3 Mandated Information This directory is intended to give you a list of agencies, telephone, fax, toll-free numbers and websites for services offered to children, women and families. Included in this publication is information about public and private agencies and services available to assist women during and after pregnancy, upon childbirth and while the child is dependent, including adoption agencies. North Dakota Century Code, Section (1) (a) states: It is unlawful for any individual to coerce a woman to undergo an abortion. If a minor is denied financial support by the minor s parent, guardian, or custodian due to the minor s refusal to have an abortion performed, the minor is deemed to be emancipated for the purposes of eligibility for public assistance benefits, except that those benefits may not be used to obtain an abortion. Any physician who performs an abortion upon a woman without her informed consent may be liable to her for damages in a civil action. Adoptive parents are allowed to pay costs of prenatal care, childbirth and neonatal care. There are many public and private agencies willing and able to help you to carry your child to term and to assist you and your child after your child is born, whether you choose to keep your child or to place your child for adoption. The state of North Dakota strongly urges you to contact one of more of these agencies before making a final decision about abortion. The law requires that your physician or your physician s agent give you the opportunity to call agencies like these before you undergo an abortion. It is the policy of the state of North Dakota that childbirth is given preference, encouragement and support as it is in the best interests of the well-being and common good of North Dakota citizens. 2

Breast milk is easy to digest and contains antibodies that can protect infants from bacterial and viral infections. Research indicates that women who breastfeed may have lower rates of certain breast and ovarian cancers. It is also recognized by the Centers for Disease Control and Prevention (CDC) as a primary strategy to reduce childhood obesity. The North Dakota Department of Health, Division of Nutrition and Physical Activity s WIC Program, Healthy Communities Program and Maternal and Child Health Nutrition Program work together to promote breastfeeding and improve support for breastfeeding in all settings. 12 North Dakota Department of Health Division of Injury Prevention and Control 600 E. Boulevard Ave., Dept nd Floor, Judicial Wing, State Capitol Bismarck, N.D Telephone: Toll-Free: Fax: Website: Child Passenger Safety Program As part of the Division of Injury Prevention and Control, the Child Passenger Safety Program exists to decrease injuries and death to children due to motor vehicle crash events. The program uses a variety of best practice strategies including data collection and analysis, design and development of initiatives, training and technical assistance, policy and advocacy and evaluation. Program activities include: Designing educational materials and developing special campaigns. Promoting proper use of car safety seat and seat belts. Providing car safety seats to local distribution programs. Coordinating and conducting car safety seat checkups (call for car seat checkup locations and schedule). Providing technical assistance to professionals and consumers. Providing training ranging in length from four to 32 hours. 11

Programs within the division include: Care Coordination CSHS supports community-based programs to help families who have children with special health care needs access services and resources. Partners include county social services and local public health. Children with Special Health Care Needs Service System CSHS supports initiatives that lead to a community-based system of services for all families, children and youth with special health care needs. Activities focus on screening, transition, medical home, family partnership and satisfaction, adequate insurance, and community-based service systems. Information Resource Center CSHS provides health-care resource information to families and service providers. Metabolic Food CSHS provides medical food and low-protein modified food products to individuals with phenylketonuria (PKU) and maple syrup urine disease. Multidisciplinary Clinics CSHS funds and administers clinics that support coordinated management of 10 different types of chronic health conditions. Clinics provide access to pediatric specialty care and enable families to see many different medical providers and health-care professionals in one place at one time. 14 Russell-Silver Syndrome Program CSHS pays for growth hormone treatment and medical food for individuals with Russell-Silver Syndrome. Specialty Care Diagnostic and Treatment Program CSHS helps families pay for medical services for eligible children, including health-care visits and tests to diagnose chronic health conditions early and specialty care needed for treatment. State Systems Development Initiative (SSDI) SSDI funding supports enhancements in Maternal and Child Health (MCH) data capacity and infrastructure, which is used to inform MCH programs, policy and practice. Boulevard Ave., Dept nd Floor, Judicial Wing, State Capitol Bismarck, N.D Telephone: Toll-Free: Fax: Website: Community Health Section The Community Health Section (CHS) is one of seven sections within the North Dakota Department of Health. The CHS is responsible for the public health of all citizens from birth to death. The section s goal is to promote health and prevent illness and disease. Mission The purpose of the Community Health Section is to support individuals, families and communities by providing quality programs that protect and enhance the health