7 PATIENT INSTRUCTIONS Fr (mafilcn A) Sft (Hydrphilic) Cntact Lenses INTRODUCTION: This Wearer's Instructin bklet has been develped fr wearers f (mafilcn A) Hydrphilic Cntact Lenses fr daily wear. lenses are sft (hydrphilic) cntact lenses which differ frm cnventinal hard cntact lenses in that they becme sft and flexible when saturated with water r tears. When fully hydrated, the lens is 62% water. Yur eye care practitiner will review the prper care and handling f yur cntact lenses. T btain maximum perfrmance and satisfactin frm yur new lenses, yu shuld carefully fllw instructins, adhere t the wearing schedule, and keep all appintments fr peridic fllw-up examinatins recmmended by yur eye care practitiner. If yu are in dubt abut any f the instructins, ask yur eye care practitiner fr clarificatin. (Omafilcn A) Sft Hydrphilic Cntact lenses may prvide imprved cmfrt fr cntact lens wearers wh experience mild discmfrt r symptms related t dryness during lens wear assciated with Evaprative Tear Deficiency r frm Aqueus Tear Deficiency (nn- Sjgren s nly). Daily wear replacement schedules may vary frm patient t patient and shuld be decided by eyecare practitiners in cnsultatin with their patients. The lens is t be cleaned, rinsed and disinfected each time it is remved frm the patients eye and discarded after the recmmended wearing perid prescribed by the eye care practitiner. DO NOT WEAR YOUR PROCLEAR DAILY WEAR CONTACT LENSES WHILE SLEEPING. CONTRAINDICATIONS (REASONS NOT TO USE) D Nt use this cntact lens when any f the fllwing cnditins exits: Previusly diagnsed primary Sjgren s Syndrme Tear Deficiency, Autimmune cnnective tissue disease which may invlve secndary Sjgren s syndrme. Such cnditins include rheumatid arthritis, plyarthritis, Wegener s granulmatsis, systemic lupus erythematsus, systemic sclersis, primary bilary cirrhsis, and mixed cnnective tissue disease.- Acute and subacute inflammatin between the lens, iris, and crnea, i.e., the anterir chamber f the eye. Any eye disease, injury, r abnrmality that affects the crnea, cnjunctiva, r eyelids. Any active crneal infectin: purulent (pus) bacterial, fungal, r viral infectin. Crneal hypesthesia (reduced crneal sensitivity), if ntaphakic. Any systemic disease which may affect the eye r be exaggerated by wearing cntact lenses. Allergy t any ingredient, such as thimersal r mercury, in a slutin which must be used t care fr the lens. Allergic reactins f cular surfaces r adnexa that may be induced r exaggerated by wearing cntact lenses r use f cntact lens slutins. If eyes becme red r irritated. Page 5
10 WARNINGS Patients were nt studied wh exceed the cnditins characterized by any f the fllwing diagnstic parameters: Rse Bengal staining > 12 n a scale f 18 Flurescein staining >12 n a scale f 15 Meibmian gland dysfunctin >3 n a scale f 0-4 PROBLEMS WITH CONTACT LENSES AND LENS CARE PRODUCTS COULD RESULT IN SERIOUS INJURY TO THE EYE. It is essential that yu fllw yur eyecare practitiner's directins and all labeling instructins fr prper use f lenses and lens care prducts, including the lens case. Eye prblems, including crneal ulcers, can develp rapidly and lead t lss f visin. Daily wear lenses are nt indicated fr vernight wear, and yu shuld nt wear lenses while sleeping. Clinical studies have shwn that the risk f serius adverse reactins is increased when these lenses are wrn vernight. Studies have shwn that cntact lens wearers wh are smkers have a higher incidence f adverse reactins than nnsmkers. If yu experience eye discmfrt, excessive tearing, visin changes, r redness f the eye, yu shuld immediately remve yur lenses and prmptly cntact yur eyecare practitiner. PRECAUTIONS Befre leaving the eyecare practitiner's ffice, yu shuld be able t prmptly remve lenses easily r shuld have smene else available t remve the lenses fr yu. Yu shuld remve the lenses immediately if the eye becmes red r irritated. Different slutins cannt always be used tgether, and nt all slutins are safe fr use with all lenses. Use nly recmmended slutins. Never use slutins recmmended fr cnventinal hard cntact lenses nly. Page 6
