2 Supplemental Application for: Admission to Doctor of Physical Therapy Program (DPT) / University of South Alabama INFORMATION AND INSTRUCTIONS FOR THE APPLICANT YOU ARE ENCOURAGED TO COMPLETE THE APPLICATION PROCESS EARLY! Notice: All the information needed to apply to the Doctor of Physical Therapy program in the Department of Physical Therapy at the University of South Alabama is listed below. Applicants must adhere to the guidelines set forth in this packet. Please read the material carefully and make sure you have completed or will be able to complete the prerequisite requirements for the appropriate category prior to the beginning of the Fall Semester for which you are applying. Please note that you must indicate on the form your chosen category of admission (Regular Graduate admission or Advanced Undergraduate admission). The application period is July 1 st December 15 th All applicants must complete PTCAS and Supplemental Applications Application Deadline: December 15 th All applicants must complete and/or submit the following items by December 15 th of the year prior to admission. A. PTCAS application ( show completed by Dec. 15th B. Supplemental Application, postmarked and/or received by Dec. 15 th ( ) C. $75.00 non-refundable application fee, (check or money order) payable to University of South Alabama, enclosed with Supplemental Application, postmarked by Dec. 15 th. D. Physical Therapy Experience 50 hours completed and must be verified and submitted electronically to PTCAS by Dec. 15. ( ) E. International Students: International applicants must have a TOEFL score > 600, TSE > 60, and TWE > 4.5 and must submit the following to the Department of Physical Therapy by October 15 of the year prior to admission. 3 Two Categories of Applicants Considered for Admission: It is highly recommended, and in most cases required, that applicants complete a bachelor s degree prior to starting the PT program. 1. Regular Graduate Applicants: (Completed BS or BA degree by matriculation) These applicants must hold or have completed a bachelor s degree in any major from an accredited college or university, complete all prerequisites for regular graduate admission as listed below with a GPA 3.00 as calculated by PTCAS, and a minimum V & Q of 300 on the GRE. 2. Advanced Undergraduate Applicants: (Not earning a BS or BA degree by matriculation) This special Advanced early-entry option is reserved for the highly motivated and academically strong student who may wish to apply during their junior year of college. Students with at least 90 semester hours of specific college prerequisites listed below, an overall GPA of 3.50 as calculated by PTCAS, and a minimum V & Q of 300 on the GRE may apply for entry to the program prior to completion of the bachelor s degree. The student applying with three years of prerequisites is then able to complete the Doctor of Physical Therapy degree with just six years of college. Applicants in this category should carefully review Additional Information for Advanced Undergraduates that follows the list of prerequisites. Admissions rankings for both groups are determined using the same criteria. Acceptance is based on the student s academic ability and aptitude for a career as a PT including cumulative and Science prerequisite GPA, GRE scores (V, Q, and writing), and interview score. Minimum Requirements for All Applicants: 1. The applicant must declare one of the above categories on the supplemental application form (deadline December 15th) and meet all applicable requirements including prerequisites. 2. Graduate Record Exam (GRE) scores must be officially submitted to PTCAS by December 15 th (Institution Code 7799). GRE must have been taken within the last 5 years. A minimum total score of 300 on the Verbal and Quantitative portion is required. Verbal, Quantitative, and Analytical Writing sections will be utilized. You may not mix/match scores from different exam attempts to achieve the minimum total score of 300 on the V & Q. 3. Regular Graduate applicants must have a minimum 3.0 (A=4.0) overall grade point average (GPA); Advanced Undergraduate applicants must have a minimum 3.5 overall GPA. 4. All applicants must have a minimum grade of "C" in each of the prerequisite courses. 5. Observation or work experience in Physical Therapy (50 hours minimum) under the supervision of a licensed Physical Therapist must be documented. (3 Social Science courses, in which 2 of them must be Psychology electives) Psychology Elective - 3 cr. Psychology Elective - 3 cr. Social Science Elective - 3 cr. MA Pre-Calculus Algebra - 2 cr. MA Pre-Calculus Trigonometry - 2 cr. or MA Pre-Calculus Algebra and Trigonometry - 4 cr. ST Statistical Reasoning - 3 cr. or BUS Applied Business Statistics - 3 cr. All Science prerequisites must have labs and must be courses a science major would take toward degree requirements. No Intro or Survey courses accepted. BLY General Biology I - 3 cr. Social Science or History Sequence* - 3 cr *Must complete at two course sequence in either Literature or History. **Electives should be concentrated in a back-up major of the student s choice NOTE: For all prerequisite courses, credits older than ten years from the application deadline must be repeated or validated by examination.

5 Recommended Courses: A number of courses are determined to be good preparation for certain components of the PT graduate program and therefore are recommended if they fit into the applicant s preparation: Microbiology or Infectious Disease (required for Advanced Undergraduate applicants), Medical Terminology, Kinesiology, Exercise Physiology, Basic Motor Learning, upper level Anatomy, Neuroscience, Pathophysiology, Abnormal Psychology, Physiological Psychology, Developmental Psychology. Additional Information for Advanced Undergraduate Applicants ONLY: Students not getting a BS or BA degree, who have completed 90 semester hours of prescribed and elective course work, who meet all admission criteria and the terms specified below are eligible for application to the program. Thus, these students may be able to complete the DPT degree in 6 years from entry into college. Admissions criteria and terms of eligibility for Advance Undergraduate ONLY: According to University policy, no more than 60 semester hours of transfer credit from two-year colleges will be accepted. They will be officially classified as an undergraduate for the first 2 semesters of the program but will be taking graduate courses of the DPT program. In the first 2 semesters of the program, while classified as undergraduate students, any financial aid is restricted to the types and amounts for which undergraduate students are eligible. Upon satisfactory completion (minimum GPA of 3.0 in all course work) of the first 2 semesters of the program, they will be awarded the Bachelor of Science in Pre-Professional Health Science degree. These students must then complete application for reclassification and admission to the Graduate School. Upon satisfactory completion of the next 6 semesters of the DPT program, students will be awarded the Doctor of Physical Therapy (DPT) degree. Tuition will be assessed at the graduate level for the entire 8 semesters of the professional program. The academic program for students in both categories is identical in its entirety. For the entire 8 semesters of the program they will be held to the same standards and policies as students enrolled in the graduate school (see the policies of the Graduate School). Selection Process Admissions rankings are determined using a calculation that includes identical components for both groups of applicants. A limited number of applicants are invited for interview with the PT faculty and PT clinicians from the community based upon academic ability including cumulative GPA, GPA emphasizing Science prerequisites, and GRE scores and aptitude for a career as a physical therapist. Final admission decisions are based upon GPA, GRE and Interview score. Students accepted into the program must submit completed health and physical exam forms to include required immunizations and background checks. Accepted students must also read, sign, and submit the Core Performance Standards document outlining fundamental tasks which applicants should be capable of performing upon entering the PT program.