and safety of all North Dakotans. To accomplish this mission, CHS staff: Advocate for healthy behaviors. Provide education, resources and services. Develop effective policies. Engage in statewide partnerships. Division of Cancer Prevention and Control Mission: The mission of the Division of Cancer Prevention and Control is to increase cancer prevention and awareness by engaging in partnerships, collecting and reporting data, assuring quality data, providing public and professional education, and assuring availability of quality services for screening, treatment, rehabilitation and palliative care. Programs and services within the division include: Behavioral Risk Factor Surveillance System (BRFSS) Tracks health conditions and risk behaviors in the United States and North Dakota via the world s largest, ongoing telephone health survey system conducted yearly since BRFSS data is used to develop health policy, design and evaluate public health programs, monitor the health risks and health status of North Dakota residents, determine the attitudes and beliefs of North Dakotans about important health issues, and meet data needs for researchers both in the state and nationwide. Comprehensive Cancer Prevention and Control Program Works with the North Dakota Cancer Coalition to reduce the incidence and impact of cancer for all North Dakotans using the North Dakota Cancer Control Plan as a guide. Areas addressed include: cancer prevention by implementing strategies that encourage healthy behaviors; increasing cancer screening; addressing needs for access to cancer treatment; and, improving quality of life for cancer survivors. 14
16 Healthy People 2020 Provides science-based, 10-year national objectives for improving the health of all Americans. North Dakota Statewide Cancer Registry (NDSCR) Is located on the campus of the University of North Dakota Grand Forks and housed with the Department of Pathology in the School of Medicine and Health Sciences. The purpose of the NDSCR, established in 1997, is to collect cancer incidence, survival and mortality data to monitor cancer trends, promote research, increase survival, develop cancer education, guide policy planning for cancer prevention and screening programs and respond to cancer concerns from patients or the public. The NDSCR is a collaborative partnership between the North Dakota Department of Health and the University of North Dakota. Data provided by NDSCR is used to guide decisions for the Department of Health cancer programs. Women s Way (the North Dakota Breast and Cervical Cancer Early Detection Program) Provides breast and cervical cancer screening services to North Dakota program-eligible women. The program supports collaborations with community-based organizations including health-care providers and public health to increase public and health-care provider education, access to care and screening services. Division of Chronic Disease Mission: To improve the health and quality of life for North Dakotans who have chronic diseases by promoting healthy behaviors, supporting health-care improvement measures, developing community policies and practices, and increasing disease risk awareness and by reducing the negative health and economic consequences of the state's number one cause of preventable disease and death tobacco use. The division accomplishes their mission by providing grants, training, community education and technical assistance to communities. Programs and services within the division include: Coordinated Chronic Disease Prevention Program (CCDPP) The CCDPP is designed to build capacity to address chronic disease prevention and health promotion in a coordinated, collaborative approach to change policies, practices and environments. This will lead to improved quality of life and health outcomes and promote education and management skills for those diagnosed with or at risk for chronic diseases. Heart Disease and Stroke Prevention Works to improve cardiovascular health of North Dakotans by facilitating effective collaborations and partnerships, monitoring critical aspects of cardiovascular disease and developing effective strategies to reduce heart disease, stroke and related risk factors. The overarching statewide emphasis is based on the ABCS of heart disease and stroke prevention with a main focus on preventing and controlling blood pressure and reducing sodium intake using population-based strategies. The ABCS include aspirin therapy, blood pressure control, cholesterol control and smoking cessation. The program provides grants, training, education, resources and technical assistance to communities, health-care providers and health organizations. North Dakota Stroke System of Care Established to improve the overall quality of care for stroke patients, from prevention of risk factors to the final stroke outcome. It is comprised of several key components: State Stroke Registry Program including the management of the grant program for local 15
