4 114,000 members in 29 counties are eligible for the MyCare Ohio program. This includes: Individuals 18 years and older Members residing in the MyCare Ohio service area Individuals entitled to benefits under Medicare Part A enrolled under Medicare Parts B and D, and receive full Medicaid benefits. Adults with disabilities and persons 65 years and older Persons with serious mental illness 4

5 January 1, 2015 Medicare Passive Enrollment All MyCare Ohio members who have not already called the Medicaid Consumer Hotline to opt-out of Medicare, or purchased a 2015 Medicare plan, will be auto-enrolled into the Medicare portion of MyCare Ohio on 1/1/15. This is also known as passive enrollment. Letters informing members of this change were mailed to all MyCare Ohio members the week of October 6 th Language from the letter, To continue to improve the way your benefits work together, we are notifying you in advance that Buckeye Health Plan MyCare Ohio will begin providing your Medicare benefits as well as your Medicaid benefits. Unless you make a different choice, this enrollment will take effect January 1, During 2014 MyCare Ohio members were passively enrolled into Medicaid only. MyCare Ohio Medicaid only benefits include standard Medicaid, longterm supports and services, and behavioral health. After Medicare passive enrollment, members will be a dual benefit member and Medicare part A, B, and D will be added to their Medicaid benefit plan. 5

6 Member s Options The Medicaid portion of MyCare Ohio is mandatory, the Medicare portion is optional. At any time, a member can call to opt-out of the Medicare portion of MyCare Ohio. To opt-out of the Medicare portion of MyCare Ohio a member must call the Ohio Medicaid Consumer Hotline: A member can also get information and questions answered. Dual benefit members, members who are enrolled for both Medicare and Medicaid with MyCare Ohio, can choose another managed care plan every month. Opt out (Medicaid only) members do not have this option. They can change managed care plans annually. 6

14 Transitions of Care Nursing Facility NF services: Provider will be retained at current rate for the life of Demonstration (42 months). 14

15 Transitions of Care - Exceptions During the transition period, change from the existing services or provider can occur in any of the following circumstances: 1. Consumer requests a change 2. Significant change in consumer s status 3. Provider gives appropriate notice of intent to discontinue services to a consumer 4. Provider performance issues are identified that affect an individual s health & welfare Plan-initiated change in service provider can only occur after an in-home assessment and development of a plan for the transition to a new provider 15

16 The Integrated Care Team Works Together with the Member & Their Provider to Find the Best Health Solutions All MyCare members have a Care Manager responsible for coordination and integration of care & services. Optum, formerly known as Evercare, will serve as Buckeye s Care Managers in the nursing facility. They will be conducting the assessments, care coordination, and in-person visits. 21 All SNF and LTC services require prior authorization Prior authorization IS NOT REQUIRED for skilled services when Buckeye is secondary (Medicaid Opt Out member) New Services: Services will be based on the member s care plan. Care Coordinator will be in contact with both the member and provider. Once services are approved, prior authorization will be entered into the system by Care Coordinator. Care Coordinator will contact service providers with a prior authorization number, confirming service can now take place. Existing Services: Services that are currently in place for member will remain for the life of the demonstration (42 months). Providers will receive a notice from Buckeye explaining transition process, and members identified as currently in facility or LTC. If you have questions if a service is authorized for the member, contact the MyCare care coordination team at All out of network non-emergent services and providers require prior authorization. 21

22 Claim Submission Options Authorization is required for all services including bed hold days Buckeye will accept standard Medicare and Medicaid billing codes RUGS etc. No payor specific codes required Buckeye will reimburse based upon current Medicare & Medicaid fee schedules including bed hold days Program Exclusions Bed hold days policy will be consistent with current regulatory policies and rates (Buckeye has current rates including occupancy variances) Inpatient hospice Buckeye will reimburse hospice provider who will in turn reimburse SNF for room & board 22

23 Patient Liability Effective 11/1/2014, Buckeye will change the way patient liability is deducted from nursing facility claims for MyCare Ohio members. The amount of patient liability deducted will be sourced from the 834 file that Buckeye receives from the Ohio Department of Medicaid, rather than the amount entered on the claim. This is consistent with the method used by other Managed Care Plans and the Ohio Department of Medicaid. Claims Continue to submit the amount of patient liability as a value code on claims, preferably using VALUE CODE D3. This amount will Program be used for Exclusions reporting and reconciliation purposes, but will not be used to deduct the amount of patient liability from the claim. Adjustments Buckeye will initiate adjustments for nursing facility claims when the incorrect amount of patient liability was deducted for dates of service back to 5/1/14. We will utilize the previously received 834 files from the State to adjust claims on your behalf. No further action is required on your part. If you need to report that the amount of patient liability on the 834 file is incorrect for a member and you have documentation of the correct amount, please call provider services at (866)

24 Bad Debt Policy Bad Debt applies to member liability for skilled level of care days of single stay Buckeye will not require SNF to file annual bad debt report Buckeye will aggregate bad debt detail from adjudicated claims by facility Buckeye will review and determine Program Exclusions liability using the following methodology Services 5/1/14 through 9/30/14 76% of bad debt Services 10/1/4 through 12/31/14 65% of bad debt Reimbursement will be paid as a lump sum payment in the 2 nd quarter of each year. 24

29 Paper Claim format All services first time claims and corrected claims must be billed to Buckeye using a UB 04 or CMS 1500 form. Forms cannot be filled out by hand. Must be completed using computer software or a typewriter. Corrected claims Previous claim number should be referenced in field 64 of the UB-04 and 22 of the Program Exclusions CMS 1500 as outlined in the NUCC guidelines. The appropriate frequency code/resubmission code should also be billed. Claims should be submitted to the following address: Buckeye Health Plan ATTN: Claims 3060 Farmington, MO
