1 Welcome Package (Regina) Welcome to Nursing! Please read all sections of this information package carefully as there are several required tasks to complete with varying deadlines. Task 1: Submit the following Risk Management Documents by the specified due date. 3 Task 3: Purchase your textbooks and supplies (May-August 2014) Textbooks First year textbooks should be available by early August. You may access your personalized list through UR Self-Service. You have two options for purchasing text books: 1) Online: Order through UR Self-Services account or at SIAST (shipping charges apply) 2) In person: Purchase at the University of Regina or SIAST bookstore Uniforms and Business Casual Dress Mandatory Program Uniforms (Scrubs) will be available in Fall All students enrolled in the SCBScN program are required to wear a standardized uniform with SCBScN logo in the practice education setting (clinical) and lab. Standardized uniforms promote professionalism, role identification as well as improve personal safety and infection control. Further information about uniforms will be provided in the Fall. Students will also need to be prepared to dress business casual for presentations and guest speakers. For women: a reasonable length skirt or pants of a non-jeans material combined with a top (such as dress shirt, or sweater set) is considered acceptable. An informal dress with appropriate skirt length is also acceptable. Revealing necklines and excessively high heels are not acceptable. For men: a combination of collared shirt (such as a dress shirt), trousers (such as khakis or blue, green, brown, or black trousers) with a belt. Jeans are NOT acceptable business casual attire. A blazer or business jacket can be added as an option. Electronic Mobile Device This is a generic term for devices like the ipod touch and Smartphone (iphone/android/blackberry/windows Phone). The device will be used for an application called Nursing Central which has been licensed for your use in class and clinical environments. For more information about the different devices Nursing Central runs, follow this link: A license for this software has already been paid for in your student fees and you will be provided with an opportunity to download the software once your first semester begins so please do not purchase a copy of Nursing Central ahead of time. Please note that you will find yourself in situations where carrying a bag will be impractical or not allowed so it is important that the device you choose fit into your pocket, for this reason an ipad or other large screen device is not a good choice. 3 SCBScN Welcome Package (Regina)

4 Also note, Nursing Central does not require that you be connected to the internet continuously and as such there is no requirement that you commit to a wireless data plan for use in our program. A device that is able to connect to the internet using Wi-Fi is sufficient. Other Required Resources These items are available at the SIAST and U of R bookstores. Protective eyewear Stethoscope with bell and diaphragm head Watch with second hand or digital readout in seconds Writing style guide for APA formatting (may be on your textbook list, you do not need more than one style guide) Task 4: Learn about your campuses and access available support services (May-August 2014) Regina nursing students will attend all four years of their nursing degree at two campuses: SIAST Wascana Campus and the University of Regina. It is a good idea to familiarize yourself with logistics like parking, bus routes and location prior to your first day of programming. You should also become familiar with the student supports available; counselling, library services, recreation and athletic activities, housing registries and many more services that are available to students free of charge! Review both institutions websites ( and for information on services and programs. Familiarize yourself with the SCBScN program website as important information will be posted here throughout the year. Want to get a head start on developing your study skills? 6 Task 6: Attend the mandatory Program Week (September 3 to 5, :30 am to 4:30 pm) Your SCBScN program begins on Wednesday, September 3 at 9:00 am sharp. All students are to pick up their welcome package outside Room 100, SIAST Wascana Campus Parkway Centre, between 8:30 and 9:00 am, prior to the beginning of programming. You must attend all scheduled programming from Wednesday, September 3 to Friday September 5. You can expect to be on campus from 8:30 am to 4:30 pm on these days. Your regular SCBScN class schedule (on UR Self-Service) will begin on Monday, September 8. *Note: September 4 you will have classes at the U of R* Remember, it is important to become familiar with your UR , UR Self- Service and UR Courses accounts (you will have gone over these at your Group Advising Session). ALL communication to students goes through these accounts and they must be checked daily. 