B UILDING EFFECTIVE T RANSITION PARTNERSHIPS Transition services ultimate goal is to provide a seamless transition from high school support to adult agency supports for students. In order to ensure there is not a gap in employment services, planning and coordination between the school representative(s) and the Vocational Rehabilitation Counselor is critical. Suggestions for building effective partnerships and strengthening collaboration are contained within this booklet. By sharing creative strategies and delineating promising practices of Vocational Rehabilitation Counselors across the state, hopefully you will find useful ideas for your own communities. 12 1

4 ANNUAL PROJECTION PLANNING The primary source of guidance for Vocational Rehabilitation Counselors (VRC) transition services is VRS Public Policy Manual 510. One transition activity outlined in PPM 510 is an annual projection meeting between the VRC and the school representative(s). Below you will find the annual projection meeting defined, its purpose, and how it can be implemented to benefit all transition partners including students. Annual Projection Meeting An annual projection meeting between a school representative and the Vocational Rehabilitation Counselor (VRC) provides essential communication concerning: Anticipating referrals to VRS For a new VRC, contact with the appropriate school staff within 12 weeks of being hired. The VRC learns the school s calendar for case conference such as fall, spring or monthly. The VRC discusses with the school staff the specific amount of notice s/he would need in order to attend the case conferences. School staff and the VRC meet to identify students who may be referred to Vocational Rehabilitation Services (VRS) at the beginning of each school year. Prioritizing and scheduling the VRC s attendance at case conferences (IEP meetings) Scheduling case conferences (IEP meetings) Planning for distribution of VRS written information at case conferences 2 11

5 CHECKLIST FOR SUCCESS Many Vocational Rehabilitation Counselors (VRC) acknowledge a key component to effective transition services is the individual relationship with the school staff. Below are suggestions for building the foundation for strong partnerships and strengthening collaboration. The VRC contacts the various personnel and provides them with VRS information and his/ her business card. The VRC records the contact information for his/her future use for the: Initially, face-to-face meetings may be the best strategy for building relationship and strengthening knowledge about each other s programs. Over time, the annual projection meeting may be held face-toface, over the phone, or via . VRC across the state have used multiple approaches to effectively exchange information. The following VRC Projection Timeline can be used during this discussion to help identify anticipated referrals and guide the prioritization of VRC case conference attendance and the timeline for application. It is important to remember that the projection meeting does not screen out students from VRS. Any student may apply to VRS at any time. Special Education Directors Guidance Counselors Special education department chairs Transition Coordinators Job Coaches Anticipated Referrals It helps a VRC organize her/his activities and caseload responsibilities if s/he knows how many students will be referred in the upcoming year for services. It is helpful for the school representatives to discuss students who will be seeking assistance with an employment goal requiring post-secondary education or training as well as students who will be seeking employment upon exiting high school. It may also be worthwhile for school representatives to identify students who are likely to drop out, be placed in alternative learning programs or be homebound and in need of VRS services. 10 3

6 Prioritizing VRC Attendance While a VRC would like to attend all case conferences, it is usually not feasible. The annual projection meeting enables the VRC and school representatives to identify students that may be seeking employment-related services prior to exiting high school or upon exit (i.e., community-based employment, integrated supported employment, assistive technology, career assessment, etc.). For these students, it would be essential for the VRC to attend their junior (year prior to exit) case conference and set an appointment to complete the VRS application as quickly as possible or by January or February of the junior year (year prior to exit). For students with an employment goal requiring post-secondary education or training (i.e., GED, vocational training, technical training, etc.), it may not be a priority for the VRC to attend the junior case conference. Providing VRS written information may be more appropriate at the junior case conference. A VRC presence is most important at these students senior year conference. It is helpful if the conference is held in the fall in order to ensure seamless services from high school to the post-secondary setting. At the latest, an appointment to complete the VRS application should be scheduled by January-February of their exiting year. SHARING INFORMATION These are successful strategies used by numerous VRC to connect and build partnerships with students, parents, and teachers: Set-up a booth at a transition fair, job fair and/or college night. Join and attend local transition council meetings. Attend local open house events to meet students and parents. Provide ideas for job shadowing and work experience sites to transition coordinator and/or other school personnel. Present VRS information to large groups of students at high schools who are interested in services. Meet one-on-one with the student so he/she understands the role of a VRC. This helps establish a relationship by getting to know him/her and offer support to achieve his/her employment goals. Share information between school staff and VRC in regards to a specific student i.e., school employment experiences and career exploration activities, assessment information, portfolio, etc. 4 9