11 Chemical disinfectin slutins shuld nt be used with heat unless specifically indicated n prduct labeling fr use in bth heat and chemical disinfectin. Always wash and rinse yur hands befre handling lenses. D nt get csmetics, ltins, saps, creams, dedrants, r sprays in the eyes r n the lenses. It is best t put n lenses befre putting n makeup. Water-base csmetics are less likely t damage lenses than il-base prducts. D nt tuch cntact lenses with the fingers r hands if the hands are nt free f freign materials, as micrscpic scratches f the lenses may ccur, causing distrted visin and/r injury t the eye. Always fllw directins in the package inserts fr the use f cntact lens slutins. Sterile unpreserved slutins, when used, shuld be discarded after the time specified in the labeling directins. Carefully fllw the handling, insertin, remval, cleaning, disinfecting, string and wearing instructins in the fr the Cntact Lens and thse prescribed by the eyecare practitiner. Always use FRESH unexpired lens care slutins. Never wear lenses beynd the perid recmmended by the eyecare practitiner. D nt use saliva r anything ther than the recmmended slutins t wet yur lenses. If aersl prducts such as hair spray while are used wearing lenses, exercise cautin and keep eyes clsed until the spray has settled. Page 7
12 T prevent yur lenses frm becming dry (dehydrated), always keep them cmpletely immersed in the recmmended strage slutin when the lenses are nt being wrn. Fllw the lens care directins fr Care fr a Dried Out Lens if yur lens surface des becme dry (dehydrated). If the lens sticks (stps mving) n the eye, fllw the directins n Care fr a Sticking Lens. The lens must mve freely n the eye fr cntinued health f the eye. If nn-mvement f the lens cntinues, yu shuld immediately cnsult yur eyecare practitiner. Avid all harmful r irritating vaprs and fumes while wearing yur lenses. Never use tweezers r ther tls t remve the lens frm the lens cntainer. Pur the lens int the hand. D nt tuch the lens with fingernails. Always handle lenses carefully and avid drpping them. Ask the eyecare practitiner abut wearing lenses during sprting activities. Always discard lenses wrn n a frequent replacement schedule after the recmmended wearing schedule prescribed by the eyecare practitiner. Always infrm the dctr (health care practitiner) that yu wear cntact lenses. Always cnsult yur eyecare practitiner befre using any medicine in yur eyes. Page 8
13 Always infrm emplyer f being a cntact lenses wearer. Sme jbs may require use f eye prtectin equipment r may require that patient nt wear cntact lenses. As with any cntact lens, fllw-up visits are necessary t assure health. Yu shuld be instructed as t a recmmended fllw-up schedule. ADVERSE REACTIONS (POTENTIAL PROBLEMS) The fllwing adverse effects have been recgnized as ptential prblems with the use f sft cntact lenses: Eyes sting, burn, r itch (irritatin) r ther eye pain Cmfrt is less than when lens was first placed n eye Feeling f smething in the eye (freign bdy, scratched area) Excessive watering (tearing) f the eyes Unusual eye secretins Redness f the eyes Reduced sharpness f visin (pr visual acuity) Blurred visin, rainbws, r hals arund bjects Sensitivity t light (phtphbia) Dry eyes If yu ntice any f the abve, yu shuld IMMEDIATELY REMOVE YOUR LENSES. If the discmfrt r prblem stps, lk clsely at the lens. If the lens is in any way damaged, DO NOT put the lens back n yur eye. Place the lens in the lens case and cntact yur eyecare practitiner. If the lens has dirt, an eyelash, r ther freign bdy n it, r the prblem stps and the lens appears undamaged, yu shuld thrughly clean, rinse, and disinfect the lenses; then reinsert them. If the prblem cntinues, DO NOT put the lens back n yur eye; immediately cnsult yur eyecare practitiner. WHEN ANY OF THE ABOVE SYMPTOMS OCCUR, A SERIOUS CONDITION SUCH AS INFECTION, ABRASION, CORNEAL ULCER, NEOVASCULARIZATION, UVEITIS, OR IRITIS MAY BE PRESENT. Yu shuld keep lens ff the eye and seek immediate prfessinal identificatin f the prblem and prmpt treatment t avid serius eye damage. Page 9
