1 Memorandum of Consortium Among Metropolitan St. Louis Psychiatric Center St. Louis Psychiatric Rehabilitation Center Hawthorn Children's Psychiatric Hospital Children s Advocacy Services of Greater St. Louis The Family Therapy Center, St. John s Mercy Medical Center The Alvin J. Siteman Cancer Center, Washington University School of Medicine Missouri Institute of Mental Health It is mutually agreed that Metropolitan St. Louis Psychiatric Center (MPC), St. Louis Psychiatric Rehabilitation Center (SLPRC), Hawthorn Children's Psychiatric Hospital (Hawthorn), Children s Advocacy Services of Greater St. Louis (CASGSL), The Family Therapy Center, St. John s Mercy Medical Center (SJMMC), Missouri Institute of Mental Health (MIMH) and The Alvin J. Siteman Cancer Center (Cancer Center) at the Washington University School of Medicine shall affiliate for the purpose of maintaining a training program for pre-doctoral psychology interns. This affiliation shall be known as the St. Louis Psychology Internship Consortium (SLPIC). MPC, Hawthorn and SLPRC are operated by the Missouri Department of Mental Health. MPC is a 125-bed inpatient psychiatric hospital accredited by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO). It provides both emergency and acute care for adult inpatients in the Eastern Region of Missouri. MPC also operates the Community Linkage Program, a pilot program for Dual Diagnosis clients. Hawthorn has 36 beds for inpatient psychiatric treatment of children and adolescents. Hawthorn also has a 24 bed residential unit and 16 slots for day treatment. Hawthorn is accredited by JCAHO and serves children and adolescents ages 6 to 17+ from a 31 county region in Eastern Missouri. SLPRC is a JCAHO accredited 212 bed psychiatric facility providing intermediate and long-term inpatient psychiatric and psychosocial rehabilitation services to adults with severe, persistent mental illness and personality disorders. Also, patients requiring longterm inpatient care for forensic evaluation and treatment/rehabilitation are committed to the hospital. Page 1
2 CASGSL is a not-for-profit agency whose mission is to address the many needs of children and families who are impacted by child sexual abuse. The agency will provide assessment, intervention (individual counseling, group therapy, family therapy) and prevention services in addition to outreach, community education and continuing education. SJMMC is an outpatient mental health service that provides assessment and treatment to children, adolescents and families. MIMH is a center for policy, research and training, whose mission is to provide evaluation and research in the areas of public policy, ethics and mental health care delivery. MIMH provides continuing education to mental health professionals and maintains the MIMH Library. The CANCER CENTER. I. OBJECTIVES SLPIC is designed to prepare interns for the professional practice of psychology. The major goal of the training program is to have the intern develop a sound foundation in clinical skills of psychological assessment and therapeutic intervention. SLPIC adheres to a generalist model of training, giving interns experience across a range of client ages, psychopathology and treatment settings. Training in ethical and professional standards will be incorporated in supervision and seminar presentations to prepare interns for the highest standards of professional conduct. SLPIC s core facilities, adjunct facilities, and affiliates are committed to the objectives, goals, and philosophy of SLPIC. The Consortium s members are strongly committed to offering a generalist internship experience with intensive supervision, didactic seminars, and opportunities for direct clinical experience. Specific goals for the intern include: 1) developing and/or refining of diagnostic skills with children and adults using structured interviewing techniques and traditional objective and projective assessment techniques in the areas of cognitive, personality and neuropsychological functioning; 2) developing competence to evaluate and report diagnostic findings appropriate to a variety of referral sources such as in-hospital staff, courts, community agencies, parents and teachers, etc. 3) increasing familiarity and skill with a variety of psychotherapeutic techniques relevant for individuals, groups, couples and families; 4) increasing understanding of psychopathology and levels of functioning as they relate to long-term, short-term, and brief therapy using supportive, behavioral, systemic and insight-oriented techniques; Page 2
3 5) acquiring knowledge of the roles of other disciplines' contributions in psychiatric and community settings and the ability to function effectively on an interdisciplinary treatment team, setting goals and planning for patient care. II. PROGRAM STRUCTURE Participation by agencies in SLPIC can be in the form of either core facilities, adjunct facilities or Consortium affiliates. Core facilities are voting members of the Consortium and have representatives on the Training and Coordinating Committees. Core facilities contribute to the financial cost of the Consortium and provide major training rotations to the Consortium. Currently MPC, a core facility, assumes responsibility for administering the Consortium. Other core facilities include Hawthorn and SLPRC. Adjunct facilities provide major training rotations to the Consortium and contribute to the financial costs of the Consortium. Adjunct facilities provide representatives to the Training and Coordinating Committees. Adjunct facilities include CASGSL, SJMMC, MIMH, the