7 Coordinate a short presentation to teachers during in-service on community services for students with disabilities i.e., Bureau of Developmental Disabilities Services (BDDS), Medicaid, Medicaid Waiver, WorkOne, Social Security Administration, service providers and VRS. (Jodi Isaacs, Seymour) Work with school personnel to understand why a referral was appropriate or inappropriate. (Joyce Spayd, Indianapolis) Develop transition packets and distribute at case conferences. Each packet contains information specific to the student s interests and needs (i.e. post-secondary training and financial aid, BDDS, etc.). (Jodi Isaacs, Seymour) Fax and a one-page fact sheet on VRS giving information about what is VRS, timeline for referral, how to refer, and local VRC names and phone numbers to local school administrators and special education department chairs. (Gary VRS Office) With the support of the high school AV department, presentations by representatives from social service agencies, BDDS, Social Security, VRS, etc. were videotaped at a local transition night. This video is now shared with students and parents throughout the year to learn more about local resources. (Pam Thompson, Valparaiso) Scheduling Case Conferences When scheduling case conferences the VRC may find it helpful to give the school representatives the timeline for the application process, eligibility determination and service delivery. There are various calendars for scheduling case conferences in Indiana. However, first semester case conferences benefit a student who is seeking VRS services in the following summer or next fall. Through discussions and creative collaboration, schools and VRS may be able to schedule conferences in the first semester for specific students. Another helpful strategy is for school representatives and VRC to block conferences together that a VRC will attend. This will enable the VRC to efficiently schedule conferences and will more likely result in the VRC being able to attend case conferences. Planning for Distribution of Written Information When the VRC is not attending a student s case conference, written information about VRS should be provided to the students and family members. Information that should be shared may include: Business Card Moving On...Life After High School Booklet VRS Brochure: Vocational Opportunities for People with Disabilities Information on how to connect with other services such as: Bureau of Developmental Disabilities Services (BDDS), Medicaid, Medicaid Waiver, Social Security Administration, Indiana Works, and WorkOne 8 5

8 VRC TRANSITION STRATEGIES Vocational Rehabilitation Counselors across the state are sending one message: Keep the Lines of Communication Open! Below are creative strategies some VRC are using to build effective transition partnerships with school personnel. Meet in informal settings such as breakfast, lunch or after working hours. (Shirley Olwine, Fort Wayne; Linda Brown, Jasper) Attend case conferences and IEP meetings outside of normal business hours to demonstrate commitment to the transition process. (Mike Murphy, Elkhart) Meet with the guidance counselors) each fall to identify any general education students who might benefit from VR services. (Linda Vicory, Terre Haute) Schedule appointments to take applications with as many students as possible during the week of spring break (junior year or year prior to exit.) Students are out of school, there is no class time conflict. (Pamela Montgomery, Gary) Coordinate with school personnel to schedule appointments with students/parents to take applications at the school. (Pamela Montgomery, Gary) Schedule office hours in the high school. Notify students and parents of dates and times via a letter or other available means. Parents and students may schedule an appointment with the VRC at the school with the school contact person. (Denise Smith, Columbus) Visit co-op office to meet personnel who send school records. Now VRC feels like they know each other and have made the record request process work smoother. (Linda Brown, Jasper) Pick up referral packets/iep information from the school to expedite the referral process if necessary. (Pamela Montgomery, Gary) Ensure invitation to case conference meetings by cultivating relationships with special education department chair/coordinator and the secretary.
