Jace Wolfe, Ph.D. No relevant financial relationship Jace Wolfe is the Director of Audiology at the Hearts for Hearing Foundation. He also is an adjunct Assistant Professor in the Audiology Department at the University of Oklahoma Health Sciences Center and Salus University. He previously served as the editor for the American Speech Language Hearing Association s Division 9 journal and is currently a co editor for the Plural Publishing, Inc. Core Clinical Concept Series on Cochlear Implants. Dr. Wolfe is a member of the Better Hearing Institute s Pediatric Advisory Board as well as the Audiology Advisory Boards for Cochlear Americas, Advanced Bionics, and the Phonak Hearing Aid Company. He is also serves on the Editorial Board of The Hearing Journal, and he is a reviewer for several peer reviewed journals. Additionally, Dr. Wolfe co authors a periodic column entitled Small Talk in The Hearing Journal, and he has published numerous articles in professional peer reviewed and trade journals and is the first author of the textbook entitled Programming Cochlear Implants. His areas of interests are pediatric amplification and cochlear implantation, personal FM systems, and signal processing for children. He provides clinical services for children and adults with hearing loss and is also actively engaged in research in several areas pertaining to hearing aids, cochlear implants, hybrid cochlear implants, and personal FM systems. John T. Roland, M.D. Financial relationship with Cochlear Americas Advance Bionics, receives a consulting fee and is a member of Cochlear Advisory Board Dr. Roland is the chairman of the Department of Otolaryngology Head and Neck Surgery at the NYU Langone Medical Center. His clinical work is in the disciplines of Otology/Neurotology and Skull Base Surgery. He is responsible for training of fellows and residents and has a very active clinical practice. He is also Co director of the NYU Cochlear Implant Center, one of the largest and most productive centers in the world. He clinically manages patients and conducts research in Cochlear Implants, Auditory Brainstem Implants and the management of Acoustic Neuroma and other Skull Base Lesions. He also has an interest in the management of facial paralysis and has developed a modification of a technique for nerve grafting. More recently, with the assistance of Dr. Allen in pediatric neurology and Dr. Golfinos in neurosurgery, he has initiated a Center of Excellence in Neurofibromatosis. Dr. Roland was a Lt. Commander U.S. Public Health Service at the Fort Yuma PHS Indian Hospital where he was Clinical Director He is currently the inactive reserves.

She has authored over 40 peer reviewed papers and has delivered over 100 podium presentations at national and international scientific conferences. Her research has been funded by the NIH NIDCD for over 10 years and focuses on combined of electric and acoustic hearing including basic auditory function and speech perception. Susan B. Waltzman, PhD is the Marica F. Vilcek Professor of Otolaryngology and Co Director of the NYU Cochlear Implant Center at NYU Langone Medical Center/NYU School of Medicine. Her research is focused on two main areas: outcomes with cochlear implants and isolating the variables which affect performance with cochlear implants. Current and on going investigations include the impact of new technology on different ci populations and outcomes in children, adolescents, older adults and special populations. Dr. Waltzman is the author of more than 90 peer reviewed articles, numerous book chapters and a textbook on cochlear implants. She also has been invited as a Visiting Professor at many institutions world wide and is a frequent contributor and invited speaker at national and international conferences. William H. Shapiro, AuD Financial relationship: Receives a consulting fee and is a Member of the Cochlear Surgeons Advisory Board William H. Shapiro received his Master of Arts degree in Audiology in 1978 from Queens College of CUNY. He received his Doctorate of Audiology from the Arizona School of Health Sciences, A.T. Still University. He has worked at the New York University Medical Center since 1984, where he has been involved with all aspects of diagnostic and rehabilitative audiology, including the dispensing and fitting of assistive hearing technology such as hearing aids, cochlear implants, and FM systems. He currently holds the position of Clinical Associate Professor in the Department of Otolaryngology, and is the Director of Faculty Group Practice Audiology. He also is the Supervising Audiologist at the New York University Cochlear Implant Center, a nationally recognized center of excellence in the field of cochlear implants. Dr. Shapiro has co authored cochlear implant related book chapters, as well as publications in peer reviewed journals. He is a frequent lecturer at professional conventions, both national, and international. He currently serves on the advisory boards of two cochlear implant companies. He is the 2008 recipient of the Outstanding Clinician Award received from the New York State Speech, Language and Hearing Association.

