1 Slide 1 SANE/SART Concept YWCA-Oklahoma City Created by Shannon Liew, RN, BSN, SANE-A & LaRae Huycke, RN, MS, APRN-BC, DNP Slide 2 /SART Programs Evolved out of the feminist movement of the 1970 s: Growing out of concern for disparity in post-trauma care Rape crisis movement Increased awareness of rape and sexual assault on the local and national level Primarily staffed by volunteers Slide 3 The Need for SANE Programs Services for victims of sexual assault were inadequate: Long wait time in the ER (4-12 hours) Victims seen as less critically ill in the health care setting Insufficient training of medical personnel to perform medico-legal examination Insufficient training of medical personnel to address emotional/safety needs of the victim
2 Slide 4 First SANE programs established in: Memphis 1975 Minneapolis 1977 Amarillo 1979 First list published identifying SANE programs not until 1991 in the Journal of Emergency Nursing with 20 programs listed International Association of Forensic Nursing founded in 1992 Slide 5 Violence Against Women Act (VAWA) signed into law in 1994 Doubled penalties for repeat offenders, required date rape to be treated the same as stranger rape, and made $800 million available to fund program development over a six year period Tulsa SANE Program received the Innovations in State and Local Government Award from the Ford Foundation and John F. Kennedy School of Government at Harvard University in 1994 Increased interest and publicity in SANE Slide 6 American Nurses Association officially recognized forensic nursing as a new specialty of nursing in 1995 SANE is the largest subspecialty of forensic nursing First certification test for SANE-A offered by the IAFN in 2002 VAWA issued federal mandate to ensure victims of sexual assault have the right to a free medico-legal examination without seeking prosecution in 2005 November 1 st, 2007, Oklahoma was in compliance
3 Slide 7 Current SANE Programs Hospital based, community based, a combination of hospital and community based, or based within law enforcement Programs have the following components: Float pool of SANE s (some volunteer, some paid) Relationship with law enforcement and the district attorney s office Crisis hotline Crisis advocates Slide 8 SART Concept SART concept is based on community/team response to victim s of sexual assault SART = Sexual Assault Response Team Components include: SANE Performs medico-legal exam Advocate Provides support to victim Law enforcement Ensures victim s safety, conducts investigation Prosecution Files charges, brings case to court Other Hotline, forensics, victim compensation, medical facility, CPS/APS, counselors Slide 9 SART Concept Goals of SART Minimize the re-victimization and re-traumatization of victim Victim-centered and victim friendly response Ensure appropriate care provided to victim Ensure timely and quality evidence collection Increase chance of successful prosecution
4 Slide 10 Victim accesses services by: Calling law enforcement/911 YWCA partnered with multiple law enforcement agencies in Oklahoma County, largest being Oklahoma City Police Department Will accept victims from other law enforcement agencies and counties Calling YWCA Rape Crisis Hotline ( RAPE) Victim presents to local ER Slide 11 Victim directed to one of three participating hospitals in Oklahoma City Metro Area: Midwest Regional Medical Center Integris Baptist Medical Center Integris Southwest Medical Center Each hospital on-call as SANE hospital on a monthly basis Participating hospitals provide medical screening exam, private space for the examination and waiting area for victim and family, and prophylactic medication Examination and medication are offered at no expense to the victim Slide 12 YWCA SANE and Hospital Advocate dispatched to hospital for medicolegal examination SANE and advocate must respond within 60 minutes Time frame for examination is within 120 hours Perform exams on victims 13 years old and up Victim must consent to examination Examination paid for by Victim s Compensation
