4 PRESENTATION OF THE HOSPITAL The Highly Specialized Rehabilitation Hospital of Motta di Livenza (O.R.A.S. S.p.a.) was set up in January 2004 in line with the legislation of the reorganization of the National Health Service, which provides for the Regions the right to experiment with innovative solutions in order to provide better health care (Article 9 bis of the Legislative Decree of 30 December 1992). Specifically, the Hospital of Motta di Livenza, a health care facility for acute patients, has been converted into a rehabilitation hospital of excellence with the establishment of a Joint Venture based on public and private capital (The Local Health Care Facility no. 9 of Treviso, the Health Care Home of Abano Terme and the Town Hall of Motta di Livenza), credited with the NHS and with the same purpose of a public facility. Today the O.R.A.S. has consolidated its leadership in the field of rehabilitation of High Specialization also beyond the confines of the region, with predominant activity in cardiological and neurological rehabilitation (severe brain and spinal cord injury), servicing 1 and a half million inhabitants, representing a virtuous example of public-private collaboration in the public health sector, with the strategic aim to bring innovation and competitiveness to the structure. The hospital receives under the inpatient and day care (Day Hospital) patients in need of rehabilitation projects for disabilities of various origins, whether they are transient or permanent. It takes over from a wide area of the Veneto Region patients with increasingly complex conditions requiring both clinical and strictly rehabilitative care, ensuring, through the experience gained in recent years by the multi-professional and multi-disciplinary rehabilitation team, the implementation of rehabilitative well-constructed paths which provide diversified programs based on pathology, clinical stability and the needs of the subject and a high quality management as a whole, including particularly fragile and complex patients. In particular, the Unit of Preventive and Rehabilitation Cardiology of the hospital takes over, in sub-intensive inpatient care, people having very recently been subjected to cardiac surgery (4-5 days after cardiac surgery) and patients with disabling diseases of cardiac, pulmonary and angiological origin. The Operative Unit of Physical Rehabilitation Medicine - the Serious Cerebral and Spinal cord Injuries Unit and the Physical Medicine and Functional Recovery Unit are engaged in the rehabilitation activity in inpatient day care (day hospital) and outpatient care, for people of all ages with transitional or permanent disabilities, with physical and cognitive impairments due to different causes (musculoskeletal, neurological, visceral disorders,). The Operative Unit of Physical Rehabilitation Medicine and the Serious Cerebral and Spinal cord Injuries Unit include a highly specialized form of rehabilitation for severe brain injury (head trauma, cerebral anoxia and severe bleeding in the brain) and for spinal cord injury. Inside the operative unit of Rehabilitation and Prevention Cardiology there are a number of beds committed to activity of General Medicine and Long Term care, which is dedicated to support patients with a high complexity that can arrive from other operative unit of the hospital. These type of patients need a high level of nursing and an important medical assistance, also considering the future dehospitalization. A corresponding Outpatient Service guarantees specialist multidisciplinary activities in addition to servicing a large area, approximately 250,000 services / year, primarily in support of patients admitted to the various operating units as well as those coming from the territory of Motta di Livenza, from the province of Treviso and from the neighboring provinces, all in a vision of total care of the patient. The outpatient clinic provides all the major specialist services, particularly with regard to services for imaging diagnostic and ultrasonography and in any case all those benefits 4
5 and medical specialist care whose demand is more relevant to the pathologies of rehabilitative interest. MISSION The Highly Specialized Rehabilitation Hospital of Motta di Livenza guarantees admissions to ordinary inpatient and day care (day hospital) and outpatient care, pursuing excellence in rehabilitation services and acting as a point of reference for local, regional and national users. The Hospital welcomes people who need rehabilitation projects for complex disabilities of various origins, in order to ensure recovery assistance interventions aimed at achieving the highest possible level of independence, social integration and quality of life given the illness. The interventions are carried out through an individualized acceptance for each person on the basis of a Rehabilitation Project developed according to the specific health problems and needs of the person and their family. The acceptance takes place