3 Cochlear Americas Presenters: Alexandra Hatton, Au.D., CCC A Sr. Alex Hatton received her Bachelor of Science degree in Speech/Language Pathology from the University of Nevada in She then went on to graduate school to study Audiology at Idaho State University. Her area of interests was always cochlear and auditory brainstem implantation and rehabilitation of patients. After graduating in 1997, Alex worked as a clinical audiologist in the cochlear implant program at Oregon Health and Science University in Portland, Oregon for 5 1/2 years. As of May of 2003, Alex joined Cochlear Americas as a clinical application specialist for the Pacific Northwest. She provides technical/clinical support for clinics in this region. Allison Mester, Au.D., CCC A, Ohio River Valley Allison Mester is a certified and licensed audiologist. Allison earned her Bachelor of Science degree in Hearing, Speech and Language Sciences and her doctorate degree in Audiology from Ohio University. Allison is a for Cochlear Americas serving the Ohio River Valley Territory. In her role, she provides surgical and clinical support for both cochlear implant and Baha products. Amy Popp, Au.D., FAAA, CCC/A Nonfinancial relationship: Publications in the field of Cochlear Implants Amy Popp, Au.D., FAAA, CCC/A is a at Cochlear. Since joining Cochlear in September 2001 she has also held the roles of Principal and Clinical Educator. While at MEETH/LHH, Amy followed over 500 pediatric and adult cochlear implant recipients. Amy has collaborated in numerous publications and presentations regarding cochlear implants. She received her bachelor's degree from Miami University in Oxford, Ohio, her master's degree from Washington University in St. Louis, Missouri and her clinical doctorate from Central Michigan University. Barbara Buck, Au.D., CCC A Barbara Buck, Au.D., CCC A is a Regional Clinical Technical Manger. Prior to her current role, she was in roles including Principal, Territory Manager, and Clinical Educator with Cochlear. Prior to joining Cochlear in May 2001, Barbara was the Cochlear Implant Coordinator at Allegheny General Hospital in Pittsburgh. She received her Bachelor's & Master s degrees from Clarion University, and her Au.D. from the University of Florida Distance Learning Program.

4 Carla M. Facer, AuD., CCC A Nonfinancial relationship: Publications in the field of Cochlear Implants Carla joined Cochlear Americas in October of 2000 as a Clinical Application Specialist. Carla has served in several job roles while at Cochlear including;, Clinical Territory Manager, Territory Manager, and Territory Outreach Specialist. Carla s interest in cochlear implants began after her father initiated the cochlear implant program at Mayo Clinic, Rochester, in She completed her CFY in 1997 at Mayo Clinic, Rochester, working in vestibular testing, pre implant evaluation, and cochlear implant programming and worked with the first several patients with auditory dys synchrony who received cochlear implants. She has co authored several articles concerning cochlear implantation. Carla received her Masters of Science degree from Purdue University in 1996 and her Au.D. from the University of Florida in Prior to joining Cochlear Americas, she worked in a private practice in Nashville, TN, providing diagnostic and electrophysiologic testing for adult and pediatric populations. Chanel Coleman, AuD, CCC A, FAAA Chanel Coleman is a for Cochlear Americas serving the New York City Metro area. In this role, she provides training and education related to cochlear implants, in addition to assistance with the clinical care of CI and Baha recipients. She earned her Doctor of Audiology degree at The University of North Carolina at Chapel Hill where her clinical training was focused in pediatric diagnostics and management. Prior to joining Cochlear in January 2012, she gained her cochlear implant experience as a clinical audiologist at The Cochlear Implant Center of New Jersey at The Rutgers University Hospital and as a doctoral fellow at The New York Eye & Ear Infirmary. She was closely involved with the management of pediatric, adult and multi handicapped cochlear implant recipients and uses the skills she gained to serve the needs of the centers within her region. Christine Menapace VP, Clinical, Quality, Regulatory and Education Christine has focused her career in the field of implantable auditory technology including cochlear implants, implantable middle ear devices and bone anchored hearing devices. She has a Master of Arts in communication Disorders with an emphasis in Audiology from San Diego State University. Christine is currently Vice President of Clinical, Quality, Regulatory, and Education for Cochlear Americas. Janet Clarke, Au.D., CCC A, FAAA Operations Learning Manager Prior to her current role, Janet was in the roles of Principal as well as Product Validation Audiologist with Cochlear Americas. Janet was on the cochlear implant team at the University of Illinois at Chicago for five years prior to joining Cochlear Americas in June She received her Bachelor's & Master s degrees from Purdue University, and her Au.D. from the University of Florida Distance Learning Program.