17 hospitals and aggregate registry database; department designation of hospitals as Primary Stroke Centers; implementation of statewide health communications to increase the awareness of the signs and symptoms of stroke and urgency to seek immediate medical care by calling 9-1-1; and provision of training and technical assistance to local hospitals, health-care providers and pre-hospital personnel on acute stroke care. Stroke System of Care Task Force Facilitates the sharing of best and promising practices to reduce duplication, identify gaps and advocate for positive policy and systems change. Provides leadership for maintaining an inclusive and coordinated statewide system of care and education that continuously improves the knowledge, diagnosis, treatment and rehabilitation of stroke patients and reduces the overall stroke risk for all North Dakota citizens. The Heart Disease and Stroke Prevention program director provides leadership for the implementation of the task force recommendations and serves on the task force as the state health officer designee. Tobacco Cessation Services Provides grants for cessation programs addressing pregnant women, incorporating tobacco into health-care systems and dental offices and city, county and state employee cessation programs. Also supports the statewide NDQuits cessation service for all tobacco users who want to quit. NDQuits offers assistance via several options telephone (North Dakota Tobacco Quitline), online (North Dakota QuitNet) and via mobile devices. Tobacco Prevention and Control Program Provides grants, training and technical assistance for tobacco prevention and control efforts in communities including American Indian tribes. The Goal is to reduce disease, disability and death related to tobacco use by: Preventing initiation among youth and young adults. Promoting quitting among adults and youth. Eliminating exposure to secondhand smoke. Identifying and eliminating tobacco-related disparities among specific population groups. Tobacco Surveillance Commissions multiple surveys designed to measure a variety of factors related to tobacco use among North Dakota citizens and assess how these factors change over time. Division of Family Health Mission: The Division of Family Health administers state and federal programs designed to improve the health of North Dakota families. The division provides funding, technical assistance, training, needs assessment, data, educational materials and other resources to local public health units, schools and other public and private entities that offer health services in North Dakota communities. Programs and services administered by the division include: Coordinated School Health Works in conjunction with the Department of Public Instruction to address the close relationship between health and learning and provides consultation, technical assistance and resources for schools and school nurses to use in organizing and managing school health and wellness initiatives. Cribs for Kids Program Provides infant safe sleep education and portable cribs to pregnant women and new mothers through partner agencies. 16
18 Family Planning Program Provides reproductive health-care services to men and women, giving preference to low income, adolescent and women-in-need populations. Services include Pap smear, breast exam, testicular exam, infertility level-one services, pregnancy planning, a broad range of birth control methods including abstinence, sexually transmitted diseases (STD) and human immunodeficiency virus (HIV) testing, and counseling. Fetal Alcohol Syndrome Provides grant oversight to the University of North Dakota s Fetal Alcohol Syndrome Center for program activities. Maternal and Child Health (MCH)/Oral Health Epidemiology Provides epidemiological support on all matters concerning the development, evaluation and prioritization of MCH and oral health programs. Newborn Screening Program Identifies infants at risk and in need of more definitive testing to diagnose and treat affected newborns. Provides regional coordination for North Dakota, South Dakota and Iowa focusing on quality assurance and education. Optimal Pregnancy Outcome Program (OPOP) Provides nursing, social and nutritional services to pregnant women. Oral Health Program Provides prevention programs, education, access, screening and consultation to address the oral health needs of North Dakotans. DentaQuest Foundation A collaborative process with stakeholders to create A Blueprint for Oral Health improvement in North Dakota. Donated Dental Services Provides grant oversight to services that provide essential dental care for disabled, elderly and medically compromised individuals who cannot afford care. Healthy Smiles Fluoride Varnish Program Provides fluoride varnish for infants, children, adolescents and young adults to age 21 years in various settings such as local public health, Head Start, clinics and schools. Seal! North Dakota Provides sealants to second and sixth grade students through schoolbased and school-linked programs to prevent dental decay in molar (back) teeth. Sudden Infant Death Syndrome (SIDS) Program Provides support, education and follow-up to those affected by a sudden infant death. Title V Maternal and Child Health Provides consultation, technical assistance and comprehensive services to improve the health, safety and well-being of mothers and children. Women s Health Services Collaborates with public and private programs that provide and/or advocate for women s health. Provides national, state and local women s health information across the state. Division of Injury Prevention and Control Mission: The Division of Injury Prevention and Control is dedicated to reducing injuries to North Dakotans through leadership, education, resources and policy. 17
