The following amendments, additions, andlor corrections shall govern this work. This Addendum incorporates the following items: 1. Meeting minutes and sign-in sheet from pre-bid meeting. 2. 2 Solicitation: Subject: Bids/Proposals Due: Carpet and Upholstery Cleaning, Repair and Water Restoration Services - Request for Proposals Camden Yards Sports Complex Meeting Minutes from Pre-Bid Meeting Tuesday, May 16, :OO p.m. - Maryland Stadium Authority's Lobby Thursday, June 1,2006 no Inter than 2:00 p.m. MBE Goal: Twenty Five Percent (25%) Procurement Officer: John Samoryk, Procurement Officer MSA Attendees: John Samoryk Jeff Provenzano Susan Thorman Jane Smith Theresa Masilek The meeting was opened by John Samoryk who acknowledged the above referenced MSA attendees. Discussion of the Request for Proposals procurement method took place. All technical proposals wil be evaluated and interviews may be conducted. Price Proposals will be opened only from those contractors considered to be technically qualified to perform the scope of work. The overall award of the contract will be based upon the best value of the combined technical and price proposals. All requested technical information must be completed in its entirety to enable a proper evaluation of the contractor's technical requirements. Incomplete submissions may be rejected as non-responsive. Potential bidders were advised to allow an adequate amount of time for the submission of the RFP -no late submissions will be considered. A twenty-five percent (25%) MBE goal has been established for this project. Participants were advised that the submission of the MBE forms, as outlined in the Proposal, is mandatory; incomplete submissions will be rejected as non-responsive. All were advised to contact Mr. Sarnoryk should they have any problems in fulfilling the MBE goal. In addition, bidders were advised that the awardee would be responsible for the payment of the emarylandmarketplace fee, as outlined in the bid documents. Any questions regarding this procurement should be submitted, in writing, to the Procurement Officer within a reasonable amount of time to respond to all interested contractors (likely no less than five (5) business days prior to the deadline for submission of proposals). It is anticipated that a contract should be in place by July 1, A tour of the areas to be cleaned at Oriole Park at Carnden Yards, the Warehouse and M&T Bank Stadium took place. For assistance in locating certfied, State of Maiyland, minority btrsinesses please refer to the Mnryland Deparinrent of Tranporiation 's, Minority Business Enterprise website at Does the Maryland Stadium Authority recognize Federal MBE certification? No. Are the frequencies for cleaning specified? Anticipatedfrequencies for the cleaning of the caipet are pecified. However, these are estimates only. There is not a regular cleaning schedule that will be followed by the contractor. Maryland Stadium Authority will direct contractor when services are required.