10 Identified Purposes and Handling of Personal Information and Personal Health Information in HSPnet Updated: July 1, 2013 Background The Health Sciences Placement Network (HSPnet) is a secure web-enabled application that is used by several jurisdictions in Canada. The HSPnet database contains information about students in clinical placements within health agencies and other locations. Students authorize their educational program to use and disclose their personal information (name, student profile) and to use (but not disclose) their personal health information via HSPnet for the purpose of locating and coordinating placements as required for an educational program. This document provides a summary of the national HSPnet Policies on Privacy, Security and Data Access, relating to the protection of student information within HSPnet. The full Policies can be viewed on the HSPnet website at Collection, Use, and Disclosure of Personal Information (PI) and Personal Health Information (PHI) in HSPnet HSPnet policies ensure that PI and PHI in HSPnet: Are collected, used, and disclosed only for purposes consistent with identifying and coordinating a student s clinical placements; Cannot be used or disclosed without the consent of the student whose PI or PHI is to be collected; and Are used by or disclosed only to authorized individuals on a need-to-know basis, by/to staff involved in student placements within the student s educational program or placement site. PHI is never disclosed via HSPnet to users who are external to the student s educational program. PI Collected May include any or all of: Student names Student home address, phone numbers or addresses Student number Student photograph Placement Preferences (1 st, 2 nd and 3 rd choices if offered) Student gender limited to students placed in locations that accommodate patient/ client preference for the gender of their care provider gender (e.g. homecare visits). Student Prerequisites as required by Placement Sites (e.g. criminal records check, CPR or other certifications) Student Profile of educational or work history relevant to placement requests Uses of PI BY authorized users only within Student s Educational Program To contact students regarding placement needs or status, or regarding urgent issues such as labour disruption at the placement destination To generate class placement lists, confirmation notices and schedules To maintain a student history of placements To track student compliance with each site s published requirements for criminal records check, CPR certification, etc. Disclosure of PI TO authorized users only within the Placement Site being asked to accept the Student Student name is disclosed upon confirmation of an accepted placement, for the purpose of facilitating placement arrangements (orientation, preceptor assignment) and as a record of students received by the Site. Name may be disclosed, at the discretion of the educational program, prior to acceptance if the Receiving Site has a need to know (e.g. to arrange a student interview, if the student is an employee). The student s school-issued address may be released for the limited purpose of delivering passwords to Site computer networks. No other student PI (besides name) is disclosed under any circumstances, excluding Student gender which may be disclosed on specific request by a placement site that requires this information to accommodate patient/client preference (e.g. placements in homecare agencies). Not disclosed under any circumstances To facilitate a good fit between the student and Placement Site, learning experiences offered, and supervisor/preceptor to be assigned.

11 Identified Purposes and Handling of Personal Information in HSPnet Page 2 Updated: July 1, 2013 PHI Collected May include any or all of: Status of indicators for safety and/or infection control as required by Placement Sites prior to accepting students: Information on a student s immunity or immunization status for vaccinepreventable diseases such as Varicella, Diptheria/Tetanus, Influenza, and Measles/Mumps or Rubella Information on Tuberculosis status including TB test and/or chest X-ray results Uses of PHI BY authorized users only within Student s Educational Program To track status of a student s eligibility according to the requirements of Receiving Agency sites where students may be placed Disclosure of PHI Not disclosed under any circumstances Safeguards The accuracy and completeness of personal information within HSPnet is maintained through the use of system tools such as mandatory fields and formatting rules, and through periodic reviews of data quality to identify the need for interventions such as user training or system modifications. HSPnet data is physically and logically secured in accordance with industry standards and best practices, including enforcement of strict rules for physical security and backups, password protection at all points of access, and use of anti-virus software, firewall protection, and data encryption. Periodic audits of HSPnet transactions are carried out to ensure there are no problems and/or gaps in the user interface that might permit inappropriate access to or update of data. Personal information on each student, along with their placement history, is retained for a housekeeping period of180 days after the student s completion of or withdrawal from the educational program, or after the consent expiry period of six years, whichever occurs first. A copy of their personal information is available to a student upon request to their jurisdiction s Privacy Officer or the national HSPnet Privacy Officer. Openness, Access, and Challenging Compliance An individual can access their own information as well as a complete description of the type of PI or PHI used/disclosed and the purposes for using or disclosing the information. Such requests can be made in writing by the student to the national HSPnet Privacy Officer and/or to the local Privacy Officer within the student s jurisdiction (contact information for each province or jurisdiction is available on the HSPnet website at An individual may request changes to their PI or PHI contained in HSPnet, or may register a complaint or challenge regarding the handling of their information in HSPnet, by submitting a request in writing to the national HSPnet Privacy Officer or local Privacy Officer within their jurisdiction.

