1 MONTANA STATE UNIVERSITY COLLEGE OF NURSING POLICY # A-25 TITLE: POLICY: RATIONALE: PROCEDURE: FACULTY SHORT AND LONG FORM CURRICULUM VITAE Each faculty member will develop short and long form curriculum vitae (c.v.) and revise annually. The short form c.v. is submitted electronically to the Dean s Administrative Assistant each spring. The long form c.v. is submitted to the Campus Director each year with the annual review materials. This c.v. is stored in the faculty personnel file on each Campus with a copy sent to Bozeman as needed for preparation of reports, accreditation self-study, etc. The long form document serves as a standardized current faculty data base for College of Nursing administrators and committees to prepare timely, accurate reports, summaries, etc., regarding the College. It can also be used as the faculty member s current vita or related document. The short form document is required for the annual report submitted to the Montana State Board of Nursing each spring. Updates are to be done by all faculty members once each year during spring semester. The templates for the Short and Long Form documents are located at: ATTACHMENT #1: Directions for Long Form C.V. ATTACHMENT #2: Directions for Short Form C.V. Reviewed/Approved by: Level I: Executive Council (May, 2006) Level II: Dean (August, 2006)
2 POLICY #A-25 ATTACHMENT #1 DIRECTIONS LONG FORM C.V. Throughout, 1. List chronologically, beginning with most recent. 2. Do not abbreviate names of professional organizations, journals, etc. 3. Under year or date sections indicate beginning and ending years or dates (e.g., or June 2-6, 2005) or beginning year/date and on-going status (e.g., 2004-current or June 6, current). Title Page: Basically self-explanatory. Title: Give current university rank, e.g., Assistant Professor or rank and administrative title, e.g., Assistant Professor and Campus Director. Date of Appointment to MSU: Should be month and year. Date of Current Rank: Should be month and year and indicates when one acquired the present rank, e.g., Associate Professor. National Certifications: List years and Certification Board(s). Professional Education: should be year degree was awarded. If currently pursuing a degree, note in process under. Previous Teaching Experience: List dates, organization(s), and position(s) of teaching appointments prior to the current MSU appointment. Professional Experience: List dates, organizations, and positions of prior or current professional nursing positions other than teaching positions. Includes Faculty Practice. Courses Taught (past 10 years only): List courses taught at MSU in the past 10 years. Include only those in which you had a major teaching responsibility. Give full course titles. Guest lectures may be listed if they were one's major teaching activity and if identified as guest lectures. Graduate Student Thesis/Project Committees: List Semester/, Course No. (N575, N590), Title of Thesis/Project, Student Name, Position on Committee (chair, member). Professional Memberships: List all organizational activities separately by position.