according to a multi-professional approach, by a team of dedicated professionals, and includes therapeutic interventions, welfare and educationalinformative activities. The rehabilitation intervention is therefore aimed towards these fundamental goals: the recovery of a functional competence that for pathological reasons has been lost; the need to put a barrier to functional regression trying to modify the natural history of chronic degenerative disorders and reduce the risk factors controlling their progression; the ability to identify solutions, also customized, to ease the patient s journey to recovery; the beginning of rehabilitation possibly at the very moment when the damage is established or according to a term defined by the precise balance between the stabilization of the outcome and the presence of the recovery potential. In keeping with the guidelines of the Ministry of Health for the rehabilitation activities of 1998 and with the National Plan to Address Rehabilitation of 2011, the reality of O.R.A.S. baes the rehabilitation process on a concept of total care of the patient, with a multidisciplinary and multi-professional view of the centrality of the person and the humanization of care and, as hospital with a prevailing rehabilitation vocation, it carries out social care and education activity and the protection of persons with disabilities, as well as training, retraining, research and teaching in rehabilitation, in collaboration with other institutions. TECHNICAL DATA OF THE HOSPITAL The Rehabilitation Hospital is located in the urban center of the town of Motta di Livenza and covers a total of 18,600 m2 of the total indoor area and 10,460 m2 of outdoor overall area, of which 1,700 m2 are dedicated to the green area. The hospital has four pavilions, three of which are used for inpatient treatment, diagnostic services, outpatient clinics, gyms and one dedicated specifically to the activity of hydrokinetic therapy with its swimming pool for the execution of specific programs of rehabilitation in the aquatic environment. 6 The hospital, in order to ensure the high standards of preset care and assistance levels and in consideration of the evolution of demand for health services by users, has in place a continuous updating and revision of the positioning logic of premises dedicated to the acceptance of patients and the provision of health services as defined in its service chart. In detail, the hospital provides for the organization of health activities in the following Units and Services:. Unit of Preventive and Rehabilitation Cardiology;. Rehabilitation Cardiology. Rehabilitation Angiology. Rehabilitation Pneumology. General Medicine and long-term care. Unit of Physical Rehabilitation Medicine - Serious Cerebral and Spinal cord Injuries Unit;. Unit of Physical Medicine and Functional Recovery;. Outpatient Service;. Service Of Diagnostic Radiology. their family, as well as the resources available. The individual rehabilitation project is carried out by a team of professionals with different skills and set up according to the rehabilitation needs of the individual patient and sharing the same with the patient to verify the achievement of the targets. Depending on the clinical situation and the needs of the person, the rehabilitation activities can be set: in ordinary hospitalization (the person is accepted in the hospital both during day time and night time); in day hospitalization (day hospital, the person is accepted only during the day); on an outpatient basis. The rehabilitation process can take place in one of the above ways (inpatient treatment, ordinary or day hospitalization, or on an outpatient basis) or in more than one method at a later time. The following describes the activities performed by different operating units and services pertaining to the hospital. OPERATING UNIT OF REHABILITATION AND PREVENTIVE CARDIOLOGY Main entrance of the Hospital. 6 HEALTH ACTIVITIES REHABILITATION UNITS The interventions carried out by the Rehabilitation Units are made on the basis of an individual rehabilitation project, which takes into account the patient s clinical condition, the recovery potential, the needs and preferences of the patient and Rehabilitation Cardiology It welcomes people with recent acute myocardial infarction or acute coronary syndrome, postsurgical myocardial revascularization (angioplasty or coronary artery bypass), post-replacement surgery or valve repair, patients with chronic heart failure or post-surgery in case of heart transplant. The treatments provide for the total care of the cardiac patient undergoing post-acute or chronic illness and hospitalization in sub-intensive care for people with cardiac surgery. There will also be a preventive rehabilitation activity suitable for the prevention of cardiovascular disease in subjects who are still asymptomatic, but at high cardiovascular risk. The Hospital conducts its rehabilitation activity in ordinary inpatient day care (Day Hospital) and outpatient care.