Community Linkage Program and the Cancer Center. Organizations will be considered for inclusion as adjunct facilities in SLPIC based on their ability to enhance the training opportunities for interns within the generalist internship philosophy of the program and their willingness to adhere to the philosophy, objectives, and model of the Consortium. Adjunct facilities will meet the supervisory requirements as outlined from time to time by the American Psychological Association in their Guidelines and Principles for Accreditation of Programs in Professional Psychology. Acceptance of an adjunct facility will require a consensus of all current facilities. Adjunct facilities will be considered annually for continued participation in the Consortium based on current training needs of the Consortium. Adjunct facilities that fail to fulfill the training expectations of the Consortium may be removed from the Consortium by consensus of the Coordinating Committee. Consortium affiliates will include those agencies that have important training personnel and resources to offer the Consortium but do not contribute funding for interns, do not provide major rotations to the interns, nor sit on the Training or Coordinating Committees. Under the current funding and Consortium agreement, interns will participate in three mandatory rotations for four months each: MPC, Hawthorn and SLPRC, requiring 32 hours per week (including 4 education hours) and an eight-hour-per-week rotation throughout the year at one of the following sites: CASGSL, SJMMC, MIMH, the Community Linkage Program or the Cancer Center. The Training Committee based upon the preferences of the interns and the interns training and experience makes assignments to supervisors and units at each site. Page 3
4 The core facilities, adjunct facilities and affiliates will participate in the presentation of Psychology Intern Seminars which will meet one time per week for 2-hour sessions. Any additional didactic training offered by a Consortium agency will be available to all Consortium interns. Supervisors from all participating agencies will facilitate the attendance of interns at Consortium-wide training opportunities whenever possible. The core facilities, adjunct facilities and affiliates will provide interns support and access to such resources as libraries, computers, and observation experiences in settings not directly controlled by SLPIC supervisors (such as in Emergency Rooms or special program settings) whenever possible. In general, support and access to resources will be assumed to parallel those typically provided to employees with a probationary or provisional employment status. Nothing in this paragraph will be construed to affect fringe benefits such as sick leave, vacation time, health insurance, etc., which are determined by Department of Mental Health guidelines applicable to the SLPIC administrator, MPC, which is the legal employer of the interns. Intern recruitment and selection will be coordinated by the Training Committee and Training Director. The Training Director will submit intern ranking to the APPIC Match Program on behalf of the Consortium. The Coordinating Committee will closely oversee the recruitment process and the selection process to assure equitable treatment of all applicants and adherence to the standards of both the American Psychological Association and the Association of Psychology Postdoctoral and Internship Centers. III. ADMINISTRATIVE STRUCTURE The Coordinating Committee will have the authority and responsibility for insuring the quality of the SLPIC training program. The Training Director will serve as the designated administrative head of the Consortium and will be the chairperson of both the Coordinating Committee and the Training Committee. Consortium Coordinating Committee 1. Membership: The Coordinating Committee will consist of the Psychology Directors or representatives from MPC, Hawthorn SLPRC, CASGSL, SJMMC, MIMH, the Community Linkage Program and the Cancer Center, or their designees. Adjunct facilities may provide only one representative to the Training and Coordinating Committees if they so choose. In the event the SLPIC Training Director is also a Psychology Director from a facility, that facility will provide a designee for voting purposes. Each core or adjunct facility will appoint an Intern Training Coordinator who will also serve on the Coordinating Committee. These two representatives from each core or adjunct facility (the Intern Training Coordinator will not simultaneously serve as SLPIC Training Director) will constitute the voting members of the Coordinating Committee. Voting rights of the core/adjunct facilities will be proportional to the number of interns engaged in training at Page 4
