennsylvania (VBH-PA), is seeking to develop community-based outpatient substance abuse services for Fayette County HealthChoices members. These outpatient services are to be physically located in Connellsville, Fayette County, Pennsylvania. Additionally, this RFP opportunity is limited to currently licensed substance abuse outpatient treatment providers who are currently physically located in Fayette County and currently providing Drug and Alcohol outpatient services to Fayette County members. BACKGROUND Outpatient drug and alcohol treatment utilization in Fayette County has historically been available in the Connellsville area. The rural nature of the county and the lack of available public transportation, contribute to low utilization numbers overall in substance abuse outpatient appointments. The provider selected through this process will be responsible for marketing their services to area residents and for working with other community-based services to implement drug and alcohol services in the Connellsville area. Monthly statistics on contacts and services provided will be required to be submitted to the Fayette County Behavioral Health Administration. DRUG & ALCOHOL OUTPATIENT SERVICES IN CONNELLSVILLE The time frame for opening a drug & alcohol outpatient treatment facility in Connellsville will be agreed upon in writing by Fayette County Behavioral Health Administration and the selected provider, prior to the award of the contract. TARGET POPULATION: These outpatient services are specifically designed to serve HealthChoices eligible adults, adolescents and children who are residents of Fayette County and in need of outpatient substance abuse treatment services. SERVICE OBJECTIVES: Increase the availability and access to community-based treatment services in Connellsville, Be capable of providing substance abuse services at varying levels of intensity, based on the needs of the individuals, Meet the treatment needs, and assist with the recovery needs, of adults and adolescents as they move toward long-term recovery. DEFINITION-COMPLIANCE: Substance Abuse Outpatient Services must adhere to the regulations outlined in the COMMONWEALTH OF PENNSYLVANIA DEPARTMENT OF HEALTH CHAPTER 711, SUBCHAPTER H. STANDARDS FOR OUTPATIENT ACTIVITIES These services must also adhere to the regulations outlined in the COMMONWEALTH OF PENNSYLVANIA DEPARTMENT OF HEALTH CHAPTER 704, STAFFING REQUIREMENTS FOR DRUG AND ALCOHOL TREATMENT ACTIVITIES and any other related chapter of this manual. 3 These services must also adhere to the regulations outlined in the COMMONWEALTH OF PENNSYLVANIA DEPARTMENT OF WELFARE CHAPTER 1223, OUTPATIENT DRUG AND ALCOHOL CLINIC SERVICES and any other related DPW regulations. MINIMUM QUALIFICATIONS Possess a full (not provisional) Pennsylvania license from the Department of Health, Division of Program Licensure for outpatient substance abuse services; Be able to become credentialed for these services with Value Behavioral Health of PA; Be a currently licensed substance abuse outpatient treatment provider who is physically located in Fayette County; Be currently providing Drug and Alcohol outpatient services to Fayette County members; Be willing and able to provide substance abuse outpatient treatment services in Connellsville, Fayette County; Applicant agency must be recovery focused, offering addiction treatment and recovery support to enhance pre-recovery engagement, recovery initiation, long-term recovery maintenance, and the quality of personal/family life in long-term recovery; Applicant agency must not be on a corrective action plan, performance improvement plan, or payment recovery plan with VBH-PA. DISQUALIFIED AGENCIES The following conditions would disqualify an agency from responding to this RFP: Agency has a provisional Pennsylvania license from the Department of Health, Division of Program Licensure for outpatient substance abuse services; Agency is on a corrective action plan with VBH-PA; Agency is on a performance improvement plan with VBH-PA; Agency is on a payment recovery plan with VBH-PA. DIRECTIONS: 1. Be sure your agency meets all the minimum qualifications prior to completing your response. 2. Respond to each question as completely as possible. 3. Respond to the questions in the sequence that they appear in the RFP. 4. There is no page limit to responses. 5. All pages must be sequentially numbered. 6. All responses must be typed using 12 pt. font. 7. Additional supporting or requested documentation should be attached as Appendices. PROPOSAL REVIEW: VBH-PA and Fayette County Behavioral Health Administration reserve the right to reject any and all proposals received as a result of this RFP, and to negotiate separately with competing applicants. If all proposals are unacceptable, the County reserves the right to reject the proposals and to issue a new RFP. The County also reserves the right to reject a proposal at any time during the process. Applicant agencies may be requested to do oral presentations for VBH-PA and Fayette County. September
4 UPON AWARD: Secure written support and approval from Fayette County Behavioral Health Administration to locate outpatient substance abuse services in Connellsville. The provider can expect to be reimbursed at the standard VBH-PA rates for substance abuse treatment services at the outpatient level of care. If deemed necessary, the provider may be asked to contract for these substance abuse services from the Fayette County Single County Authority, Fayette County Drug and Alcohol Commission, Inc. There is no guarantee by the County or VBH-PA of any anticipated level of service need for this population. There is no commitment by the County or VBH-PA on anything beyond what is directly stated in this RFP. The award resulting from this RFP does not ensure that the need in the community is sufficient to support the development and/or sustainability of this service. September