19 Programs and services administered by the division include: Child Passenger Safety Program Promotes child passenger safety activities through educational campaigns, car seat distribution programs, car safety seat checkups and trainings. Domestic Violence/Rape Crisis Supports programs to develop effective law enforcement and prosecution strategies to combat domestic violence, sexual assault, dating violence and stalking crimes and strengthen victim services, and partner with stakeholders to develop and implement policies, provide training and technical assistance and evaluate programs. Injury/Violence Prevention Programs Promotes the reduction and elimination of intentional and unintentional injuries using best practice strategies, including primary prevention theories, data collection and analysis, intervention design and development, training and technical assistance, policy advocacy and evaluation. Suicide Prevention Collaborates with the North Dakota Suicide Prevention Coalition to implement programs on suicide prevention and early intervention to reduce the number of attempted and completed suicides. Division of Nutrition and Physical Activity Mission: To support growth and development; prevent overweight and obesity; and prevent and control diabetes through programs designed to improve healthful eating and physical activity. Programs and services within the division include: Breastfeeding Promotion and Support Breastfeeding is universally endorsed by the world s health and scientific organizations as the best way of feeding infants and young children. It is also recognized by the Centers for Disease Control and Prevention (CDC) as a primary strategy to reduce childhood obesity. The Nutrition and Physical Activity Programs work together to promote breastfeeding and improve support for breastfeeding in all settings by providing leadership for the statewide breastfeeding coalition, providing technical assistance to the local breastfeeding coalitions and coordinating the infant-friendly workplace designation program. Diabetes Prevention and Control Supports diabetes prevention, early diagnosis and disease management by working with communities, health professionals and health systems in the areas of policy, disease management, quality improvement and education. Healthy Communities Healthy Communities Chronic Disease is responsible for collaboration with local and statewide governmental and non-governmental agencies/organizations to build proficiency and capacity for policy, systems and environmental (PSE) changes that support health behaviors to prevent chronic disease. Healthy Communities Maternal and Child Health Nutrition promotes nutritional well-being across the lifespan for women, infants and children. By providing leadership and support to local public health nutritionists, the program encourages networking and information exchange at the local, state and national levels. 18
20 Special Supplemental Nutrition Program for Women, Infants and Children (WIC) Offers healthy foods for proper growth and development and helps families choose healthier ways of eating. WIC is for eligible pregnant, breastfeeding and postpartum women, infants and children younger than 5 and is available in all North Dakota counties. Worksite Wellness The North Dakota Department of Health s Worksite Wellness Program promotes a holistic wellness culture by providing opportunities for employees to improve their health and wellbeing. This position schedules plans and facilitates monthly meetings and activities in order to foster a healthier lifestyle for all of the Department of Health employees. Healthy North Dakota The Community Health Section is linked closely to Healthy North Dakota, which is a partnership that brings together partners and stakeholders to identify common strategies to address health issues. Services Family planning services are available regardless of age, gender, race, nationality, religion, sexual preference, disability or ability to pay. Participation is voluntary. Services include: Physical exams for men and women. Education and information about all birth control methods, including abstinence and natural family planning. Birth control methods and referral for sterilization procedures. Breast, cervical and testicular cancer screening. Diagnosis and treatment of sexually transmitted infections. Preconception and life plan counseling. Pregnancy testing. Community education and outreach. WIC offers healthy food for proper growth and development and helps families choose healthier ways of eating. What is Women s Way? Women s Way is the North Dakota Breast and Cervical Cancer Early Detection Program. Women s Way may provide a way to pay for most breast and cervical cancer screenings and is available for eligible North Dakota women. Who is eligible for Women s Way? A woman who: Is a resident of North Dakota. Is between ages 40 through 64. Meets income guidelines. North Dakota local public health units have a long history of providing personal and population based health services to residents in their city and/or county jurisdictions. Relay TTY: Fax: Website: Developmental Disabilities Division Description Developmental Disabilities provides support and training to individuals and families in order to maximize community and family inclusion, independence, and self-sufficiency; to prevent institutionalization; and to enable institutionalized individuals to return to the community. To achieve this goal, Developmental Disabilities contracts with private, nonprofit, and for-profit organizations to provide an array of residential services, day services, and family support services. Funding to pay for services may come from a number of sources including personal resources, Medicaid and Department of Human Services. Early Intervention North Dakota Early Intervention serves infants and toddlers with disabilities, developmental delays, or who are high-risk for developing delays birth to age 3. These services and supports are guided by Part C of the federal Individuals with Disabilities Education Act (IDEA). Eligibility for the program is determined through the Developmental Disability Program Management Units at each of the regional human service centers. Every child who qualifies is part of a team, which also includes their parents or guardian, a service coordinator from the family s regional human service center and, in most cases, an infant development provider. The team develops an Individualized Family Service Plan (IFSP), which drives outcomes and