5 the site during the training year. (For example, sites who train the whole intern class will have one vote, those who train three interns will have 3/5 vote, and those who train one intern will have 1/5 vote). Voting will require a quorum of one member from each core/adjunct facility and there will be a maximum of one designee or alternate from each core/adjunct facility attending the meetings. Acceptance of an adjunct facility, removal of a facility or changes in programmatic structure of the Consortium will require a consensus of the participating facilities. All other issues will require a simple majority with tie-breaking votes cast by the Training Director. 2. Leadership: The Training Director by the consent and agreement of the core facilities will be a 40% funded position at MPC. However, the administrative decision regarding whether an employee may be considered to fill or continue as the Training Director resides with the agency or facility which employs the individual as Training Director. Whenever possible, such decisions will be made in consultation with the other Consortium facilities. In situations other than described above, a vote of two-thirds of the Coordinating Committee will be required to approve changes in or to re-elect the Training Director for the Consortium. The Training Director is the major representative of the SLPIC training program. The Training Coordinators will assist the Training Director with the implementation of the Consortium training program in their respective agencies. 3. Frequency of Meetings: The Coordinating Committee will meet quarterly at the call of the chairperson. A special meeting may be called by any of the members of the Coordinating Committee. 4. Duties and Responsibilities: The Coordinating Committee, under the leadership of the Training Director, will be responsible for maintaining SLPIC compliance with the criteria for accreditation of the American Psychological Association and with the guidelines of the Association of Psychology Postdoctoral and Internship Centers. In this role, the Committee will be responsible for maintaining the resources, evaluating prospective core facilities, evaluating core facilities for continued participation in the Consortium and ensuring the quality of the clinical supervision within the internship. The Coordinating Committee will be responsible for providing feedback as deemed necessary and on a written basis annually to the supervisor of the Director of Training for consideration in the performance appraisal of the Director of Training. Consortium Training Director The Training Director, as the designated administrative head and administrator of SLPIC, serves as chairperson of both the Coordinating Committee and the Training Committee. The Training Director will be responsible for recruitment of prospective interns and coordination of the selection process; for orientation of interns; for advising and supervising interns; for maintaining an intern advocacy system; for administering the intern evaluation procedures; for planning and implementing the Intern Seminars; for the scheduling of internship rotations; for the development of representative publications of the Consortium; for the preparation of relevant reading lists; for maintaining records of Page 5
6 interns; for communicating with the interns university training programs throughout the training year; for representing the Consortium to accrediting agencies and for other duties as related to the overall operation of the SLPIC program. The Training Director will work with the Administrative Coordinator, a 25% clerical position at MPC, to accomplish these duties. Consortium Training Committee 1) Membership: the Training Committee will consist of the Training Director, the Training Coordinators as appointed by the respective Directors of Psychology, or other administrative authority. 2) Intern Representative: a designated intern will serve as a representative to the Training Committee and will be included in meetings where matters that are relevant and appropriate to their training experience are discussed. 3) Leadership: the Chairperson of the Training Committee is the Training Director. 4) Frequency of meetings: the Training Committee will meet on a frequent basis as scheduled by the Consortium Training Director. 5) Source of authority: the activities of the Training Committee derive from the authority of the Training Director to implement the policies and the programs of the SLPIC. The members are appointed by the Coordinating Committee with the stipulation that their respective Directors of Psychology or other agency authority authorize their time commitment to the work of the committee. 6) Duties and responsibilities: the Training Committee functions as the working staff for the Training Director in the implementation of the SLPIC program. The Training Committee will be responsible for maintaining a grievance evaluation procedure that will be available to supervisors and interns. IV. FINANCIAL STRUCTURE Funding for five interns' salaries, the 40% Director of Training and 25% of the Administrative Coordinator's salary will be apportioned among the core and adjunct Saint Louis Psychology Internship Consortium facilities based on the number of interns they train during a training year. (For example, sites that train all five interns shall contribute 25% of the costs, sites that train three of the five interns would contribute 3/5 of 25% of the cost which is 15% of the total cost, sites that train one intern would contribute 1/5 of 25 % of the cost which is 5% of the total cost. For outpatient sites, the cost for the training year will be approximately $ subject to legislative approval. Page 6

