18 Acupuncture Needles Park Sham Device Research Tools The Park Sham Device is an apparatus used to administer the sham needle and real needles. The Park Sham Needle appears similar to the real needle, but is blunt and free to slide within the handle so that when pressed, it telescopes into the handle instead of penetrating the skin. The device holds the un-inserted sham needle in place, and maintains the sterility of a genuine acupuncture needle. This device consists of a guide tube: a special oversized tube with base flange which is equipped with double-sided tape, and can therefore stick on target skin. (Fig 1) Since the Park Sham Device was first developed in , several teams of researchers in UK 2, 3, Japan 4, Korea 5 and USA 6 have tested its credibility and validity in various settings. Based on these findings, over the past 10 years, it has been frequently used in research projects, often funded by government agencies including the US National Institute of Health Park and his fellow colleagues, who developed this device, state that this device is indistinguishable from the same procedure using real needles in acupuncture naïve subjects, and inactive, where the specific needle sensation is taken as a surrogate measure of activity. 2 However, due to the nature of minimal physiological activity required to achieve successful subject blinding in acupuncture trial, they emphatically call for caution in application of sham acupuncture control 11 and in interpretation of the findings. 12 For this reason, Park is keen to call this a bona fide version of sham control rather than final version. 19 Park Sham Device How To Use Park Sham Deive Currently either real or sham needles are packed in a single guide tube in a blister package. To avoid compromising the sterilisation you could peel off only the top of the package to expose the handle of the needle. Then introduce that handle through the bottom of the plastic flange. After introducing the handle you tilt the needle and Park tube over to allow the needle to slide down to the other end of the tube. Then you will be able to hold the needle handle together with the top end of the Park tube. After this, peel off the cover of the double-sided sticky tape and place the device on the skin while keeping the needle handle projecting about 5mm from the top of the Park tube. Then tap the needle gently to insert it about 5mm followed by moving down the upper telescopic guide tube to allow sufficient exposure of the handle for manipulation of the needle. Manipulate the needle as desired. To remove: first take out the needle and dispose of it safely, then peel the flange from the skin, with the guide tube in place. Doing this quickly will minimise discomfort. Unlike standard athletic taping, which often involves wrapping a joint for support and compression, kinesiology tape is placed in a variety of patterns depending on the injury, It is pulled to differing degrees of tension to create the desired effect and is typically worn for two-to-five days, unlike standard tape, which is used mainly during an activity. Kinesiology tape can be used to pull back a shoulder that is hunching forward, or to reduce swelling in a joint, tape can be used to pull up the skin and create an area of low pressure where fluid can move and drain. Some clinicians believe the tape provides stimulation to skin cells that affects pain pathways-similar to rubbing a spot that hurts. (Summarized from the Wall Street Journal Apr 27, 2010) Suggested Instructions for Use Always shape the cut ends of the tape in such a way that they are curved with no sharp corners. This helps to reduce the problem of the corners catching in clothing, causing the tape to become dislodged. To correctly apply the tape, first measure the required length while the backing tape is still in place, then shape the corners as outlined above. Position the area to be taped so that the underlying muscles are fully stretched. Remove the backing and lay the tape on the skin sticking it in place. Do not stretch the tape, simply lay it on the skin. As the affected muscles relax the tape will take on a slightly corrugated form. Acu-Tape should stay in place for 3 to 7 days. The exact length of time will depend on how greasy the skin is and how much the subject perspires. It is possible to shower carefully, keeping the tape in place. The area should be patted dry and not rubbed.