services. Services are delivered in the family s home and other natural environments for that child. North Dakota utilizes the Developmental Disabilities unit at each regional human service center to serve as the single point of entry for North Dakota s Early Intervention services. Relay TTY: Fax: Website: Medical Services Division Health Tracks North Dakota Health Tracks is a free preventive health program for children ages 0 to 21 who are eligible for Medicaid. Health Tracks pays for screenings, diagnosis and treatment services to help prevent health problems from occurring or to help keep health problems from becoming worse. Health Tracks also pays for orthodontics (teeth braces), glasses, hearing aids, vaccinations, counseling and other important health services. Health Tracks staff will help schedule appointments for services and will also help locate transportation. Some services require prior authorization, so be sure to check with your screener about these requirements. Healthy Steps (CHIP Children s Health Insurance Program) Healthy Steps insurance is for children who: Do not have health insurance coverage. Are 18 years of age or younger. Medicaid was authorized in 1966 for the purpose of extending medical care and services to people whose resources are limited. Medicaid pays for health services for qualifying families with children, women who are pregnant, as well as the elderly and individuals who are disabled. Medicaid provides coverage to: Children in foster care or subsidized adoption. Children with disabilities (birth to 19). Other children up to age 21. Pregnant women. This division is the single state authority responsible for overseeing a statewide network of substance abuse and mental health treatment, recovery support services, mental health promotion, and substance abuse prevention services. This translates into licensing, training, development and implementation of appropriate mental health and substance abuse services throughout the state. The division writes grants for mental health, substance abuse treatment and substance use prevention, and homelessness, and monitors the grant activities for compliance, and reports on challenges, accomplishments, and client and program outcomes. The division manages the traumatic brain injury (TBI) efforts, community-based high risk sex offender treatment, problem gambling treatment, the contract for long-term residential treatment services for individuals addicted to methamphetamine and other controlled substances, and licenses psychiatric residential treatment centers and regional human service centers. Lastly, the division offers prevention specialist services to targeted communities, implements population-focused prevention strategies, and offers a clearinghouse of prevention materials regarding substance abuse. 54

Parent Resource Centers The North Dakota Parent Education Network provides for ongoing training, collaboration, delivery and evaluation of consistent, research-based programming. Many of the women involved with these deliveries have risk factors that could result in poor birth outcomes. Also, many Medicaid enrolled women lack the necessary resources to ensure that their newborns receive all the care that is necessary for these infants to reach their full potential. The provision of the Targeted Case Management service is to assist at-risk pregnant women in their desire to achieve a healthy birth outcome by: Reinforcing the need for each woman to receive early, comprehensive and continuous prenatal care; Helping each pregnant woman understand her risk factors that could affect birth outcomes and what actions can be taken to lessen those risks; and Assisting women in obtaining other necessary support services that will contribute to a healthy birth outcome. 117

153 Housing Services: Community Housing Development Organizations (CHDO) CAAs are nonprofit developers of affordable housing for low-income individuals and can assist with acquisition and rehabilitation of existing rental housing; rehab of existing rental housing owned by another entity; construction of new rental housing; acquisition and rehab of existing housing for sale of first-time homebuyers; and building new housing for sale to first-time homebuyers. Home/Owner Occupied Rehabilitation Intended to preserve and extend the life of existing housing in North Dakota. The goal is to provide assistance to low- and moderate-income homeowners in maintaining their homes as safe, sanitary dwellings. Priorities include correcting structural deficiencies, meeting code standards, weatherization and general home improvements. Supportive Services: Food and Nutrition Numerous food pantries are administered and supported by CAAs across the state. Individual Development Accounts (IDAs) IDAs allow participants an opportunity to acquire a lasting asset after saving for an extended period of time. The three eligible assets are college tuition, a first home or small business capitalization. Participants are required to save for at least six months. At the end of the program, every dollar deposited into the IDA by the participant is matched 2:1. In North Dakota, participants are able to save up to $2,000 for a match of $4,000. Self Reliance Program This program is designed to help individuals and families become economically self-sufficient. A case manager assists program participants on a one-to-one basis in setting goals and achieving them through: Identifying and resolving problems. Identifying community resources. Exploring educational and employment opportunities. Evaluating career choices and job-seeking skills. Building and improving self-confidence. The types of infants treated in Level II and Level III NICUs often overlap and are inconsistent from region to region.