FACILITY RESPONSIBILITIES The core facilities, adjunct facilities, and affiliates acknowledge their commitment to uniform administration and implementation of the program s training principles, policies, and procedures addressing trainee/student admission, financial support, training resource access, potential performance expectations, and evaluations. VI. MEMORANDUM OF CONSORTIUM RENEWAL Automatic renewal for the training year shall be scheduled for September 1, 2001, for core facilities and November 1, 2001, for adjunct facilities. Facilities not wishing to renew shall notify the Training Director in writing by these dates. This agreement will go into effect and be binding for the training year September 1, 2001, through September 1, 2002, following ratification and acceptance by the undersigned. VII. TERM AND TERMINATION This Agreement shall continue for a term of one (1) year from September 1, 2001, ( Initial Term ) and shall continue thereafter for a period of one (1) additional year ( Renewal Term ) unless terminated as provided herein. Each party to the Agreement shall notify the Training Director by September 1 for core facilities and November 1 for adjunct facilities of the Initial term and any Renewal Term of its intent to participate in the Consortium for the following term. Unless mutually agreed upon, either party may terminate this Agreement with at least 90 (ninety) days prior written notice of such intention given to the other parties, but in no event, shall such termination be on a date other than the end of a training year. VIII. MISCELLANEOUS A. Each party agrees to maintain appropriate licensure and accreditation with respect to the facilities or programs required to be in existence in order to effectuate the obligations of the parties under this Agreement, including but not limited to licensure required by any federal, state or local agencies and accreditation by JCAHO. Should either party become aware of the loss or threatened loss of any licensure or accreditation that would affect the performance of its obligations under this Agreement, it shall immediately so notify the other parties. B. This Agreement shall be governed by and construed in accordance with the laws of the State of Missouri. The parties agree that this Agreement is subject to all applicable state, local and federal laws and regulations, and any amendments thereto during the term Page 7
8 of this Agreement. In the event any provision in this Agreement shall be deemed, by either party, to be a violation of law or regulation, enacted after the execution of this Agreement, or to be inconsistent with the laws or regulations existing at the date of this Agreement but interpreted by a court or regulatory authority of competent jurisdiction after the execution of this Agreement, then the parties shall proceed in good faith to renegotiate this Agreement to eliminate such violation. Any party wishing to withdraw from the Agreement due to a failure to renegotiate its terms to that party s satisfaction may do so upon ten (10) business days written notice to the other parties. Such withdrawal shall not serve to terminate the Agreement as to any of the other parties. C. Any party withdrawing from the Agreement pursuant to Section VII.B. shall remain obligated, for the remainder of the contract term, to the financial commitment made pursuant to Section IV of this Agreement. D. In the event any term or provision of this Agreement is found to be unenforceable or void, in whole or in part, as drafted, then the offending term or provisions shall be construed as valid and enforceable to the maximum extent permitted by law, and the balance of this Agreement shall remain in full force and effect. E. Neither party hereto may assign its interest, delegate the performance of its obligations under this Agreement to, or subcontract with, any person, form or entity without first obtaining the prior written consent of the other parties. F. The waiver by either party of a breach or violation of any provisions of this Agreement shall not operate as or be construed to be a waiver of any such party s right with respect to any subsequent breach thereof. G. School of Medicine are independent contractors and this Agreement shall not constitute the formation of a partnership, joint venture or principal-agent relationship or any relationship other than that of independent parties contracting with each other solely for the purpose of carrying out the terms and conditions of this Agreement. I. In the event that this Agreement is subject to Section 1861 (v) (1) (I) of the Social Security Act as amended by the Omnibus Reconciliation Act of 1980, the parties hereby agree that: a. Each party shall, until the expiration of four (4) years after the furnishing of the services pursuant to this Agreement, make available upon written request, to the Secretary of the U.S. Department of Health and Human Services, or upon request, to the U.S. Comptroller General, or any of their duly authorized representatives, the contract and books, documents and records of such party that are necessary to certify the nature and extent of the cost under this Agreement. b. If either party carries out any of the duties of this Agreement through a subcontract with a value or cost of $ 10,000 or more over a twelve (12) month period with a related organization, such subcontract shall contain a clause to the effect that, until the expiration of four (4) years after the furnishing of such services pursuant to such subcontract, the related organization shall make available upon written request to the Secretary of the U.S. Department of Health and Human Services, or upon request to the U.S. Comptroller General, or any of their duly authorized representatives, the subcontract and books, documents and records of such party that are necessary to certify the nature and extent of such cost; and c. If Section 1861 (v) (1) (I) of the Social Security Act is found to be inapplicable to this Agreement, then the aforementioned clauses shall be deemed inoperative and without force and effect. Page 9

This Agreement constitutes the entire understanding and agreement among the parties with respect to the subject matter thereof, and may not be modified without the written agreement of said parties.
