1 ADDENDUM #3 REVISION to Hosted VOIP RFP DATE: December 3, 2013 TO: HOLDERS OF BID DOCUMENTS FOR HOSTED VOIP RFP FROM: Kasey Segraves, Information Systems Supervisor Escondido Union District 302 North Midway Drive This Addendum #3, forms a part of the Contract Documents and modifies the original bidding documents. Acknowledge receipt of this addendum by including a copy of it with your Bid Submittal. Failure to do so may subject the bidder to disqualification. This addendum answers questions from bidders via and/or those attending the site walk on Monday November 25, 2013 and modifies various sections of the RFP 1. Define Support, Warranty and Maintenance using the following: Support, Warranty, & Maintenance System Support Period The Respondent must present options for annual and multi-year support contracts, tied to system implementation schedule. It is required that support and warranty on all components will not start until such time as the component is implemented and accepted by the District. The District shall have the option of purchasing support contracts during a period of at least 3 years with two one year optional extensions after the effective date of the Contract. Warranty It is required that the Support and Manufacturer Warranty on all system components will not begin until acceptance of the equipment. Acceptance is defined as the point in time when equipment is 100% operational and service levels have met defined parameters and the District accepts the equipment and installation in writing. The expectation is that delivery will be taken on a "just in time" basis following burn-in testing by the Respondent and will also allow for reasonable programming, configuration, and testing prior to District deployment. Software & Updates Respondent must include the schedule (frequency) for doing system maintenance such as software upgrades, including anticipated impact that such upgrades have on platform availability. Discuss your strategy for implementing patches and point releases, including who is responsible for installing and testing these along with availability of technical support. Include supported product lifecycles per release. Explain your system for notification of identified vulnerabilities, patches, upgrades, etc. as they pertain to our proposed solution. Address the current release as well as the product roadmap for the next 18 months.

3 3. Clarification of Various Items on Page 10 Quotation Page Item 8 The District requires 911 location awareness down to the classroom level. Item 12 In addition to a voic box for each extension, the District has 10 voic boxes that are not tied to an extension. Item 13 Please describe your Fax solution and provide ala carte pricing for adding any additional fax lines outside of e-rate. Item 16 Provide ala carte pricing for additional equipment that may be needed to make the system work but is not eligible for e-rate discounts. 4. Correction to Letter of Agreement. 4 Letter of Agreement REQUEST FOR PROPOSAL Escondido Union District ERATE 2014 VOIP Hosted System RFP # Escondido Union District and (Name of Company) Pursuant to the terms of Escondido Union District RFP # for VOIP Hosted System (Name of Company) s response to RFP # dated (mm/dd/yyyy), (Name of Company) will provide the equipment and services per RFP # effective the date of issuance of Escondido Union District Purchase Order(s). (Name of Company) and Escondido Union District acknowledge that this agreement is for E-Rate eligible products and services, which are contingent on funding by the School and Libraries Division of USAC/FCC and the Escondido Union District for E-Rate Year 2014, and the Board Members of the Escondido Union District approval. The Escondido Union District reserves the right to terminate the referenced Request for Proposal (RFP) and all documents associated with the Request for Proposal, including but not limited to this Letter of Agreement, in its sole discretion at any time, with or without cause, upon written notice to the other party. In the event of termination, notice shall be deemed served on the date of mailing and shall be effective immediately. The Escondido Union District shall not be responsible for any costs to Bidder prior to termination.
