1 OFFICIAL REFERENDUM BALLOT TOWN OF LEBANON, MAINE JUNE 12, 2012 REFERENDUM 1: Shall the Town of Lebanon vote to authorize the Board of Selectmen to expend up to $70, for structural repairs, including lifting and installing a new concrete foundation under the Old Lebanon Town Hall on Center Road. Budget Committee recommend: Yes 7 No 3 Abstain 0 REFERENDUM 2: EXPLANATION: This is a two part Warrant Article. The first section A, seeks an appropriation of funds to hire a Town Manager under the existing vote of the Inhabitants of the Town of Lebanon to adopt a Town Manager Plan. The second section B, is a vote to rescind the Town Manager Plan that was previously adopted. If you vote YES on section A, then you should not vote on section B. If you vote NO on section A, then please vote on section B. A. Shall the Town of Lebanon vote to fund a position of Town Manager in the amount of $93, Selectmen recommend: Yes 0 No 3 Abstain 0 Budget Committee recommend: Yes 0 No 10 Abstain 0 NOTE: If you voted YES on 2A, DO NOT vote on 2B. B. Shall the Town of Lebanon vote to revoke the statutory Town Manager Plan, which had previously been adopted by a prior town vote. Budget Committee recommend: Yes 4 No 6 Abstain 0 REFERENDUM 3: Shall the Town of Lebanon vote to raise and appropriate the sum of $319, for government expenses for the town office including the Selectmen and Animal Control Officers. REFERENDUM 4: Shall the Town of Lebanon vote to raise and appropriate the sum of $80, for the contingency fund. REFERENDUM 5: Shall the Board of Selectmen be authorized to sell property or grant an easement to Central Maine Power for the expansion of their transmission line through Town owned property. This authorization shall remain in effect until rescinded by town meeting vote.

2 REFERENDUM 6: Shall the Town of Lebanon vote to raise and appropriate the sum of $8, for General Assistance for residents in need. Budget Committee recommend: Yes 9 No 1 Abstain 0 REFERENDUM 7: Shall the Town of Lebanon vote to raise and appropriate the sum of $116, for all insurances for the town, except for Lebanon Rescue Department. REFERENDUM 8: Shall the Town of Lebanon vote to raise and appropriate the sum of $25, for legal fees. REFERENDUM 9: Shall the Town of Lebanon vote to authorize funding at last year s ( ) level for the town office and essential services whose new appropriation is not approved by this year s referendum vote, until such time that any new funding is authorized. (Essential services are: Fire, Codes Enforcement, Animal Control, Transfer Station and Highway Departments) Budget Committee recommend: Yes 4 No 4 Abstain 2 REFERENDUM 10: Shall the Town of Lebanon vote to raise and appropriate the sum of $8, for the updating of tax maps. REFERENDUM 11: Shall the Town of Lebanon vote to raise and appropriate the sum of $313, for the operation of the Lebanon Transfer Station. REFERENDUM 12: Shall the Town of Lebanon vote to authorize the Board of Selectmen to enter into a lease agreement for a new fire truck to replace Engine 1. Once in service, the current Engine 1 would be sold to the highest bidder. The price of the new fire truck and equipment would not exceed $375, and the lease payment would be $80, per year for five years with a $1.00 buyout at the end. Selectmen recommend: Yes 0 No 3 Abstain 0 Budget Committee recommend: Yes 7 No 3 Abstain 0

3 REFERENDUM 13: Shall the Town of Lebanon vote to raise and appropriate the sum of $109, for the operation, upkeep and purchase of equipment and supplies for the Lebanon Fire Department and the Lebanon Fire Station. REFERENDUM 14: Shall the Town of Lebanon vote to raise and appropriate the sum of $1, for the fire hydrants in South Lebanon per contract. REFERENDUM 15: Shall the Town of Lebanon vote to raise and appropriate the sum of $10, to fund the No Spray agreements with Central Maine Power and the Department of Transportation. Budget Committee recommend: Yes 8 No 2 Abstain 0 REFERENDUM 16: Shall the Town of Lebanon vote to raise and appropriate the sum of $978, for summer and winter maintenance and repair of highways, plus State Department of Transportation revenues. Budget Committee recommend: Yes 7 No 2 Abstain 1 REFERENDUM 17: Shall the Town of Lebanon vote to raise and appropriate the sum of $15, for the operation of the Martha Sawyer Community Library. Budget Committee recommend: Yes 5 No 5 Abstain 0 REFERENDUM 18: Shall the Town of Lebanon vote to raise and appropriate the sum of $8, for the operation of the Planning Board and for dues to the Southern Maine Regional Planning Commission. REFERENDUM 19: Shall the Town of Lebanon vote to raise and appropriate the sum of $3, for the Lebanon Recreation Department. Budget Committee recommend: Yes 8 No 1 Abstain 1 REFERENDUM 20: Shall the Town of Lebanon vote to raise and appropriate the sum of $1,100 for Summer Rec. Camp.