7 Rehabilitation Angiology Carries out rehabilitation in patients with documented arterial disease of the lower limbs. The angiological rehabilitation is complementary to pharmacological and surgical treatments and is aimed in particular at patients with arterial disease of the lower extremities with claudicatio intermittens (Fontaine stage II), i.e. pain or cramping in one or both calves while walking. The goals of rehabilitation treatment in these cases are the control and / or elimination of ischemic rest pain, improvement and / or healing of trophic lesions, the functional recovery of the lower limbs and autonomy of movement of the patient. The angiological rehabilitation takes place mainly in inpatient day care (Day Hospital). Rehabilitation Pneumology It welcomes patients diagnosed with respiratory failure from chronic obstructive pulmonary disease, chronic degenerative disease or bronchopulmonary disorder who have suffered a recent thoracic surgery. The pulmonary rehabilitation aims to achieve the highest level of activity and independence of the patient in the post-acute or chronic phase of the disease correlated with the deficits arising from the above mentioned diseases and the results of surgeries that involve precisely the thoracic region. The rehabilitation activities are provided in inpatient, day care (day hospital) and outpatient service. There is also an inpatient service at night (Night Hospital), for patients with suspected respiratory sleep disorder (sdr of obstructive sleep apnea, OSAS). General Medicine and long-term care The activities of General Medicine and Long Term care are performed within the operative unit of Rehabilitation and Prevention Cardiology of the hospital. Its activity is addressed to the persons affected of multiple pathology and persons which has the necessity of diagnostic and therapeutic assessment either in the acute as in the exacerbation phase of disease, which can t be treated by the general practitioner (GP). For that type of activity, some diagnostic-therapeutic pathways has been adopted, shared with the specialist department of our associated hospitals, aimed to reach the overall management of the patients and to guarantee the optimal treatment of Some patients during rehabilitation activities. 7

A number of beds are dedicated for the patients that are still unstable and has a high level of nursing needs and require specialist care and assistance at the moment of dehospitalization. The activity of General Medicine and Long Term care is performed only inpatient. THE OPERATIVE UNIT OF PHYSICAL REHABILITATION MEDICINE - THE SERIOUS CEREBRAL AND SPINAL CORD INJURIES UNIT This Operative Unit includes a inpatient rehabilitation highly specialized facility, dedicated to severe brain and spinal cord injury, carrying out its activities in inpatient rehabilitation and building pathways for rehabilitation: Severe brain injury (head trauma, cerebral anoxia and severe bleeding in the brain); Spinal cord injury; Recent stroke and other disabilities of neurological origin (eg. Polyneuropathy, multiple sclerosis during exacerbation phase...); Outcomes of hip fractures, outcomes of joint replacement implant, outcomes of polytrauma or other disabilities of skeletal muscle origin. OPERATING UNIT OF PHYSICAL MEDICINE AND FUNCTIONAL RECOVERY The Unit carries out its business rehabilitation activity in inpatient day care (day hospital) and outpatient care: Severe brain injury (head trauma, cerebral anoxia and severe bleeding in the brain); Spinal cord injury; Recent stroke, and other disabilities of neurological origin (eg. Polyneuropathy, multiple sclerosis exacerbation phase); Outcomes of skeletal fractures; Outcomes of surgical procedures on the musculo skeletal system; Osteoarticular degenerative disorders; Paradysmorphisms (scoliosis, curved spine and other postural dysfunctions); Musculoskeletal pain syndromes such as low back pain and neck pain. As part of the various rehabilitative areas, each physician is responsible for a defined number of rehabilitation projects to safeguard the quality of the medical act, playing a leading role in supervising the rehabilitation team dedicated to the individual patient. According to the same principles of O.R.A.S. rehabilitation is a process of problem solving and education, the course of which leads a person to achieve the best possible quality of life on the physical, functional, social and emotional level, with the least possible restriction of their operational choices. As a result, the rehabilitation process concerns, besides strictly clinical aspects, psychological and social aspects. To achieve a good level of effectiveness, each rehabilitation project, for any individual, must therefore be focused on multiple targets, planned in an orderly manner, so that the autonomy reached in the various fields can be translated into the independence of the person as a whole and in any case a better quality of life. In relation with and in the light of this, the professionals actively involved in the implementation of the rehabilitation project at the ORAS can be: Physician (Cardiologist, physiatrist, Pulmonologist ); Professional Nurse; O.S.S.; Physiotherapist; Speech Therapist; Occupational Therapist; Psychologist; Neuropsychologist. 8