14 CLINICAL TEST RESULT Dry Eye patients have been categrized in the Reprt f the Natinal Eye Institute/Industry Wrkshp n Clinical Trials in Dry Eyes, published in the CLAO Jurnal, Octber 1995, Vl. 21, N. 4. The Safety and Effectiveness f the (mafilcn A) sft cntact lens in defined Dry Eye patients was demnstrated by the results f a clinical trial which are summarized here. The study patients dry eye cnditin is defined as arising frm Evaprative Tear Deficiency r frm Aqueus Tear Deficiency (nn-sjgren s nly). Lenses were nt tested in subjects having severe cases f Dry Eye syndrme. All subjects were existing cntact lens wearers, wh met the Dry Eye diagnstic inclusin criteria. See yur eye care practitiner fr details f the inclusin criteria. Overall the, subjects wearing (mafilcn A) lenses reprted imprved lens perfrmance t that reprted fr the Cntrl lenses, with the differences in perfrmance becming mre marked as the perid f pst-fit time increased. This perfrmance data was cllected using patient ratings f descriptins f eye symptms. Fr each symptm the subject was asked t indicate the frequency r severity f the symptm. Test data shws patients wearing (mafilcn A) lenses reprted wearing lenses lnger, mre cmfrtably with mre feelings f mistness f the eye, and less frequent dry eye symptms. The results fr cmfrt are further supprted by the reduced frequency f ccurrence f varius symptms which detail Cmfrt: eye irritatin, eye itchiness, burning, grittiness, watering and light-sensitivity. Variatin in eye sensatins thrughut the day and nticeability f eye symptms were categrized as ccurring less frequently while wearing the (mafilcn A) cntact lenses. * See glssary fr definitins Page 10
15 PERSONAL CLEANLINESS FOR LENS HANDLING: 1. Preparing the Lens fr Wearing It is essential that yu learn and use gd hygienic methds in the care and handling f yur new lenses. Cleanliness is the first and mst imprtant aspect f prper cntact lens care. In particular, yur hands shuld be clean and free f any freign substances when yu handle yur lenses. The prcedures are: Always wash yur hands thrughly with a mild sap, rinse cmpletely, and dry with a lint-free twel befre tuching yur lenses. Avid the use f saps cntaining cld cream, ltin, r ily csmetics befre handling yur lenses, since these substances may cme int cntact with the lenses and interfere with successful wearing. Handle yur lenses with yur fingertips, and be careful t avid cntact with fingernails. It is helpful t keep yur fingernails shrt and smth. Start ff crrectly by getting int the habit f always using prper hygienic prcedures s that they becme autmatic. 2. Handling the Lenses Develp the habit f always wrking with the same lens first t avid mixups. Remve the lens frm its case and examine it t be sure that it is mist, clean, clear, and free f any nicks r tears. 3. Placing the Lens n the Eye The insertin methd described belw is recmmended because it reduces the chances f eye injury and/r lens damage. Hwever, if yu find this methd difficult, yur eyecare practitiner will give yu additinal instructin r suggest anther methd. Page 11
16 After thrughly washing, rinsing, and drying yur hands, and inspecting the lens, place the right lens n the tip f yur right index finger (left index finger if yu are left-handed). Make sure yur index finger is dry (if the finger is wet, surface tensin will make the lens adhere t yur finger preventing the lens frm being transferred frm the finger t the eye). Pull the lwer lid f yur right eye dwnward with the middle finger f yur right hand. Pull the upper lid f yur right eye upward with the middle finger f yur left hand. Lk straight ahead and gently place the lens directly n the crnea. Lk dwn and slwly remve yur right hand, releasing yur lwer lid. If there is an initial freign bdy sensatin, lk tward yur nse and slide yur lens ff the crnea away frm the nse. Then lk away frm yur nse tward the lens until the lens repsitin itself n the crnea. If the freign bdy sensatin persists, remve the lens, rinse it with a recmmended rinsing slutin, and re-insert. If the freign bdy sensatin still persists, remve the lens and cntact yur eyecare practitiner. Check fr the prper lens centratin by cvering yur left eye. If the visin f yur right eye is crisp and clear, the lens is in its prper psitin. T insert yur left lens, fllw the same prcedure. If, after insertin f ne f yur lenses, the visin in that eye is blurred, check fr the fllwing: The lens is nt centered n the eye. Csmetics r ils are n the lens. The lens is n the wrng eye. The lens is inside-ut. (It wuld als nt be as cmfrtable as nrmal) Page 12