4 REFERENDUM 21: Shall the Town of Lebanon vote to pay the Selectmen $12.00 per hour plus expenses. Selectmen recommend: Yes 0 No 0 Abstain 3 REFERENDUM 22: Shall the Town of Lebanon vote to raise and appropriate the sum of $12, This includes $ for Memorial Day and $12, for the restoration and maintenance of cemeteries throughout the town which includes Veteran s Cemeteries. Budget Committee recommend: Yes 7 No 1 Abstain 2 REFERENDUM 23: Shall the Town of Lebanon vote to raise and appropriate the sum of $4, for the operation of the Emergency Management Agency. REFERENDUM 24: Shall the Town of Lebanon vote to raise and appropriate the sum of $38, to cover the cost of the Emergency Dispatching for the Rescue and Fire Departments through Sanford Dispatch. REFERENDUM 25: Shall the Town of Lebanon vote to charge interest on unpaid taxes at the rate of 7% per annum, and to set the date when taxes are committed for the Fiscal Year (July 1, 2012 through June 30, 2013) become due and payable as of October 15, 2012 and April 15, Said interest to be collected after October 15, 2012 and April 15, 2013, and to allow the Tax Collector to accept prepayment of taxes prior to the Tax Commitment Date. REFERENDUM 26: Shall the Town of Lebanon vote to set an interest rate of 3% as allowed by state law as the rate to be paid to taxpayers who pay amounts in excess of amounts finally assessed and authorize any such interest paid or abatements granted to be charged against the annual overlay. REFERENDUM 27: Shall the Town of Lebanon authorize the Selectmen to sell and dispose of any real estate acquired by the Town for the non payment of taxes, such sale to be by public sale in a manner to be determined by the Selectmen, and to give the Selectmen the authority to execute any deed to clear title for former owners who settle their tax debt in a manner satisfactory to the Selectmen. This authorization shall remain in effect until rescinded by town meeting vote. REFERENDUM 28: Shall the Town of Lebanon vote to raise and appropriate the sum of $35, for the combined position of Code Enforcement Officer/Health Officer/911 Director, which includes estimated mileage.

5 REFERENDUM 29: Shall the Town of Lebanon vote to authorize the Board of Selectmen, on behalf of the Town, to apply for, accept and expend Federal, State and other sources of revenue for Town purposes during the current and previous fiscal year. This does not include any monetary local match the Town may need for these grants. This authorization shall remain in effect until rescinded by town meeting vote. REFERENDUM 30: Shall the Town of Lebanon vote to raise and appropriate the sum of $18, to support the following agencies: York County Community Action $ 4,450 Caring Unlimited $ 1,568 So. Maine Agency on Aging $ 2,800 So. Maine Agency Senior Lunch Program $ 3,000 Southern Maine Parent Awareness $ 250 Counseling Services, Inc. $ 2,500 York County Juvenile Fire Safety $ 500 York County Shelter Programs, Inc $ 1,507 Child Abuse Prevention Council York Cty $ 300 Woodford Family Services $ 1,100 Sexual Assault Response Services $ 250 Total: $18,225 REFERENDUM 31: Shall an ordinance entitled Special Amusement Ordinance be enacted. A copy of the ordinance is available from the Town Clerk. REFERENDUM 32: Shall an ordinance entitled Exempting Eligible Active Duty Military Personnel from Vehicle Excise Tax be enacted. A copy of this ordinance is available from the Town Clerk. REFERENDUM 33: Shall the Town of Lebanon vote to appropriate all of the monies received from the State for snowmobile registrations to the Lebanon Trail Riders Club for the maintenance of their system or network of snowmobile trails, on the condition that those trails are open in snow season to the public for outdoor recreation purposes at no charge, and to authorize the municipal officers to enter into an agreement with the Club, under such terms and conditions as the municipal officers may deem advisable, for that purpose. REFERENDUM 34: Shall the Town of Lebanon vote to place any unexpended funds for the prior year into the general fund. The unexpended funds will be used to decrease or eliminate tax anticipation borrowing. Amounts up to 30% of the expended funds may be used to reduce the amount of commitment.

6 REFERENDUM 35: Shall the Town of Lebanon vote to accept Laverne Lane, off Heath Road, as a town road as approved by the Planning Board and Road Commissioner. REFERENDUM 36: Shall the Town of Lebanon vote to authorize the Board of Selectmen, on behalf of the Town, to waive tax lien foreclosures on properties when it is deemed to be in the best interest of the town to do so. This authorization will remain in effect until rescinded by a future town meeting vote. REFERENDUM 37: Shall the Town of Lebanon vote to accept a.25 acre parcel of land on Milton Mills Road donated by Ronald B. Newell, successor Trustee of the Russell D. Newell Revocable Trust of 2007, Map R15, Lot 5.