Ballard, Au.D, CCC A Clinical Territory Manager Jessica Ballard, Au.D., received her undergraduate training in Communication Disorders at Eastern Washington University, and obtained a Doctor of Audiology degree from the University of Oklahoma Health Sciences Center in Following completion of her education, Jessica worked as a cochlear implant audiologist at Hearts for Hearing in Oklahoma City for three years specializing in pediatric audiology services and cochlear implants. In September 2009, Jessica joined Cochlear Americas as Outreach Specialist for the North Texas and Oklahoma Territory. In this position, Jessica works to provide education and training to many hearing healthcare professionals, as well as clinical support. Jessica Melton, AuD Jessica is a licensed and certified Audiologist. She trained at the University of Illinois Champaign Urbana and earned a Doctorate of Audiology at Rush University Medical Center in Since 2007, Jessica has served Cochlear Americas as both a Territory Outreach Specialist and a Clinical Territory Manager. Today, she provides clinical and technical support for cochlear implant centers in the Upper Central Territory. Jillian Crosson, Ph.D., CCC A Principal Clinical Project Manager Jillian has a dual role of Principal for the Midwest Territory, where she applies a focus on customer care and product education and Manager of Clinical Studies at Cochlear, responsible for studies that address marketing or clinical needs. She received her Masters in Audiology as well as her Ph.D. in Speech and Hearing Science from Washington University, St. Louis, MO. Prior to Cochlear, Jillian was a Clinical Audiologist on the Pediatric Cochlear Implant Team at Tygerberg Hospital in CapeTown, South Africa. After two years, she became Chief Audiologist. Later, Jillian was a Clinical Audiologist at the Central Institute for the Deaf plus an Assistant Professor in the Department of Speech and Hearing, administered by Central Institute for the Deaf at Washington University. Leigh Ann Monthey, M.A., CCC A Principal Leigh Ann Monthey, M.A. CCC A received her Bachelors of Science degree from the University of Nebraska and her Masters of Arts from the University of Kansas. She has been working with cochlear implants since She began working with cochlear implants at the Midwest Ear Institute and then at the University of Kansas Medical Center Otolaryngology Department. She is currently a Principal Clinical Applications Specialist with Cochlear Americas. In this position, she works with clinics and schools throughout the country to help individuals who have or are considering obtaining a cochlear implant, Baha osseointegrated implant, Auditory Brainstem Implant or VistaFix implant. She participates in counseling, programming, teaching, research and rehabilitation. Leigh Ann joined the Clinical Services department at Cochlear in August 2000.

6 Linda Figini MA, CCC A, FAAA Sr. Professional Education Manager Linda Figini has been an audiologist with Cochlear Americas for 4.5 years. She has 14 years audiology experience, including working at the League for the Hard of Hearing, as adjunct faculty at Hunter College, and as a cochlear implant audiologist at the New England Center for Hearing Rehabilitation. She has experience in programming, educational consulting, and rehabilitation for pediatric and adult cochlear implant recipients. She has participated in clinical research and presented on a variety of topics related to cochlear Implants, Baha, and pediatric audiology. She obtained her MA in audiology at the University of Florida in 1993 and joined Cochlear in June Megan A. Narron, Au.D., CCC A Megan joined Cochlear as a in In this role, she provides training, education, and technical/clinical support related to cochlear implants and Baha in Maryland, Virginia, and Washington, DC. She earned her Bachelor of Science at the State University of New York College at Geneseo in 1998, Master of Arts degree in Audiology at the University of Akron in 2000, and clinical doctorate at A.T. Still University in She has over 10 years of clinical experience with specialization in cochlear implants and pediatric audiology. Prior to joining Cochlear she was a cochlear implant audiologist at NYU Cochlear Implant Center. Megan currently resides in Pasadena, Maryland. Melissa Owings Wilson, Au.D. CCC A, FAAA Melissa is currently employed by Cochlear Americas as the for the Southeast Region. Prior to this role, she was a for the states of Georgia and Alabama. Melissa provides training and education related to Cochlear Implants and assistance with the clinical care of CI and Baha recipients. She earned both her Bachelor of Science and Master of Science degrees from East Carolina University. Melissa received her Doctorate degree in Audiology from the University of Florida in Before joining Cochlear, she worked as a clinical audiologist on the Implant Team at Wake Forest University Baptist Medical Center for four years. She is a member of the American Speech, Language and Hearing Association, a fellow with the American Academy of Audiology and holds licensure with the North Carolina Board of Examiners for Speech and Language Pathologists and Audiologists. Melissa resides in Winston Salem, NC. Peter N. Arkis, Au.D., CCC A Pete joined Cochlear Americas in 1999 and has been in multiple roles in the clinical department. Prior to his employment at Cochlear, he was Director of Audiology at the Warren Otologic Group in Ohio where he was a co founder of their cochlear implant program in 1985 and participated in numerous clinical trials. Dr. Arkis has also served as an instructor at West Virginia University, Kent State University and American Academy of Otolaryngology conferences. Dr. Arkis received his B.A. and Masters degrees from Kent State University, Kent, Ohio and an Au.D. from A. T. Still University, Arizona School of Health Sciences in Mesa, AZ.

7 Reynita Sagon Alcisto, Au.D., CCC A Reynita is originally from Wahiawa, HI and earned a Bachelor of Science Degree in S/L Pathology and Audiology from the University of Hawai i and her doctorate degree in Audiology from Washington University in St. Louis, MO. During this time, Reynita's capstone study, "The Developement of Word Regonition Materials in Ilocano" was published in the Philippine Journal of Otolaryngology, Head and Neck Surgery. She has since then been an editor for this journal. Reynita spent the fourth year of her audiology program as an extern at Tripler Army Medical Center then accepted a position as a pediatric audiologist at the House Research Institute CARE Center where she worked for nearly 6 years. Reynita joined Cochlear in Stacey Butler, Au.D., CCC A Stacey Butler joined Cochlear Americas in June 2012, and is the clinical application specialist for the Rocky Mountains territory. Prior to joining Cochlear she worked for 10 years as a clinical audiologist in both adult and pediatric cochlear implant programs in the United States and in New Zealand. Stacey earned her Masters in Audiology at the University of Arizona in her hometown of Tucson, AZ and completed her Doctorate of Audiology at the University of Utah.