17 If yu find that yur visin is still blurred after eliminating all f the abve pssibilities, remve bth lenses and cnsult yur eyecare practitiner. CENTERING THE LENS Very rarely, a lens that is n the crnea will be displaced nt the white part f the eye during lens wear. This can als ccur during placement and remval f the lenses if the crrect techniques are nt perfrmed prperly. T recenter a lens which has becme displaced, first wash, thrughly rinse and dry yur hands, then fllw ne f the three prcedures listed belw: 1. Lk in the directin f the displaced lens. The lens will usually slide tward the center f the eye and recenter itself n the crnea. 2. Clse yur eyelids and gently massage the lens int place thrugh the clsed lid, using yur index finger. 3. Gently slide the displaced lens nt the crnea while the eye is pen, using finger pressure n the edge f the upper r lwer lid. REMOVING THE LENS Always remve the same lens first. Wash, thrughly rinse and dry yur hands with a lint-free twel. Always be sure the lens is n the crnea befre trying t remve it. Remve the right lens first (then the left lens), fllwing the steps belw: With yur head straight, lk upward as far as yu can. Place the middle finger f yur right hand (left hand if yu are left-handed) n the lwer eyelid f yur right eye and pull the eyelid dwn; then tuch the lwer edge f the lens with the tip f yur right index finger. 18 While still lking up, slide the lens dwn nt the white part f yur eye with yur index finger. Still lking up, mve yur index finger slightly t yur left and mve yur thumb up s that yu can pinch the lens gently between yur thumb and the index finger releasing the surface tensin between the lens and eye. Remve the lens frm the eye. NOTE: In remving the lenses, be sure that yu use yur fingers and nt yur fingernails. A fingernail can injure yur eye and/r tear the lens Fllw the recmmended lens care prcedures as described under the heading, CARING FOR YOUR LENSES (CLEANING, RINSING, DISINFECTING, ENZYMING, STORAGE AND REWETTING/LUBRICATING). If this methd f remving yur lens is difficult fr yu, yur eyecare practitiner will prvide yu with an alternate methd. CARING FOR YOUR LENSES (CLEANING, RINSING, DISINFECTING, ENZYMING, STORAGE AND REWETTING/LUBRICATING) 1. Basic Instructins: Fr cntinued safe and cmfrtable wearing f yur lenses, it is imprtant that yur first clean and rinse, then disinfect (and neutralize fr hydrgen perxide systems) yur lenses after each remval, using the care regimen recmmended by yur eyecare practitiner. Cleaning and rinsing are necessary t remve mucus, secretins, films, r depsits which may have accumulated during wearing. The ideal time t clean yur lenses is immediately after remving them. Disinfecting is necessary t destry harmful germs. Yu shuld adhere t a recmmended care regimen. Failure t fllw the regimen may result in develpment f serius cular cmplicatins as discussed in the WARNINGS sectin abve Page 14
19 If yu require nly visin crrectin, but will nt r cannt adhere t a recmmended care regimen fr yur lenses, r are unable t place and remve lenses r have smene available t place and remve them, yu shuld nt attempt t get and wear cntact lenses. When yu first get yur lenses, be sure yu have put the lenses n and remve them while yu are in yur eyecare practitiner's ffice. At that time yu will prvided with a recmmended cleaning and disinfectin regimen and instruct and warnings fr lens care, handling, cleaning, and disinfectin. Yur eyecare practitiner shuld instruct yu abut apprpriate and adequate prcedures and prducts fr yur use, and prvide yu with a cpy f the Patient Instructins fr the (mafilcn A) Cntact Lens. Fr safe cntact lens wear, yu shuld knw and always practice yur lens care rutine: Always wash, rinse, and dry hands befre handling cntact lenses. Always use fresh unexpired lens care slutins. Use the recmmended system f lens care, either perxide r chemical (nt heat) and carefully fllw instructins n slutin labeling. Different slutins cannt always be used tgether, and nt all slutins are safe fr use with all lenses. D nt alternate r mix lens care systems unless indicated n slutin labeling. Page 15