Permission to Use and Disclose Your Student Related Personal Information and Personal Health Information By signing this consent, you authorize your educational Program Collect, use and/or disclose your personal information (name and student profile information that is under the custody and control of your Program) to authorized staff of Receiving Agencies for the purpose of locating and coordinating an appropriate placement experience (e.g. clinical practica, fieldwork, or preceptorship) as required by your educational program; Use your student related personal information and personal health information relating to placement prerequisites, for the purpose of tracking your compliance against Receiving Agency safety and infection control prerequisites for accepting students. Placement prerequisites that may be tracked include personal information such as CPR certification or criminal records check status, and personal health information such as immunity/immunization status of vaccine-preventable diseases. Placement prerequisite information is used only by staff involved with your educational program, and is never disclosed to users external to your educational program. Disclose your personal information to the owner and administrator of the HSPnet system, namely Provincial Health Services Authority British Columbia (PHSA), to allow PHSA to indirectly collect your personal information to provide HSPnet student placement services. 2. Consent Period This consent is effective immediately and shall remain valid for up to six years, or shall be voided upon your completion of the Program, your formal withdrawal from the Program, or upon written request as described below. 3. Your Rights With Respect to This Consent 3.1 Right to Refuse Consent - You have the right to refuse to sign this consent, and if you refuse your placement will be processed manually at the earliest convenience of the Program and Receiving Agency. 3.2 Right to Review Privacy & Security Policies - A copy of the document entitled Identified Purposes and Handling of Personal Information in HSPnet, which summarizes Privacy and Security policies relating to how we may use and disclose your personal information via HSPnet, is distributed with this Consent Form. You may wish to review the complete Privacy and Security Policies for HSPnet before signing this consent. The Privacy and Security Policies may be amended from time to time, and you may obtain an updated copy by contacting privacy@hspcanada.net. 3.3 Right to Request Restrictions on Use/Disclosure - You have the right to request that we restrict how we use and/or disclose your personal information or personal health information via HSPnet for the purpose of locating and coordinating a suitable placement experience. Such requests must be made in writing to the placement coordinator for your Program. If we agree to a restriction you have requested, we must restrict our use and/or disclosure of your personal information in the manner described in your request. If this restriction precludes our ability to coordinate your placement via HSPnet, then your placement will be processed manually at the earliest convenience of the placement coordinator and receiving agency. 3.4 Right to Revoke Consent - You have the right to revoke this consent at any time. Your revocation of this consent must be in writing to the placement coordinator for your Program. Note that your revocation of this consent, or the voiding of this consent upon your completion or withdrawal from the Program, would not be retroactive and would not affect uses or disclosures we have already made according to your prior consent. 3.5 Right to Receive a Copy of This Consent Form - You may request a copy of your signed consent form. Collection of your personal information is done under the authority of the privacy legislation that applies to educational institutions in your province. 13 Immunization Requirements- Wascana Campus (Regina) As part of your enrolment in the Saskatchewan Collaborative Bachelor of Science in Nursing Program, you must obtain and submit a record of immunization. Please follow the steps listed below. OBTAIN AND SUBMIT YOUR RECORDS OF IMMUNIZATION BY AUGUST 1, 2014 Upon receipt of this notice, submit your immunization record to SIAST Wascana Campus. If you do not already have a copy of your childhood immunization record, call your local public health office where you attended elementary and high school or your doctor s office. If you have received vaccines for travel, employment or as a student, include a copy of those records If you cannot locate your record or they are not available, call the SIAST Campus Health Nurses at and ask to be transferred to the Health Office at Wascana Campus Fax, scan or your immunization record to SIAST Wascana Campus Health Centre: o Fax: o o wascanahealth@siast.sk.ca Mail: SIAST Wascana Health Centre, Room 107, 4500 Wascana Parkway, PO Box 556, Regina, SK, S4P 3A3 AFTER YOUR PROGRAM BEGINS To receive your immunizations on campus, please drop into the Health Centre, Room 107 (Main Building) to see what you require after you have submitted your records. Please note you do not need to update your immunizations before the program begins. All immunizations need to be completed by October 31, 2014 ADDITIONAL INFORMATION 1. Those failing to complete the required immunizations may be excluded from some areas of clinical practice until these requirements have been met. If you choose not to receive a specific vaccine your program requires, you will need to sign a waiver absolving the University of Regina and SIAST of any liability. You are required to contact the Campus Health Nurse if you choose not to receive a vaccine. Annual influenza vaccine is strongly recommended and may be a requirement of some clinical agencies. 2. Cost of Vaccines: Any costs associated with vaccines will be the responsibility of the student. Most vaccines are provided free. March, 2014