5 Please provide the following information regarding your interest in, or ability to develop and offer, substance abuse outpatient treatment services for HealthChoices members residing in Fayette County. 1. Please provide a brief overview of the agency, its history, mission, and experience serving this target population. Please describe in detail the agency s background, experience and present activity, as well as the philosophy of the organization which will enable it to successfully provide the proposed service to this target population in the following areas: a. At the administrative level (5 pts) b. At the direct service level (5 pts) 2. Describe the agency s statement of purpose in offering outpatient substance abuse treatment services to this population. Outline how the outpatient program would fit into the agency s mission and articulate the agency s goals and objectives in meeting the needs of individuals identified as benefiting from these treatment services. Describe any of the agency s unique characteristics that would directly benefit these individuals. (10 pts) 3. Describe in detail some of the characteristics of the HealthChoices members to be served through these services and how these characteristics would be addressed. Also include a statement of nondiscrimination. (5 pts) 4. Describe your agency s commitment and experience in collaborating and coordinating with other community service agencies, supports and collateral organizations. (10 pts) 5. If the agency considers itself to be a specialist in treating certain diagnostic categories or disorders, please describe (explain why) and provide any relevant supporting documentation. Also provide any documentation/verification of expertise in serving special populations. (5 pts) 6. Address the following program details for the drug & alcohol outpatient levels of care at the agency s current location in Fayette County and at the projected location in Connellsville: a. Describe the agency s treatment staffing structure for each of these levels (e.g. number of administrative staff, direct treatment staff, clerical support staff and all other staff to be related to the delivery of this service), and the job responsibilities of each program staff member. (10 pts) b. Please attach an organizational chart depicting the structure of this program. Also attach an organizational chart depicting where this program fits into the agency s overall organizational structure. (5 pts) c. Provide a complete service description for this substance abuse program. Describe in detail the program components (e.g. engagement efforts, criteria for admission, September
6 referral and intake process, length of service, provision of individual vs. group therapeutic sessions, availability, family engagement, assessment and involvement, and direct treatment service modalities) to be employed with this target population and how these will be managed throughout the facility. (10 pts) d. Describe how the agency expects to conduct treatment planning processes (e.g. What instruments will be used to perform ongoing continued care assessments? What will be the agency s treatment review process and discharge planning process? What mechanism will be used to ensure that the member and other agencies [if relevant] will be included in the treatment planning process?). (10 pts) e. Describe how the agency will work closely with the individual s family or significant others to both address clinical issues and build upon specific strengths within the relationship, as a part of treatment planning. Outline how each program will engage and utilize the family in developing and implementing an individual s treatment plan. (10 pts) f. Describe the agency s experience and approach to addressing individuals with cooccurring disorders (i.e. mental health and/or intellectual disability, in addition to substance abuse). (10 pts) g. Describe the role cultural competency plays in the delivery of the proposed services and throughout the agency. Outline specific activities in the proposed program which will address and improve cultural relevance of the program for individuals. Explain how cultural differences will be integrated into these programs and in what capacity can cultural competency serve as the mechanism for addressing behavioral health disparities in these programs and in the agency. (5 pts) h. Define the basic elements of the outpatient program, include program specific goals and objectives, and describe how these goals and objectives will be measured. Clearly outline the specific services to be provided, including hours of activity, number of HealthChoices members to be served and staff/consumer ratios. Describe the provider s plan to develop other program related resources. (10 pts) i. Describe the agency s plan for continuous quality monitoring and improvement. Outline what outcome measures will be in place which demonstrate the effectiveness of your program. (10 pts) Outcome measures should be based on resiliency-oriented principles such as: Improving quality of life Individual and family satisfaction Improving community and educational integration Participation in meaningful activities and social relationships Discharge Choice September
7 Peer and Community supports Achieving positive clinical outcomes j. Describe what measures will be in place to identify discharge indicators. Be sure to fully explain how the agency will identify and address treatment progress, quality of life indicators and individual feedback in an evaluation process. (5 pts) k. Describe the agency s philosophy, experience and plans for incorporating recovery into the outpatient services. (5 pts) l. Provide a copy of the agency s current drug-free outpatient PA Department of Health license. (5 pts) 7. Describe the agency s proposed plan for obtaining any relevant certifications or required training of the outpatient treatment staff and for ensuring that other staff development and training opportunities will be ongoing. (5 pts) 8. Provide any other information the organization would like to offer, such as letters of recommendation, memorandums of understanding, potential location of facility, etc. to support the proposal.