20 Always remve, clean, rinse, enzyme and disinfect yur lenses accrding t the schedule prescribed by yur eyecare practitiner. The use f an enzyme r any cleaning slutin des nt substitute fr disinfectin. D nt use saliva r anything ther than the recmmended slutins fr lubricating r rewetting yur lenses. D nt put lenses in yur muth. Lenses prescribed in a frequent replacement prgram shuld be thrwn away after the recmmended wearing perid prescribed by yur eyecare practitiner. Never rinse yur lenses in water frm the tap. There are tw reasns fr this: a. Tap water cntains many impurities that can cntaminate r damage yur lenses and may lead t eye infectin r injury. b. Yu might lse the lens dwn the drain. The lens care prducts listed belw are recmmended by CperVisin fr use with yur (mafilcn A) Cntact Lens. Yur eyecare practitiner may recmmend alternate prducts that are apprpriate fr yu t use with yur Cntact Lens. Page 16

Read the label n the slutin bttle, and fllw instructins Clean ne lens first (always the same lens first t avid mixups), rinse the lens thrughly with recmmended saline r disinfecting slutin t remve the cleaning slutin, mucus, and film frm the lens surface. Fllw the instructins prvided in the cleaning slutin labeling. Put that lens int the crrect chamber f the lens case. Then repeat the prcedure fr the secnd lens. After cleaning, disinfect lenses using the system recmmended by yur eyecare practitiner and/r the lens manufacturer. Fllw the instructins prvided in the disinfectin slutin labeling. T stre lenses, disinfect and leave them in the clsed/unpened case until ready t wear. If lenses are nt t be used immediately fllwing disinfectin, yu shuld cnsult the package insert r yur eyecare practitiner fr infrmatin n strage f yur lenses. 22 After remving yur lenses frm the lens case, empty and rinse the lens case with slutin(s) recmmended by the lens case manufacturer; then allw the lens case t air dry. When the case is used again, refill it with fresh strage slutin. Replace lens case at regular intervals. Yur eyecare practitiner may recmmend a lubricating/rewetting slutin fr yur use. Lubricating/Rewetting slutins can be used t wet (lubricate) yur lenses while yu are wearing them t make them mre cmfrtable. 2. Care Fr A Sticking Lens - If the lens sticks (stps mving) n the eye, apply 2-3 drps f the recmmended lubricating/rewetting slutin. Wait until the lens begins t mve freely n eye befre remving it. If nn-mvement f the lens cntinues, yu shuld immediately cnsult yur eyecare practitiner. CLEANING Yur lenses shuld always be cleaned immediately when remved, t clean any tear secretins r ther debris which may have accumulated during wear. Never clean yur lenses directly ver a sink with the drain pen, as the lenses may be lst. Fr prper cleaning, fllw the instructins belw fr each lens. 1. Wash and rinse yur hands thrughly. 2. Apply a few drps f the recmmended cleaning slutin t each side f the lens and clean thrughly by rubbing each side f the lens against the palm f ne hand with the index finger f the ppsite hand. 3. Rinse the lens thrughly with the recmmended rinsing slutin, place it in the apprpriate cmpartment f the lens case, and fill each cmpartment with the recmmended disinfecting slutin DISINFECTION AND STORAGE Either Chemical (nt heat) r Perxide disinfectin system is recmmended fr use with yur Hydrphilic Cntact Lenses. Yur eyecare practitiner will recmmend the best system fr yu. Page 18
23 CHEMICAL (NOT HEAT) DISINFECTION Clean yur cntact lenses with a recmmended cleaning slutin and thrughly rinse them with a recmmended rinsing slutin. After cleaning, t disinfect, carefully fllw the instructins accmpanying the disinfecting slutin in the care regimen recmmended by the lens manufacturer r yur eyecare practitiner. When using hydrgen perxide lens care systems, lenses must be neutralized befre wearing. Fllw the recmmendatins n the hydrgen perxide system labeling. Thrughly rinse lenses with a fresh slutin recmmended fr rinsing befre inserting and wearing, r fllw the instructins n the disinfectin slutin labeling. D nt heat the disinfectin slutin and