1 FIRE SCIENCE TECHNOLOGY CREDIT PURCHASE PROGRAM Thank you for your interest in the Fire Science Technology credit purchase program. The information contained within this package will provide you with the answers to many of your questions regarding this program and how to participate in it. Contents of this program application package: Form #1- Notice of Application for Credit Purchase under the Fire Science Technology Credit Purchase Program Form #2- Student Request for Official Transcript and/or Training Record Procedural Steps for the Fire Science Technology Credit Purchase Program toward the Associate of Applied Science in Fire Science Technology: Step 1. All persons interested in applying under this credit purchase program must first apply for and be admitted to. The credit purchase application cannot be processed until the applicant is admitted to. This program is currently available to Missouri residents only. Step 2. Once admission has been granted, those students applying under this program are required to complete the following two forms: Form #1- Notice of Application for Credit Purchase under the Fire Science Technology Credit Purchase Program

2 This form must be completed and submitted to the Director of the Department of Public Safety (MACDPS) before or at the same time as the Student Request for Official Transcript and/or Training Record is completed. There is a $25 non-refundable training record evaluation fee that must accompany the Notice of Application form. Form #2- Student Request for Official Transcript and/or Training Record This form must be submitted by the applicant to each training center that issued a certificate under consideration as part of this credit purchase program. Once completed, this form is submitted by the applicant to each fire training entity or academy where training has been completed that meets or exceeds the certification standards of the Missouri Division of Fire Safety (MDFS). A list of the MDFS certifications currently recognized as eligible for consideration under this credit purchase program are listed below. As noted in the request for training records and/or transcripts, these documents must be sent directly from the applicant s training entity or academy to the Director of MACDPS for evaluation. Any fee required by the applicant s training entity or academy for this service is the responsibility of the applicant. Step 3. Once the necessary training records are received, the application for credit purchase is evaluated. A completed Evaluation for Credit Purchase form is completed by the Director of MACDPS and sent back to the applicant. The applicant will identify and verify by signature which credit hours will be purchased from those listed on the eligibility form. This completed form must then be returned by the student through either the mail or by fax to the Director of MACDPS for further processing. Step 4. Once the verified and signed Credit Purchase Evaluation form is received by the Director of MACDPS, a billing will be generated to the applicant that indicates the total cost of the credit purchase that must be paid prior to the awarding of credit. Credit approved for purchase under this program is purchased at the rate of $25 per credit hour. The billing will indicate that the applicant has 180 days from the date of billing to submit the credit purchase amount. Once the 180 day period has expired the credit purchase option is no longer available. Any fees paid as part of the credit purchase program are nonrefundable. Step 5. Once the credit purchase amount is remitted in full, the Dean of Career and Technical Education (CTE) will be sent the approved credit purchase program package requesting the awarding of credit. Upon approval of the Dean, a letter requesting the awarding of the specified credit is issued to the Mineral

3 Area College Registrar. The Registrar will award the credit to the applicant s transcript, pending the completion of at least 15 credit hours through. Upon completion of 15 credit hours at, the Registrar s Office will make a permanent entry in the applicant s college transcript reflecting the college credits purchased. Note: Credits purchased under this program will reflect a grade of P for pass, and will not count toward the applicant s grade point average. Degree-FST Printed Student Name (First, MI, Last): Student JICS #: Home Address: Home or Cell Phone: Read and Sign this Notice of Application: Please accept this application for credit purchase evaluation under the provisions of the Fire Science Technology Credit Purchase Program toward the Associate of Applied Science Degree in Fire Science Technology. I understand this application process will involve the following steps: 1. I must first complete and be accepted for admission to, and have paid the $15 admission fee. 2. I must complete this Notice of Application for the Credit Purchase Program, and enclose a check or money order in the amount of $25 with this application, payable to, for the purpose of evaluating the training records and/or transcripts of my fire science training, to determine what credit I may be eligible to purchase under this program. 3. I understand that any fees paid under the provisions of this program are non-refundable. 4. I understand that I am responsible for arranging to have any training records and/or transcripts related to my fire academy or other fire entity training sent directly to the Director of the Department of Public Safety from the academy where the training was received, and that any fees related to that process are my responsibility. 5. I understand that once these records have been received, an evaluation will be made to determine what credit I may be eligible to purchase under this program. The results of this evaluation will be sent to me, and at that time I must select the credit I intend to purchase. I will return that completed credit evaluation form, and a billing will be for the credit purchase I have indicated. 6. I understand that once that billing is generated, I will have 180 days to pay the amount in full to secure my credit purchase, after which time the credit purchase program is no longer available. I also understand that I may not revisit the application process for additional credit once the initial application process is completed. 7. I understand that I must complete at least 15 credit hours at before those credits will permanently appear on my transcript, and that these course grades will indicate only a letter of P for pass and will not count towards my grade point average.
