An additional 50 physicians and specialists receive referrals. The NCBC is strategically located in the Post-Franklin neighborhood of Battle Creek. The Clinic's location allows for service to the target population comprising adults staying at shelters (The Haven, Inasmuch House, S.A.F.E. Place) and families and individuals who access limited financial assistance programs. The NCBC provides a comfortable and non-threatening health care environment for economically disadvantaged citizens of Battle Creek. Clients come via word of mouth, referrals from health care sources, Human Department, and County Health Plan. NCBC is a partnership with physicians, Battle Creek Health System, and Calhoun County Public Health Department, which serves as fiduciary agent. are provided by a multi-disciplinary team of volunteers, including general and specialty physicians, podiatrists, dieticians, pharmacists, therapists, and nurses. The NCBC provides primary health care, health education, and substance abuse and mental health referrals. Mental health and substance abuse services are provided on-site. have been developed with the Battle Creek Health System, Family Independence Agency (Calhoun County Department of Social ), Summit Pointe (Calhoun County Mental Health), Fieldstone Hospital, Veterans Administration Medical Center, City of Battle Creek, and a wide variety of other human service providers. Laboratory, radiology, and diagnostic services are provided via linkages with Regional Medical Laboratories and Radiology Consultants. Substance abuse assessment and treatment is coordinated through South Central Michigan Substance Abuse Commission. Specialty medical and surgical diagnosis and treatment is arranged on an individual basis with physicians who accept responsibility for indigent care without reimbursement. Eyeglasses are provided through the local Lions Club. The HCH program operates on the premises of the Ministry with Community Center which houses the HCH clinic, a drop-in center, an emergency overnight shelter, a soup kitchen, and a food and clothing bank. The Family Health Center HCH Program provides basic health, diagnostic, preventive, acute or urgent care, and pharmaceutical services. Primary care services are provided by a physician and mid-level providers. HCH also conducts intensive outreach, case management, housing, income, and transportation linkage activities, as well as referrals and funding for radiology, laboratory, and dental services. HCH assesses mental health and substance abuse issues and provides treatment and/or referral when necessary. HCH has referral arrangements with Borgess Medical Center, Bronson Methodist Hospital, a number of specialty care providers in the area, and Central Assessment Screening for substance abuse counseling. Other linkages include surrounding shelters and the Health Disparities Collaborative sponsored by HRSA. Estimated homeless population: 6,600 Patients served: 2,

An outreach coordinator, mid-level provider, caseworker, and a medical assistant staff the HCHN homeless program. include family planning, prescription medication, laboratory testing, preventive dental services, transportation, OB program, TB testing, substance abuse, and mental health referrals. A Family Independence Agency eligibility worker, stationed at the main clinic, assists clients in applying for benefits and entitlements. In addition to providing medical services, the mobile health team places strong emphasis on providing health education on the non-medical factors that influence homelessness in the community. This approach is an attempt to treat the whole person both physically and mentally as part of the case management process. Partnerships and linkages include Shelter of Flint, REACH, Community Mental Health, Salvation Army, Transition House, Odyssey House, Inner City Outreach, and Carriage Town Mission. It is the designated site for the provision of health care services to homeless individuals. Outreach and service delivery is provided at homeless shelters by a mobile health team. Primary care medical services comprise the bulk of services rendered. Augmenting these services are participation in Title X (Family Planning), Title XV (Breast and Cervical Cancer Control Program), Vaccines for Children, onsite substance abuse treatment, and smoking cessation counseling. STI/HIV screening and treatment is available as well as enabling services by a social worker, nutritionist, and other support staff. Other services, such as dental, are provided through referral within Ingham County s Community Health Center Network. Networking with a local neighborhood center has yielded improved access to health care. Involvement with Michigan State University (MSU) Extension service offers a referral source to assist with chronic disease management. Attending physicians from MSU provide volunteer after hours medical services and limited access to specialty care on-site. Estimated homeless population: 1,200 Patients served: 3,

Heartside Clinic provides primary health care and support services to persons who are homeless, mainly those living in shelters or on the street in the downtown area of Grand Rapids. Clinica Santa Maria provides primary health care, OB/GYN, podiatry, dental, dietician, maternal and infant support services, and WIC to persons who are homeless, many of whom are transient or living with relatives and are Spanish speaking. Both sites offer "open access," offering both pre-scheduled and same-day appointments. Emergency care, laboratory, specialty clinic services, and prescription medicine are provided at a nearby hospital campus. The HCH program has established liaisons with Kent County Community Mental Health, Kent County Health Department, Family Independence Agency, local missions and shelters, Salvation Army, an array of substance abuse facilities, Catholic Social, and Dwelling Place of Grand Rapids, which is the major provider of SROs offering temporary housing. The stationary clinic operates five days per week. Direct services include prevention, medical, nursing, dental, social work, and pharmacy. Mental health and substance abuse services are provided on-site by DHCH staff in addition to referrals to community agencies and treatment facilities. Subspecialty, hospital, emergency, laboratory, and radiology services are provided by community resources. Social service problems are addressed by clinic social/outreach workers and a network of referrals in the community. Substance abuse treatment is available to individuals with alcohol/drug abuse problems who are homeless through community agreements and the Detroit Health Department Bureau of Substance Abuse. Psychiatric treatment is available through an agreement with Managed Care Group of Detroit and Wayne County (formally Detroit Central City Community Mental Health). DHCH also has linkages with New Center Mental Health, Detroit Health Department, and Visiting Nurses Association. St. John Health (System) provides specialty referral services and hospitalization.