9 OUTPATIENT SERVICE The continuous improvement of the care system and of the quality of health services provided is one of the main goals of the Hospital of Motta di Livenza, including for the different specialized outpatient activities that can be carried out by the Hospital given the ability to identify and hire highly qualified medical professionals. The Outpatient Service provides support for specialist advice of patients admitted to the various operating units of the hospital as well as a specialized multidisciplinary activity for patients who come from the territory of Motta di Livenza, from the province of Treviso and the neighboring provinces, all highly integrated with respect to the network structures of the Local Health Care Facility No. 9 of Treviso, whether public or private. The outpatient activity ensures the taking in charge of the patient with the execution of any further diagnostic according to high quality standards in relation to the professionalism of medical specialists, the continuous updating of the medical equipment, and the set up of control instruments designed to certify the professionalism of all those involved, with continuous monitoring of the quality of the service provided, including by forwarding to other units of the same Local Health Care Facility No. 9 of Treviso. The activities include all those medical specialized outpatient services of interest and as integration of the rehabilitation activities, as well as all the major specialized services, according to a work schedule from Monday to Saturday inclusive and on the even Sundays of the month (DGRV n. 320 del ). Among the other specialty branches involved, the Outpatient Service of O.R.A.S. guarantees visits in the specialties of: Specialistic branch Allergology Angioloy; Cardiology; Surgery; Dermatology; Ultrasonography; Endocrinology; Physiatry; Gynecology; Internal Medicine; Neurology; Neuropsychology; Ophthalmology; Orthopedics; Otolaryngology; Proctology; Pneumology; Diagnostic Radiology; Rheumatology; Urology. DEPARTMENT OF DIAGNOSTIC RADIOLOGY The Radiology performs conventional radiological exams, mammography, echography, bone densitometry, MR of musculoskeletal and vertebral segments (cervical and lumbar), with particular attention to the ultrasonogram diagnostic of the breast with the related mammographic and ultrasonographic findings, biopsy insights and follow-up after surgery and / or cancer treatment. The Radiology is integrated within Department of Radiology of Azienda ULSS 9 di Treviso with intention to offer the diagnostic service through high quality imaging and evaluation by radiology specialist, which permits the best management of the diagnostic process in cases of major complexity. 10 Also according to the standards of quality and professionalism of O.R.A.S. and referred to the service rendered, there is the continuous commitment to ensure not only the high standard of diagnostic radiology services provided but, for example, even the containment of waiting lists and sending diagnostic reports in health care facilities more suitable to the needs of the patient. information related to preparation for check-ups are delivered to the address provided by the patient; the final report drawn up on paper is also delivered to the address provided upon completion. Among the standardized tests provided by the check-up service we list: The check-up allows a clinical eavaluetion of the subjet, through specialist conultation, laboratory test and instrumental test. 10 CHECK-UP SERVICE The check-up allows a clinical evaluation of the subject, through specialist consultations, laboratory tests and instrumental tests, in order to provide an early diagnosis of any undetected disorder and to provide guidelines for improving the quality of life of the patient by means of any treatment recommended and the prescribed pharmacological therapies. The check-up is done by specialists and by skilled health personnel and its diagnostic process includes a written final clinical evaluation.