lenses. Leave the lenses in the unpened lens case until ready t put n the eyes. Cautin: Lenses that are chemically disinfected may absrb ingredients frm the disinfecting slutin which may be irritating t the eyes. A thrugh rinse in fresh sterile saline slutin prir t placement n the eye shuld reduce the ptential fr irritatin. ENZYMATIC CLEANING Enzyme cleaning may be recmmended by yur eyecare practitiner. Enzyme cleaning remves prtein depsits n the lens. These depsits cannt be remved with regular cleaners. Remving prtein depsits is imprtant fr the well-being f yur lenses and eyes. If these depsits are nt remved, they can damage the lenses and cause irritatin. Page 19
24 Enzyme cleaning des NOT replace rutine cleaning and disinfecting. Fr enzyme cleaning, yu shuld carefully fllw the instructins in the enzymatic cleaning labeling. LENS CASE CLEANING AND MAINTENANCE Cntact lens cases can be a surce f bacterial grwth. Lens cases shuld be emptied, cleaned, rinsed with slutins recmmended by the lens case manufacturer, and allwed t air dry. Lens cases shuld be replaced at regular intervals as recmmended by the lens case manufacturer r yur eyecare practitiner. CARE FOR A DRIED OUT (DEHYDRATED LENS) If yur sft cntact lens is ff yur eye and expsed t air fr 10 minutes r lnger, it will becme dry and brittle. Handle the lens carefully. T rewet yur lens: Place the lens in its lens case and SOAK the lens in the recmmended rinsing and strage slutin fr at least ne hur. Sak the lens until it returns t a sft state. Clean and disinfect the rewetted (rehydrated) lens using the lens care system recmmended by yur eyecare practitiner. If, after saking, the lens des nt becme sft, DO NOT USE THE LENS, but cntact yur eyecare practitiner. EMERGENCIES If chemicals f any kind (husehld prducts, gardening slutins, labratry chemicals, etc.) are splashed int the eyes, yu shuld: FLUSH YOUR EYES IMMEDIATELY WITH TAP WATER AND IMMEDIATELY CONTACT yur eyecare practitiner OR VISIT A HOSPITAL EMERGENCY ROOM WITHOUT DELAY. INSTRUCTIONS FOR THE MONOVISION WEARER Yur shuld be aware that as with any type f lens crrectin, there are advantages and cmprmises t mnvisin cntact lens therapy. The benefit f clear near visin in straight ahead and upward gaze that is available with mnvisin may be accmpanied by a visin cmprmise that may reduce yur visual acuity and depth perceptin fr distance and near tasks. Sme patients have experienced difficulty adapting t it. Symptms, such as mild blurred visin, dizziness, headaches and a feeling f slight imbalance, may last fr a brief minute r fr several weeks as Page 20
25 adaptin takes place. The lnger these symptms persist, the prer yur prgnsis fr successful adaptatin. Yu shuld avid visually demanding situatins during the initial adaptatin perid. It is recmmended that yu first wear these cntact lenses in familiar situatins, which are nt visually demanding. Fr example, it might be better t be a passenger rather than a driver f an autmbile during the first few days f lens wear. It is recmmended that yu nly drive with mnvisin crrectin if yu pass yur state drivers license requirements with mnvisin crrectin. Sme mnvisin patients will never be fully cmfrtable functining under lw levels f illuminatin, such as driving at night. If this happens, yu may want t discuss with yur eyecare practitiner having additinal cntact lenses prescribed s that bth eyes are crrected fr distance when sharp distance bincular visin is required. If yu require very sharp near visin during prlnged clse wrk, yu may want t have additinal cntact lenses prescribed s that bth eyes are crrected fr near when sharp near bincular visin is required. Sme mnvisin patients require supplemental spectacles t wear ver the mnvisin crrectin t prvide the clearest visin fr critical tasks. Yu shuld discuss this with yur eyecare practitiner. It is imprtant that yu fllw yur eyecare practitiner's suggestins fr adaptatin t mnvisin cntact lens therapy. Yu shuld discuss any cncerns that yu may have during and after the adaptin perid. The decisin t be fit with a mnvisin crrectin is mst apprpriately left t the eyecare practitiner in cnjunctin with yu, after carefully cnsidering and discussing yur needs. Page 21
